| 1 | Entecavir add-on Peg-interferon therapy plays a positive role in reversing hepatic fibrosis in treatment-naive chronic hepatitis B patients: a prospective and randomized controlled trial显示文摘Background:The efficacy of entecavir(ETV)add-on peg-interferon therapy compared with ETV monotherapy in treatment-naive hepatitis B virus(HBV)patients remains controversial.We investigated whether adding peg-interferon to ongoing ETV treatment leads to a better curative effect or not.Methods:All patients have been recruited between August 2013 and January 2015 from the Shanghai Public Health Clinical Center and Zhongshan Hospital(China).Eligible HBV patients(n=144)were randomly divided(1:1)to receive either ETV monotherapy(n=70)or peg-interferon add-on therapy from week 26 to 52(«=74).Patients were followed-up for at least 2 years.Indexes including hepatitis B surface antigen(HBsAg)and hepatitis B e antigen(HBeAg)seroconversion rate,sustained virologic response,transient elastography value,and histological scores were evaluated every 3 months until the end of the study.The rate of patients with HBsAg loss was defined as the primary endpoint criteria.Results:At week 26,no patient achieved HBsAg seroconversion in either group.At week 52,one patient in the monotherapy group was HBsAg-negative but there was none in the combination therapy group.The monotherapy group showed significantly better liver function recovery results than the combination therapy group.At week 78,one patient in the combination group had HBsAg seroconverted.At week 104,only three patients in the combination therapy group were HBsAg-negative compared with one patient in monotherapy.The mean alanine aminotransferase and aspartate aminotransferase levels and transient elastography values decreased significantly compared with baseline.Both groups showed a favorable decrease in alpha-fetoprotein(monotherapy:4.5[2.8,7.1]vs.2.2[1.8,3.1]ng/mL,P<0.001;combination therapy:5.7[3.0,18.8]vs.3.2[2.0,4.3]ng/mL,P<0.001)and an improved result of liver biopsy examination scores.The combination group showed a better improvement in histology compared with the monotherapy group(mean transient elastography value 6.6[4.9,9.8]vs.7.8[5.4,11.1]kPa,P=0.028).But there was no significant difference in HBsAg conversion rate(1.8%[1/56]i^s.4.1%[3/73],P=0.809)and HBeAg conversion rate(12.5%[7/56]i/s.11.0%[8/73],P=0.787),as well as HBV-DNA,sustained virologic response(93.2%vs.98.5%,P=0.150)between the two groups.Conclusions:Both therapies supported liver function recovery and histology improvement.Combination therapy did not show better anti-viral efficacy in HBsAg or HBeAg seroconversion compared with monotherapy.However,combination therapy played a more positive role in reversing hepatic fibrosis compared with monotherapy.Trial registration:ClinicalTrials.gov:NCT02849132;http://gffzzd3f38a9638994581skk0f9nxf9cbq6ofc.ffgz.tsg.suse.edu.cn/ct2/show/NCT02849132. | Jing-Mao Yang Li-Ping Chen Ya-Jie Wang Bei Lyu Hong Zhao Zhi-Yin Shang Jun Li Zhen-Yu Fan Sheng-Di Wu Xiao Ming Xian Li Shao-Ping Huang Ji-Lin Cheng | 2020 | Chinese Medical Journal2020,,14: | 11 |
| 2 | Standard Protocol Items for Clinical Trials with Traditional Chinese Medicine 2018: Recommendations, Explanation and Elaboration(SPIRIT-TCM Extension 2018)显示文摘Traditional Chinese Medicine(TCM) is one of the oldest systems of medicine. More and more attention has been paid to TCM application, but the variable quality of clinical trials with TCM impedes its widespread acceptance. The Standard Protocol Items: Recommendations for Interventional Trials(SPIRIT) 2013 Statement has established guidelines for designing clinical trials to ensure that the trial results are accurate and reliable. However, there are difficulties when applying SPIRIT 2013 Statement to trials with TCM, due to the unique theory and the characteristic of TCM intervention. An Extension to the original SPIRIT was developed to ensure the quality of trial design with TCM. As Chinese herbal formulae, acupuncture and moxibustion are common and representative interventions in TCM practice, the executive working group determined that the SPIRIT-TCM Extension focus on these three interventions. Extension was developed through initiation, 3 rounds of Delphi consensus survey, and finalizing expert meeting. Seven items from the SPIRIT 2013 Statement were modified, namely, 'title', 'background and rationale', 'objectives', 'eligibility criteria', 'interventions', 'outcomes', and 'data collection methods'. The Extension includes the introduction of the concept of TCM pattern and 3 major TCM interventions, with examples and explanations. The SPIRIT-TCM Extension 2018 provides suggestion for investigators in designing high quality TCM clinical trials. | 3 | Radiation balance and the response of albedo to environmental factors above two alpine ecosystems in the eastern Tibetan Plateau显示文摘Understanding the energy balance on the Tibetan Plateau is important for better prediction of global climate change. To characterize the energy balance on the Plateau, we examined the radiation balance and the response of albedo to environmental factors above an alpine meadow and an alpine wetland surfaces in the eastern Tibetan Plateau, using 2014 data. Although our two sites belong to the same climatic background, and are close geographically, the annual incident solar radiation at the alpine meadow site(6,447 MJ/(m2·a)) was about 1.1 times that at the alpine wetland site(6,012 MJ/(m2·a)),due to differences in the cloudiness between our two sites. The alpine meadow and the alpine wetland emitted about 38%and 42%, respectively, of annual incident solar radiation back into atmosphere in the form of net longwave radiation; and they reflected about 22% and 18%, respectively, of the annual incident solar radiation back into atmosphere in the form of shortwave radiation. The annual net radiation was 2,648 and 2,544 MJ/(m2·a) for the alpine meadow site and the alpine wetland site, respectively, accounting for only about 40% of the annual incident solar radiation, significantly lower than the global mean. At 30-min scales, surface albedo exponentially decreases with the increase of the solar elevation angle; and it linearly decreases with the increase of soil-water content for our two sites. | 5 | Establishment of a predictive model for inpatient sudden cardiac death in a Chinese cardiac department population:a retrospective study显示文摘Background:Little is known about the risk factors for sudden cardiac death (SCD) in the overall hospitalized cardiac department population.This study was conducted to investigate the risk factors and develop a predictive model for SCD in a hospitalized cardiac department population.Methods:We conducted a retrospective study of patients admitted to the cardiac department of the First Affiliated Hospital of Xinjiang Medical University from June 2015 to February 2017.We collected the clinical data from medical records.Multiple stepwise logistic regression analysis was carried out to confirm the risk factors for SCD and develop a predictive risk model.The risk score was assessed by the area under receiver operating characteristic (AUROC) curve and the Hosmer-Lemeshow goodness-of-fit test.Results:A total of 262 patients with SCD and 4485 controls were enrolled in our study.Logistic regression modeling identified eight significant risk factors for in-hospital SCD:age,main admitting diagnosis,diabetes,corrected QT interval,QRS duration,ventricular premature beat burden,left ventricular ejection fraction,and estimated glomerular filtration rate.A predictive risk score including these variables showed an AUROC curve of 0.774 (95% confidence interval:0.744–0.805).The Hosmer-Lemeshow goodness-of-fit test showed the chi-square value was 2.527 (P= 0.640).The incidence of in-hospital SCD was 1.3%,4.1%,and 18.6% for scores of 0 to 2,3 to 5 and ≥6,respectively (P<0.001).Conclusions:Age,main admitting diagnosis,diabetes,QTc interval,QRS duration,ventricular premature beat burden,left ventricular ejection fraction,and estimated glomerular filtration rate are factors related to in-hospital SCD in a hospitalized cardiac department population.We developed a predictive risk score including these factors that could identify patients who are predisposed to in-hospital SCD. | Lu-Xiang Shang Xian-Hui Zhou Jiang-Hua Zhang Wen-Hui Zhang ZuKe-La TuEr-Hong Yang Zhao Wen-Kui Lyu Yao-Dong Li Bao-Peng Tang | 2019 | Chinese Medical Journal2019,,1: | 2 |
| 6 | An improvement of soil temperature simulations on the Tibetan Plateau显示文摘The simulation of soil temperature on the Tibetan Plateau(TP) plays a dominant role in the performance of both global climate and numerical weather forecast models. To improve the simulation of soil temperature on the TP, the Johansen soil thermal conductivity parameterization scheme was introduced into Community Land Model 3.5(CLM3.5) and Regional Climatic Model 4(Reg CM4). The improved CLM3.5 and Reg CM4-CLM were utilized to conduct offline and regional simulation experiments on the TP. Comparison of the new and old schemes revealed that CLM3.5 provides high thermal conductivity parameters of mineral soil solid on the TP. The Johansen scheme is more practical for the TP than the soil thermal conductivity parameterization in CLM3.5. The simulation of soil temperature and liquid water content was improved in offline experiment. | 7 | Going below and beyond the surface: Microneedle structure, materials, drugs, fabrication, and applications for wound healing and tissue regeneration显示文摘Microneedle, as a novel drug delivery system, has attracted widespread attention due to its non-invasiveness, painless and simple administration, controllable drug delivery, and diverse cargo loading capacity. Although microneedles are initially designed to penetrate stratum corneum of skin for transdermal drug delivery, they, recently, have been used to promote wound healing and regeneration of diverse tissues and organs and the results are promising. Despite there are reviews about microneedles, few of them focus on wound healing and tissue regeneration. Here, we review the recent advances of microneedles in this field. We first give an overview of microneedle system in terms of its potential cargos (e.g., small molecules, macromolecules, nucleic acids, nanoparticles, extracellular vesicle, cells), structural designs (e.g., multidrug structures, adhesive structures), material selection, and drug release mechanisms. Then we briefly summarize different microneedle fabrication methods, including their advantages and limitations. We finally summarize the recent progress of microneedle-assisted wound healing and tissue regeneration (e.g., skin, cardiac, bone, tendon, ocular, vascular, oral, hair, spinal cord, and uterine tissues). | 12 | IgG4 and IgE co-positive group found in idiopathic orbital inflammatory disease显示文摘AIM: To reveal the cytokines involved in idiopathic orbital inflammatory disease(IOID) and the relationship between Th17 cells, IgE and IOID pathogenesis.METHODS: Whole blood samples were processed immediately after collection and serological IgG4, IgG, and IgE antibodies were tested using ELISA. IOID and orbital cavernous hemangioma(CH) tissue samples underwent Bio-Plex multiplex cytokine detection. Hematoxylin-Eosin(HE) staining of all paraffin samples suggested the histological features of IOIDs, and expressions of IgG4 and IL-17 A in affected tissues were detected by immunohistochemistry. RESULTS: Among 40 IOID plasma samples, 52.5%(21/40) were positive for IgG4 and 25%(10/40) were positive for IgE. Overlapped IgG4 or IgE positive samples accounted for 22.5%(9/40). Therefore, IOID samples were separated into three groups. The IgE+/IgG4+ group had a relevantly lower level of pro-inflammatory cytokine expression. IL-4(Th2 cell related), IL-10 and TGF-β1(Treg cell immunity related) were elevated in all three groups. Some of the Th17 cell related cytokines(i.e. IL-17 A/F, IL-25, IL-23, and IL-33) displayed higher expression levels in the IgE-/IgG4-group compared to the other two groups.CONCLUSION: We discovered an IgG4-IgE co-positive group as well as Th17 cell immune involvement in IgG4-IgE co-negative subgtroup in IOID for the first time. The pathogenesis of IOID could differ from different subgroups according to the IgG4 and IgE detection. In recent years, the areas of TP lakes underwent drastic changes and have become a research hotspot. However, the characteristics of the lake-atmosphere interaction over the high-altitude lakes are still unclear, which inhibits model development and the accurate simulation of lake climate effects. The source region of the Yellow River(SRYR) has the largest outflow lake and freshwater lake on the TP and is one of the most densely distributed lakes on the TP. Since 2011,three observation sites have been set up in the Ngoring Lake basin in the SRYR to monitor the lake-atmosphere interaction and the differences among water-heat exchanges over the land and lake surfaces. This study presents an eight-year(2012–19), half-hourly, observation-based dataset related to lake–atmosphere interactions composed of three sites. The three sites represent the lake surface, the lakeside, and the land. The observations contain the basic meteorological elements,surface radiation, eddy covariance system, soil temperature, and moisture(for land). Information related to the sites and instruments, the continuity and completeness of data, and the differences among the observational results at different sites are described in this study. These data have been used in the previous study to reveal a few energy and water exchange characteristics of TP lakes and to validate and improve the lake and land surface model. | 20 | Effects of the inlet particle spatial distribution on the performance of a gas-solid cyclone separator显示文摘The present study was aimed at the inlet particle spatial distribution of a gas-solid separator with high solid loading and investigated its effects on cyclone performance and the inner flow pattern.The flow fields of a cyclone with different rectangular particle flow areas on the inlet surface were numerically simulated using a four-way coupling method.The simulated results indicate that reducing the inlet particle flow area and lowing the inlet particle position can effectively reduce the scattered particles inside the cyclone separator and enhance the separation performance.Vertically gathering the particles to the centerline can also weaken the particle back-mixing.The particles near the roof account for the swirling particle ceiling phenomenon.The inlet particle spatial distribution affects the pressure drop mainly by affecting the gas tangential velocity in the cylinder body.Moreover,compared to the hori-zontal particle distribution on the inlet surface,the vertical particle distribution has greater effects on cyclone performance. | Yuge Yao Manxia Shang Xiwei Ke Zhong Huang Tuo Zhou Junfu Lyu | 2024 | Particuology2024,85,2: | 0 |
