1Systematic review of anastomotic complications of esophagojejunostomy after laparoscopic total gastrectomy显示文摘AIM: To investigate the anastomotic complications ofesophagojejunostomy(EJS) after laparoscopic total gastrectomy(LTG), we reviewed retrospective studies.METHODS: A literature search was conducted in Pub Med for studies published from January 1, 1994 through January 31, 2015. The search terms included 'laparoscopic,' 'total gastrectomy,' and 'gastric cancer.' First, we selected 16 non-randomized controlled trials(RCTs) comparing LTG with open total gastrectomy(OTG) and conducted an updated meta-analysis of anastomotic complications after total gastrectomy. The Newcastle-Ottawa scoring system(NOS) was used to assess the quality of the non-RCTs included in this study. Next, we reviewed anastomotic complications in 46 case studies of LTG to compare the various procedures for EJS. RESULTS: The overall incidence of anastomotic leakage associated with EJS was 3.0%(30 of 984 patients) among LTG procedures and 2.1%(31 of 1500 patients) among OTG procedures in the 16 non-RCTs. The incidence of anastomotic leakage did not differ significantly between LTG and OTG(odds OR = 1.42, 95%CI: 0.86-2.33, P = 0.17, I2 = 0%). Anastomotic stenosis related to EJS was reported in 72(2.9%) of 2484 patients, and the incidence was 3.2% among LTG procedures and 2.7% among OTG procedures. The incidence of anastomotic stenosis related to EJS was slightly, but not significantly, higher in LTG than in OTG(OR = 1.55, 95%CI: 0.94-2.54, P = 0.08, I2 = 0%). The various procedures for LTG were classified into six categories in the review of case studies of LTG. The incidence of EJS leakage was similar(1.1% to 3.2%), although the incidence of EJS stenosis was relatively high when the Or VilTM device was used(8.8%) compared with other procedures(1.0% to 3.6%).CONCLUSION:The incidence of anastomotic complications associated with EJS was not different between LTG and OTG. 3Endoscopic ultrasound-guided fine needle aspiration in the differentiation of type 1 and type 2 autoimmune pancreatitis显示文摘AIM:To investigate the usefulness of endoscopic ultrasound-guided fine needle aspiration(EUS-FNA) in the differentiation of autoimmune pancreatitis(AIP).METHODS:We retrospectively reviewed 47 of 56 AIP patients who underwent EUS-FNA and met the Asian diagnostic criteria.On 47 EUS-FNA specimens,we evaluated the presence of adequate material and characteristic features of lymphoplasmacytic sclerosing pancreatitis(LPSP) and idiopathic duct-centric pancreatitis(IDCP) mentioned in the International Consensus Diagnostic Criteria and examined if these findings make a contribution to the differential diagnosis of type 1 and type 2 AIP.A disposable 22-gauge needle was used for EUS-FNA.RESULTS:Adequate specimens including pancreatic tissue for differentiating AIP from cancer were obtained from 43 of 47 patients who underwent EUSFNA.EUS-FNA was performed from the pancreatic head in 21 cases,which is known to be technically difficult when performed by core biopsy;there was no significant difference in the results compared with pancreatic body-tail.Nine of 47 patients met level 1 findings of LPSP and 5 patients met level 2 findings of LPSP.No one met level 1 findings of IDCP,but 3 patients met level 2 findings of IDCP.Of 10 seronegative cases,2 cases were diagnosed with 'definitive type 1 AIP',and 3 cases were diagnosed with 'probable type 2 AIP' when considering both the level 2 histological findings and response to steroids.CONCLUSION:EUS-FNA is useful in the differentiation of type 1 and type 2 AIP,particularly in seronegative cases.Takuya Ishikawa Akihiro Itoh Hiroki Kawashima Eizaburo Ohno Hirosh Matsubara Yuya Itoh Yosuke Nakamura Takeshi Hiramatsu Masanao Nakamura Ryoji Miyahara Naoki Ohmiya Hidemi Goto Yoshiki Hirooka 2012World Journal of Gastroenterology2012,18,29:8
4Clinical significance of MET in gastric cancer显示文摘Chemotherapy has become the global standard treatment for patients with metastatic or unresectable gastric cancer(GC),although outcomes remain unfavorable.Many molecular-targeted therapies inhibiting signaling pathways of various tyrosine kinase receptors have been developed,and monoclonal antibodies targeting human epidermal growth factor receptor 2 or vascular endothelial growth factor receptor 2 have become standard therapy for GC.Hepatocyte growth factor and its receptor,c-MET(MET),play key roles in tumor growth through activated signaling pathways from receptor in GC cells.Genomic amplification of MET leads to the aberrant activation found in GC tumors and is related to survival in patients with GC.This review discusses the clinical significance of MET in GC and examines MET as a potential therapeutic target in patients with GC.Preclinical studies in animal models have shown that MET antibodies or smallmolecule MET inhibitors suppress tumor-cell proliferation and tumor progression in MET-amplified GC cells.These drugs are now being evaluated in clinical trials as treatments for metastatic or unresectable GC.Mikito Inokuchi Sho Otsuki Yoshitaka Fujimori Yuya Sato Masatoshi Nakagawa Kazuyuki Kojima 2015World Journal of Gastrointestinal Oncology2015,7,11:5
5Cellular Caspase-3 Contributes to EV-A71 2Apro-Mediated Down-Regulation of IFNAR1 at the Translation Level显示文摘Enterovirus A71(EV-A71) is the major pathogen responsible for the severe hand, foot and mouth disease worldwide, for which few effective antiviral drugs are presently available. Interferon-a(IFN-a) has been used in antiviral therapy for decades;it has been reported that EV-A71 antagonizes the antiviral activity of IFN-a based on viral 2 Apro-mediated reduction of the interferon-alpha receptor 1(IFNAR1);however, the mechanism remains unknown. Here, we showed a significant increase in IFNAR1 protein induced by IFN-a in RD cells, whereas EV-A71 infection caused obvious downregulation of the IFNAR1 protein and blockage of IFN-a signaling. Subsequently, we observed that EV-A71 2 Apro inhibited IFNAR1 translation by cleavage of the eukaryotic initiation factor 4 GI(eIF4GI), without affecting IFNAR1 m RNA levels induced by IFN-a. The inhibition of IFNAR1 translation also occurred in puromycin-induced apoptotic cells when caspase-3 cleaved e IF4 GI. Importantly, we verified that 2 Aprocould activate cellular caspase-3, which was subsequently involved in e IF4 GI cleavage mediated by 2 Apro. Furthermore, inhibition of caspase-3 activation resulted in the partial restoration of IFNAR1 in cells transfected with 2 A or infected with EV-A71, suggesting the pivotal role of both viral 2 Aproand caspase-3 activation in the disturbance of IFN-a signaling. 7A Combined Computational and Experimental Study on the Structure-Regulation Relationships of Putative Mammalian DNA Replication Initiator GINS显示文摘GINS, a heterotetramer of SLD5, PSF1, PSF2, and PSF3 proteins, is an emerging chromatin factor recognized to be involved in the initiation and elongation step of DNA replication. Although the yeast and Xenopus GINS genes are well docu- mented, their orthologous genes in higher eukaryotes are not fully characterized. In this study, we report the genomic structure and transcriptional regulation of mammalian GINS genes. Serum stimulation increased the GINS mRNA levels in human cells. Reporter gene assay using putative GINS promoter sequences revealed that the expression of mammalian GINS is regulated by 17β-Estradiol- stimulated estrogen receptor α, and human PSF3 acts as a gene responsive to transcription factor E2F1. 9Sediment organic matter source estimation and ecological classification in the semi-enclosed Batan Bay Estuary, Philippines显示文摘The large organic matter flow in tropical coastal areas is recognized as an important process in the global carbon(C)cycle.However,the nature of organic matter flow in semi-enclosed tropical estuaries remains unclear due to the various environmental processes(tidal change,river flow,waves from the sea,and internal circulation)and organic matter sources therein.Thus,sediment organic matter(SOM)sources,and their distribution pattern,are key to understanding ecosystem material flow.Our research in the Batan Bay Estuary,Philippines,a semi-enclosed estuary under large mangrove deforestation,was conducted to determine ecosystem properties through analysis of C and nitrogen stable isotope ratios and environmental factors.First,we determined that mangrove litter,microphytobenthos,and phytoplankton are the main SOM sources in the Batan Bay Estuary.Second,the estuary was classified into three ecological zones(the Bay zone,Back-barrier zone,and River zone).In addition,we estimated SOM source ratios using the Stable Isotope Analysis in R package and determined different organic matter sources in different zone.The high ratios of mangrove litter as SOM indicate that a large amount of terrestrial plant organic matter remains despite the heavy mangrove deforestation that has occurred since the 1980s,and that the Back-barrier zone consists of a different type of ecosystem that promotes accumulation of C from mangrove litter and microphytobenthos.Yuya Ogawa Yuki Okamoto Resurreccion Bitoon Sadaba Mamoru Kanzaki 2021International Journal of Sediment Research2021,36,1:3

16The deep-space multi-object orbit determination system and its application to Hayabusa2’s asteroid proximity operations显示文摘The deep-space multi-object orbit determination system(DMOODS)and its application in the asteroid proximity operation of the Hayabusa2 mission are described.DMOODS was developed by the Japan Aerospace Exploration Agency(JAXA)for the primary purpose of determining the trajectory of deep-space spacecraft for JAXA’s planetary missions.The weighted least-squares batch filter is used for the orbit estimator of DMOODS.The orbit estimator supports more than 10 data types,some of which are used for relative trajectory measurements between multiple space objects including natural satellites and small bodies.This system consists of a set of computer programs running on Linux-based consumer PCs on the ground,which are used for orbit determination and the generation of radiometric tracking data,such as delta differential one-way ranging and doppler tracking data.During the asteroid proximity phase of Hayabusa2,this system played an essential role in operations that had very strict navigation requirements or operations in which few optical data were obtained owing to special constraints on the spacecraft attitude or distance from the asteroid.One example is orbit determination during the solar conjunction phase,in which the navigation accuracy is degraded by the effect of the solar corona.The large range bias caused by the solar corona was accurately estimated with DMOODS by combining light detection and ranging(LIDAR)and ranging measurements in the superior solar conjunction phase of Hayabusa2.For the orbiting operations of target markers and the MINERVA-II2 rover,the simultaneous estimation of six trajectories of four artificial objects and a natural object was made by DMOODS.This type of simultaneous orbit determination of multi-artificial objects in deep-space has never been accomplished before.Hiroshi Takeuchi Kent Yoshikawa Yuto Takei Yusuke Oki Shota Kikuchi Hitoshi Ikeda Stefania Soldini Naoko Ogawa Yuya Mimasu Go Ono Fuyuto Terui Naoya Sakatani Manabu Yamada Toru Kouyama Shingo Kameda Takanao Saiki Yuichi Tsuda 2020Astrodynamics2020,4,4:2
17Detection of active bleeding from gastric antral vascular ectasia by capsule endoscopy显示文摘Gastric antral vascular ectasia(GAVE) has been recognized as one of the important causes of occult and obscure gastrointestinal bleeding.The diagnosis is typically made based on the characteristic endoscopic features,including longitudinal row of flat,reddish stripes radiating from the pylorus into the antrum that resemble the stripes on a watermelon.These appearances,however,can easily be misinterpreted as moderate to severe gastritis.Although it is believed that capsule endoscopy(CE) is not helpful for the study of the stomach with its large lumen,GAVE can be more likely to be detected at CE rather than conventional endoscopy.CE can be regarded as 'physiologic' endoscopy,without the need for gastric inflation and subsequent compression of the vasculature.The blood flow of the ecstatic vessels may be diminished in an inflated stomach.Therefore,GAVE may be prominent in CE.We herein describe a case of active bleeding from GAVE detected by CE and would like to emphasize a possibility that CE can improve diagnostic yields for GAVE.Tetsuya Ohira Akira Hokama Nagisa Kinjo Manabu Nakamoto Chiharu Kobashigawa Yuya Kise Satoshi Yamashiro Fukunori Kinjo Yukio Kuniyoshi Jiro Fujita 2013World Journal of Gastrointestinal Endoscopy2013,5,3:2
