| 1 | Practice guidelines for the pathological diagnosis of primary liver cancer: 2015 update显示文摘In 2010, a panel of Chinese pathologists reported the first expert consensus for the pathological diagnosis of primary liver cancers to address the many contradictions and inconsistencies in the pathological characteristics and diagnostic criteria for PLC. Since then considerable clinicopathological studies have been conducted globally, prompting us to update the practice guidelines for the pathological diagnosis of PLC. In April 18, 2014, a Guideline Committee consisting of 40 specialists from seven Chinese Societies(including Chinese Society of Liver Cancer, Chinese Anti-Cancer Association; Liver Cancer Study Group, Chinese Society of Hepatology, Chinese Medical Association; Chinese Society of Pathology, Chinese Anti-Cancer Association; Digestive Disease Group, Chinese Society of Pathology, Chinese Medical Association; Chinese Society of Surgery, Chinese Medical Association; Chinese Society of Clinical Oncology, Chinese Anti-Cancer Association; Pathological Group of Hepatobiliary Tumor and Liver Transplantation, Chinese Society of Pathology, Chinese Medical Association) was created for the formulation of the first guidelines for the standardization of the pathological diagnosis of PLC, mainly focusing on the following topics: gross specimen sampling, concepts and diagnostic criteria of small hepatocellular carcinoma(SHCC), microvascular invasion(MVI), satellite nodules,and immunohistochemical and molecular diagnosis. The present updated guidelines are reflective of current clinicopathological studies, and include a novel 7-point baseline sampling protocol, which stipulate that at least four tissue specimens should be sampled at the junction of the tumor and adjacent liver tissues in a 1:1 ratio at the 12, 3, 6 and 9 o'clock reference positions. For the purposes of molecular pathological examination, at least one specimen should be sampled at the intratumoral zone, but more specimens should be sampled for tumors harboring different textures or colors. Specimens should be sampled at both adjacent and distant peritumoral liver tissues or the tumor margin in order to observe MVI, satellite nodules and dysplastic foci/nodules distributed throughout the background liver tissues. Complete sampling of whole SHCC ≤ 3 cm should be performed to assess its biological behavior, and in clinical practice, therapeutic borders should be also preserved, even in SHCC. The diagnostic criteria of MVI and satellite nodules, immunohistochemical panels, as well as molecular diagnostic principles, such as clonal typing, for recurrent HCC and multinodule HCC were also proposed and recommended. The standardized process of pathological examination is aimed at ensuring the accuracy of pathological PLC diagnoses as well as providing a valuable frame of reference for the clinical assessment of tumor invasive potential, the risk of postoperative recurrence, long-term survival, and the development of individualized treatment regimens. | 2 | Anisotropic coding metamaterials and their powerful manipulation of differently polarized terahertz waves显示文摘Metamaterials based on effective media can be used to produce a number of unusual physical properties(for example,negative refraction and invisibility cloaking)because they can be tailored with effective medium parameters that do not occur in nature.Recently,the use of coding metamaterials has been suggested for the control of electromagnetic waves through the design of coding sequences using digital elements‘0’and‘1,'which possess opposite phase responses.Here we propose the concept of an anisotropic coding metamaterial in which the coding behaviors in different directions are dependent on the polarization status of the electromagnetic waves.We experimentally demonstrate an ultrathin and flexible polarization-controlled anisotropic coding metasurface that functions in the terahertz regime using specially designed coding elements.By encoding the elements with elaborately designed coding sequences(both 1-bit and 2-bit sequences),the x-and y-polarized waves can be anomalously reflected or independently diffused in three dimensions.The simulated far-field scattering patterns and near-field distributions are presented to illustrate the dual-functional performance of the encoded metasurface,and the results are consistent with the measured results.We further demonstrate the ability of the anisotropic coding metasurfaces to generate a beam splitter and realize simultaneous anomalous reflections and polarization conversions,thus providing powerful control of differently polarized electromagnetic waves.The proposed method enables versatile beam behaviors under orthogonal polarizations using a single metasurface and has the potential for use in the development of interesting terahertz devices. | Shuo Liu Tie Jun Cui Quan Xu Di Bao Liangliang Du Xiang Wan Wen Xuan Tang Chunmei Ouyang Xiao Yang Zhou Hao Yuan Hui Feng Ma Wei Xiang Jiang Jiaguang Han Weili Zhang Qiang Cheng | 2016 | Light(Science & Applications)2016,5,1: | 36 |

| 7 | Epidemiology,clinical and laboratory characteristics of currently alive HIV-1 infected former blood donors naive to antiretroviral therapy in Anhui Province,China显示文摘Background Unregulated commercial blood/plasma collection among farmers occurred between 1992 and 1995 in central China and caused the second major epidemic of human immunodeficiency virus type 1 (HIV-1) infection in China. It is important to characterize HIV-1-infected former blood donors and to study characteristics associated with disease progression for future clinical intervention and vaccine development. Methods A cross-sectional study was performed on HIV-1-infected former blood donors (FBDs) and age-matched HIV-seronegative local residents. Demographic, epidemiologic, clinical and key laboratory data were collected from all study participants. Both unadjusted and adjusted multivariate linear regressions were employed to analyze the association of the decrease of CD4+ T-cell counts with other characteristics. Results Two hundred and ninety-four HIV-1-infected FBDs and 59 age-matched HIV-seronegative local residents were enrolled in this study. The unregulated blood/plasma collection occurred more than a decade (10.8 -12.8 years) ago, which caused the rapid spread of HIV-1 infection and the high prevalence of co-infection with hepatitis C virus (HCV, 89.5%); hepatitis B virus (HBV) co-infection was observed in only 11 HIV+participants (3.7%). Deterioration in both clinical manifestation and laboratory parameters and increase of viral loads were observed in parallel with the decrease of CD4+ T-cell counts. The decrease of total lymphocyte counts (P<0.001) and hemoglobin levels (P<0.001) and the appearance of dermatosis (P=0.03) were observed in parallel with the decrease of CD4+ T-cell counts whereas viral loads (P<0.001) and CD8+ T-cell counts (P=0.01) were inverselyassociated with CD4+ T-cell counts. Conclusions Co-infection with HCV but not HBV is highly prevalent among HIV-1-infected FBDs. CD4+ T-cell counts is a reliable indicator for disease progression among FBDs. | 8 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. | 9 | H2 strain attenuated live hepatitis A vaccines:protective efficacy in a hepatitis A outbreak显示文摘AIM To investigate the protective efficacy ofH2 strain attenuated live hepatitis A vaccines(H2-strain vaccines)in hepatitis A(HA)outbreaks.METHODS With the permission of theirparents,5551 pre-school and grade 1-3 primaryschool children were inoculated with 1 dose(106.5TCID50)of H2-strain vaccines in anonrandomized,controlled trial conducted inFucheng County,Hebei Province in May 1997.Another 6485 children in the same grades andcompatible in gender and age were enrolled ascontrols.Epidemiological and serological surveywas conducted to evaluate the protectiveefficacy of the vaccines.ELISA was used todetect serum IgM anti-HAV.RESULTS HA outbreak started in early May1998,peaked in the middle of the same month,and lasted about 80 days.Overall 302 HA caseswere found,192(53.58%)were 5-9 years old.One vaccinee and 25 control cases were found tohave hepatitis A,which account for 0.28%(1/356)and 5.92%(25/422)of all vaccinees andcontrols in the 14 villages,respectively.The protective efficacy of vaccines was 95.27%(95% Cl:85.83%-104.72%).In subjects testedfor anti-HAV IgM from 13 villages,1(0.40%)overt and 11(4.06%)asymptomatic HAV caseswere found in 271 vaccinees,but 21(6.69%)ofovert and asymptomatic ones were found in 314controls.CONCLUSION H2 strain vaccines were excellent in preventing overt hepatitis A, but not so effective in preventing asymptomatic hepatitis A virus infection. | 10 | A draft sequence of the rice (Oryza sativa ssp. indica) genome显示文摘The sequence of the rice genome holds fundamental information for its biology, including physiology, genetics, development, and evolution, as well as information on many beneficial phenotypes of economic significance. Using a 'whole genome shotgun' approach, we have pro-duced a draft rice genome sequence of Oryza sativa ssp. in-dica, the major crop rice subspecies in China and many other regions of Asia. The draft genome sequence is constructed from over 4.3 million successful sequencing traces with an accumulative total length of 2214.9 Mb. The initial assembly of the non-redundant sequences reached 409.76 Mb in length, based on 3.30 million successful sequencing traces with a total length of 1797.4 Mb from an indica variant cultivar 93-11, giving an estimated coverage of 95.29% of the rice genome with an average base accuracy of higher than 99%. | 12 | A Scan of Obesogenic Environments and a Spatial Inference of Obesity Prevalence in Chinese Children and Adolescents: Based on the Chinese Health and Nutrition Survey 2011 Data显示文摘Objective To identify the characteristics of Chinese obesogenic environments at a provincial level, infer a spatial distribution map of obesity prevalence in 31 provinces, and provide a foundation for development of policy to reduce obesity in children and adolescents. Methods After scanning obesity data on subjects aged 7-17 years from 12 provinces in the China Health and Nutrition Survey 2011 and environmental data on 31 provinces from the China Statistical Yearbook 2011 and other sources, we selected 12 predictors. We used the 12 surveyed provinces as a training sample to fit an analytical model with partial least squares regression and prioritized the 12 predictors using variable importance in projection. We also fitted a predictive model with Bayesian analysis. Results We identified characteristics of obesogenic environments. We fitted the predictive model with a deviance information criterion of 61.96 and with statistically significant(P < 0.05) parameter estimates of intercept [95% confidence interval(CI): 329.10, 963.11], log(oil)(CI: 13.11, 20.30), log(GDP)(CI: 3.05, 6.93), log(media)(CI:-234.95,-89.61), and log(washing-machine)(CI: 0.92, 5.07). The total inferred average obesity prevalence among those aged 7-17 was 9.69% in 31 Chinese provinces in 2011. We also found obvious clustering in occurrences of obesity in northern and eastern provinces in the predicted map. Conclusion Given complexity of obesity in children and adolescents, concerted efforts are needed to reduce consumption of edible oils, increase consumption of vegetables, and strengthen nutrition, health, and physical activity education in Chinese schools. The dimensionless maximum thermal resistance(DMTR) and the dimensionless equivalent thermal resistance(DETR) defined based on the entransy dissipation rate(EDR) are taken as performance evaluation indexes. According to constructal theory, the variations of the two indexes with the geometric parameters of the fin are analyzed by using a finite-volume computational fluid dynamics code, the effects of the fin-material fraction on the two indexes are analyzed. It is found that the two indexes decrease monotonically as the ratio between the front height and the back height of the fin increases subjected to the non-uniform height rectangular fin. When the model is reduced to the uniform height fin, the two indexes increase first and then decrease with increase in the ratio between the height of the fin and the fin space. The fin-material fraction has no effect on the change rule of the two indexes with the ratio between the height of the fin and the fin space. The sensitivity of the DETR to the geometric parameters of the fin is higher than that of the DMTR to the geometric parameters. Methods A cross-sectional study was conducted in adult populations of areas differing in iodine nutrition from three provinces(Autonomous Region) in China. Serum levels of thyroid hormones and Tg as well as thyroid autoantibodies were measured. The thyroid volume and nodule were measured by ultrasound. A multivariate linear regression analysis was used to assess iodine intake and other indeterminate factors associated with the serum Tg level. Results A total of 573 adults were recruited for this study. The serum Tg levels differed significantly among the three groups(22.27 μg/L, 9.73 μg/L and 15.77 μg/L in the excess, more-than-adequate, and deficient groups, respectively). The results of multivariate linear regression analysis indicate that excess and deficient iodine intake, goiter, thyroid nodule, hypothyroidism are significantly related with higher Tg level, and Tg Ab positivity is significantly related with lower serum Tg. The PtNW and multiwalled carbon nanotubes (CNT) are then dispersed into chitosan (CHIT) solution. The resulting PtNW-CNT-CHIT material brings new capabilities for electrochemical devices by using the synergistic action of the electrocatalytic activity of PtNW and CNT. By dropping the PtNW-CNT-CHIT film onto the glassy carbon (GC) electrode surface, and after evaporation an amperometric sensor for the determination of indole-3-acetic acid (IAA) was developed. The oxidation current of IAA increased significantly at the PtNW-CNT-CHIT film coated GC electrode, in contrast to that at the CNT-CHIT modified GC. | 20 | Further understanding of an uncommon disease of combined small cell lung cancer: clinical features and prognostic factors of 114 cases显示文摘Objective: Combined small cell lung cancer(C-SCLC) is an uncommon subgroup of small cell lung cancer(SCLC) and few clinical data can be referred.Our study is to investigate the clinical features and prognostic factors of C-SCLC, as well as the role of multimodality treatment.Methods: Between January 2004 and December 2012, patients with histologically diagnosed C-SCLC were retrospectively analyzed.The survivals were evaluated with the Kaplan-Meier method.Univariate and multivariate analyses were used to evaluate potential prognostic factors.Results: One hundred and fourteen patients were enrolled, with a median age of 59(range: 20-79) years old.The most common combined component was squamous cell carcinoma(52.6%).Among these patients, the disease was stage Ⅰ, Ⅱ, Ⅲ and Ⅳ in 9.6%, 19.3%, 46.5% and 24.6% of the patients, respectively.Eighty patients(70.2%)received at least two of the three modalities containing chemotherapy, radiotherapy and surgery.The median follow-up was 32.5 months.The median time of overall survival(OS) was 26.2 months.On univariate analysis,smoking(P=0.029), Karnofsky performance score(KPS) <80(P=0.000), advanced TNM stage(P=0.000), no surgery(P=0.010), positive resection margin(P=0.000), positive lymph nodes ≥4(P=0.000), positive lymph node ratio >10%(P=0.000) and non-multimodality treatment(P=0.004) were associated with poor OS.Multivariate analysis confirmed that smoking, advanced TNM stage, positive resection margin and positive lymph nodes ratio>10% were poor prognostic features.Conclusions: C-SCLC has a relatively early stage and good prognosis, which may due to the underestimated diagnosis in non-surgical patients.Multimodality therapy is recommended, especially for limited disease.Smoking,advanced TNM stage, positive resection margin and positive lymph nodes ratio >10% are poor prognostic factors. | Yu Men Zhouguang Hui Jun Liang Qinfu Feng Dongfu Chen Hongxing Zhang Zefen Xiao Zongmei Zhou Weibo Yin Luhua Wang | 2016 | Chinese Journal of Cancer Research2016,28,5: | 2 |
