5Feasibility and efficacy of delayed pharmacoinvasive therapy for STelevation myocardial infarction显示文摘BACKGROUND ST-elevation myocardial infarction(STEMI)refers to a clinical syndrome that features symptoms of myocardial ischemia with consequent ST-elevation on electrocardiography and an associated rise in cardiac biomarkers.Rapid restoration of brisk flow in the coronary vasculature is critical in reducing mortality and morbidity.In patients with STEMI who could not receive primary percutaneous coronary intervention(PCI)on time,pharmacoinvasive strategy(thrombolysis followed by timely PCI within 3-24 h of its initiation)is an effective option.AIM To analyze the role of delayed pharmacoinvasive strategy in the window period of 24-72 h after thrombolysis.METHODS This was a physician-initiated,single-center prospective registry between January 2017 and July 2017 which enrolled 337 acute STEMI patients with partially occluded coronary arteries.Patients received routine pharmacoinvasive therapy(PCI within 3-24 h of thrombolysis)in one group and delayed pharmacoinvasive therapy(PCI within 24-72 h of thrombolysis)in another group.The primary endpoint was major adverse cardiac and cerebrovascular events(MACCE)within 30 d of the procedure.The secondary endpoints included major bleeding as defined by Bleeding Academic Research Consortium classification,angina,and dyspnea within 30 d.RESULTS The mean age in the two groups was comparable(55.1±10.1 years vs 54.2±10.5 years,P=0.426).Diabetes was present among 20.2%and 22.1%of patients in the routine and delayed groups,respectively.Smoking rate was 54.6%and 55.8%in the routine and delayed groups,respectively.Thrombolysis was initiated within 6 h of onset of symptoms in both groups(P=0.125).The mean time from thrombolysis to PCI in the routine and delayed groups was 16.9±5.3 h and 44.1±14.7 h,respectively.No significant difference was found for the occurrence of measured clinical outcomes in the two groups within 30 d(8.7%vs 12.9%,P=0.152).Univariate analysis of demographic characteristics and risk factors for patients who reported MACCE in the two groups did not demonstrate any significant correlation.Secondary endpoints such as angina,dyspnea,and major bleeding were non-significantly different between the two groups.CONCLUSION Delayed PCI pharmacoinvasive strategy in a critical diseased but not completely occluded artery beyond 24 h in patients who have been timely thrombolyzed seems a reasonable strategy.Rishi Sethi Lalit Mohan Pravesh Vishwakarma Abhishek Singh Swati Sharma Monika Bhandari Ayush Shukla Akhil Sharma Gaurav Chaudhary Akshyaya Pradhan Sharad Chandra Varun Shankar Narain Sudhanshu Kumar Dwivedi 2023World Journal of Cardiology2023,15,1:1

9Study of coronary sinus anatomy during levophase of coronary angiography显示文摘BACKGROUND Coronary sinus(CS)imaging has recently gained importance due to increasing need for mapping and ablation of electrophysiological arrhythmias and left ventricular(LV)pacing during cardiac resynchronization therapy(CRT).Retrograde venogram is the current standard for imaging CS and its tributaries.AIM To evaluate CS anatomy during levophase of routine coronary angiography to aid LV lead implantation during CRT.METHODS In this prospective observational study,164 patients undergoing routine coronary angiography for various indications(Chronic stable angina-44.5%,acute coronary syndrome-39.5%,Dilated cardiomyopathy-11%,atypical chest pain-5%)were included.Venous phase(levophase)of left coronary injection was recorded in left anterior oblique-cranial and right anterior oblique-cranial views.Visibility of coronary veins,width and shape of CS ostium,angulations of proximal CS with body of CS were noted.Presence,size,take-off angle and tortuosity of posterolateral vein(PLV),anterior interventricular veins(AIV)and middle cardiac vein(MCV)were also noted.RESULTS During levophase,visibility grade(Muhlenbruch grade)for coronary veins was 3 in 74%and 2 in 26%of cases.Visibility of CS did not correlate with body mass index.The diameter of CS ostium was<10 mm,10-15 mm and>15 mm in 48%,42%and 10%of patients respectively.Proximal CS was tubular in 136(83%)patients and funnel-shaped in 28(17%)patients.Sharp take-off angulation between ostium and body of CS was seen in 16(10%)patients.Two or more PLV were present in 8 patients while PLV was absent in 52(32%)patients.Angle of take-off of PLV with body of CS was favourable(0°-45°)in 65(40%)patients.The angle was 45°-90°in 36 patients and difficult take-off angle(>90°)was seen in 8 patients.Length of PLV reached distal third of myocardium in 84 cases and middle third in 11 cases.There was no tortuosity in 79 cases,a single bend in 29 cases and more than 2 bends in 4 cases.Thirty nine(24%)patients had other veins supplying posterior/Lateral wall of LV.There was a single vein supplying lateral/posterior wall in 31(19%)patients.Diameter of MCV and AIV was significantly larger in patients with absent PLV as compared to patients with a PLV.CONCLUSION Levophase study of left coronary injection is effective in visualization of the CS in almost all patients undergoing coronary angiography and may be an effective alternative to retrograde venogram in patients with LV dysfunction or LBBB.Akshyaya Pradhan Vrishank Bajaj Pravesh Vishwakarma Monika Bhandari Akhil Sharma Gaurav Chaudhary Sharad Chandra Rishi Sethi Varun Shankar Narain Sudhanshu Dwivedi 2022World Journal of Cardiology2022,14,6:1

13Co-relation of SARS-CoV-2 related 30-d mortality with HRCT score and RT-PCR Ct value-based viral load in patients with solid malignancy显示文摘BACKGROUND Coronavirus disease 2019(COVID-19)patients with malignancy are published worldwide but are lacking in data from India.AIM To characterize COVID-19 related mortality outcomes within 30 d of diagnosis with HRCT score and RT-PCR Ct value-based viral load in various solid malignancies.METHODS Patients included in this study were with an active or previous malignancy and with confirmed severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection from the institute database.We collected data on demographic details,baseline clinical conditions,medications,cancer diagnosis,treatment and the COVID-19 disease course.The primary endpoint was the association between the mortality outcome and the potential prognostic variables,specially,HRCT score,RT-PCR Ct value-based viral load,etc.using logistic regression analyses treatment received in 30 d.RESULTS Out of 131 patients,123 met inclusion criteria for our analysis.The median age was 57 years(interquartile range=19-82)while 7(5.7%)were aged 75 years or older.The most prevalent malignancies were of GUT origin 49(39.8%),hepatopancreatobiliary(HPB)40(32.5%).109(88.6%)patients were on active anticancer treatment,115(93.5%)had active(measurable)cancer.At analysis on May 20,2021,26(21.1%)patients had died.In logistic regression analysis,independent factors associated with an increased 30-d mortality were in patients with the symptomatic presentation.Chemotherapy in the last 4 wk,number of comorbidities(≥2 vs none:3.43,1.08-8.56).The univariate analysis showed that the risk of death was significantly associated with the HRCT score:for moderate(8-15)[odds ratio(OR):3.44;95%confidence interval(CI):1.3-9.12;P=0.0132],severe(>15)(OR:7.44;95%CI:1.58-35.1;P=0.0112).CONCLUSION To the best of our knowledge,this is the first study from India reporting the association of HRCT score and RT-PCR Ct value-based 30-d mortality outcomes in SARS-CoV-2 infected cancer patients.Satya Narayan Vineet Talwar Varun Goel Krushna Chaudhary Anurag Sharma Pallavi Redhu Satyajeet Soni Arpit Jain 2022World Journal of Clinical Oncology2022,13,5:0
14Newly identified genetic variant rs2294693 in UNC5CL gene is associated with decreased risk of esophageal carcinoma in the J&K Population–India显示文摘Esophageal cancer is the second most common type of cancer after lung carcinoma in the state of Jammu and Kashmir(J&K).The understanding of genetics in Esophageal cancer development is poor in the state.Genome wide association studies(GWAS)has proved to be unsurpassed tool in identification of new loci associated with different cancers.GWAS in Chinese population has identified SNP rs2294693 present in UNC5CL(UNC-5 Family C-Terminal like)to be associated with non-cardia gastric cancer.We performed a case control association study and genotyped the SNP rs2294693 using Taqman allele discrimination assay in 566 individuals(166 esophageal cancer patients and 400 controls)belonging to the J&K population.A statistically significant protective association with allelic odds ratio of 0.73(0.56–0.94 at 95%CI)and p value=0.016 was observed.This is the first study in relation to esophageal cancer in the Jammu and Kashmir population,so far it has been studied in association with gastric carcinoma in the Chinese population only.The results indicate that the polymorphism rs2294693 is associated with esophageal cancer susceptibility and the mutant(T)allele might be a protective factor for esophageal cancer among Jammu and Kashmir population.Further the functional characterization of the variation is also warranted.RUCHI SHAH SONALI VERMA AMRITA BHAT GH RASOOL BHAT VARUN SHARMA INDU SHARMA HEMENDER SINGH SANDEEP KAUL EKTA RAI SWARKAR SHARMA 2021BIOCELL2021,45,3:0
15Using focused pharmacovigilance for ensuring patient safety against antileishmanial drugs in Bangladesh’s National Kala-azar Elimination Programme显示文摘Background:Adverse effects of antileishmanial drugs can affect patients’quality of life and adherence to therapy for visceral leishmaniasis(VL)and post-kala-azar dermal leishmaniasis(PKDL).In Bangladesh,there are 26 treatment centers that manage leishmaniasis cases coming from 100 endemic upazilas(subdistricts)of 26 districts(these include VL,PKDL,treatment failure,and relapse VL and cutaneous leishmaniasis cases).This study aimed to investigate the feasibility of using focused pharmacovigilance for VL(VLPV)in Bangladesh’s National Kala-azar Elimination Programme for the early detection and prevention of expected and unexpected adverse drug reactions(ADRs).Methods:This activity has been going on since December 2014.Activity area includes secondary public hospital or Upazila health complex(UHC)in hundred sub districts and Surya Kanta Kala-azar Research Center(SKKRC)in Mymensingh District,a specialized center for management of complicated VL and PKDL cases.Communicable Disease Control(CDC)of the Directorate General of Health Services(DGHS)assigned twenty five of hundred UHCs and SKKRC(total 26)as treatment centers depending on their suitable geographical location.This was implemented for better management of VL cases with Liposomal Amphotericin B(AmBisome®)to ensure patient convenience and proper utilization of this expensive donated drug.A VLPV expert committee and a UHC VLPV team were established,an operational manual and pharmacovigilance report forms were developed,training and refresher training of health personnel took place at UHCs and at the central level,collected information such as patient data including demographics,treatment history and response,adverse events were analyzed.This report includes information for the period from December 2014 to December 2016.Results:From December 2014 to December 2016,1327 leishmaniasis patients were treated and 1066(80%)were available for VLPV.Out of these,57,33,9,and 1%were new VL,PKDL,VL relapse,and other cases,respectively.Liposomal amphotericin B was mostly used(82%)for case management,followed by miltefosine(20%)and paromomycin(3%).Out of the 1066 patients,26%experienced ADRs.The most frequent ADR was fever(17%,176/1066),followed by vomiting(5%,51/1066).Thirteen serious adverse events(SAEs)(eight deaths and five unexpected SAEs)were observed.The expert committee assessed that three of the deaths and all unexpected SAEs were possibly related to treatment.Out of the five unexpected SAEs,four were miltefosine-induced ophthalmic complications and the other was an AmBisome^(■)-induced avascular necrosis of the nasal alae.The Directorate General of the Drug Administration entered the ADRs into the World Health Organization Uppsala Monitoring Centre(WHO-UMC)VigiFlow database.Conclusions:This study found that VLPV through NKEP is feasible and should be continued as a routine activity into the public health system of Bangladesh to ensure patient safety against anti-leishmanial drugs.Md.Sakhawat Hossain Amresh Kumar A.F.M Akhtar Hossain Md.Mahshin Abhijit Sharma Md.Akter Hossain Varun Sharma Rashidul Haque A.K.M Shamsuzzaman Shomik Maruf Prakash Ghosh Vivek Ahuja Dinesh Mondal 2018Infectious Diseases of Poverty2018,7,1:0
16Evaluation of fundus autofluorescence patterns in age-related macular degeneration显示文摘AIM: To study the various morphological patterns of fundus autofluorescence(FAF) images in patients with age-related macular degeneration(AMD) in Indian population.· METHODS: Totally 179 eyes of 104 patients with clinical diagnosis of AMD were recruited into the study.Autofluorescence images were captured using confocal scanning laser ophthalmoscope and the patterns of FAF were classified.·RESULTS: Of 179 eyes, 27(15.08%) were early AMD,58(32.41%) were intermediate AMD, 94 eyes(52.51%)were late AMD. Of 94 eyes with late AMD, 79(84.04%)were neovascular AMD and 15(15.96%) were central geographic atrophy. In eyes with early and intermediate AMD, 9 patterns of FAF were noted. Six patterns(normal,minimal change, focal increased, patchy increased,linear, reticular) were similar to that in the published classification. Two patterns(lacelike and speckled)described in the published classification were not found.Three new patterns(focal hypo-fluorescence, patchy hypo-fluorescence, mixed focal hypo-fluorescence and hyper-fluorescence) were detected. In eyes with neovascular AMD, 6 morphological patterns of FAF were noted. Two patterns(mixed hypo-fluorescence and hyper-fluorescence, central hypo-fluorescence with hyper-fluorescent rim) were similar to that in published classification. Two patterns(normal, near normal or normal background fluorescence in the centre of hypo-fluorescent area) described in the published classification were not found. Four new patterns(minimal change, hypo-fluorescent patch, central hypo-fluorescence with surrounding reticular, bull's eye) were recognized.
