1Exenatide improves hepatic steatosis by enhancing lipid use in adipose tissue in nondiabetic rats显示文摘AIM:To investigate the metabolic changes in skeletal muscle and/or adipose tissue in glucagon-like peptide-1-induced improvement of nonalcoholic fatty liver disease(NAFLD).METHODS:Male Wistar rats were fed either a control diet(control group)or a high-fat diet(HFD).After 4wk,the HFD-fed rats were subdivided into two groups;one group was injected with exenatide[HFD-Ex(+)group]and the other with saline[HFD-Ex(-)group]every day for 12 wk.The control group received saline and were fed a control diet.Changes in weight gain,energy intake,and oxygen consumption were analyzed.Glucose tolerance tests were performed after 8 wk of treatment.Histological assessments were performed in liver and adipose tissue.RNA expression levels of lipid metabolism related genes were evaluated in liver,skeletal muscle,and adipose tissue.RESULTS:Exenatide attenuated weight gain[HFDEx(-)vs HFD-Ex(+)]and reduced energy intake,which was accompanied by an increase in oxygen consumption and a decrease in the respiratory exchange ratio[HFD-Ex(-)vs HFD-Ex(+)].However,exenatide did not affect glucose tolerance.Exenatide reduced lipid content in the liver and adipose tissue.Exenatide did not affect the expression of lipid metabolism-related genes in the liver or skeletal muscle.In adipose tissue,exenatide significantly upregulated lipolytic genes,including hormone-sensitive lipase,carnitine palmitoyltransferase-1,long-chain acyl-CoA dehydrogenase,and acyl-CoA oxidase 1[HFD-Ex(-)vs HFD-Ex(+)].Exenatide also upregulated catalase and superoxide dismutase 2[HFD-Ex(-)vs HFD-Ex(+)].CONCLUSION:In addition to reducing appetite,enhanced lipid use by exenatide in adipose tissue may reduce hepatic lipid content in NAFLD,most likely by decreasing lipid influx into the liver.Kosuke Tanaka Yuko Masaki Masatake Tanaka Masayuki Miyazaki Munechika Enjoji Makoto Nakamuta Masaki Kato Masatoshi Nomura Toyoshi Inoguchi Kazuhiro Kotoh Ryoichi Takayanagi 2014World Journal of Gastroenterology2014,20,10:6
2Antithrombin Ⅲ injection via the portal vein suppresses liver damage显示文摘AIM:To investigate the effects of antithrombin Ⅲ(AT Ⅲ) injection via the portal vein in acute liver failure.METHODS:Thirty rats were intraperitoneally challenged with lipopolysaccharide(LPS) and D-galactosamine(GalN) and divided into three groups:a control group;a group injected with AT Ⅲ via the tail vein;and a group injected with AT Ⅲ via the portal vein.AT Ⅲ(50 U/kg body weight) was administrated 1 h after challenge with LPS and GalN.Serum levels of inflammatory cytokines and fibrin degradation products,hepatic fibrin deposition,and hepatic mRNA expression of hypoxiarelated genes were analyzed.RESULTS:Serum levels of alanine aminotransferase,tumor necrosis factor-α and interleukin-6 decreased significantly following portal vein AT Ⅲ injection compared with tail vein injection,and control rats.Portal vein AT Ⅲ injection reduced liver cell destruction and decreased hepatic fibrin deposition.This treatment also significantly reduced hepatic mRNA expression of lactate dehydrogenase and heme oxygenase-1.CONCLUSION:A clinically acceptable dose of AT Ⅲ injection into the portal vein suppressed liver damage,probably through its enhanced anticoagulant and antiinflammatory activities.Masayuki Miyazaki Masaki Kato Masatake Tanaka Kosuke Tanaka Shinichiro Takao Motoyuki Kohjima Tetsuhide Ito Munechika Enjoji Makoto Nakamuta Kazuhiro Kotoh Ryoichi Takayanagi 2012World Journal of Gastroenterology2012,18,16:2

8Accuracy of shoulder joint injections with ultrasound guidance:Confirmed by magnetic resonance arthrography显示文摘BACKGROUND Intra-articular glenohumeral joint injections are essential procedures for treating various shoulder disorders. Fluoroscopy-guided injections have been extensively used;however, they pose a risk of radiation exposure and are expensive and timeconsuming. Recently, it has been suggested that ultrasound-guided injections are accurate and cost effective procedures.AIM To evaluate the accuracy of ultrasound-guided glenohumeral injections using a posterior approach that is confirmed using magnetic resonance arthrography(MRA).METHODS The study included 179 shoulders of patients with recurrent anterior instability(150 patients;103 and 76 right and left shoulders, respectively;160 males and 19 females;average age = 20.5 years;age range: 14-63 years) who underwent MRA for preoperative diagnosis. They were injected with 12 m L lidocaine(1%) using the ultrasound-guided posterior approach and then underwent magnetic resonance imaging. Two shoulder surgeons, except for the injector, evaluated the transverse relaxation(T2)-weighted images of axial planes and classified the intraarticular condition of injected contrast into three groups based on one of the three following scenarios: no leakage, injection into the glenohumeral joint without leakage;minor leakage, practical intra-articular injection with some leakage outside the posterior rotator cuffs;and major leakage, inaccurate injection with mass leakage without any contrast into the joint. The inter-rater reliability between two assessors was also evaluated by calculating Cohen’s kappa coefficient. The learning curve was assessed regarding the inaccurate injection rate by analyzing Spearman’s rank correlation coefficient.RESULTS Of the 179 injections, 163 shoulders(91.0%) had no leakage, 10 shoulders(5.6%) had minor leakage, and six shoulders(3.4%) had major leakage. In total, 173 shoulders(96.6%) were intraarticularly injected;thus, we could detect anterior labrum and capsular pathologies. Regarding the inter-rater reliability, the kappa coefficient was 0.925, indicating consistency in the evaluations by both examiners. Regression analysis of the inaccurate injection rate for assessingtechnical learning showed a logarithmic curve with a downward trend(R;= 0.887, P < 0.001). Three(50%) of the six inaccurate injections classified into “major leakage” were observed in the first 30 injections, indicating that the accurate injection showed a leaning effect.CONCLUSION Ultrasound-guided intra-articular glenohumeral injections using a posterior approach had high accuracy;however, injection accuracy depends on clinical experience.Kosuke Kuratani Makoto Tanaka Hiroto Hanai Kenji Hayashida 2022World Journal of Orthopedics2022,13,3:1

11Hepatocellular carcinoma or interferon-based therapy history attenuates sofosbuvir/ribavirin for Japanese genotype 2 hepatitis C virus显示文摘AIM To investigate the real-world efficacy and safety of sofosbuvir/ribavirin(SOF/RBV) therapy for Japanese patients with genotype 2 hepatitis C virus(GT2-HCV).METHODS A total of 182 patients with GT2-HCV infection who received SOF/RBV therapy for 12 wk at our hospital were enrolled. The patients comprised 122 men and 60 women(age range: 17-84 years; mean age ± SD: 60.1 ± 12.1 years). Relationships between virological response and clinical data were examined by logistic regression analyses. RESULTS The proportions of patients with liver cirrhosis and history of hepatocellular carcinoma(HCC) were 29.0% and 17.3%, respectively. The proportion of patients with prior interferon(IFN)-based therapy was 25.6%. SOF/RBV therapy rapidly decreased HCV RNA levels. Several patients required RBV dose reduction because of anemia or fatigue. Four patients discontinued the therapy. The rates of sustained virological response at 12 wk after the end of treatment were 87.9%(intention to treat: 160/182) and 94.1%(per protocol: 159/169). Multivariate analyses showed that history of HCC or IFN-based therapy independently reduced the efficacy of SOF/RBV therapy. CONCLUSION SOF/RBV therapy for GT2-HCV is safe, highly tolerated, and effective. 12Oral tolerance is inducible during active dextran sulfate sodium-induced colitis显示文摘AIM:To investigate whether oral tolerance is inducible during the active phase of dextran sulfate sodium(DSS)-induced colitis.METHODS:Colitis was induced in 6-to 8-wk-old female BALB/c mice by the administration of 2%DSS.To induce oral tolerance,mice that received water with DSS[DSS(+)]and mice that received autoclaved water[DSS(-)]were intragastrically(i.g.)administered ovalbumin(OVA)as a tolerogen before systemic challenge with OVA.Following this,serum levels of OVA-specific Ig E antibodies were measured.In mice with activecolitis,CD4+CD25+Foxp3+cell and B10 cell frequencies were evaluated using flow cytometry.Cytokine m RNA expression profiles were evaluated by reverse transcription real-time polymerase chain reaction.RESULTS:Regardless of the presence of DSS colitis,OVA-specific immunoglobulin E concentrations were significantly reduced in mice that were i.g.administered OVA compared to mice that were i.g.administered PBS[DSS(+):4.4(4.2-9.5)ng/m L vs 83.9(66.1-123.2)ng/m L,P<0.01;DSS(-):27.7(0.1-54.5)ng/m L vs116.5(80.6-213.6)ng/m L,P<0.01].These results demonstrated that oral tolerance was induced in both the presence and absence of colitis.In the spleen and mesenteric lymph nodes(MLN),the frequencies of CD4+CD25+Foxp3+cells and B10 cells,both of which are associated with oral tolerance,did not significantly change.In the spleen,interferon-γm RNA expression significantly decreased in mice with colitis[DSS(+):0.42(0.31-0.53)vs DSS(-):1.00(0.84-1.39),P<0.01].The expression levels of other cytokines did not significantly change.CONCLUSION:Oral tolerance is inducible during active DSS colitis.The stability of regulatory cell populations in the spleen and MLN in colitis might correlate with these results.Satoshi Ino Chikara Kohda Kosuke Takeshima Hiroki Ishikawa Tomoko Norose Toshiko Yamochi Masafumi Takimoto Hiroshi Takahashi Kazuo Tanaka 2016World Journal of Gastrointestinal Pharmacology and Therapeutics2016,7,2:1

20Multicentric recurrence of intraductal papillary neoplasm of bile duct after spontaneous detachment of primary tumor:A case report显示文摘BACKGROUND Intraductal papillary neoplasm of the bile duct(IPNB)rarely recurs in a multicentric manner.We encountered a patient with multiple recurrences of the gastric subtype of IPNB one year after spontaneous detachment of the primary tumor during peroral cholangioscopy(POCS).CASE SUMMARY A 68-year-old woman on maintenance hemodialysis because of lupus nephritis had several cardiovascular diseases and a pancreatic intraductal papillary mucinous neoplasm(IPMN).She was referred to our department for dilation of the common bile duct(CBD)and a tumor in the lumen,detected using ultrasonography.She had no complaints,and blood tests of hepatobiliary enzymes were normal.Magnetic resonance cholangiopancreatography(MRCP)showed a papillary tumor in the CBD with a filling defect detected using endoscopic retrograde cholangiography(ERC).Intraductal ultrasonography revealed a papillary tumor and stalk at the CBD.During POCS,the tumor spontaneously detached with its stalk into the CBD.Pathology showed low-intermediate nuclear atypia of the gastric subtype of IPNB.After 1 year,follow-up MRCP showed multiple tumors distributed from the left hepatic duct to the CBD.ERC and POCS showed multicentric tumors.She was alive without hepatobiliary symptoms at least two years after initial diagnosis of IPNB.CONCLUSION The patient experienced gastric subtype of IPNB without curative resection.Observation may be reasonable for patients with this subtype.Hiroki Fukuya Akifumi Kuwano Shigehiro Nagasawa Yusuke Morita Kosuke Tanaka Masayoshi Yada Akihide Masumoto Kenta Motomura 2022World Journal of Clinical Cases2022,10,3:0
