| 1 | 基于时序NDVI的露天煤矿区土地损毁与复垦过程特征分析显示文摘露天煤矿区是人类活动强扰动地区之一。该文以阿巴拉契亚煤田区韦恩县为研究区域,应用遥感时序分析法分析了像元尺度的土地损毁和复垦过程特征。得出结论:1984-2010年间,韦兹县露天开采扰动区域占采矿权范围的45.80%,其中植被恢复区域占开采范围的66.45%,开采时间越早,植被恢复像元比例越高;开采造成的地表无植被覆盖期时长中位数为6 a,均值为7 a;已充分复垦的区域,NDVI值恢复至采前水平的加权平均时长为12 a。基于像元变化轨迹的研究,除揭示土地损毁-复垦过程特征外,能较好地反映空间异质性,可以为土地复垦管理和相关政策决策提供科学依据。 | 李晶 Zipper Carl E. 李松 Donovan Patricia F. | 2 | Proton pump inhibitor-associated pneumonia:Not a breath of fresh air after all?显示文摘Over the past two decades,proton pump inhibitors (PPIs)have emerged as highly effective and relatively safe agents for the treatment of a variety of gastrointestinal disorders.Unfortunately,this desirable pharmacological profile has also contributed to superfluous and widespread use in both the inpatient and outpatient settings.While generally well-tolerated,research published over the last decade has associated these agents with increased risks of Clostridium difficile disease, fractures likely due to calcium malabsorption and both community-acquired(CAP)and hospital-acquired pneumonias(HAP).The mechanism behind PPI-associated pneumonia may be multifactorial,but is thought to stem from compromising the stomach's'acid mantle'against gastric colonization of acid-labile pathogenic bacteria which then may be aspirated.A secondary postulate is that PPIs,through their inhibition of extra-gastric H+/K+ATPase enzymes,may reduce the acidity of the upper aerodigestive tract,thus resulting in increased bacterial colonization of the larynx,esophagus and lungs.To date,several retrospective case control studies have been published looking at the association between PPI use and CAP.Some studies found a temporal relationship between PPI exposure and the incidence of pneumonia,but only two could define a dose-response re-lationship.Furthermore,other studies found an inverse correlation between duration of PPI use and risk of CAP. In terms of HAP,we reviewed two retrospective cohort studies and one prospective study.One retrospective study in a medical ICU found no increased association of HAP in PPI-exposed patients compared to no acid-lowering therapy,while the other in cardiothoracic surgery patients showed a markedly increased risk compared to those receiving H2RAs.The one prospective study in ICU patients showed an increased risk of HAP with PPIs, but not with H2RAs.In conclusion,the current literature shows a slight trend toward an association between PPI use and pneumonia and an increased risk with PPIs over H2RAs,but the findings are not consistent across all studies.Larger controlled trials still need to be done to better identify the risk that PPIs impart towards patients contracting CAP or HAP.Until these are completed, we will have to continue to extrapolate across smaller controlled trials to predict the associated risks in our respective patient populations.In the interim,it appears prudent to limit the use of PPIs to situations where they are clinically indicated and,in such cases,use them at the lowest effective dose.In the case of prescribing for stress ulcer prophylaxis in ICU patients,perhaps H2RAs should be used as the preferred agents over PPIs. | Alexander L Fohl Randolph E Regal | 2011 | World Journal of Gastrointestinal Pharmacology and Therapeutics2011,2,3: | 12 |
| 3 | Augmenting peripheral nerve regeneration using stem cells: A review of current opinion显示文摘Outcomes following peripheral nerve injury remain frustratingly poor. The reasons for this are multifactorial, although maintaining a growth permissive environment in the distal nerve stump following repair is arguably the most important. The optimal environment for axonal regeneration relies on the synthesis and release of many biochemical mediators that are temporally and spatially regulated with a high level of incompletely understood complexity. The Schwann cell(SC) has emerged as a key player in this process. Prolonged periods of distal nerve stump denervation, characteristic of large gaps and proximal injuries, have been associated with a reduction in SC number and ability to support regenerating axons. Cell based therapy offers a potential therapy for the improvement of outcomes following peripheral nerve reconstruction. Stem cells have the potential to increase the number of SCs and prolong their ability to support regeneration. They may also have the ability to rescue and replenish populations of chromatolytic and apoptotic neurons following axotomy. Finally, they can be used in non-physiologic ways to preserve injured tissues such as denervated muscle while neuronal ingrowth has not yet occurred. Aside from stem cell type, careful consideration must be given to differentiation status, how stem cells are supported following transplantation and how they will be delivered to the site of injury. | 4 | 考虑转捩影响的翼型动态失速数值模拟显示文摘用数值求解雷诺平均NS方程的方法来对考虑转捩影响的中低雷诺数下振荡翼型动态失速进行数值模拟,计算中采用了k-ωSST两方程湍流模式,并加入Chen-Thyson转捩模型来模拟流动中的转捩效应。采用该方法分别对雷诺数Re=1.35×105和Re=7.7×104情况下NACA0012翼型的动态失速进行了数值模拟。计算结果显示:计算出的翼型动态失速气动力系数迟滞曲线与实测结果符合较好;对于Re=7.7×104的工况,实测的升力系数迟滞曲线中出现了高频振荡,计算结果有效地捕捉到了这一流动现象,并通过分析瞬时流线的计算结果揭示出尾缘涡的涡脱落是引起该高频振荡的主要原因。此外,通过数值计算分析了中低雷诺数下减缩频率对升力系数迟滞曲线的影响,从结果中看到,随着减缩频率的增加,翼型的失速攻角值会增加,升力系数峰值会增加;当减缩频率减小时,升力系数曲线中的高频振荡频率会增加。通过进一步计算分析知,升力曲线中高频振荡的产生不仅取决于减缩频率,还取决于流动的雷诺数,只有在中低雷诺数、较小的减缩频率下翼型动态失速的升力迟滞曲线中才有可能出现高频振荡。 | 钱炜祺 Randolph C. | 5 | A dual positional specific lipoxygenase functions in the generation of flavor compounds during climacteric ripening of apple显示文摘Lipoxygenase(LOX)is an important contributor to the formation of aroma-active C6 aldehydes in apple(Malus×domestica)fruit upon tissue disruption but little is known about its role in autonomously produced aroma volatiles from intact tissue.We explored the expression of 22 putative LOX genes in apple throughout ripening,but only six LOXs were expressed in a ripening-dependent manner.Recombinant LOX1:Md:1a,LOX1:Md:1c,LOX2:Md:2a and LOX2:Md:2b proteins showed 13/9-LOX,9-LOX,13/9-LOX and 13-LOX activity with linoleic acid,respectively.While products of LOX1:Md:1c and LOX2:Md:2b were S-configured,LOX1:Md:1a and LOX2:Md:2a formed 13(R)-hydroperoxides as major products.Site-directed mutagenesis of Gly567 to an alanine converted the dual positional specific LOX1:Md:1a to an enzyme with a high specificity for 9(S)-hydroperoxide formation.The high expression level of the corresponding MdLOX1a gene in stored apple fruit,the genetic association with a quantitative trait locus for fruit ester and the remarkable agreement in regio-and stereoselectivity of the LOX1:Md:1a reaction with the overall LOX activity found in mature apple fruits,suggest a major physiological function of LOX1:Md:1a during climacteric ripening of apples.While LOX1:Md:1c,LOX2:Md:2a and LOX2:Md:2b may contribute to aldehyde production in immature fruit upon cell disruption our results furnish additional evidence that LOX1:Md:1a probably regulates the availability of precursors for ester production in intact fruit tissue. | Doreen Schiller Carolina Contreras Jörg Vogt Frank Dunemann Bruno G Defilippi Randolph Beaudry Wilfried Schwab | 2015 | Horticulture Research2015,2,1: | 6 |

| 7 | Hepatic intra-arterial infusion of yttrium-90 microspheres in the treatment of recurrent hepatocellular carcinoma after liver transplantation: A case report显示文摘Hepatocellular carcinoma (HCC) recurs with a reported frequency of 12%-18% after liver transplantation. Recurrence is associated with a mortality rate exceeding 75%. Approximately one-third of recurrences develop in the transplanted liver and are therefore amenable to local therapy. A variety of treatment modalities have been reported including resection, transarterial chemo- embolization (TACE), radiofrequency ablation (RFA), ethanol ablation, cryoablation, and external beam irradiation. Goals of treatment are tumor control and the minimization of toxic effect to functional parenchyma. Efficacy of treatment is mitigated by the need for ongoing immunosuppression. Yttrium-90 microspheres have been used as a treatment modality both for primary HCC and for pre-transplant management of HCC with promising results. Twenty-two months after liver transplantation for hepatitis C cirrhosis complicated by HCC, a 42-year old man developed recurrence of HCC in his transplant allograft. Treatment of multiple right lobe lesions with anatomic resection and adjuvant chemotherapy was unsuccessful. Multifocal recurrence in the remaining liver allograft was treated with hepatic intra-arterial infusion of yttrium-90 microspheres (SIR-Spheres, Sirtex Medica Inc., Lake Forest, IL, USA). Efficacy was demonstrated by tumor necrosis on imaging and a decrease in alpha-fetoprotein (AFP) level. | 10 | Cockcroft-gault revisited: New de-liver-ance on recommendations for use in cirrhosis显示文摘The Cockcroft-Gault(CG) equation has become perhaps the most popular practical approach for estimating renal function among health care professionals. Despite its widespread use, clinicians often overlook not only the limitations of the original serum creatinine(SCr) based equation, but also may not appreciate the validity of the many variations used to compensate for these limitations. For cirrhotic patients in particular, the underlying pathophysiology of the disease contributes to a falsely low SCr, thereby overestimating renal function with use of the CG equation in this population. We reviewed the original CG trial from 1976 along with data surrounding clinician specific alterations to the CG equation that followed through time. These alterations included different formulas for body weight in obese patients and the 'rounding up' approach in patients with low SCr. Additionally, we described the pathophysiology and hemodynamic changes that occur in cirrhosis; and reviewed several studies that attempted to estimate renal function in this population. The evidence we reviewed regarding the most accurate manipulation of the original CG equation to estimate creatinine clearance(Cr Cl) was inconclusive. Unfortunately, the homogeneity of the patient population in the original CG trial limited its external validity. Elimination of body weight in the CG equation actually produced the estimate closest to the measure Cr Cl. Furthermore, 'rounding up' of SCr values often underestimated Cr Cl. This approach could lead to suboptimal dosing of drug therapies in patients with low SCr. | 15 | Prevalence of Barrett's esophagus in patients with moderate to severe erosive esophagitis显示文摘AIM: To investigate the proportion of patients with moderate-severe erosive esophagitis (EE) who will have Barrett's esophagus (BE) after healing of inflammation. METHODS: Patients with EE of Los Angeles (LA) class B, C and D who underwent follow-up endoscopy documenting complete mucosal healing. RESULTS: A total of 86/169 patients were suspected of having BE (38 before healing and 48 after healing of EE) and, 46/86 eventually had the histological confirmation. At index esophago-gastro-duodenoscopy (EGD), BE was suspected in 38/169 (22%), and ultimately, histologically confirmed in 20 of these. In 11 patients where biopsies were performed in the presence of inflammation, BE was detected in 2 and missed in 5 (including 2 dysplasias). In 131/169 patients (77.5%), BE was not suspected at index EGD. After healing of EE though, 48 patients had suspicion of BE who underwent biopsies, and in 26 of these histology was positive for BE. The length of inflammation had a linear correlation with the length of BE (P = 0.01). Out of multiple variables to predict BE, only the suspicion at index endoscopy was statistically significant (P = 0.01). CONCLUSION: BE was seen in 46/169 (27%) patients with EE of LA class B, C and D.
