1Utility of different glycemic control metrics for optimizing management of diabetes显示文摘The benchmark for assessing quality of long-term glycemic control and adjustment of therapy is currently glycated hemoglobin(Hb A1c). Despite its importance as an indicator for the development of diabeticcomplications, recent studies have revealed that this metric has some limitations; it conveys a rather complex message, which has to be taken into consideration for diabetes screening and treatment. On the basis of recent clinical trials, the relationship between Hb A1 c and cardiovascular outcomes in long-standing diabetes has been called into question. It becomes obvious that other surrogate and biomarkers are needed to better predict cardiovascular diabetes complications and assess efficiency of therapy. Glycated albumin, fructosamin, and 1,5-anhydroglucitol have received growing interest as alternative markers of glycemic control. In addition to measures of hyperglycemia, advanced glucose monitoring methods became available. An indispensible adjunct to Hb A1 c in routine diabetes care is selfmonitoring of blood glucose. This monitoring method is now widely used, as it provides immediate feedback to patients on short-term changes, involving fasting, preprandial, and postprandial glucose levels. Beyond the traditional metrics, glycemic variability has been identified as a predictor of hypoglycemia, and it might also be implicated in the pathogenesis of vascular diabetes complications. Assessment of glycemic variability is thus important, but exact quantification requires frequently sampled glucose measurements. In order to optimize diabetes treatment, there is a need for both key metrics of glycemic control on a day-to-day basis and for more advanced, user-friendly monitoring methods. In addition to traditional discontinuous glucose testing, continuous glucose sensing has become a useful tool to reveal insufficient glycemic management. This new technology is particularly effective in patients with complicated diabetes and provides the opportunity to characterize glucose dynamics. Several continuous glucose monitoring(CGM) systems, which have shown usefulness in clinical practice, are presently on the market. They can broadly be divided into systems providing retrospective or real-time information on glucose patterns. The widespread clinical application of CGM is still hampered by the lack of generallyaccepted measures for assessment of glucose profiles and standardized reporting of glucose data. 3Significance of an additional unenhanced scan in computed tomography angiography of patients with suspected acute aortic syndrome显示文摘AIM To assess potential benefits of an additional unenhanced acquisition in computed tomography angiography(CTA) in patients with suspected acute aortic syndrome(AAS).METHODS A total of 103 aortic CTA(non-electrocardiography-gated, 128 slices) performed due to suspected AAS were retrospectively evaluated for acute aortic dissection(AAD), intramural hematoma(IMH), or penetrating aortic ulcer(PAU). Spiral CTA protocol consisted of an unenhanced acquisition and an arterial phase. If AAS was detected, a venous phase(delay, 90 s) was added. Images were evaluated for the presence and extent of AAD, IMH, PAU, and related complications. The diagnostic benefit of the unenhanced acquisition was evaluated concerning detection of IMH.RESULTS Fifty-six(30% women; mean age, 67 years; median, 68 years) of the screened individuals had AAD or IMH. A triphasic CT scan was conducted in 76.8%(n =43). 56% of the detected AAD were classified as Stanford type A, 44% as Stanford type B. 53.8% of the detected IMH were classified as Stanford type A, 46.2% as Stanford type B. There was no significant difference in the involvement of the ascending aorta between AAD and IMH(P = 1.0) or in the average age between AAD and IMH(P = 0.548), between Stanford type A and Stanford type B in general(P = 0.650) and between Stanford type A and Stanford type B within the entities of AAD and IMH(AAD: P = 0.785; IMH: P = 0.146). Only the unenhanced acquisitions showed a significant density difference between the adjacent lumen and the IMH(P = 0.035). Subadventitial hematoma involving the pulmonary trunk was present in 5 patients(16%) with Stanford A AAD. The difference between the median radiation exposure of a triphasic(2737 mGy*cm) compared to a biphasic CT scan(2135 mGy*cm) was not significant(P = 0.135).CONCLUSION IMH is a common and difficult to detect entity of AAS. 14Tender evaluation through efficiency analysis for public construction contracts显示文摘Given the European Public Procurement Directive 2014/24/EU,policymakers have ordered the inclusion of various criteria,such as the price,life-cycle costs,environmental,and social aspects,in the evaluation of tenders for public construction projects.Consequently,the relevance of non-monetary award criteria has gained significant value.However,the established evaluation formulas,which are used to obtain the best value for money procurement,have resulted in legal disputes.The existing evaluation formulas exhibit mathematical weaknesses,wherein scoring indices do not express economic efficiency adequately.Thus,a conflict is observed between the political requirement of non-monetary award criteria and their evaluation by contracting authorities.To overcome such dilemma,an extensive literature review is conducted.Specifically,this study explores the essential problems of existing evaluation formulas and develops a more reliable method.The technique from the field of efficiency analysis,i.e.,Data Envelopment Analysis(DEA),is adopted in this study.For contract awarding,the DEA is extended by introducing a decision theoretical framework.For public procurement,the proposed method combines two advantages.First,the proposed method ensures the derivation of a robust tender ranking given that with respect to clients’preferences,irrelevant and insufficiently tailored tenders do not influence the scoring.Second,the proposed method supports the intention of policy makers to promote public goals,such as sustainable aspects.By disclosing the strengths and weaknesses of bidders with respect to their competitors,all bidders can obtain a precise overview of their performance regarding the award criteria.In sum,the proposed method allows a targeted improvement of certain criteria values in future tenders and consequently leads to an enhancement of public goals.Stefan NIEWERTH Peter VOGT Markus THEWES 2022Frontiers of Engineering Management2022,9,1:1
