1Gastric adenocarcinoma of the fundic gland(chief cell-predominant type): A review of endoscopic and clinicopathological features显示文摘Gastric adenocarcinoma of the fundic gland(chief cellpredominant type, GA-FG-CCP) is a rare variant of welldifferentiated adenocarcinoma, and has been proposed to be a novel disease entity. GA-FG-CCP originates from the gastric mucosa of the fundic gland region without chronic gastritis or intestinal metaplasia. The majority of GA-FG-CCPs exhibit either a submucosal tumor-like superficial elevated shape or a flat shape on macroscopic examination. Narrow-band imaging with endoscopic magnification may reveal a regular or an irregular microvascular pattern, depending on the degree of tumor exposure to the mucosal surface. Pathological analysis of GA-FG-CCPs is characterized by a high frequency of submucosal invasion, rare occurrences of lymphatic and venous invasion, and low-grade malignancy. Detection of diffuse positivity for pepsinogen-I by immunohistochemistry is specific for GA-FG-CCP. Careful endoscopic examination and detailed pathological evaluation are essential for early and accurate diagnosis of GA-FG-CCP. 2Gastric adenocarcinoma of fundic gland type: Five cases treated with endoscopic resection显示文摘Recently,a new disease entity termed gastric adenocarcinoma of fundic gland type(GA-FG) was proposed.We treated five cases of GA-FG with endoscopic submucosal dissection.All tumors were small and located in the upper third of the stomach.Four tumors were macroscopically identified as 0-IIa and one was identified as 0-Ⅱb.Narrow-band imaging with magnifying endoscopy showed an irregular microvascular pattern in 2 cases and a regular microvascular pattern in the remainder.All tumors arose from the deep layer of the lamina propria mucosae and showed submucosal invasion.Lymphatic invasion was seen only in one case,while no venous invasion was recognized.All tumors were positive for pepsinogen-Ⅰ and MUC6 by immunohistochemistry.None showed p53 overexpression,and the labeling index of Ki-67 was low in all cases.All cases have been free from recurrence or metastasis.Herein,we discussed the clinicopathological features of GA-FG in comparison with past reports.Masaki Miyazawa Mitsuru Matsuda Masaaki Yano Yasumasa Hara Fumitaka Arihara Yosuke Horita Koichiro Matsuda Akito Sakai Yatsugi Noda 2015World Journal of Gastroenterology2015,21,26:16
3Comprehensive and innovative techniques for livertransplantation in rats: A surgical guide显示文摘AIM: To investigate our learning curves of orthotopic liver transplantation (OLT) in rats and the most important factor for successful surgery. METHODS: We describe the surgical procedures for our rat OLT model, and determined the operator learning curves. The various factors that contributed to successful surgery were determined. The most important surgical factors were evaluated between successful and unsuccessful surgeries.RESULTS: Learning curve data indicated that 50 cases were required for operator training to start a study. Operative time, blood loss, warm ischemic time, anhepatic phase, unstable systemic hemodynamic state, and body temperature after surgery significantly affected surgery success by univariate analysis, while the anhepatic phase was the most critical factor for success by multivariate analysis. CONCLUSION: OLT in rats is the only liver transplantation model that provides clinically relevant and reliable results. 4Hepatitis C virus enhances incidence of idiopathic pulmonary fibrosis显示文摘AIM: To investigate the cumulative development incidence and predictive factors for idiopathic pulmonary fibrosis in hepatitis C virus (HCV) positive patients. METHODS: We studied 6150 HCV infected patients who were between 40-70 years old (HCV-group). Another 2050 patients with hepatitis B virus (HBV) were selected as control (HBV-group). The mean observation period was 8.0 ± 5.9 years in HCV-group and 6.3 ± 5.5 years in HBV-group. The primary goal is the development of idiopathic pulmonary fibrosis (IPF) in both groups. The cumulative appearance rate of IPF and independent factors associated with the incidence rate of IPF were calculated using the Kaplan- Meier method and the Cox proportional hazard model. All of the studies were performed retrospectively by collecting and analyzing data from the patient records in our hospital. RESULTS: Fifteen patients in HCV-group developedIPF. On the other hand, none of the patients developed IPF in HBV-group. In HCV-group, the cumulative rates of IPF development were 0.3% at 10th year and 0.9% at 20th year. The IPF development rate in HCV-group was higher than that in HBV-group (P = 0.021). The IPF development rate in patients with HCV or HBV was high with statistical significance in the following cases: (1) patients ≥ 55 years (P < 0.001); (2) patients who had smoking index (package per day × year) of ≥ 20 (P = 0.002); (3) patients with liver cirrhosis (P = 0.042). 5Gastric adenocarcinoma of fundic gland type spreading to heterotopic gastric glands显示文摘Herein, we present a case of gastric adenocarcinoma of fundic gland type(GA-FG) spreading to heterotopic gastric glands(HGG) in the submucosa. A 58-year-old man with epigastric pain was referred to our hospital and underwent an esophagogastroduodenoscopy. A Borrmann type II gastric cancer at the antrum and a 10 mm submucosal tumor-like lesion in the lesser curvature of the upper third of the stomach were detected. Histological examination of the biopsy specimens obtained from the submucosal tumorlike lesion suggested a GA-FG. Therefore, endoscopic submucosal dissection was performed as excisional biopsy, and histopathological examination of the resected specimen confirmed a GA-FG and HGG proximal to the GA-FG. Although the GA-FG invaded the submucosal layer slightly, the submucosal lesion of the GA-FG had a poor stromal reaction and was located just above the HGG in the submucosa. 6Endoscopic submucosal dissection as excisional biopsy for anorectal malignant melanoma:A case report显示文摘BACKGROUND Anorectal malignant melanoma (AMM) is a rare disorder with an extremely poor prognosis. Although there is currently no consensus on the treatment methods for AMM, surgical procedures have been the most common treatment methods used until now. We recently encountered a case of AMM that we diagnosed using endoscopic submucosal dissection (ESD). To our knowledge, this is the first case of ESD for AMM, suggesting that ESD can potentially be a diagnostic and treatment method for AMM. CASE SUMMARY A 77-year-old woman visited our hospital with a chief complaint of anal bleeding and a palpable rectal mass. Colonoscopy revealed a 20-mm protruded lesion in the lower rectum. After obtaining biopsy specimens from the lesion, although a malignant rectal tumor was suspected, a definitive diagnosis was not made. Endoscopic ultrasonography revealed tumor invasion into the submucosal layer but not the muscular layer. Therefore, we performed an excisional biopsy using ESD. Immunohistochemical examination of the ESD-resected specimen revealed tumor cells positive for Human Melanin Black-45, Melan-A, and S-100. Moreover, the tumor cells lacked melanin pigment;thus, a diagnosis of amelanotic AMM was made. Although the AMM had massively invaded the submucosal layer and both lymphatic and venous invasion were present, we closely monitored the patient without any additional therapy on the basis of her request. Six months after ESD, local recurrence was detected, and the patient consented to wide local excision. 8Reaction mechanism of phosphorus with multi phase fluxes during hot metal dephosphorization显示文摘The formation of solid solution combined with tricalcium phosphate and dicalcium silicate could promote a considerable removal of phosphorus from liquid slag to solid during hot metal dephosphorization,and thus the dephosphorization by using multi phase fluxes could significantly decrease the consumption of lime. 10The effect of basic fibroblast growth factor on regeneration in a surgical wound model of rat submandibular glands显示文摘This study developed an animal model of surgically wounded submandibular glands(SMGs)and investigated the effects of collagen gel with basic fibroblast growth factor(b FGF)on tissue regeneration of surgically wounded SMGs in vivo.The animal model was produced by creating a surgical wound using a 3-mm diameter biopsy punch in SMGs.The wound was filled with collagen gel with b FGF(b FGF group)or without b FGF(control group).In the animal model of surgically wounded SMGs,salivary glands without scar tissue around the wound area were observed with smaller areas of collagen gel.Small round and spindle-shape cells invaded the collagen gel in both groups after operation day(AOD)5,and this invasion dramatically increased at AOD 7.Host tissue completely replaced the collagen gel at AOD 21.The invading immune cells in the group treated with collagen gel with b FGF were positive for vimentin,a-smooth muscle actin(a SMA),CD49f,c-kit and AQP5 at AOD 7.Similarly,the m RNA expression of vimentin,a SMA,CD49f,keratin19 and AQP5 was also increased.This study suggests that the use of collagen gels with b FGF improves salivary gland regeneration.Fumitaka Kobayashi Kenichi Matsuzaka Takashi Inoue 2016International Journal of Oral Science2016,8,1:4

13Protocol for laparoscopic cholecystectomy: Is it rocket science?显示文摘Laparoscopic cholecystectomy(LC) does not require advanced techniques, and its performance has therefore rapidly spread worldwide. However, the rate of biliary injuries has not decreased. The concept of the critical view of safety(CVS) was first documented two decades ago. Unexpected injuries are principally due to misidentification of human factors. The surgeon's assumption is a major cause of misidentification, and a high level of experience alone is not sufficient for successful LC. We herein describe tips and pitfalls of LC in detail and discuss various technical considerations.Finally, based on a review of important papers and our own experience, we summarize the following mandatory protocol for safe LC:(1) consideration that a high level of experience alone is not enough;(2) recognition of the plateau involving the common hepatic duct and hepatic hilum;(3) blunt dissection until CVS exposure;(4) Calot's triangle clearance in the overhead view;(5) Calot's triangle clearance in the view from underneath;(6) dissection of the posterior right side of Calot's triangle;(7) removal of the gallbladder body; and(8) positive CVS exposure. 14Laparoscopic-assisted catheter insertion for continuous ambulatory peritoneal dialysis:A case report of simple technique for optimal placement显示文摘A 40-year-old male underwent tube placement surgery for continuous ambulatory peritoneal dialysis(CAPD).A2-cm skin incision was made,and the peritoneum was reflected enough to perform secure fixation.A swannecked,double-felted silicone CAPD catheter was inserted,and the felt cuff was sutured to the peritoneum to avoid postoperative leakage.An adequate gradient for tube fixation to the abdominal wall was confirmed.The CAPD tube was passed through a subcutaneous tunnel.Aeroperitoneum was induced to confirm that there was no air leakage from the sites of CAPD insertion.Two trocars were placed,and we confirmed that the CAPD tube led to the rectovesical pouch.Tip position was reliably observed laparoscopically.Optimal patency of the CAPD tube was confirmed during surgery.Placement of CAPD catheters by laparoscopic-assisted surgery has clear advantages in simplicity,safety,flexibility,and certainty.Laparoscopic technique should be considered the first choice for CAPD tube insertion.Tomohide Hori Masaya Nakauchi Kazuhiro Nagao Fumitaka Oike Takahiro Tanaka Daigo Gunji Noriyuki Okada 2013World Journal of Gastrointestinal Surgery2013,5,10:2

16STAT3 deficiency prevents hepatocarcinogenesis and promotes biliary proliferation in thioacetamide-induced liver injury显示文摘AIM To elucidate the role of STAT3 in hepatocarcinogenesis and biliary ductular proliferation following chronic liver injury. METHODS We investigated thioacetamide(TAA)-induced liver injury, compensatory hepatocyte proliferation, and hepatocellular carcinoma(HCC) development in hepatic STAT3-deficient mice. In addition, we evaluated TAAinduced biliary ductular proliferation and analyzed the activation of sex determining region Y-box9(SOX9) and Yes-associated protein(YAP), which regulate the transdifferentiation of hepatocytes to cholangiocytes.RESULTS Both compensatory hepatocyte proliferation and HCC formation were significantly decreased in hepatic STAT3-deficient mice as compared with control mice. STAT3 deficiency resulted in augmentation of hepatic necrosis and fibrosis. On the other hand, biliary ductular proliferation increased in hepatic STAT3-deficient livers as compared with control livers. 17Diffusion-weighted magnetic resonance imaging in management of bladder cancer, particularly with multimodal bladder-sparing strategy显示文摘Bladder-sparing strategy for muscle-invasive bladdercancer(MIBC) is increasingly demanded instead ofradical cystectomy plus urinary diversion. Multimodaltherapeutic approaches consisting of transurethralresection, chemotherapy, radiotherapy and/or partialcystectomy improve patients' quality of life by preserv-ing their native bladder and sexual function withoutcompromising oncological outcomes. Because a favor-able response to chemoradiotherapy(CRT) is a prereq-uisite for successful bladder preservation, predictingand monitoring therapeutic response is an essentialpart of this approach. Diffusion-weighted magneticresonance imaging(DW-MRI) is a functional imagingtechnique increasingly applied to various types of can-cers. Contrast in this imaging technique derives fromdifferences in the motion of water molecules among tissues and this information is useful in assessing the biological behavior of cancers. Promising results in pre-dicting and monitoring the response to CRT have been reported in several types of cancers. Recently, growing evidence has emerged showing that DW-MRI can serve as an imaging biomarker in the management of blad-der cancer. The qualitative analysis of DW-MRI can be applied to detecting cancerous lesion and monitoring the response to CRT. Furthermore, the potential role of quantitative analysis by evaluating apparent diffu-sion coefficient values has been shown in character-izing bladder cancer for biological aggressiveness and sensitivity to CRT. 20Dissolution of Al_2TiO_5 inclusions in CaO-SiO_2-Al_2O_3 slags at 1823 K显示文摘Al-Ti-O inclusions always clog submerged nozzles in Ti-bearing Al-killed steel.A typical synthesized Al2TiO5 inclusion was immersed in a CaO-SiO2-Al2O3 molten slag for different durations at 1823 K.The Al2TiO5 dissolution paths and mechanism were revealed by scanning electron microscopy (SEM) and energy dispersive spectroscopy (EDS).Decreased amounts of Ti and Al and increased amounts of Si and Ca at the dissolution boundary prove that inclusion dissolution and slag penetration simultaneously occur.SiO2 diffuses or penetrates the inclusion more quickly than CaO,as indicated by the w(CaO)/w(SiO2) value in the reaction region.A liquid product (containing 0.7-1.2 w(CaO)/w(SiO2),15wt%-20wt% Al2O3,and 5wt%-15wt% TiO2) forms on the inclusion surface when Al2TiO5 is dissolved in the slag.Al2TiO5 initially dissolves faster than the diffusion rate of the liquid product toward the bulk slag.With increasing reaction time,the boundary reaches its largest distance,the Al2TiO5 dissolution rate equals the liquid product diffusion rate,and the dissolution process remains stable until the inclusion is completely dissolved.De-yong Wang Jun Liu Mao-fa Jiang Fumitaka Tsukihashi Hiroyuki Matsuura 2011International Journal of Minerals,Metallurgy and Materials2011,18,6:2
