| 1 | 混合的自然的孔 transluminal 为 ileocecal 的内视镜的外科切除术显示文摘 Although laparoscopic colectomy is commonly performed around the world,an operative wound formed during the surgery is large but not sufficient enough to convert for the majority of open surgery.Thus,a certain sized skin incision is required to remove the resected colon.Here we report the case of a pure laparoscopic ileocecal resection which involves transanal specimen extraction.We present a case characterized by a laterally spreading type of tumor of the cecum.We performed a pure laparoscopic ileocecal resection and the resected specimen was removed transanally using colonoscopy.Intracorporeal functional anastomosis was then performed using a flexible linear stapling device under supporting barbed suture traction.The patient was discharged without complications on postoperative day 4.Laparoscopic colectomy performed with minimal incision could essentially increase the usage of this surgical technique.Although our method is restricted to flat or small lesions,we think it is a feasible and realistic solution for minimization of operative invasion because it involves specimen extraction through a natural orifice. | Satoru Takayama Masayasu Hara Mikinori Sato Hiromitsu Takeyama | 2012 | World Journal of Gastrointestinal Surgery2012，4，2： | 2 |

| 13 | 开腹手术所致手术中氧化应激反应的预防显示文摘背景有观点认为,各种活性氧成分早期即参与了手术应激和损伤,但尚未得到证实;医疗手段对氧化应激反应的作用也不明确。方法测定血中氢过氧化物的水平,作为细胞损伤的氧化应激指标;同时测定血浆铁还原能力,作为总抗氧化能力的指标。拟行乙状结肠切除术的患者分为4组:七氰烷麻醉下开腹乙状结肠切除术组;七氟烷麻醉下腹腔镜乙状结肠切除术组;丙泊酚麻醉下开腹乙状结肠切除术组;丙泊酚麻醉下腹腔镜乙状结肠切除术组。结果七氟烷开腹乙状结肠切除术组患者的血浆铁还原能力显着降低,由387mmol/L降至153mmol/L,氢过氧化物水平没有变化,这表明手术中患者的氧化应激反应有所增加。氢过氧化物,是一种典型的细胞氧化产物,它并没有明显变化,可能是由于手术中增加的氧化应澈反应还不足以损伤细胞。七氟烷腹腔镜乙状结肠切除术组中,氢过氧化物水平和铁还原能力都没有明显改变,表明腹腔镜手术不会增加手术的氧化应激反应。丙泊酚麻醉2个组,氢过氧化物术毕都明显下降,分别为120±73UCarr和144±107UCarr(1UCarr=0.8mg/L H2O2)。结论开腹手术会增加手术中的氧化应激反应,腹腔镜手术则不会;丙泊酚作为抗氧化剂能够有效地减少手术中的氧化应激反应。 | Masahiko Tsuchiya Eisuke F. | 14 | Sofosbuvir/Ribavirin therapy for patients experiencing failure of ombitasvir/paritaprevir/ritonavir + ribavirin therapy: Two cases report and review of literature显示文摘BACKGROUND The effectiveness of sofosbuvir/ribavirin(SOF/RBV) combination therapy,which is one of the 1 st-choice therapeutic options for patients with hepatitis C virus(HCV) genotype 2(HCV-G2) in Japan according to the most recent version of the Japan Society of Hepatology guideline, for patients who experienced failure of the ombitasvir/paritaprevir/ritonavir plus ribavirin(OBV/PTV/r+RBV) combination therapy, which was another option for patients with HCV-G2, is unknown.CASE SUMMARY We evaluated the effects of SOF/RBV combination therapy in two patients with genotype 2 a who could not achieve a sustained virological response(SVR) by OBV/PTV/r+RBV combination therapy. One patient was complicated with VogtKoyanagi-Harada(VKH) disease. Resistance-associated variations before SOF/RBV combination therapy were not detected in two patients. Both patients had an SVR at 12 wk after the treatment(SVR12). Regarding adverse events(AEs), itching, chill, a dull feeling in the throat and cough as well as increase of alanine transaminase level were shown in one patient, while a headache and deterioration of light aversion probably due to the recurrence of VKH disease were shown in the other patients.
