| 1 | Sphingosine kinase 1 is upregulated with lysophosphatidic acid receptor 2 in human colorectal cancer显示文摘AIM: To examine the expression of Sph K1, an oncogenic kinase that produces sphingosine 1-phosphate(S1P), and its correlation with the expression of LPAR2, a major lysophosphatidic acid(LPA) receptor overexpressed in various cancers, in human colorectal cancer.METHODS: Real-time reverse-transcription polymerase chain reaction was used to measure the m RNA expression of Sph K1, LPAR2, and the three major S1 P receptors in 27 colorectal cancer samples and corresponding normal tissue samples. We also examined the correlation between the expression of Sph K1 and LPAR2.RESULTS: C o l o r e c t a l c a n c e r t i s s u e i n 2 2 o f 2 7 patients had higher levels of Sph K1 m RNA than in normal tissue. In two-thirds of the samples, Sph K1 m RNA expression was more than two-fold higher than in normal tissue. Consistent with previous reports, LPAR2 m RNA expression in 20 of 27 colorectal cancer tissue samples was higher compared to normal tissue samples. Expression profiles of all three major S1 P receptors, S1PR1, S1PR2, and S1PR3, varied without any trend, with no significant difference in expression between cancer and normal tissues. A highly significant positive correlation was found between Sph K1 and LPAR2 expression [Pearson's correlation coefficient(r) = 0.784 and P < 0.01]. | 2 | A unique case of collagenous colitis presenting as protein-losing enteropathy successfully treated with prednisolone显示文摘A 76-year-old woman with a 5-mo history of recurrent diarrhea and generalized edema was admitted to our hospital. Colonoscopy revealed edematous mucosa,and histopathological examination was compatible with collagenous colitis. Protein leakage from the colon,particularly in the ascending portion,was identified on 99mTc-human serum albumin scintigraphy. Collagenous colitis associated with protein-losing enteropathy (PLE) without small bowel disease was diagnosed. Prednisolone treatment ameliorated diarrhea and hypoproteinemia. | 4 | Water-assisted self-sustained burning of metallic single-walled carbon nanotubes for scalable transistor fabrication显示文摘Although aligned arrays of semiconducting single-walled carbon nanotubes(s-SWNTs) are promising for use in next-generation electronics owing to theirultrathin bodies and ideal electrical properties, even a small portion of metallic(m-) counterparts causes excessive leakage in field-effect transistors (FETs).To fully exploit the benefits of s-SWNTs for use in large-scale systems, it isnecessary to completely eliminate m-SWNTs from as-grown SWNT arrays andthereby obtain purely semiconducting large-area arrays, wherein numerousFETs can be flexibly built. In this study, we performed electrical burning ofm-SWNTs assisted by water vapor and polymer coating to eliminate m-SWNTsover a long length for the scalable fabrication of transistors from the remainings-SWNT arrays. During the electrical-breakdown process, the combination ofwater vapor and the polymer coating significantly enhanced the burning ofthe SWNTs, resulting in a self-sustained reaction along the nanotube axis. Wefound that m-SWNT segments partially remaining on the anode side resultedfrom one-way burning from the initial breakdown position, where Joule-heating-induced oxidation first occurred. The s-SWNT-enriched arrays obtained wereused to fabricate multiple FETs with a high on-off current ratio. | 18 | Stage at diagnosis of colorectal cancer through diagnostic route:Who should be screened?显示文摘BACKGROUND Colorectal cancer(CRC)is a global health concern,with advanced-stage diagnoses contributing to poor prognoses.The efficacy of CRC screening has been well-established;nevertheless,a significant proportion of patients remain unscreened,with>70%of cases diagnosed outside screening.Although identifying specific subgroups for whom CRC screening should be particularly recommended is crucial owing to limited resources,the association between the diagnostic routes and identification of these subgroups has been less appreciated.In the Japanese cancer registry,the diagnostic routes for groups discovered outside of screening are primarily categorized into those with comorbidities found during hospital visits and those with CRC-related symptoms.AIM To clarify the stage at CRC diagnosis based on diagnostic routes.METHODS We conducted a retrospective observational study using a cancer registry of patients with CRC between January 2016 and December 2019 at two hospitals.The diagnostic routes were primarily classified into three groups:Cancer screening,follow-up,and symptomatic.The early-stage was defined as Stages 0 or I.Multivariate and univariate logistic regressions were exploited to determine the odds of early-stage diagnosis in the symptomatic and cancer screening groups,referencing the follow-up group.The adjusted covariates were age,sex,and tumor location.RESULTS Of the 2083 patients,715(34.4%),1064(51.1%),and 304(14.6%)belonged to the follow-up,symptomatic,and cancer screening groups,respectively.Among the 2083 patients,CRCs diagnosed at an early stage were 57.3%(410 of 715),23.9%(254 of 1064),and 59.5%(181 of 304)in the follow-up,symptomatic,and cancer screening groups,respectively.The symptomatic group exhibited a lower likelihood of early-stage diagnosis than the follow-up group[P<0.001,adjusted odds ratio(aOR),0.23;95%confidence interval(95%CI):0.19-0.29].The likelihood of diagnosis at an early stage was similar between the follow-up and cancer screening groups(P=0.493,aOR for early-stage diagnosis in the cancer screening group vs follow-up group=1.11;95%CI=0.82-1.49).CONCLUSION CRCs detected during hospital visits for comorbidities were diagnosed earlier,similar to cancer screening.CRC screening should be recommended,particularly for patients without periodical hospital visits for comorbidities. | Nobukazu Agatsuma Takahiro Utsumi Yoshitaka Nishikawa Takahiro Horimatsu Takeshi Seta Yukitaka Yamashita Yukari Tanaka Takahiro Inoue Yuki Nakanishi Takahiro Shimizu Mikako Ohno Akane Fukushima Takeo Nakayama Hiroshi Seno | 2024 | World Journal of Gastroenterology2024,30,10: | 0 |
| 19 | Self-expanding metal stent placement and pathological alterations among obstructive colorectal cancer cases显示文摘BACKGROUND Experimental studies suggest that self-expanding metal stents(SEMSs)enhance the aggressive behavior of obstructive colorectal cancer.The influence of SEMS placement on pathological alterations remains to be elucidated.AIM To determine whether SEMS placement is associated with molecular or pathological features of colorectal carcinoma tissues.METHODS Using a nonbiased molecular pathological epidemiology database of patients with obstructive colorectal cancers,we examined the association of SEMS placement with molecular or pathological features,including tumor size,histological type,American Joint Committee on Cancer(AJCC)-pTNM stage,and mutation statuses in colorectal cancer tissues compared with the use of transanal tubes.A multivariable logistic regression model was used to adjust for potential confounders.RESULTS SEMS placement was significantly associated with venous invasion(P<0.01),but not with the other features examined,including tumor size,disease stage,mutation status,and lymphatic invasion.In both the univariable and multivariable models with adjustment for potential factors including tumor location,histological type,and AJCC-pT stage,SEMS placement was significantly associated with severe venous invasion(P<0.01).For the outcome category of severe venous invasion,the multivariable odds ratio for SEMS placement relative to transanal tube placement was 19.4(95%confidence interval:5.24–96.2).No significant differences of disease-free survival and overall survival were observed between SEMS and transanal tube groups.CONCLUSION SEMS placement might be associated with severe venous invasion in colorectal cancer tissue,providing an impetus for further investigations on the pathological alterations by SEMSs in colorectal cancer development. | Keisuke Kosumi Kosuke Mima Kosuke Kanemitsu Takuya Tajiri Toru Takematsu Yuki Sakamoto Mitsuhiro Inoue Yuji Miyamoto Takao Mizumoto Tatsuo Kubota Nobutomo Miyanari Hideo Baba | 2022 | World Journal of Gastrointestinal Endoscopy2022,14,11: | 0 |
| 20 | Validation of the Japan Coma Scale for the prediction of mortality in children:analysis of a nationwide trauma database显示文摘Objective The Japan Coma Scale(JCS)is widely used in clinical practice to evaluate levels of consciousness in Japan.There have been several studies on the usefulness of JCS in adults.However,its usefulness in evaluating children has not been reported.Therefore,this study aimed to assess the usefulness of the JCS for the prediction of mortality in children.Methods This is a multicenter cohort study which used data from a national trauma registry(Japan Trauma Data Bank).This study included patients under 16 years of age who were treated between 2004 and 2015.The primary outcome measure was in-hospital mortality.Two models were used to examine each item of the Glasgow Coma Scale(GCS)and the JCS.Model A included the discrete levels of each index.In model B,data regarding age,sex,vital signs on arrival to hospital,the Injury Severity Score,and blunt trauma were added to each index.The effectivity of the JCS score was then evaluated using the area under the curve(AUC)for discrimination,a calibration plot,and the Hosmer-Lemeshow test for calibration.Results A total of 9045 patients were identified.The AUCs of the GCS and JCS were 0.929(95%confidence interval(CI)0.904 to 0.954)and 0.930(95%CI 0.906 to 0.954)in model A and 0.975(95%CI 0.963 to 0.987)and 0.974(95%CI 0.963 to 0.985)in model B,respectively.The results of the Hosmer-Lemeshow test were 0.00(p=1.00)and 0.00(p=1.00)in model A and 4.14(p=0.84)and 8.55(p=0.38)in model B for the GCS and JCS,respectively.Conclusions We demonstrated that the JCS is as valid as the GCS for predicting mortality.The findings of this study indicate that the JCS is a useful and relevant tool for pediatric trauma care and future research. | Yuki Enomoto Yusuke Tsutsumi Asuka Tsuchiya Takahiro Kido Koji Ishigami Masahito Togo Susumu Yasuda Yoshiaki Inoue | 2022 | World Journal of Pediatric Surgery2022,5,2: | 0 |
