1Programmed death ligand-1 expression and its prognostic role in esophageal squamous cell carcinoma显示文摘AIM To investigate the expression and prognostic role of programmed death ligand-1(PD-L1) in locally advanced esophageal squamous cell carcinoma(ESCC).METHODS A total of 200 patients with ESCC who underwent radical esophagectomy with standard lymphadenectomy as the initial definitive treatment in Seoul National University Hospital from December 2000 to April 2013 were eligible for this analysis. Tissue microarrays were constructed by collecting tissue cores from surgical specimens, and immunostained with antibodies directed against PD-L1, p16, and c-Met. Medical records were reviewed retrospectively to assess clinical outcomes. Patients were divided into two groups by PD-L1 status, and significant differences in clinicopathologic characteristics between the two groups were assessed. RESULTS Tumor tissues from 67 ESCC patients(33.5%) were PDL1-positive. Positive p16 expression was observed in 21 specimens(10.5%). The H-score for c-Met expression was ≥ 50 in 42 specimens(21.0%). Although PDL1-positivity was not significantly correlated with any clinical characteristics including age, sex, smoking/alcoholic history, stage, or differentiation, H-scores for c-Met expression were significantly associated with PDL1-positivity(OR = 2.34, 95%CI: 1.16-4.72, P = 0.017). PD-L1 expression was not significantly associated with a change in overall survival(P = 0.656). In contrast, the locoregional relapse rate tended to increase(P = 0.134), and the distant metastasis rate was significantly increased(HR = 1.72, 95%CI: 1.01-2.79, P = 0.028) in patients with PD-L1-positive ESCC compared to those with PD-L1-negative ESCC.CONCLUSION PD-L1 expression is positively correlated with c-Met expression in ESCC. 2Risk prediction platform for pancreatic fistula after pancreatoduodenectomy using artificial intelligence显示文摘BACKGROUND Despite advancements in operative technique and improvements in postoperative managements,postoperative pancreatic fistula(POPF)is a life-threatening complication following pancreatoduodenectomy(PD).There are some reports to predict POPF preoperatively or intraoperatively,but the accuracy of those is questionable.Artificial intelligence(AI)technology is being actively used in the medical field,but few studies have reported applying it to outcomes after PD.AIM To develop a risk prediction platform for POPF using an AI model.METHODS Medical records were reviewed from 1769 patients at Samsung Medical Center who underwent PD from 2007 to 2016.A total of 38 variables were inserted into AI-driven algorithms.The algorithms tested to make the risk prediction platform were random forest(RF)and a neural network(NN)with or without recursive feature elimination(RFE).The median imputation method was used for missing values.The area under the curve(AUC)was calculated to examine the discriminative power of algorithm for POPF prediction.RESULTS The number of POPFs was 221(12.5%)according to the International Study Group of Pancreatic Fistula definition 2016.After median imputation,AUCs using 38 variables were 0.68±0.02 with RF and 0.71±0.02 with NN.The maximal AUC using NN with RFE was 0.74.Sixteen risk factors for POPF were identified by AI algorithm:Pancreatic duct diameter,body mass index,preoperative serum albumin,lipase level,amount of intraoperative fluid infusion,age,platelet count,extrapancreatic location of tumor,combined venous resection,co-existing pancreatitis,neoadjuvant radiotherapy,American Society of Anesthesiologists’score,sex,soft texture of the pancreas,underlying heart disease,and preoperative endoscopic biliary decompression.We developed a web-based POPF prediction platform,and this application is freely available at http://gffzz52cab37b8d4e4d2chp9u5n0cukxkw6cu9.ffgz.tsg.suse.edu.cn This study is the first to predict POPF with multiple risk factors using AI.This platform is reliable(AUC 0.74),so it could be used to select patients who need especially intense therapy and to preoperatively establish an effective treatment strategy.In Woong Han Kyeongwon Cho Youngju Ryu Sang Hyun Shin Jin Seok Heo Dong Wook Choi Myung Jin Chung Oh Chul Kwon Baek Hwan Cho 2020World Journal of Gastroenterology2020,26,30:12
3Analysis of Herbal Medicine Prescriptions for Patients in An Academic Korean Medical Hospital:A Cross Sectional Study of Electronic Medical Records(2010–2013)显示文摘Objective: To obtain fundamental information for the standardization of herbal medicine in Korea. Methods: We analyzed the herbal medicine prescription data of patients at the Pusan National University Korean Medicine Hospital from March 2010 to February 2013. We used the Dongui-Bogam(Dong Yi Bao Jian) to classify prescribed herbal medicines. Results: The study revealed that the most frequently prescribed herbal medicine was 'Liuwei Dihuang Pill(LWDHP, 六味地黄丸)' which was used for invigorating 'Shen(Kidndy)-yin'. 'LWDHP' was most frequently prescribed to male patients aged 50–59, 60–69, 70–79 and 80–89 years, and 'Xionggui Tiaoxue Decoction(XGTXD, 芎归调血饮)' was most frequently prescribed to female patients aged 30–39 and 40–49 years. According to the International Classification of Diseases(ICD) codes, 'Diseases of the musculoskeletal system and connective tissue' showed the highest prevalence. 'LWDHP' and 'XGTXD' was the most frequently prescribed in categories 5 and 3, respectively. Based on the percentage of prescriptions for each sex, 'Ziyin Jianghuo Decoction(滋阴降火汤)' was prescribed to mainly male patients, and 'XGTXD' with 'Guima Geban Decoction(桂麻各半汤)' were prescribed to mainly female patients. 8Quantum secure direct communication network with hyperentanglement显示文摘We propose a quantum secure direct communication protocol with entanglement swapping and hyperentanglement.Any two users, Alice and Bob, can communicate with each other in a quantum network, even though there is no direct quantum channel between them. The trust center, Trent, who provides a quantum channel to link them by performing entanglement swapping, cannot eavesdrop on their communication. 14A Quantum Network System of QSS-QDC Using χ-Type Entangled States显示文摘A multiuser quantum direct communication network system for N users utilizing χ-type entangled states is prupused. The network system is composed of a communication center,N users,and N quantum lines linking the center and the N users.There is no quantum line among users,and therefore only N quantum lines are necessary for communication between users.Using one χ-type entangled state, in this protocol we are able to send two bits of information through direct communication and,at the same time,share two bits of quantum keys.
