| 2 | Fatigue and fracture behavior of nickel-based superalloy Inconel 718 up to the very high cycle regime显示文摘The fatigue and fracture behavior of nickel-based superalloy Inconel 718 was investigated up to the very high cycle regime under rotary bending tests at room temperature. It was found that this superalloy can still fracture after exceeding 107 cycles. Fractographic analysis revealed that there was a transition from fatigue crack initiation at multi-sites to single initiation with decreasing stress levels. The fracture surface can be divided into four areas according to the appearance, associated with fracture mechanics analysis of the corresponding stress intensity factors. The fracture mechanism dominant in each area was disclosed by scanning electron microscope examination and analyzed in comparison with those obtained from the crack growth tests. Subsequently, life prediction modeling was proposed by estimating the crack initiation and propagation stage respectively. | 6 | Post-polypectomy bleeding and thromboembolism risks associated with warfarin vs direct oral anticoagulants显示文摘AIM To verify the validity of the endoscopy guidelines for patients taking warfarin or direct oral anticoagulants(DOAC).METHODS We collected data from 218 patients receiving oral anticoagulants(73 DOAC users, 145 warfarin users) and 218 patients not receiving any antithrombotics(age-and sexmatched controls) who underwent polypectomy.(1) We evaluated post-polypectomy bleeding(PPB) risk in patients receiving warfarin or DOAC compared with controls;(2) we assessed the risks of PPB and thromboembolism between three AC management methods: Discontinuing AC with heparin bridge(HPB)(endoscopy guideline recommendation), continuing AC, and discontinuing AC without HPB.RESULTS PPB rate was significantly higher in warfarin users and DOAC users compared with controls(13.7% and 13.7% vs 0.9%, P < 0.001), but was not significantly different between rivaroxaban(13.2%), dabigatran(11.1%), and apixaban(13.3%) users. Two thromboembolic events occurred in warfarin users, but none in DOAC users. Compared with the continuing anticoagulant group, the discontinuing anticoagulant with HPB group(guideline recommendation) had a higher PPB rate(10.8% vs 19.6%, P = 0.087). These findings were significantly evident in warfarin but not DOAC users. One thrombotic event occurred in the discontinuing anticoagulant with HPB group and the discontinuing anticoagulant without HPB group; none occurred in the continuing anticoagulant group.CONCLUSION PPB risk was similar between patients taking warfarin and DOAC. Thromboembolism was observed in warfarin users only. | 8 | Epidemiology,clinical characteristics,and management of chronic kidney disease in human immunodeficiency virusinfected patients显示文摘Antiretroviral therapy has markedly reduced acquired immune deficiency syndrome-related deaths and opportunistic infectious diseases.This has resulted in prolonged survival of individuals infected with the human immunodeficiency virus(HIV).However,this improvement in survival has been accompanied by an increase in the incidence of chronic kidney disease(CKD) and end-stage renal disease.CKD is now epidemic among HIV-infected populations in both Western and Eastern countries.Risk factors associated with CKD in HIV-infected populations include aging,hypertension,diabetes mellitus,co-infection with hepatitis C virus,a low CD4 cell count,and a high HIV viral load.Clinical experience has shown that HIVinfected individuals often have one or more concurrent risk factors for CKD.The cumulative effect of multiple risk factors on the development of CKD should be noted in this population.Glomerular disease directly related to HIV infection,so-called HIV-associated nephropathy,remains an important cause of CKD among a limited HIV population of African descent,but is less likely to be common among other urban HIV populations.The impact of exposure to nephrotoxic antiretroviral agents on the development of kidney disease is both an old and a new concern.In particular,the association of tenofovir with kidney tubular injury has been an area of great interest.The findings regarding tenofovir's adverse effect on long-term kidney function vary among studies.The early identification and treatment of CKD is recommended for reducing the burden of patients requiring dialysis in HIV-infected populations.Periodic monitoring of urinary concentrations of albumin,protein,and tubular injury markers such as low-molecular-weight proteins may be useful for the early diagnosis of patients at risk for incident CKD.This review focuses on recent epidemiology,clinical characteristics,and management of CKD in a contemporary HIV-infected population. | Minoru Ando Naoki Yanagisawa | 2015 | World Journal of Nephrology2015,4,3: | 5 |

| 11 | Efficient hemostatic method for endoscopic submucosal dissection of colorectal tumors显示文摘AIM:To evaluate a new hemostatic method using hemostatic forceps to prevent perforation and perioperative hemorrhage during colonic endoscopic submucosal dissection(ESD).METHODS:We studied 250 cases,in which ESD for colorectal tumors was performed at the Kyoto Prefectural University of Medicine or Nara City Hospital between 2005 and 2010.We developed a new hemostatic method using hemostatic forceps in December 2008 for the efficient treatment of submucosal thick vessels.ESD was performed on 126 cases after adoption of the new method(the adopted group)and the new method was performed on 102 of these cases.ESD was performed on 124 cases before the adoption of the new method (the unadopted group).The details of the new method are as follows:firstly,a vessel was coagulated using the hemostatic forceps in the soft coagulation mode according to the standard procedure,and the coagulated vessel was removed using the forceps in the'endocut' mode without perioperative hemorrhage.Secondly,the partial surrounding submucosa was dissected using the forceps in the endocut mode.In the current study,we evaluated the efficacy of this method.RESULTS:Coagulated vessels were successfully removed using the hemostatic forceps in all 102 cases without severe perioperative hemorrhage.Moderate perioperative hemorrhage occurred in five cases(4.9%);however,it was stopped by immediately reuse of the hemostatic forceps.The partial surrounding submucosa was dissected using the forceps in all 102 cases.In the adopted group,the median operation time was 105 min.The proportion of endoscopic en bloc resection was 92.8%(P<0.01)compared to 80.6%in the unadopted group.The postoperative hemorrhage and perforation rates were 2.3%and 2.3%.The rate of perforation was significantly lower than that in the unadopted group (9.6%,P<0.01).We evaluated the ease of use of this method by allowing our three trainees to performed ESD on 46 cases,which were accomplished without any severe hemorrhage.CONCLUSION:The new method effectively treated submucosal thick vessels and shows promise for the prevention of perforation and perioperative hemorrhage in colonic ESD. | Naohisa Yoshida Yuji Naito Munehiro Kugai Ken Inoue Naoki Wakabayashi Nobuaki Yagi Akio Yanagisawa Toshikazu Yoshikawa | 2010 | World Journal of Gastroenterology2010,16,33: | 4 |

| 13 | Image-enhanced endoscopy for diagnosis of colorectal tumors in view of endoscopic treatment显示文摘Recently,image-enhanced endoscopy(IEE) has been used to diagnose gastrointestinal tumors.This method is a change from conventional white-light(WL) endoscopy without dyeing solution,requiring only the push of a button.In IEE,there are many advantages in diagnosis of neoplastic tumors,evaluation of invasion depth for cancerous lesions,and detection of neoplastic lesions.In narrow band imaging(NBI) systems(Olympus Medical Co.,Tokyo,Japan),optical filters that allow narrow-band light to pass at wavelengths of 415 and 540 nm are used.Mucosal surface blood vessels are seen most clearly at 415 nm,which is the wavelength that corresponds to the hemoglobin absorption band,while vessels in the deep layer of the mucosa can be detected at 540 nm.Thus,NBI also can detect pit-like structures named surface pattern.The flexible spectral imaging color enhancement(FICE) system(Fujifilm Medical Co.,Tokyo,Japan) is also an IEE but different to NBI.FICE depends on the use of spectral-estimation technology to reconstruct images at different wavelengths based on WL images.FICE can enhance vascular and surface patterns.The autofluorescence imaging(AFI) video endoscope system(Olympus Medical Co.,Tokyo,Japan) is a new illumination method that uses the difference in intensity of autofluorescence between the normal area and neoplastic lesions.AFI light comprises a blue light for emitting and a green light for hemoglobin absorption.The aim of this review is to highlight the efficacy of IEE for diagnosis of colorectal tumors for endoscopic treatment. | Naohisa Yoshida Nobuaki Yagi Akio Yanagisawa Yuji Naito | 2012 | World Journal of Gastrointestinal Endoscopy2012,4,12: | 3 |
| 14 | Cardiac-specific knockout of ET_(A)receptor mitigates low ambient temperature-induced cardiac hypertrophy and contractile dysfunction显示文摘Cold exposure is associated with oxidative stress and cardiac dysfunction.The endothelin(ET)system,which plays a key role in myocardial homeostasis,may participate in cold exposure-induced cardiovascular dysfunction.This study was designed to examine the role of ET-1 in cold stress-induced cardiac geometric and contractile responses.Wild-type(WT)and ETA receptor knockout(ETAKO)mice were assigned to normal or cold exposure(48C)environment for 2 and 5 weeks prior to evaluation of cardiac geometry,contractile,and intracellular Ca21 properties.Levels of the temperature sensor transient receptor potential vanilloid(TRPV1),mitochondrial proteins for biogenesis and oxidative phosphorylation,including UCP2,HSP90,and PGC1a were evaluated.Cold stress triggered cardiac hypertrophy,depressed myocardial contractile capacity,including fractional shortening,peak shortening,and maximal velocity of shortening/relengthening,reduced intracellular Ca21 release,prolonged intracellular Ca21 decay and relengthening duration,generation of ROS and superoxide,as well as apoptosis,the effects of which were blunted by ETAKO.Western blotting revealed downregulated TRPV1 and PGC1a as well as upregulated UCP2 and activation of GSK3b,GATA4,and CREB in cold-stressed WT mouse hearts,which were obliterated by ETAKO.Levels of HSP90,an essential regulator for thermotolerance,were unchanged.The TRPV1 agonist SA13353 attenuated whereas TRPV1 antagonist capsazepine mimicked cold stress-or ET-1-induced cardiac anomalies.The GSK3b inhibitor SB216763 ablated cold stress-induced cardiac contractile(but not remodeling)changes and ET-1-induced TRPV1 downregulation.These data suggest that ETAKO protects against cold exposure-induced cardiac remodeling and dysfunction mediated through TRPV1 and mitochondrial function. | Yingmei Zhang Linlin Li Yinan Hua Jennifer M.Nunn Feng Dong Masashi Yanagisawa Jun Ren | 2012 | Journal of Molecular Cell Biology2012,4,2: | 2 |
