1HBV cccDNA in patients' sera as an indicator for HBV reactivation and an early signal of liver damage显示文摘AIM: To evaluate the covalently closed circle DNA (cccDNA)level of hepatitis B virus (HBV) in patients' liver and sera. METHODS: HBV DNA was isolated from patients' liver biopsies and sera. A sensitive real-time PCR method, which is capable of differentiation of HBV viral genomic DNA and cccDNA, was used to quantify the total HBV cccDNA. The total HBV viral DNA was quantitated by real-time PCR using a HBV diagnostic kit (PG Biotech, LTD, Shenzhen, China)described previously. RESULTS: For the first time, we measured the level of HBV DNA and cccDNA isolated from ten HBV patients' liver biopsies and sera. In the liver biopsies, cccDNA was detected from all the biopsy samples. The copy number of cccDNA ranged from from 0.03 to 173.1 per ceil, the copy number of total HBV DNA ranged from 0.08 to 3 717 per cell. The ratio of total HBV DNA to cccDNA ranged from 1 to 3 406. In the sera,cccDNA was only detected from six samples whereas HBV viral DNA was detected from all ten samples. The ratio of cccDNA to total HBV DNA ranged from 0 to 1.77%. To further investigate the reason why cccDNA could only be detected in some patients' sera, we performed longitudinal studies. The cccDNA was detected from the patients' sera with HBV reactivation but not from the patients' sera without HBV reactivation. The level of cccDNA in the sera was correlated with ALT and viral load in the HBV reactivation patients. CONCLUSION: HBV cccDNA is actively transcribed and replicated in some patients' hepatocytes, which is reflected by a high ratio of HBV total DNA vs cccDNA. Detection of cccDNA in the liver biopsy will provide an end-point for the anti-HBV therapy. The occurrence of cccDNA in the sera is an early signal of liver damage, which may be another important clinical parameter.Johnny Sze 2004World Journal of Gastroenterology2004,10,1:107

3Impacts of different drying strategies on drying characteristics,the retention of bio-active ingredient and colour changes of dried Roselle显示文摘The drying kinetics of Roselle(Hibiscus sabdariffa L) of variety Terengganu(UMKL-1) and the quality attribution of Roselle were studied. The experiments were conducted using four different drying methods, including solar greenhouse drying(SD), solar greenhouse with intermittent heat pump drying(SIHP), hot air drying(HA) and heat pump drying(HP). Among the four drying methods, HP achieved the highest drying rate at a range from0.054 g H_2 O·(g DM)^(-1) ·min^(-1) to 0.212 g H_2 O·(g DM)^(-1)·min^(-1) while SD had the lowest drying rate, measured at 0.042 g H_2 O·(g DM)^(-1)·min^(-1).The analysis on colour kinetics revealed that there is no significant colour loss(p > 0.05) observed from HP's dried Roselle. 4儿童NTRK重排间叶源性肿瘤临床病理学观察显示文摘目的:探讨儿童NTRK重排间叶源性肿瘤临床和病理学特征,以提高对该类疾病的认识。方法:收集上海交通大学医学院附属上海儿童医学中心及新加坡KK Women′s and Children′s Hospital从2017年1月至2019年9月5例手术切除标本,采用EnVision法检测免疫组织化学表型,荧光原位杂交(FISH)检测相关基因并对NTRK基因重排进行克隆性分析。结果:该组5例患儿,3例男性,2例女性。年龄从3个月到13岁,部位包括软组织(膝、胸腔、腹壁)和肾,肿瘤大小为4.5~12.5 cm。组织学检查主要呈梭形细胞肿瘤,浸润性生长,可伴有炎性细胞。免疫表型上,肿瘤细胞阳性表达Pan-TRK,分子检测均存在NTRK基因重排,包括DCTN1-NTRK1的发现。除2例目前接受靶向药物治疗,其余均为无疾病进展病例,随访时间9~29个月。结论:NTRK重排间叶源性肿瘤,其肿瘤发生部位广泛,组织学图像多变。免疫组织化学Pan-TRK可以帮助诊断,NTRK基因检测确认其存在重排是诊断的金标准,对不能完整切除或有复发、转移病例建议靶向药物治疗。殷敏智 马靖 何巧 沈萍 陈洁枫 金晓婷 张忠德 Chik Hong Kuick Chen Huiyi Eileen Hui Qi Ng Sze Jet Aw Kenneth Tou En Chang 2020中华病理学杂志2020,49,7:6
5Capecitabine with radiation is an effective adjuvant therapy in gastric cancers显示文摘AIM:To analyze the outcome of patients who received concurrent capecitabine(Xeloda) and radiation(XRT) compared to the established concurrent 5-fluorouracil(5-FU) with radiation(5FU-RT) and fluoropyrimidine-based chemotherapy alone as adjuvant treatment in gastric cancers.METHODS:All patients with gastric cancers who received adjuvant treatment at the National Cancer Centre Singapore between 1996 and 2006 were reviewed.Treatment outcomes of patients who received XRT were compared with those who had 5FU-RT or chemotherapy alone as adjuvant therapy for gastric cancers.RESULTS:A total of 108 patients were reviewed.Median age at diagnosis was 60.The majority of the patients(64.8%) had advanced stage Ⅲ and Ⅳ disease(with no distant metastasis).All except 4 patients had D2 gastrectomy.Twenty one patients(19.4%) had positive surgical resection margins.Thirty three patients received XRT compared with 52 who had 5FU-RT and 23 who received chemotherapy alone.For the patients in the chemotherapy-only group,all had fluoropyrimidine-based therapy,with added cisplatin in 7 patients and epirubicin in 2 patients.Median recurrence-free survival was longer for the XRT group(52 mo) compared to the 5FU-RT(35 mo) and chemotherapy-only groups(25 mo)(P=0.48).The patients in the XRT group achieved similar median overall survival(53 mo) as the 5FU-RT(54 mo) and the chemotherapy-only groups(44 mo)(P=0.5).CONCLUSION:Capecitabine with concurrent radiation was as effective as concurrent 5FU with radiation or fluoropyrimidine-based chemotherapy alone when used as adjuvant treatment in patients with gastric cancers.Chee Kian Tham Su Pin Choo Donald Yew Hee Poon Han Chong Toh Simon Yew Kuang Ong Sze Huey Tan Michael Lian Chek Wang Kian Fong Foo 2010World Journal of Gastroenterology2010,16,29:6

8Randomised controlled trial comparing modified Sano's and narrow band imaging international colorectal endoscopic classifications for colorectal lesions显示文摘AIM To assess the utility of modified Sano′s(MS) vs thenarrow band imaging international colorectal endoscopic(NICE) classification in differentiating colorectal polyps.METHODS Patients undergoing colonoscopy between 2013 and 2015 were enrolled in this trial.Based on the MS or the NICE classifications,patients were randomised for real-time endoscopic diagnosis.This was followed by biopsies,endoscopic or surgical resection.The endoscopic diagnosis was then compared to the final(blinded) histopathology.The primary endpoint was the sensitivity(Sn),specificity(Sp),positive predictive value(PPV) and negative predictive value(NPV) of differentiating neoplastic and non-neoplastic polyps(MSⅡ/Ⅱo/Ⅲa/Ⅲb vs I or NICE 1 vs 2/3).The secondary endpoints were 'endoscopic resectability'(MSⅡ/Ⅱo/Ⅲa vs Ⅰ/Ⅲb or NICE 2 vs 1/3),NPV for diminutive distal adenomas and prediction of post-polypectomy surveillance intervals.RESULTS A total of 348 patients were evaluated.The Sn,Sp,PPV and NPV in differentiating neoplastic polyps from non-neoplastic polyps were,98.9%,85.7%,98.2% and 90.9% for MS;and 99.1%,57.7%,95.4% and 88.2% for NICE,respectively.The area under the receiver operating characteristic curve(AUC) for MS was 0.92(95%CI:0.86-0.98);and AUC for NICE was 0.78(95%CI:0.69,0.88).The Sn,Sp,PPV and NPV in predicting 'endoscopic resectability' were 98.9%,86.1%,97.8% and 92.5% for MS;and 98.6%,66.7%,94.7% and 88.9% for NICE,respectively.The AUC for MS was 0.92(95%CI:0.87-0.98);and the AUC for NICE was 0.83(95%CI:0.75-0.90).The AUC values were statistically different for both comparisons(P = 0.0165 and P = 0.0420,respectively).The accuracy for diagnosis of sessile serrated adenoma/polyp(SSA/P) with high confidence utilizing MS classification was 93.2%.The differentiation of SSA/P from other lesions achieved Sp,Sn,PPV and NPV of 87.2%,91.5%,89.6% and 98.6%,respectively.The NPV for predicting adenomas in diminutive rectosigmoid polyps(n = 150) was 96.6% and 95% with MS and NICE respectively.The calculated accuracy of post-polypectomy surveillance for MS group was 98.2%(167 out of 170) and for NICE group was 92.1%(139 out of 151).CONCLUSION The MS classification outperformed the NICE classification in differentiating neoplastic polyps and predicting endoscopic resectability.Both classifications met ASGE PIVI thresholds.Leonardo Zorrón Cheng Tao Pu Kuan Loong Cheong Doreen Siew Ching Koay Sze Pheh Yeap Amanda Ovenden Mahima Raju Andrew Ruszkiewicz Philip W Chiu James Y Lau Rajvinder Singh 2018World Journal of Gastrointestinal Endoscopy2018,10,9:4
9Feasibility and accuracy of coronary imaging in elderly patients using the 64-row multi-detector computed tomography: a correlation study with conventional coronary angiography显示文摘Background Elderly patients generally have higher occurrence of coronary calcification, increased heart rate and difficulty with prolonged breath-holding. The aim of our study was to investigate the feasibility and accuracy of using 64-row multi-detector computed tomography (MDCT) in the assessment of coronary artery stenoses in elderly patients. Methods One hundred and fifty two patients with suspected or known coronary artery disease were divided into 4 groups according to their age (Group A: 40-49 years,n=34; Group B: 50-59 years, n=57; Group C: 60-69 years, n=48; Group D: 70 years and above; n=13). Coronary CT angiography (CTA) using a 64-row MDCT was performed and the findings were compared with that of conventional coronary angiography (CCA).Using axial images, multi-planar reconstructions (MPR) and maximum intensity projections (MIP), coronary segments of lumen diameter = 1.5mm were analyzed for the presence of significant stenosis (= 50% ). Results Percentages of poor image quality from coronary CTA preventing reliable correlations with CCA were 21%, 14%, 19% and 62% in Groups A to D respectively. Patients in Group D had significantly higher calcium scores compared with the other groups (P<0.001). In patients where CTA images were of acceptable quality, percentages of accurate correlations with CCA were 89.8%, 93.4%, 86.6% and 78.0% for Groups A to D respectively.There were no significant difference in serum creatinine, heart rate and contrast volume between the 4 groups. Conclusions The 64-row MDCT coronary angiography was less accurate and feasible for patients aged 70 years or above due to heavy coronary calcification and inability to perform a satisfactory breath-hold. 12Phase Ⅰ/Ⅱ randomized controlled trial of autologous bone marrow-derived mesenchymal stem cell therapy for chronic stroke显示文摘AIM To examine the safety and efficacy of mesenchymal stem cell(MSC) therapy for intracerebral haemorrhage with neurological dysfunctions for a year.METHODS MSC were ex vivo expanded from 29 mL(17-42 mL) autologous bone marrow. Patients were randomized to have two intravenous injections of autologous MSC or placebos in four weeks apart. Neurological functions and clinical outcomes were monitored before treatment and at 12^(th), 16^(th), 24^(th), 36^(th) and 60^(th) week upon completion of^(th)e treatment. RESULTS A mean of 4.57 × 10~7(range: 1.43 × 10~7-8.40 × 10~7) MSC per infusion was administered accounting to 8.54 × 10~5(2.65 × 10~5-1.45 × 10~6) per kilogram body weight in two occasions. There was neither adverse event at time of administration nor sign of de novo tumour development among patients after monitoring for a year post MSC therapy. Neuro-restoration and clinical improvement in terms of modified Barthel index, functional independence measure and extended Glasgow Outcome Scale were evident among patients having MSC therapy compared to patients receiving placebos. 17Predictive control of plug-in electric vehicle chargers with photovoltaic integration显示文摘This paper presents a model predictive control(MPC) for off-board plug-in electric vehicle(PEV)chargers with photovoltaic(PV) integration using two-level four-leg inverter topology. The PEV charger is controlled by a unified controller that incorporates direct power and current MPC to dynamically control decoupled active-reactive power flow in a smart grid environment as well as to control PEV battery charging and discharging reliably. PV power generation with maximum power tracking is seamlessly integrated with the power flow control to provide additional power generation. Fast dynamic response and good steady-state performance under all power flow modes and various environmental conditions are evaluated and analyzed. From the results obtained, the charger demonstrates less than 1.5% total harmonic distortion as well as low active and reactive power ripple of less than 7% and 8% respectively on the grid for all power flow modes. The PEV battery also experiences a low charging and discharging current ripple of less than 2.5%. 19Growth arrest-specific gene 2 suppresses hepatocarcinogenesis by intervention of cell cycle and p53-dependent apoptosis显示文摘BACKGROUND Growth arrest-specific gene 2(GAS2)plays a role in modulating in reversible growth arrest cell cycle,apoptosis,and cell survival.GAS2 protein is universally expressed in most normal tissues,particularly in the liver,but is depleted in some tumor tissues.However,the functional mechanisms of GAS2 in hepatocellular carcinoma(HCC)are not fully defined.AIM To investigate the function and mechanism of GAS2 in HCC.METHODS GAS2 expression in clinic liver and HCC specimens was analyzed by real-time PCR and western blotting.Cell proliferation was analyzed by counting,MTS,and colony formation assays.Cell cycle analysis was performed by flow cytometry.Cell apoptosis was investigated by Annexin V apoptosis assay and western blotting.RESULTS GAS2 protein expression was lower in HCC than in normal tissues.Overexpression of GAS2 inhibited the proliferation of HCC cells with wide-type p53,while knockdown of GAS2 promoted the proliferation of hepatocytes(P<0.05).Furthermore,GAS2 overexpression impeded the G1-to-S cell cycle transition and arrested more G1 cells,particularly the elevation of sub G1(P<0.01).Apoptosis induced by GAS2 was dependent on p53,which was increased by etoposide addition.The expression of p53 and apoptosis markers was further enhanced when GAS2 was upregulated,but became diminished upon downregulation of GAS2.In the clinic specimen,GAS2 was downregulated in more than 60%of HCCs.The average fold changes of GAS2 expression in tumor tissues were significantly lower than those in paired non-tumor tissues(P<0.05).CONCLUSION GAS2 plays a vital role in HCC cell proliferation and apoptosis,possibly by regulating the cell cycle and p53-dependent apoptosis pathway.Ran-Xu Zhu Alfred Sze Lok Cheng Henry Lik Yuen Chan Dong-Ye Yang Wai-Kay Seto 2019World Journal of Gastroenterology2019,25,32:2
20Total Ginsenoside Extract from Panax ginseng Enhances Neural Stem Cell Proliferation and Neuronal Differentiation by Inactivating GSK-3β显示文摘Objective:To study the effects of total ginsenosides(TG)extract from Panax ginseng on neural stem cell(NSC)proliferation and differentiation and their underlying mechanisms.Methods:The migration of NSCs after treatment with various concentrations of TG extract(50,100,or 200μg/mL)were monitored.The proliferation of NSCs was examined by a combination of cell counting kit-8 and neurosphere assays.NSC differentiation mediated by TG extract was evaluated by Western blotting and immunofluorescence staining to monitor the expression of nestin and microtubule associated protein 2(MAP2).The GSK-3β/β-catenin pathway in TG-treated NSCs was examined by Western blot assay.The NSCs with constitutively active GSK-3βmutant were made by adenovirus-mediated gene transfection,then the proliferation and differentiation of NSCs mediated by TG were further verified.Results:TG treatment significantly enhanced NSC migration(P<0.01 or P<0.05)and increased the proliferation of NSCs(P<0.01 or P<0.05).TG mediation also significantly upregulated MAP2 expression but downregulated nestin expression(P<0.01 or P<0.05).TG extract also significantly induced GSK-3βphosphorylation at Ser9,leading to GSK-3βinactivation and,consequently,the activation of the GSK-3β/β-catenin pathway(P<0.01 or P<0.05).In addition,constitutive activation of GSK-3βin NSCs by the transfection of GSK-3βS9 A mutant was found to significantly suppress TG-mediated NSC proliferation and differentiation(P<0.01 or P<0.05).Conclusion:TG promoted NSC proliferation and neuronal differentiation by inactivating GSK-3β.LIN Kai-li ZHANG Ji CHUNG Hau-lam WU Xin-yi LIU Bin ZHAO Bo-xin SZE Stephen Cho-wing ZHOU Ping-zheng YUNG Ken Kin-lam ZHANG Shi-qing 2022Chinese Journal of Integrative Medicine2022,28,3:2
