| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 2 | The intracellular mechanism of alpha-fetoprotein promoting the proliferation of NIH 3T3 cells显示文摘AIM The existence and properties of alpha-fetoprotein (AFP) receptor on the surface of NIH 3T3 cells and the effects of AFP on cellular signal transduction pathway were investigated. METHODS The effect of AFP on the proliferation of NIH 3T3 cells was measured by incorporation of 3H-TdR. Receptor-binding assay of 125I-AFP was performed to detect the properties of AFP receptor in NIH 3T3 cells. The influences of AFP on the [cAMP]i and the activities of protein kinase A (PKA) were determined. Western blot was used to detect the change of K-ras P21 protein expression. RESULTS The proliferation of NIH 3T3 cells treated with 0-80 mg/L of AFP was significantly enhanced. The Scatchard analysis indicated that there were two classes of binding sites with KD of 2.722×10-9M (Bmax=12810 sites per cell) and 8.931× 10-SM (Bmax=l19700 sites per cell) respectively. In the presence of AFP (20 mg/L), the content of cAMP and activities of PKA were significantly elevated . The level of K-ras P21 protein was upregulated by AFP at the concentration of 20 mg/L. The monoclonal antibody against AFP could reverse the effects of AFP on the cAMP content, PKA activity and the expression of K-ras p21 gene. | 3 | Association of Dietary Pattern during Pregnancy and Gestational Diabetes Mellitus: A Prospective Cohort Study in Northern China显示文摘Objective To examine the association of maternal dietary patterns during pregnancy with gestational diabetes mellitus(GDM) in northern China.Methods The dietary intakes of pregnant women were recorded twice by 24‐hour dietary recalls for three days prior to having been diagnosed with GDM, at 5‐15 and 24‐28 gestational weeks, respectively.GDM was diagnosed, and serum glycosylated hemoglobin(HbA1c) was measured at 24‐28 weeks.Dietary patterns were assessed by factor analysis. The association of the dietary pattern with GDM and HbA1c was examined by multiple logistic models.Results Of 753 participants, 64(8.5%) were diagnosed with GDM. Four dietary patterns were identified: Western pattern(dairy, baked/fried food and white meat), traditional pattern(light‐colored vegetables, fine grain, red meat and tubers), mixed pattern(edible fungi, shrimp/shellfish and red meat)and prudent pattern(dark‐colored vegetables and deep‐sea fish). Compared with the prudent pattern,both the Western pattern and the traditional pattern were associated with an increased risk of GDM(a OR = 4.40, 95% CI: 1.58‐12.22; a OR = 4.88, 95% CI: 1.79‐13.32) and a high level of HbA1c(a OR = 12.37,95% CI: 1.47‐103.91; a OR = 26.23, 95% CI: 2.54‐270.74). | 5 | A Systematic Assessment of Blood Lead Level in Children and Associated Risk Factors in China显示文摘In this study, we searched multiple databases for all relevant original articles (1996-2013). To investigate blood lead levels (BLL) and possible risk factors for lead exposure among children in China A total of 388 articles met our inclusion criteria. The overall geometric mean (GM) BLL was 71 IJ.g/L, and the prevalence of elevated BLL (EBLL, defined as BLL 〉_100 Ixg/L) was l&.4&% among children. The prevalence of EBLL remained significantly higher among boys. In children less than 6 years of age, there were significantly increasing trends in both BLL and prevalence of EBLL in an age-dependent manner. The ban on leaded gasoline significantly reduced the BLL as well as EBLL prevalence; however, children whose parents had lower educational levels or were exposed to lead in the workplace had a higher EBLL prevalence. Despite its decline over time, the average BLL among children in China remains higher than the average level most recently reported in the United States. | 12 | Quality control measures for lowering the seroconversion rate of hemodialysis patients with hepatitis B or C virus显示文摘BACKGROUND: Hemodialysis (HD) patients are at high risk of infection by hepatitis B virus (HBV) or hepatitis C virus (HCV). The present study was designed to determine the impact of quality control measures on the prevention of transmission of blood-borne viruses. METHODS: A total of 6182 adult maintenance HD patients from all HD units in Zhejiang Province were recruited on January 1, 2007. The baseline demographic and clinical characteristics were recorded and all patients were followed up until death or survival at 4 years later. The Quality Control Standards of Hemodialysis were gradually implemented in HD units. The HBV or HCV seroconversion rates of the recruited patients were calculated and compared every year during the observation period. RESULTS: The prevalence of HBV was 8.3% at the beginning of the study, and 6.6% for HCV. With the implementation of the HD quality control measures, the HBV seroconversion rate tended to decrease year by year (χ 2 =6.620, P=0.085), and the HCV seroconversion rate decreased significantly (χ 2 =10.41, P=0.015). Compared with the data in 2007, the HBV seroconversion rate (χ 2 =4.204, P=0.040, relative risk ratio 0.393, 95% CI 0.156-0.991) and the HCV seroconversion rate (χ 2 =7.373, P=0.007, relative risk ratio 0.386, 95% CI 0.189-0.787) decreased significantly in 2010. | 13 | LMP2-DC Vaccine Elicits Specific EBV-LMP2 Response to Effectively Improve Immunotherapy in Patients with Nasopharyngeal Cancer显示文摘Objective To evaluate the safety and effectiveness of a vaccine based on latent membrane protein 2(LMP2)modified dendritic cells(DCs)that boosts specific responses of cytotoxic T lymphocytes(CTLs)to LMP2 before and after intradermal injection in patients with nasopharyngeal carcinoma(NPC).Methods DCs were derived from peripheral blood monocytes of patients with NPC.We prepared LMP2-DCs infected by recombinant adenovirus vector expressing LMP2(rAd-LMP2).NPC patients were immunized with 2×105 LMP2-DCs by intradermal injection at week 0 and after the second and fourth weeks.Specific responses to LMP2 were detected by enzyme-linked immunospot(ELISPOT)assay at week 0 and at the fifth and eighth weeks.Local clinicians performed the follow-up and tracking of patients.Results We demonstrated that DCs derived from monocytes displayed typical DC morphologies;the expression of LMP2 in the LMP2-DCs vaccine was confirmed by immunocytochemical assay.Twenty-nine patients with NPC were enrolled in this clinical trial.The LMP2-DCs vaccine was well tolerated in all of the patients.Boosted responses to LMP2 peptide sub-pools were observed in 18 of the 29 patients with NPC.The follow-up data of 29 immunized patients from April,2010 to April 2015 indicated a five-year survival rate of 94.4%in responders and 45.5%in non-responders.Conclusion In this pilot study,we demonstrated that the LMP2-DCs vaccine is safe and effective in patients with NPC.Specific CTLs responses to LMP2 play a certain role in controlling and preventing the recurrence and metastasis of NPC,which warrants further clinical testing. | ZENG Yi SI Yong Feng LAN Gui Ping WANG Zhan ZHOU Ling TANG Min Zhong SJ O`Brien LAN Jiao ZHOU Xiang Yang WANG Yong Li TANG Juan ZHOU Zhi Xiang DU Hai Jun LIN Hui | 2020 | Biomedical and Environmental Sciences2020,33,11: | 2 |
