| 1 | 难治性慢性鼻窦炎的相关因素探讨显示文摘目的 慢性鼻窦炎的难治性在于发病因素的复杂性,本研究拟探讨细菌生物膜、变态反应、主要炎性细胞因子与难治性慢性鼻窦炎的相关性.方法 采用视觉模拟量表评分和鼻内镜检查评分的方法 ,从340例首次接受内镜鼻窦手术和规范药物治疗1年以上的慢性鼻窦炎患者中,选择19例治疗无效者、6例临床治愈者分别作为实验组和对照组,通过电镜扫描、酶联免疫吸附法分别检测两组病例筛窦黏膜表面细菌生物膜的形成和筛窦黏膜组织中特异性IgE、白细胞介素(interleukin,IL)-4、IL-5、IL-6、IL-8的含量,统计分析比较组间差异.数据分析用SPSS13.0统计软件处理.结果 ①筛窦黏膜表而细菌生物膜:实验组有19例除上皮细胞结构破坏、纤毛脱落等病变以外,全部发现细菌生物膜形成,而对照组6例上皮细胞结构完整,纤毛整齐,未发现细菌生物膜形成,组间细菌生物膜检出率差异具有统计学意义(P〈0.01).②特异性IgE:实验组有9例筛窦黏膜组织检测到特异性IgE,而对照组6例均未检测到特异性IgE,组间差异有统计学意义(P〈0.05).③IL:实验组仪有2例标本中检测到IL-8含量升高,实验组和对照组其他病例4种IL含量均在王常参考值以下而无法检出.实验组检测到IL的病例数分别为:IL-4 7例、IL-5 14例、IL-6 15例、IL-8 12例,对照组检测到的病例数分别为:IL-4 1例、IL-5 5例、IL-6 5例、IL-8 5例,组间检出率差异无统计学意义(P值均〉0.05).结论 细菌生物膜、变应性炎性反应是导致难治性慢性鼻窦炎的重要相关因素之一. | Muneif A.S 左可军 史剑波 许庚 | 2010 | 中华耳鼻咽喉头颈外科杂志2010,45,12: | 37 |
| 2 | Ciliary parathyroid hormone signaling activates transforming growth factor-βto maintain intervertebral disc homeostasis during aging显示文摘Degenerative disc disease(DDD) is associated with intervertebral disc degeneration of spinal instability. Here, we report that the cilia of nucleus pulposus(NP) cells mediate mechanotransduction to maintain anabolic activity in the discs. We found that mechanical stress promotes transport of parathyroid hormone 1 receptor(PTH1 R) to the cilia and enhances parathyroid hormone(PTH) signaling in NP cells. PTH induces transcription of integrin α_vβ_6 to activate the transforming growth factor(TGF)-β-connective tissue growth factor(CCN2)-matrix proteins signaling cascade. Intermittent injection of PTH(iPTH) effectively attenuates disc degeneration of aged mice by direct signaling through NP cells, specifically improving intervertebral disc height and volume by increasing levels of TGF-β activity, CCN2, and aggrecan. PTH1 R is expressed in both mouse and human NP cells. Importantly,knockout PTH1 R or cilia in the NP cells results in significant disc degeneration and blunts the effect of PTH on attenuation of aged discs. | 3 | The common MTHFR C677T and A1298C variants are not associated with the risk of non-syndromic cleft lip/palate in northern Venezuela显示文摘Non-syndromic cleft lip with or without cleft palate (nsCL/P) is among the most common major birth defects, with complex inheritance involving multiple genes and environmental factors. Numerous studies of MTHFR, encoding methylenetetrahydrofolate reductase, which catalyzes the rate-limiting step of folic acid biosynthesis, have shown inconsistent association of two common hypomorphic allelic variants, C677T and A1298C, in nsCL/P patients and, in some cases, their mothers. We have studied the MTHFR C677T and A1298C polymorphisms in nsCL/P patients, their mothers, and population-matched controls from northern Venezuela. We found no evidence for contribution of the MTHFR C677T and A1298C variants to the risk of nsCL/P in northern Venezuela. | 5 | Dystrophic calcification and heterotopic ossification in fibrocartilaginous tissues of the spine in diffuse idiopathic skeletal hyperostosis(DISH)显示文摘Diffuse idiopathic skeletal hyperostosis(DISH) is a prevalent noninflammatory spondyloarthropathy characterized by ectopic mineral formation along the anterolateral aspect of the vertebral column, yet little is known about its underlying pathogenesis. Our objective was to evaluate the histopathological features and composition of ectopic mineral within spinal tissues affected by DISH in humans. Thoracic spine segments from six embalmed cadaveric donors(one female and five males;median age 82 years)meeting the radiographic diagnostic criteria for DISH were evaluated using radiological, histological, and physical analyses. Overall,the histological features of ectopic mineralization at individual motion segments were heterogeneous, including regions of heterotopic ossification and dystrophic calcification. Heterotopic ossifications were characterized by woven and lamellar bone,multifocal areas of metaplastic cartilage, and bony bridges along the anterior aspect of the intervertebral disc space. Dystrophic calcifications were characterized by an amorphous appearance, a high content of calcium and phosphorus, an X-ray diffraction pattern matching that of hydroxyapatite, and radiodensities exceeding that of cortical bone. Dystrophic calcifications were found within the anterior longitudinal ligament and annulus fibrosus in motion segments both meeting and not meeting the radiographic criteria for DISH. In summary, our findings indicate that in DISH, ectopic mineral forms along the anterior aspect of the spine by both heterotopic ossification and dystrophic calcification of fibrocartilaginous tissues.
