| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 2 | Multicenter study of the clinicopathological features and recurrence risk prediction model of early-stage breast cancer with low-positive human epidermal growth factor receptor 2 expression in China (Chinese Society of Breast Surgery 021)显示文摘Background:Breast cancer with low-positive human epidermal growth factor receptor 2(HER2)expression has triggered further refinement of evaluation criteria for HER2 expression.We studied the clinicopathological features of early-stage breast cancer with low-positive HER2 expression in China and analyzed prognostic factors.Methods:Clinical and pathological data and prognostic information of patients with early-stage breast cancer with low-positive HER2 expression treated by the member units of the Chinese Society of Breast Surgery and Chinese Society of Surgery of Chinese Medical Association,from January 2015 to December 2016 were collected.The prognostic factors of these patients were analyzed.Results:Twenty-nine hospitals provided valid cases.From 2015 to 2016,a total of 25,096 cases of early-stage breast cancer were treated,7642(30.5%)of which had low-positive HER2 expression and were included in the study.After ineligible cases were excluded,6486 patients were included in the study.The median follow-up time was 57 months(4-76 months).The disease-free survival rate was 92.1%at 5 years,and the overall survival rate was 97.4%at 5 years.At the follow-up,506(7.8%)cases of metastasis and 167(2.6%)deaths were noted.Multivariate Cox regression analysis showed that tumor stage,lymphvascular invasion,and the Ki67 index were related to recurrence and metastasis(P<0.05).The recurrence risk prediction model was established using a machine learning model and showed that the area under the receiving operator characteristic curve was 0.815(95%confidence interval:0.750-0.880).Conclusions:Early-stage breast cancer patients with low-positive HER2 expression account for 30.5%of all patients.Tumor stage,lymphvascular invasion,and the Ki67 index are factors affecting prognosis.The recurrence prediction model for breast cancer with low-positive HER2 expression based on a machine learning model had a good clinical reference value for predicting the recurrence risk at 5 years.Trial registration:ChiCTR.org.cn,ChiCTR2100046766. | Ling Xin Qian Wu Chongming Zhan Hongyan Qin Hongyu Xiang Ling Xu Jingming Ye Xuening Duan Yinhua Liu Chinese Society of Breast Surgery(CSBrS) Chinese Society of Surgery of Chinese Medical Association | 2022 | Chinese Medical Journal2022,,6: | 9 |

| 4 | Pain threshold, anxiety and other factors affect intensity of postoperative pain in gastric cancer patients: A prospective cohort study显示文摘Objective: This prospective cohort study explored factors related to postoperative pain in gastric cancer patients.Methods: A total of 236 patients who underwent gastrectomy were enrolled. All patients enrolled in the study completed the Hospital Anxiety and Depression Scale(HADS) questionnaire and Life Orientation Test-Revised(LOT-R) questionnaire on the day before surgery. Heat pain threshold(HPT), cold pain threshold(CPT) and pressure pain threshold(PPT) were measured for all patients one day prior to surgery and demographic details were collected. All patients were connected to a patient-controlled intravenous analgesia(PCIA) pump at the end of the surgery. The occurrence of postoperative pain was used as a dependent variable, and multivariate logistic regression analyses were conducted to screen for factors affecting postoperative pain.Results: In total, 83 patients(35.2%) had postoperative pain. | 8 | Clinicopathological analysis of early-stage breast cancer patients that meet indications for BRCA 1/2 genetic testing显示文摘Objective:To investigate the clinicopathological characteristics and prognostic factors of early-stage breast cancer patients with indications for breast cancer susceptibility genes 1/2(BRCA1/2)genetic testing in China.Methods:Based on the indication criteria for BRCA genetic testing specified in the National Comprehensive Cancer Network(NCCN)clinical practice guidelines in oncology,genetic/familial high-risk assessment:Breast and ovarian(Version 2.2019),a retrospective analysis was performed on patients with early-stage invasive breast cancer treated at Breast Disease Center,Peking University First Hospital between January 2008 and December 2016.Clinicopathological characteristics of all patients were analyzed,and prognoses were calculated using the KaplanMeier method and a Cox proportionate hazards model.Results:A total of 906 early-stage breast cancer patients who had indications for BRCA genetic testing and had complete clinicopathological data and follow-up information were included in the study group,accounting for34.7%of all breast cancer patients treated in Breast Disease Center,Peking University First Hospital during the study period.Compared with breast cancer patients without indications for BRCA genetic testing,the overall survival(OS)and disease-free survival(DFS)of patients with indications were not significantly different.In the study group,patients with premenopausal status,high T stage,lymph node positive,estrogen receptor(ER)negative,Ki-67>20%and presence of a vascular tumor thrombus had worse prognosis.There were more family histories of gastrointestinal cancer in patients with related indications than in patients without such indications.Conclusions:Single-center data showed that more than 30%of patients with early-stage breast cancer had indications for BRCA genetic testing.There was no prognostic difference in patients with or without indications for BRCA genetic testing.Premenopausal status,high T stage,lymph node positive,ER negative,Ki-67>20%,and presence of a vascular tumor thrombus were associated with poor prognosis. | Hongyu Xiang Ling X in Qian Liu Hong Zhang Shuang Zhang Jingming Ye Yuanjia Cheng Ting Li Yinhua Liu Ling Xu | 2020 | Chinese Journal of Cancer Research2020,32,2: | 1 |
| 9 | Chromosome-level genome assemblies of five Prunus species and genome-wide association studies for key agronomic traits in peach显示文摘Prunus species include many important perennial fruit crops,such as peach,plum,apricot,and related wild species.Here,we report de novo genome assemblies for five species,including the cultivated species peach(Prunus persica),plum(Prunus salicina),and apricot(Prunus armeniaca),and the wild peach species Tibetan peach(Prunus mira)and Chinese wild peach(Prunus davidiana).The genomes ranged from 240 to 276 Mb in size,with contig N50 values of 2.27−8.30Mb and 25,333−27,826 protein-coding gene models.As the phylogenetic tree shows,plum diverged from its common ancestor with peach,wild peach species,and apricot~7 million years ago(MYA).We analyzed whole-genome resequencing data of 417 peach accessions,called 3,749,618 high-quality SNPs,577,154 small indels,31,800 deletions,duplications,and inversions,and 32,338 insertions,and performed a structural variant-based genome-wide association study(GWAS)of key agricultural traits.From our GWAS data,we identified a locus associated with a fruit shape corresponding to the OVATE transcription factor,where a large inversion event correlates with higher OVATE expression in flat-shaped accessions.Furthermore,a GWAS revealed a NAC transcription factor associated with fruit developmental timing that is linked to a tandem repeat variant and elevated NAC expression in early-ripening accessions.We also identified a locus encoding microRNA172d,where insertion of a transposable element into its promoter was found in double-flower accessions.Thus,our efforts have suggested roles for OVATE,a NAC transcription factor,and microRNA172d in fruit shape,fruit development period,and floral morphology,respectively,that can be connected to traits in other crops,thereby demonstrating the importance of parallel evolution in the diversification of several commercially important domesticated species.In general,these genomic resources will facilitate functional genomics,evolutionary research,and agronomic improvement of these five and other Prunus species.We believe that structural variant-based GWASs can also be used in other plants,animal species,and humans and be combined with deep sequencing GWASs to precisely identify candidate genes and genetic architecture components. | Qiuping Tan Sen Li Yuzheng Zhang Min Chen Binbin Wen Shan Jiang Xiude Chen Xiling Fu Dongmei Li Hongyu Wu Yong Wang Wei Xiao Ling Li | 2021 | Horticulture Research2021,8,1: | 1 |

| 14 | NR5A2 connects zygotic genome activation to the first lineage segregation in totipotent embryos显示文摘Zygotic genome activation(ZGA)marks the beginning of the embryonic program for a totipotent embryo,which gives rise to the inner cell mass(ICM)where pluripotent epiblast arises,and extraembryonic trophectoderm.However,how ZGA is connected to the first lineage segregation in mammalian embryos remains elusive.Here,we investigated the role of nuclear receptor(NR)transcription factors(TFs),whose motifs are highly enriched and accessible from the 2-cell(2C)to 8-cell(8C)stages in mouse embryos.We found that NR5A2,an NR TF strongly induced upon ZGA,was required for this connection.Upon Nr5a2 knockdown or knockout,embryos developed beyond 2C normally with the zygotic genome largely activated.However,4–8C-specific gene activation was substantially impaired and Nr5a2-deficient embryos subsequently arrested at the morula stage.Genome-wide chromatin binding analysis showed that NR5A2-bound cis-regulatory elements in both 2C and 8C embryos are strongly enriched for B1 elements where its binding motif is embedded.NR5A2 was not required for the global opening of its binding sites in 2C embryos but was essential to the opening of its 8C-specific binding sites.These 8C-specific,but not 2C-specific,binding sites are enriched near genes involved in blastocyst and stem cell regulation,and are often bound by master pluripotency TFs in blastocysts and embryonic stem cells(ESCs).Importantly,NR5A2 regulated key pluripotency genes Nanog and Pou5f1/Oct4,and primitive endoderm regulatory genes including Gata6 among many early ICM genes,as well as key trophectoderm regulatory genes including Tead4 and Gata3 at the 8C stage.By contrast,master pluripotency TFs NANOG,SOX2,and OCT4 targeted both early and late ICM genes in mouse ESCs.Taken together,these data identify NR5A2 as a key regulator in totipotent embryos that bridges ZGA to the first lineage segregation during mouse early development. | Fangnong Lai Lijia Li Xiaoyu Hu Bofeng Liu Ziqi Zhu Ling Liu Qiang Fan Huabin Tian Kai Xu Xukun Lu Qing Li Kong Feng Lijuan Wang Zili Lin Hongyu Deng Jinsong Li Wei Xie | 2023 | Cell Research2023,33,12: | 0 |

In this paper, we propose a Perceptive Variable Bit-Rate Control(PVBRC) framework for the state-of-the-art video coding standard High-Efficiency Video Coding(HEVC)/H.265. PVBRC allocates a bit-rate to a picture while taking a Comprehensive Picture Quality Assessment(CPQA) model and perceptive target bit-rate allocation into consideration. The CPQA model calculates the objective and perceptive quality of both source and reconstructed pictures by referring to the human vision system. The coding bit-rate is then dynamically allocated by the result of the CPQA model according to differences in picture content. In PVBRC, the quantization parameter for current picture encoding is updated by an effective fuzzy logical controller to satisfy the transmission requirements of the Internet of Things. | 16 | The Effect of Xu’s Influenza Decoction Combined with Oseltamivir on Influenza A:A Propensity Score Matching Study显示文摘Background:The resurgence of seasonal influenza virus circulation has been seen in 2021-2022 after the tempo-rary suppression in 2020-2021.Neuraminidase inhibitors(NAIs)are widely applied in the clinical treatment of influenza A despite several limitations.Objective:To access the efficacy of Xu’s influenza decoction(XID)in combination therapy with oseltamivir for the treatment of influenza A.Methods:In this retrospective cohort study,the eligible participants were diagnosed with influenza A between June 1,2018,and May 30,2022,in the First Affiliated Hospital of Zhejiang Chinese Medical University.According to whether Xu’s influenza decoction was applied,patients were divided into two groups:treated with or without XID.Propensity score matching(PSM)was used to further adjust the covariates between groups.The primary outcome was to compare time to defervescence via K-M curves,Breslow tests,and Cox regression analysis.In Cox proportional hazards model,a univariate analysis was performed to obtain preliminary results,while a further multivariate analysis was conducted to study the independent factors that influence defervescence.Subgroup analysis was conducted according to body temperature and time from onset to admission.The secondary outcome consisted of routine blood and C-reactive protein(CRP),length of stay,and medical costs.Results:A total of 336 patients with influenza A were enrolled in this study(i.e.,163 patients in the XID+oseltamivir group;173 patients in the oseltamivir group).After 1:1 matching via PSM,230 patients meeting the criteria were included in the analysis,with 115 in each arm.The XID+oseltamivir group had shorter time to defervescence(36 h vs 44 h,P=0.011),shorter length of stay(3 days vs 4 days,P=0.018),and higher defervescence possibility(HR=1.384,95%CI:1.054-1.818).Subgroup analysis indicated that for patients during non-window period(≥48 h)with medium-grade fever(38.1℃-39℃),the XID+oseltamivir combination therapy reduced time to defervescence(P=0.04995/0.004)with a higher defervescence possibility(HR=1.524/1.683).Meanwhile,there’s no statistical significance but observable trends of the XID+oseltamivir group in the lower medical costs(3068.07 yuan vs 3120.68 yuan),the lower neutrophils%(48.53%vs 51.00%)and the higher lymphocyte%(39.83%vs 37.72%).Conclusion:The combination of XID and oseltamivir can shorten the time to defervescence and length of stay in influenza A.Its antipyretic effect is mainly reflected in the medium-grade and non-window periods. | Tianxi Chen Shuyan Fu Fengyuan Tian Qiushuang Li Hongyu Ling Yijie Lou Jun Tang Hong Zheng | 2024 | Clinical Complementary Medicine and Pharmacology2024,4,1: | 0 |

Additionally, the consistency of the Miller-Payne and residual cancer burden(RCB)systems in evaluating the efficacy of neoadjuvant therapy for early human epidermal growth factor receptor-2(HER2)+ breast cancer was analyzed.Methods: The clinicopathological data of female patients with early-stage HER2+ breast cancer who received dual-target neoadjuvant therapy at 26 hospitals of the Chinese Society of Breast Surgery(CSBrS) from March 2019 to December 2021 were collected. Patients were allocated to four groups: the HER2 immunohistochemistry(IHC)3+/hormone receptor(HR)-, IHC3+/HR+, IHC2+ in situ hybridization(ISH)+/HR-and IHC2+ ISH+/HR+groups. The overall pCR rate for patients, the pCR rate in each group and the factors affecting the pCR rate were analyzed. The consistency between the Miller-Payne and RCB systems in assessing the efficacy of neoadjuvant therapy was analyzed.Results: From March 1, 2019, to December 31, 2021, 77,376 female patients with early-stage breast cancer were treated at 26 hospitals;18,853(24.4%) of these patients were HER2+. After exclusion of unqualified patients, 2,395 patients who received neoadjuvant dual-target(H+P) therapy combined with chemotherapy were included in this study. The overall pCR rate was 53.0%, and the patients' HR statuses and different HER2+ statuses were significantly correlated with the pCR rate(P<0.05). The consistency of the pathological efficacy assessed by the Miller-Payne and RCB systems was 88.0%(κ=0.717, P<0.001).Conclusions: Different HER2 expression statuses and HR expression statuses are correlated with the pCR rate after dual-target neoadjuvant therapy in HER2+ breast cancer patients. | 19 | Neoadjuvant therapy for early human epidermal growth factor receptor 2 positive breast cancer in China: A multicenter real-world study (CSBrS-015)显示文摘Background:Pertuzumab has been approved for application in China by the National Medical Products Administration,and both national and international guidelines make recommendations for the use of neoadjuvant treatment with trastuzumab or trastuzumab+pertuzumab plus chemotherapy regimens for patients with indications.The goal of this study was to investigate the short-term clinical efficacy of the neoadjuvant therapies trastuzumab and trastuzumab+pertuzumab for patients with early human epidermal growth factor receptor 2(HER2)-positive breast cancer in China.Methods:A real-world study was conducted using the clinicopathological data of patients with early HER2-positive breast cancer who were admitted to the member hospitals of the Chinese Society of Breast Surgery,Chinese Surgical Society of Chinese Medical Association between March 2019 and December 2020.This study analyzed the efficacy and tolerance of trastuzumab+chemotherapy and trastuzumab+pertuzumab+chemotherapy in patients with early HER2-positive breast cancer.The Response Evaluation Criteria in Solid Tumors 1.1 was adopted to evaluate clinical efficacy.The pathological efficacy was evaluated using the MillerPayne grade.The Common Terminology Criteria for Adverse Events(version 5.0)was adopted to evaluate adverse events(AEs).The propensity scores were subjected to propensity score matching using the R language(1:1 matching with a maximum allowable difference of 0.05 between the two groups).Efficacy was compared using the chi-square test,and correlation analysis was performed using linear regression.Results:A total of 1032 patients with early HER2-positive breast cancer met the enrollment criteria and were included in this study.Among these patients,472 received neoadjuvant trastuzumab+chemotherapy(the trastuzumab group),and 560 received neoadjuvant trastuzumab+pertuzumab+chemotherapy(the trastuzumab+pertuzumab group).The overall pathologic complete response(pCR)rate was 47.2%(487/1032),while the pCR rates of the trastuzumab and trastuzumab+pertuzumab groups were 34.5%(163/472)and 57.9%(324/560),respectively,and the difference was significant(P<0.001).The incidence of grade 4 AEs was 24/321(7.5%)in the trastuzumab+pertuzumab group,and there were no cases in which the left ventricular ejection fraction decreased by more than 10%.Conclusions:Patients in the trastuzumab+pertuzumab group had a higher pCR rate than those in the trastuzumab group,and the toxic side effects were tolerable. | Yuanjia Cheng Hongyu Xiang Ling Xin Xuening Duan Yinhua Liu Chinese Society of Breast Surgery(CSBrS) Chinese Society of Surgery of Chinese Medical Association | 2022 | Chinese Medical Journal2022,135,19: | 0 |
