| 3 | Combined MELD and blood lipid level in evaluating the prognosis of decompensated cirrhosis显示文摘AIM: To evaluate the prognostic value of the combined model for end-stage liver disease (MELD) and blood lipid level in patients with decompensated cirrhosis. METHODS: A total of 198 patients with decompensated cirrhosis were enrolled into the study. The values of triglyceride (TG), cholesterol (TC), high density lipoproteins (HDL) and low density lipoprotein (LDL) of each patient on the fi rst day of admission were retrieved from the medical records, and MELD was calculated. All the patients were followed up for 1 year. The relationship between the change of blood lipid level and the value of MELD score was studied by analysis of variance. The prognostic factors were screened by multivariate Cox proportional hazard model. Draw Kaplan-Meier survival curves were drawn. RESULTS: Forty-f ive patients died within 3 mo and 83 patients died within 1 year. The levels of TG, TC, HDL and LDL of the death group were all lower than those of the survivors. The serum TG, TC, HDL and LDL levels were lowered with the increase of the MELD score. Multivariate Cox proportional hazard model showed that MELD ≥18 and TC ≤2.8 mmol/L were independent risk factors for prognosis of decompensated cirrhosis. Survival analysis showed that MELD ≥18 combined with TC ≤ 2.8 mmol/L can clearly discriminate between the patients who would survive and die in 1 year. CONCLUSION: MELD ≥18 and TC ≤2.8 mmol/L are two important indexes to predict the prognosis of patients with decompensated cirrhosis. | 6 | Chronology and Crust-Mantle Mixing of Ore-forming Porphyry of the Bangongco: Evidence from Zircon U-Pb Age and Hf Isotopes of the Naruo Porphyry Copper-Gold Deposit显示文摘The Naruo porphyry copper-gold deposit(hereinafter referred to as the Naruo deposit) in Tibet is a potentially ultra-large, typical gold-rich porphyry copper deposit, which was recently discovered in the Bangongco-Nujiang metallogenic belt. This study analyzed U-Pb chronology and Hf isotopes of the ore-bearing granodiorite porphyry in the Naruo deposit using the LA-ICPMS dating technique. The results show that the weighted average age is 124.03±0.94Ma(MSWD=1.7, n=20), and 206Pb/238 U isochron age is 126.2±2.7 Ma(MSWD=1.02, n=20), both of which are within the error. The weighted average age represents the crystallization age of the granodiorite porphyry, which indicates that the ore-bearing porphyry in the Naruo deposit area was formed in the Early Cretaceous and further implies that the Neo-tethys Ocean had not been closed before 124 Ma under a typical island-arc subduction environment. The εHf(t) of zircons from the granodiorite porphyry varies from 2.14 to 9.07, with an average of 5.18, and all zircons have εHf(t) values greater than 0; 176Hf/177 Hf ratio is relatively high(0.282725–0.282986). Combined with the zircon age―Hf isotope correlation diagram, the aforementioned data indicate that the source reservoir might be a region that is mixed with depleted mantle and ancient crust, which possibly contains more materials sourced from depleted mantle. Rock-forming ages and ore-forming ages of the Duolong ore concentrate area are 120–124 Ma and 118–119 Ma, respectively, which indicate 124–118 Ma represents the main rockforming and ore-forming stage within the area. The Naruo deposit is located in the north of the Bangongco-Nujiang suture, and it yielded a zircon LA-ICPMS age of 124.03 Ma. This indicates the Bangongco-Nujiang oceanic basin subducted towards the north at about 124 Ma, and the Neo-tethys Ocean had not been closed before the middle Early Cretaceous. | 8 | Temporal echocardiography findings in patients with fulminant myocarditis: beyond ejection fraction decline显示文摘The features of myocardial strains from speckle-tracking echocardiography (STE) have not been well defined in fulminant myocarditis (FM) patients.In this study,changes in the left ventricular ejection fraction (LVEF) and global and layer-specific myocardial strains over time were monitored.We aimed to determine the echocardiographic patterns of FM and ascertain their significance in FM treatment.Twenty patients who were clinically diagnosed with FM and received mechanical life support were prospectively enrolled.Conventional echocardiographic measurements were obtained,and serial strain echocardiography was performed from admission to hospital discharge until LVEF recovery (> 50%).Global/regional peak systolic longitudinal strains (GLS/RLS) and layer-specific longitudinal strains were quantified,and their changes with time were monitored in 14 FM patients.All patients had severely impaired cardiac function.Steep improvement in LVEF and GLS were observed within 6 days.Layer-specific strain analysis showed that reduction at admission or recovery at discharge in the endocardium and epicardium strains were equal.In conclusion,FM patients who received mechanical circulatory supports exhibited steep improvement in ventricular function within 6 days.The patchy and diffused distribution pattern of reduced RLS and equally and severely impaired strain in the endocardium and epicardium are valuable features in the diagnosis of FM. | Houjuan Zuo Rui Li Fei Ma Jiangang Jiang Kun Miao Haojie Li Eike Nagel Marijana Tadic Hong Wang Dao Wen Wang | 2020 | Frontiers of Medicine2020,14,3: | 10 |

| 12 | Primary site and regional lymph nodeinvolvement are independent prognosticfactors for early?stage extranodal nasal?typenatural killer/T cell lymphoma显示文摘Background:Nasal-type extranodal natural killer/T-cell lymphoma(ENKTCL) originates primarily in the nasal cavity or extra-nasal sites within the upper aerodigestive tract.However,it is unclear whether the primary site can serve as an independent prognostic factor or whether the varying clinical outcomes observed with different primary sites can be attributed merely to their propensities of regional lymph node involvement.The aim of this study was to investigate the prognostic implications of the primary site and regional lymph node involvement in patients with early-stage nasal-type ENKTCL.Methods:To develop a nomogram,we reviewed the clinical data of 215 consecutively diagnosed patients with early-stage nasal-type ENKTCL who were treated in Sun Yat-sen University Cancer Center with chemotherapy and radiotherapy between 2000 and 2011.The predictive accuracy and discriminative ability of the nomogram were determined using a concordance index(C-index) and calibration curve.Results:The 5-year overall survival(OS) and progression-free survival(PFS) rates of patients with nasal ENKTCL were higher than those of patients with extra-nasal ENKTCL(OS:68.2%vs.46.0%,P = 0.030;PFS:53.4%vs.26.6%,P = 0.010).The 5-year OS and PFS rates of patients with Ann Arbor stage IE ENKTCL were higher than those of patients with Ann Arbor stage HE ENKTCL(OS:66.3%vs.59.2%,P = 0.003;PFS:51.4%vs.40.3%,P = 0.009).Multivariate analysis showed that age >60 years,ECOG performance status score >2,elevated lactate dehydrogenase(LDH) level,extranasal primary site,and regional lymph node involvement were significantly associated with lower 5-year OS rate;age >60 years,elevated LDH level,extra-nasal primary site,and regional lymph node involvement were significantly associated with lower 5-year PFS rate.The nomogram included the primary site and regional lymph node involvement based on multivariate analysis.The calibration curve showed good agreement between the predicted and actual 5-year OS and PFS rates,and the C-indexes of the nomogram for the OS and PFS rates were 0.697 and 0.634,respectively.Conclusions:The primary site and regional lymph node involvement are independent prognostic factors for earlystage ENKTCL treated with chemotherapy followed by definitive radiotherapy. | Shao‑Qing Niu Yong Yang Yi‑Yang Li Ge Wen Liang Wang Zhi‑Ming Li Han‑Yu Wang Lu‑Lu Zhang Yun‑Fei Xia Yu‑Jing Zhang | 2016 | Chinese Journal of Cancer2016,35,5: | 8 |

| 15 | Association between Plasma Metal Levels and Diabetes Risk: a Case-control Study in China显示文摘Objective Many metals, some of which have been classified as environmental endocrine disruptors, are used extensively in everyday consumer products and are ubiquitous in our living environment. In the present study, we aimed to explore the associations between the prevalence risk of type 2 diabetes and plasma levels of 20 trace elements as well as those of heavy metals in a Han Chinese population. Methods We conducted a case-control study to investigate the associations between plasma concentrations of 20 metals and diabetes in Jiangsu province. A total of 122 newly diagnosed cases of type 2 diabetes and 429 matched controls were recruited from community physical examinations in Suzhou City of Jiangsu Province. Plasma metal levels were measured by inductively-coupled plasma mass spectrometry. Results After adjusting for confounders, plasma vanadium, chromium, manganese, copper, zinc, arsenic, selenium, strontium, palladium, cadmium, cesium, and barium were associated with diabetes risk(P < 0.05). The adjusted OR increased with increasing concentration of vanadium, manganese, copper, zinc, and cesium. Conclusion Many metals, including manganese, copper, zinc, arsenic, selenium, and cadmium in plasma, are associated with the morbidity of diabetes. However, this work is challenged by the pixel-based image classification method for its comparatively low accuracy. In this paper, an object-based image analysis(OBIA)method was presented to map intra-reef geomorphology of coral reefs in the Xisha Islands, China using Landsat 8satellite imagery. Following the work of the Millennium Coral Reef Mapping Project, a regional reef class hierarchy with ten geomorphic classes was first defined. Then, incorporating the hierarchical concept and integrating the spectral and additional spatial information such as context, shape and contextual relationships, a large-scale geomorphic map was produced by OBIA with accuracies generally more than 80%. | 18 | Factors associated with the incidence of ectopic pregnancy in women undergoing assisted reproductive treatment显示文摘Background:Ectopic pregnancy(EP)is a common complication in women undergoing assisted reproductive treatment,but the underlying causes for this remain unclear.This study aimed to explore factors affecting the incidence of EP in in vitro fertilization(IVF)/intracytoplasmic sperm injection(ICSI).Methods:This was a retrospective study on the incidence of EP in IVF/ICSI cycles between January 1,2013 and December 31,2017.Patient age,infertility diagnosis(tubal factor or not),primary or secondary infertility,type of cycle(frozen-thawed or fresh),type of embryo(s)transferred(cleavage embryo or blastocyst),number of embryos transferred(one,two,or three),previous history of EP,and endometrial combined thickness were analyzed to explore their relationships with the incidence of EP.Based on clinical typing results,the patients were divided into an EP group or a non-EP group.Categorical variables were analyzed using Chi-squared test or Fisher exact test.Logistic regression analysis was performed to explore their associations with the incidence of EP.Results:The percentage of patients with primary infertility in EP group was significantly lower than that in non-EP group(31.3%vs.46.7%,χ^2=26.032,P<0.001).The percentage of patients with tubal infertility in EP group was also significantly higher than that in non-EP group(89.2%vs.63.6%,χ^2=77.410,P<0.001).The percentages of patients with transfer of cleavage-stage embryo or blastocyst(91.4%vs.84.4%,χ^2=10.132,P=0.001)and different endometrial combined thickness(ECT)(χ^2=18.373,P<0.001)differed significantly between EP and non-EP groups.For patients who had a previous history of one to four EPs,the percentage of patients undergoing transfer of a cleavage-stage embryo was significantly higher in EP group than that in non-EP group(92.2%vs.77.6%,χ^2=13.737,P<0.001).In multivariate logistic regression analysis,tubal infertility was strongly associated with EP(adjusted odds ratio:3.995,95%confidence interval:2.706-5.897,P<0.001).Conclusions:In IVF/ICSI cycles,transfer of a blastocyst-stage embryo,especially for patients with a previous history of EP,reduced the rate of EP.Tubal infertility was strongly associated with EP. | Xiao-Ying Jin Chao Li Wen Xu Liu Liu Min-Ling Wei Hai-Yi Fei Jing Li Feng Zhou Song-Ying Zhang | 2020 | Chinese Medical Journal2020,,17: | 7 |
