| 1 | Prognosis of ulcerative colitis differs between patients with complete and partial mucosal healing, which can be predicted from the platelet count显示文摘AIM:To determine the difference in clinical outcome between ulcerative colitis(UC)patients with Mayo endoscopic subscore(MES)0 and those with MES 1.METHODS:UC patients with sustained clinical remission of 6 mo or more at the time of colonoscopy were examined for clinical outcomes and the hazard ratios of clinical relapse according to MES.Parameters,including blood tests,to identify predictive factors for MES 0and slight endoscopic recurrence in clinically stablepatients were assessed.Moreover,a receiver operating characteristic curve was generated,and the area under the curve was calculated to indicate the utility of the parameters for the division between complete and partial mucosal healing.All P values were two-sided and considered significant when less than 0.05.RESULTS:A total of 183 patients with clinical remission were examined.Patients with MES 0(complete mucosal healing:n=80,44%)were much less likely to relapse than those with MES 1(partial mucosal healing:n=89,48%)(P<0.0001,log-rank test),and the hazard ratio of risk of relapse in patients with MES 1 vs MES0 was 8.17(95%CI:4.19-17.96,P<0.0001).The platelet count(PLT)<26×104/μL was an independent predictive factor for complete mucosal healing(OR=4.1,95%CI:2.15-7.99).Among patients with MES 0 at the initial colonoscopy,patients of whom colonoscopy findings shifted to MES 1 showed significant increases in PLT compared to those who maintained MES 0(3.8×104/μL vs-0.6×104/μL,P<0.0001).CONCLUSION:The relapse rate differed greatly between patients with complete and partial mucosal healing.A shift from complete to partial healing in clinically stable UC patients can be predicted by monitoring PLT. | Asuka Nakarai Jun Kato Sakiko Hiraoka Toshihiro Inokuchi Daisuke Takei Yuki Moritou Mitsuhiro Akita Sakuma Takahashi Keisuke Hori Keita Harada Hiroyuki Okada Kazuhide Yamamoto | 2014 | World Journal of Gastroenterology2014，20，48： | 11 |
| 2 | Clinical significance of white gastric crypt openings observed via magnifying endoscopy显示文摘AIM:To evaluate the relationship between Helicobacter pylori(H.pylori)-induced gastritis and white gastric mucosal crypt openings(COs)in the gastric corpus.METHODS:A total of 175 consecutive patients(including 69 patients with gastric cancer)were enrolled in this study.We used magnifying endoscopy(ME)to observe the mucosa microsurface of the lesser and greater curvature of the gastric corpus(350 areas in all).We focused on areas with a round pit microstructure(primarily observed in non-atrophied areas)and evaluated the white openings of these gastric pits.We classified the whiteness of the COs as the'white-edged dark spot'type(consisting of a dark spot bordered by white);the'white'type(pure white with no dark spot);and the'dense white pit(DWP)'type(dense white,resembling a snowball).Gastritis was also histologically evaluated according to the updated Sydney System.RESULTS:We detected round COs using ME in 246 of the 350 areas examined.The histological examination showed significantly more mononuclear cells and neutrophil infiltration in the'white'and'DWP'types than the'white-edged dark spot'type(P<0.001).Furthermore,significantly high-grade inflammation and evidence of active H.pylori-induced gastritis was observed in the'DWP'type(P<0.001).Significant differences were observed in the whiteness of COs between H.pylori-positive(n=139)and negative(n=36)patients(P<0.001).The sensitivity and specificity of the'white'and'DWP'types for predicting H.pylori infection were78.5%and 81.7%,respectively.Of the patients with gastric cancer,22.5%(18/80)had'white-edged dark spots',51.3%(41/80)had'white'COs,and 26.3%(21/80)had'DWP'-type COs.'DWPs'were frequently observed among patients with undifferentiated gastric cancer[45.7%(16/35)].CONCLUSION:CO whiteness detected via ME was associated with histological evidence of gastritis and helps to predict the severity of inflammation and H.pyloriinduced activity. | Masashi Kawamura Hitoshi Sekine Shu Abe Daisuke Shibuya Katsuaki Kato Takayuki Masuda | 2013 | World Journal of Gastroenterology2013，19，48： | 7 |
| 3 | Ulcerative colitis patients in clinical remission demonstrate correlations between fecal immunochemical test results, mucosal healing, and risk of relapse显示文摘AIM: To assess the risk of relapse in ulcerative colitis(UC) patients in clinical remission using mucosal status and fecal immunochemical test(FIT) results. METHODS: The clinical outcomes of 194 UC patients in clinical remission who underwent colonoscopy were based on evaluations of Mayo endoscopic subscores(MESs) and FIT results.RESULTS: Patients with an MES of 0(n = 94, 48%) showed a ten-fold lower risk of relapse than those with an MES of 1-3(n = 100, 52%)(HR = 0.10, 95%CI: 0.05-0.19). A negative FIT result(fecal hemoglobin concentrations ≤ 100 ng/m L) was predictive of patients with an MES of 0, with a sensitivity of 0.94 and a specific of 0.76. Moreover, patients with a negative FIT score had a six-fold lower risk of clinical relapse than those with a positive score(HR = 0.17, 95%CI: 0.10-0.28). Inclusion of the distinguishing parameter, sustaining clinical remission > 12 mo, resulted in an even stronger correlation between negative FIT results and an MES of 0 with respect to the risk of clinical relapse(HR = 0.11, 95%CI: 0.04-0.23).CONCLUSION: Negative FIT results one year or more after remission induction correlate with complete mucosal healing(MES 0) and better prognosis. | 5 | Successful biliary drainage using a metal stent through the gastric stoma显示文摘We report a case of biliary drainage for malignant stricture using a metal stent with an ultrathin endoscope through the gastric stoma. A 78-year-old female was referred to our hospital for jaundice and fever. She had undergone percutaneous endoscopic gastrostomy(PEG) for esophageal obstruction after radiation therapy for cancer of the pharynx. Abdominal contrast-enhanced computed tomography showed a 3-cm enhanced mass in the middle bile duct and dilatation of the intrahepatic bile duct. We initially performed endoscopic retrograde cholangiopancreatography(ERCP) with a trans-oral approach. However, neither the side-viewing endoscope nor the ultrathin endoscope passed through the esophageal orifice. Thus, we eventually performed ERCP via the PEG stoma using an ultrathin endoscope. We performed biliary drainage with a 6F introducer selfexpanding metal stent. The cytology findings obtained by brush cytology showed malignancy. | 8 | Prevalence of gastroesophageal reflux symptoms in a large unselected general population in Japan显示文摘AIM:To examine the prevalence of gastroesophageal reflux disease (GERD) symptoms in a large unselected general population in Japan. METHODS: In Japan, mature adults are offered regular check-ups for the prevention of gastric cancer. A notice was sent by mail to all inhabitants aged > 40 years. A total of 160 983 Japanese (60 774 male, 100 209 female; mean age 61.9 years) who underwent a stomach check up were enrolled in this study. In addition, from these 160 983 subjects, we randomly selected a total of 82 894 (34 275 male, 48 619 female; mean age 62.4 years) to evaluate the prevalence of abdominal pain. The respective subjects were prospectively asked to complete questionnaires concerning the symptoms of heartburn, dysphagia, and abdominal pain for a 1 mo period. RESULTS: The respective prevalences of the symptoms in males and females were: heartburn, 15.8% vs 20.7%; dysphagia, 5.4% vs 7.8%; and abdominal pain, 6.6% vs 9.6%. Among these symptoms, heartburn was significantly high compared with the other symptoms, and the prevalence of heartburn was significantly more frequent in females than in males in the 60-89-year agegroup. Dysphagia was also significantly more frequent in female patients. The photoluminescence(PL) emission spectra and PL decay time profiles were investigated by using Quantaurus-tau(Hamamatsu). The scintillation spectra and decay time profiles were measured by using our laboratory-constructed set-up under X-ray irradiation. Finally, TSL glow curve was measured by using Nanogray TL-2000 with the heating rate of 1 °C/s. In PL and scintillation, emission peaks were observed at 493, 543, 587 and 620 nm due to the 4f-4f transitions of Tb3^+. Decay time constants of the emission by PL and scintillation appeared to be different, and the measured values were approximately 2.4 and 1.9 ms, respectively.
