2Gastric cancer: Epidemiology, risk factors and prevention strategies显示文摘Gastric cancer(GC) is a global health problem, with more than 1 million people newly diagnosed with GC worldwide each year. GC is more prevalent in less developed countries than in more developed countries. About half of all GC cases worldwide occur in East Asia, notably China. Globally, overall incidence rates of GC are declining, which is potentially attributed to a decrease in Helicobacter pylori(H. pylori) infection and the use of refrigeration to preserve foods rather than salt. GC is a multifactorial disease, and its occurrence and development were impacted by environmental and genetic factors. H. pylori infection is the primary risk factor for GC, especially for non-cardia. The prognosis of GC is poor due to stages at the first diagnosis. The 5-year survival rate is less than 10% when patients are diagnosed at an advanced stage, but the rate is as high as 85% if patients are detected at an earlier stage. Endoscopic screening can potentially prevent GC by early diagnosis and early treatment and has been widely adopted in screening programs in East Asian countries, such as Japan and Korea. 5The Association of Maternal Body Composition and Dietary Intake with the Risk of Gestational Diabetes Mellitus during the Second Trimester in a Cohort of Chinese Pregnant Women显示文摘Objective To investigate the association of maternal body composition and dietary intake with the risk of gestational diabetes mellitus(GDM). Methods A total 154 GDM subjects and 981 controls were enrolled in a prospective cohort study in 11 hospitals from May 20, 2012 to December 31, 2013. Bioelectrical impedance analysis and dietary surveys were used to determine body composition and to evaluate the intake of nutrients in subjects at 21-24 weeks' gestation(WG). Logistic regression analysis was applied to explore the relationships of maternal body composition and dietary intake with the risk of GDM morbidity. Results Age, pre-pregnant body weight(BW), and body mass index(BMI) were associated with increased risk of GDM. Fat mass(FM), fat mass percentage(FMP), extracellular water(ECW), BMI, BW, energy, protein, fat, and carbohydrates at 21-24 WG were associated with an increased risk of GDM. In contrast, fat free mass(FFM), muscular mass(MM), and intracellular water(ICW) were associated with a decreased risk of GDM. 6Learning-based lensless imaging through optically thick scattering media显示文摘The problem of imaging through thick scattering media is encountered in many disciplines of science,ranging from mesoscopic physics to astronomy.Photons become diffusive after propagating through a scattering medium with an optical thickness of over 10 times the scattering mean free path.As a result,no image but only noise-like patterns can be directly formed.We propose a hybrid neural network for computational imaging through such thick scattering media,demonstrating the reconstruction of image information from various targets hidden behind a white polystyrene slab of 3 mm in thickness or 13.4 times the scattering mean free path.We also demonstrate that the target image can be retrieved with acceptable quality from a very small fraction of its scattered pattern,suggesting that the speckle pattern produced in this way is highly redundant.This leads to a profound question of how the information of the target being encoded into the speckle is to be addressed in future studies.Meng Lyu Hao Wang Guowei Li Shanshan Zheng Guohai Situ 2019Advanced Photonics2019,1,3:28

8Posterior scleral reinforcement for the treatment of pathological myopia显示文摘AIM: To investigate the effects of posterior scleral reinforcement(PSR) in the treatment of pathological myopia.·METHODS: The study included 52 eyes in 43 patients with pathological myopia who underwent PSR(PSR group), and 52 eyes in 36 age- and myopia-matched patients who did not undergo such treatment as control group. Axial length, refraction error, best corrected visual acuity(BCVA), and macular scans by optical coherence tomography(OCT) were recorded at baseline, 6mo, 1, 3and 5y after the surgery, and the complications were noted.·RESULTS: There were no statistical differences in axial length, refractive error, or BCVA between the PSR group and the control group at baseline. At the end of the follow-up, the mean axial length was 29.79 ±1.26 mm in the PSR group, which was significantly shorter than that in the control group(30.78±1.30 mm)(P <0.01), and the mean refractive error was-16.86 ±2.53 D in the PSR group, which was significantly lower than that in the control group(-19.18 ±2.12 D)(P <0.01). A statistically significant difference in BCVA was found between the PSR group(0.51 ±0.25 log MAR) and the control group(0.62±0.26 log MAR) at the postoperative 5-year follow-up(P <0.01). 9Dexmedetomidine attenuates traumatic brain injury: action pathway and mechanisms显示文摘Traumatic brain injury induces potent inflammatory responses that can exacerbate secondary blood-brain barrier(BBB) disruption, neuronal injury, and neurological dysfunction. Dexmedetomidine is a novel α2-adrenergic receptor agonist that exert protective effects in various central nervous system diseases. The present study was designed to investigate the neuroprotective action of dexmedetomidine in a mouse traumatic brain injury model, and to explore the possible mechanisms. Adult male C57 BL/6 J mice were subjected to controlled cortical impact. After injury, animals received 3 days of consecutive dexmedetomidine therapy(25 μg/kg per day). The modified neurological severity score was used to assess neurological deficits. The rotarod test was used to evaluate accurate motor coordination and balance. Immunofluorescence was used to determine expression of ionized calcium binding adapter molecule-1, myeloperoxidase, and zonula occluden-1 at the injury site. An enzyme linked immunosorbent assay was used to measure the concentration of interleukin-1β(IL-1β), tumor necrosis factor α, and IL-6. The dry-wet weight method was used to measure brain water content. The Evans blue dye extravasation assay was used to measure BBB disruption. Western blot assay was used to measure protein expression of nucleotide-binding oligomerization domain-like receptor family pyrin domain-containing 3(NLRP3), caspase-1 p20, IL-1β, nuclear factor kappa B(NF-κB) p65, occluding, and zonula occluden-1. Flow cytometry was used to measure cellular apoptosis. Results showed that dexmedetomidine treatment attenuated early neurological dysfunction and brain edema. Further, dexmedetomidine attenuated post-traumatic inflammation, up-regulated tight junction protein expression, and reduced secondary BBB damage and apoptosis. These protective effects were accompanied by down-regulation of the NF-κB and NLRP3 inflammasome pathways. 10Status of glycosylated hemoglobin and prediction of glycemic control among patients with insulin-treated type 2 diabetes in North China:a multicenter observational study显示文摘Background:Blood glucose control is closely related to type 2 diabetes mellitus(T2DM)prognosis.This multicenter study aimed to investigate blood glucose control among patients with insulin-treated T2DM in North China and explore the application value of combining an elastic network(EN)with a machine-learning algorithm to predict glycemic control.Methods:Basic information,biochemical indices,and diabetes-related data were collected via questionnaire from 2787 consecutive participants recruited from 27 centers in six cities between January 2016 and December 2017.An EN regression was used to address variable collinearity.Then,three common machine learning algorithms(random forest[RF],support vector machine[SVM],and back propagation artificial neural network[BP-ANN])were used to simulate and predict blood glucose status.Additionally,a stepwise logistic regression was performed to compare the machine learning models.Results:The well-controlled blood glucose rate was 45.82%in North China.The multivariable analysis found that hypertension history,atherosclerotic cardiovascular disease history,exercise,and total cholesterol were protective factors in glycosylated hemoglobin(HbAlc)control,while central adiposity,family history,T2DM duration,complications,insulin dose,blood pressure,and hypertension were risk factors for elevated HbAlc.Before the dimensional reduction in the EN,the areas under the curve of RF,SVM,and BP were 0.73,0.61,and 0.70,respectively,while these figures increased to 0.75,0.72,and 0.72,respectively,after dimensional reduction.Moreover,the EN and machine learning models had higher sensitivity and accuracy than the logistic regression models(the sensitivity and accuracy of logistic were 0.52 and 0.56;RF:0.79,0.70;SVM:0.84,0.73;BP-ANN:0.78,0.73,respectively).Conclusions:More than half of T2DM patients in North China had poor glycemic control and were at a higher risk of developing diabetic complications.The EN and machine learning algorithms are alternative choices,in addition to the traditional logistic model,for building predictive models of blood glucose control in patients with T2DM.Jiao Wang Meng-Yang Wang Hui Wang Hong-Wei Liu Rui Lu Tong-Qing Duan Chang-Ping Li Zhuang Cui Yuan-Yuan Liu Yuan-Jun Lyu Jun Ma 2020Chinese Medical Journal2020,,1:19

12Occurrence of perfluoroalkyl substances in matched human serum,urine,hair and nail显示文摘The purpose of this study was to determine perfluoroalkyl substances(PFASs) in human serum, urine, hair and nail from general populations, and to investigate the possibility for human urine, hair and nail used as the biomonitoring sample for PFASs exposure. We detected the concentrations of PFHx A, PFOA, PFNA, PFDA, PFUn DA, PFDo A, PFHx S and PFOS in 39 matched human serum, urine, hair and nail samples from Shenzhen in China. The detection frequency and the median level of PFOS were all higher than that of the other PFASs in four matrices. The median concentration of PFOS in serum, urine, hair, and nail were 9.24 ng/mL, 13.96 ng/L, 0.58 ng/g and 0.63 ng/g, respectively. 13Multimorbidity patterns and association with mortality in 0.5 million Chinese adults显示文摘Background:Few studies have assessed the relationship between multimorbidity patterns and mortality risk in the Chinese population.We aimed to identify multimorbidity patterns and examined the associations of multimorbidity patterns and the number of chronic diseases with the risk of mortality among Chinese middle-aged and older adults.Methods:We used data from the China Kadoorie Biobank and included 512,723 participants aged 30 to 79 years.Multimorbidity was defined as the presence of two or more of the 15 chronic diseases collected by self-report or physical examination at baseline.Multimorbidity patterns were identified using hierarchical cluster analysis.Cox regression was used to estimate the associations of multimorbidity patterns and the number of chronic diseases with all-cause and cause-specific mortality.Results:Overall,15.8%of participants had multimorbidity.The prevalence of multimorbidity increased with age and was higher in urban than rural participants.Four multimorbidity patterns were identified,including cardiometabolic multimorbidity(diabetes,coronary heart disease,stroke,and hypertension),respiratory multimorbidity(tuberculosis,asthma,and chronic obstructive pulmonary disease),gastrointestinal and hepatorenal multimorbidity(gallstone disease,chronic kidney disease,cirrhosis,peptic ulcer,and cancer),and mental and arthritis multimorbidity(neurasthenia,psychiatric disorder,and rheumatoid arthritis).During a median of 10.8 years of follow-up,49,371 deaths occurred.Compared with participants without multimorbidity,cardiometabolic multimorbidity(hazard ratios[HR]=2.20,95%confidence intervals[CI]:2.14-2.26)and respiratory multimorbidity(HR=2.13,95%CI:1.97-2.31)demonstrated relatively higher risks of mortality,followed by gastrointestinal and hepatorenal multimorbidity(HR=1.33,95%CI:1.22-1.46).The mortality risk increased by 36%(HR=1.36,95%CI:1.35-1.37)with every additional disease.Conclusion:Cardiometabolic multimorbidity and respiratory multimorbidity posed the highest threat on mortality risk and deserved particular attention in Chinese adults.Junning Fan Zhijia Sun Canqing Yu Yu Guo Pei Pei Ling Yang Yiping Chen Huaidong Du Dianjianyi Sun Yuanjie Pang Jun Zhang Simon Gilbert Daniel Avery Junshi Chen Zhengming Chen Jun Lyu Liming Li On Behalf of the China Kadoorie Biobank Collaborative Group 2022Chinese Medical Journal2022,,6:17
14Adjuvant chemotherapy with paclitaxel and cisplatin in lymph node-positive thoracic esophageal squamous cell carcinoma显示文摘Objective:No standard postoperative adjuvant chemotherapy has ever been established in node-positive esophageal squamous cell carcinoma(ESCC).This is a study to explore the effect of postoperative paclitaxel(PTX)and cisplatin(DDP)in lymph node-positive,completely resected thoracic ESCC patients.Methods:We conducted a prospective phase II trial.Patients had pathologically node-positive thoracic ESCC with negative margins.Outcomes of disease-free survival(DFS)and overall survival(OS)were compared with a matched historical control cohort.The postoperative chemotherapy regimen consisted of 4 to 6 cycles of PTX 150mg/m2 administered intravenously on d 1 followed by DDP 50 mg/m2 on d 2 every 14 d.Results:Forty-three patients were accrued from December 2007 to May 2012 at Cancer Hospital of Chinese Academy of Medical Sciences for adjuvant chemotherapy.The historical control group consisted of 80 patients who received complete resection but no adjuvant chemotherapy during the same period of time.Of the 43 patients with adjuvant chemotherapy,37(86.0%)patients completed 4 to 6 cycles of chemotherapy.The 3-year DFS rates were56.3%in the adjuvant group and 34.6%in the control group(P=0.006).The 3-year OS rates were 55.0%in the adjuvant group and 37.5%in the control group(P=0.013).Multivariate analysis revealed that postoperative chemotherapy was the significant predictor for improved OS(P=0.005).Conclusions:Biweekly adjuvant PTX and DDP might improve 3-year DFS and OS in lymph node-positive,curatively resected thoracic ESCC patients.These conclusions warrant further study in randomized phase III clinical trials.Liangze Zhang Weiwei Li Xiao Lyu Yan Song Yousheng Mao Shaoming Wang Jing Huang 2017Chinese Journal of Cancer Research2017,29,2:15

19Three-dimensional finite-time cooperative guidance for multiple missiles without radial velocity measurements显示文摘In order to simultaneously attack a target with impact angle constraint in threedimensional(3-D) space, a novel distributed cooperative guidance law for multiple missiles under directed communication topologies is proposed without radial velocity measurements. First, based on missiles-target 3-D relative motion equations, the multiple missiles cooperative guidance model with impact angle constraint is constructed. Then, in Line-of-Sight(LOS) direction, based on multiagent system cooperative control theory, one guidance law with directed topologies is designed with strict proof, which can guarantee finite time consensus of multiple missiles' impact times. Next, in elevation direction and azimuth direction of LOS, based on homogeneous system stability theory and integral sliding mode control theory, two guidance laws are proposed respectively with strict proof, which can guarantee LOS angles converge to desired values and LOS angular rates converge to zero in finite time.
