| 1 | MELD score,insulin-like growth factor 1 and cytokines on bone density in end-stage liver disease显示文摘AIM:To determine the contributions of insulin-like growth factor 1 (IGF-1),cytokines and liver disease severity to bone mineral density in patients pre-transplantation.METHODS:Serum IGF-1,tumor necrosis factor-α (TNFα) and interleukin 6 (IL-6) were measured and the Model for End-Stage Liver Disease (MELD) score calculated in 121 adult patients referred to a single centre for liver transplantation.Bone mineral density (BMD) of the lumbar spine and femoral neck were assessed via dual energy X-ray absorptiometry.Demographics,liver disease etiology,medication use and relevant biochemistry were recorded.RESULTS:A total of 117 subjects were included,with low BMD seen in 68.6%,irrespective of disease etiol-ogy.In multivariable analysis,low body mass index (BMI),increased bone turnover and low IGF-1 were independent predictors of low spinal bone density.At the hip,BMI,IGF-1 and vitamin D status were predictive.Despite prevalent elevations of TNFα and IL-6,levels did not correlate with degree of bone loss.The MELD score failed to predict low BMD in this pre-transplant population.CONCLUSION:Osteopenia/osteoporosis is common in advanced liver disease.Low serum IGF-1 is weakly predictive but serum cytokine and MELD score fail to predict the severity of bone disease. | Rebecca Mitchell Jill McDermid Mang M Ma Constance L Chik | 2011 | World Journal of Hepatology2011，3，6： | 5 |
| 2 | Pediatric Nd:YAG laser capsulotomy in the operating room: review of 87 cases显示文摘AIM: To report a large series of children having Nd:YAG laser capsulotomy in the operating room using the lateral decubitus position. METHODS: Medical records of children who underwent Nd:YAG laser capsulotomy in the operating room at Ann & Robert H. Lurie Children's Hospital of Chicago between September 2008 and April 2017 were reviewed. Induction of general anesthesia and intubation was performed in the supine position after which the patient was placed in lateral decubitus position. The Nd:YAG laser capsulotomy was performed using a standard protocol. At the completion of the procedure, the patient was turned back into the supine position and extubated. RESULTS: This study included 87 eyes of 60 patients. Patient's age ranged from 1 to 18 y(mean 6.4±4.1 y). In most cases(84/87, 97%), the procedure was performed under general anesthesia. In all cases, good focus on the membrane was achieved, and the procedure was performed successfully. There were no intraoperative ocular or anesthesia-related complications. | 3 | NOD2 and ATG16L1 polymorphisms affect monocyte responses in Crohn's disease显示文摘AIM:To assess whether polymorphisms in NOD2 and ATG16L1 affect cytokine responses and mycobacterium avium subspecies paratuberculosis (MAP) survival in monocytes from Crohn's disease (CD) patients.METHODS:Monocytes were isolated from peripheral blood of CD patients of known genotype for common single nucleotide polymorphisms of NOD2 and ATG16L1.Monocytes were challenged with MAP and bacterial persistence assessed at subsequent time-points.Cytokine responses were assayed using a Milliplex multi-analyte profiling assay for 13 cytokines.RESULTS:Monocytes heterozygous for a NOD2 polymorphism (R702W,P268S,or 1007fs) were more permissive for growth of MAP (P=0.045) than those without.There was no effect of NOD2 genotype on subsequent cytokine expression.The T300A polymorphism ofATG16L1 did not affect growth of MAP in our model (P=0.175),but did increase expression of cytokines interleukin (IL)-10 (P=0.047) and IL-6 (P=0.019).CONCLUSION:CD-associated polymorphisms affected the elimination of MAP fromex vivo monocytes (NOD2),or expression of certain cytokines (ATG16L1),implying independent but contributory roles in the pathogenesis of CD. | Dylan M Glubb Richard B Gearry Murray L Barclay Rebecca L Roberts John Pearson Jacqui I Keenan Judy McKenzie Robert W Bentley | 2011 | World Journal of Gastroenterology2011，17，23： | 2 |
