| 2 | A robust and efficient structural reliability method combining radial-based importance sampling and Kriging显示文摘Simulation based structural reliability analysis suffers from a heavy computational burden, as each sample needs to be evaluated on the performance function, where structural analysis is performed. To alleviate the computational burden, related research focuses mainly on reduction of samples and application of surrogate model, which substitutes the performance function. However,the reduction of samples is achieved commonly at the expense of loss of robustness, and the construction of surrogate model is computationally expensive. In view of this, this paper presents a robust and efficient method in the same direction. The present method uses radial-based importance sampling (RBIS) to reduce samples without loss of robustness. Importantly, Kriging is fully used to efficiently implement RBIS. It not only serves as a surrogate to classify samples as we all know, but also guides the procedure to determine the optimal radius, with which RBIS would reduce samples to the highest degree. When used as a surrogate, Kriging is established through active learning, where the previously evaluated points to determine the optimal radius are reused. | 3 | The origin of Mosuo people as revealed by mtDNA and Y chromosome variation显示文摘The Mosuo, living in the Lugu Lake area in northwest Yunnan Province, China, is the only matriarchal population in China. The Mosuo was officially identified as Naxi nationality although its relationship with Naxi remains controversial. We studied the genetic relationship between the Mosuo and five other ethnic groups currently residing in northwest Yunnan, i.e. Naxi, Tibetan, Bai, Yi and Pumi, by typing the genetic variations in mtDNA HVS1 and 21 Y chromosome markers (13 SNPs & 8 STR markers). We showed that the maternal lineages of the Mosuo bear the strongest resemblance with those found in Naxi while its paternal lineages are more similar to those that are prevalent in Yunnan Tibetan. | 12 | Impact of satellite blood culture on early diagnosis of sepsis显示文摘Background:The aim of this study was to assess whether satellite blood culture(SBC)can improve turnaround times,antibiotic switching,and patient prognosis,relative to laboratory blood culture(LBC).Methods:Patients with sepsis treated in the intensive care units(ICUs)of Henan Provincial People’s Hospital from February 5,2018 to January 19,2019 who met the inclusion criteria were recruited to the study and divided into the SBC group and LBC group according to different blood culture methods.Patient demographics,blood culture,antibiotic adjustment,and prognosis data were collected and compared between the two groups.Results:A total of 204 blood culture sets from 52 ICU patients,including 100 from the medical microbiology LBC group and 104 from the SBC group,were analyzed in this study.There was no significant difference in the positive rates between the two groups.Time from specimen collection to incubation was significantly shorter in the SBC group than that in the LBC group(1.65 h vs.3.51 h,z=−4.09,P<0.001).The median time from specimen collection to notification of blood culture positivity was 24.83 h in the SBC group and 27.83 h in the LBC group.Median times from adjustment of antibiotics according to the first report were 26.05 h and 51.71 h in the SBC and LBC groups,respectively,while those according to the final report were 97.17 h and 111.45 h,respectively.Median ICU lengths of stay were 15.00 days and 17.00 days in the SBC and LBC groups,respectively,and median ICU lengths of stay were 18.00 days and 23.50 days,respectively.Mean hospitalization costs were 157.99 and 186.73 thousand yuan in the SBC and LBC groups,respectively.Conclusion:SBC can significantly reduce blood culture turnaround times;however,there were no significant differences between the two blood culture methods in initial reporting of positive cultures,time to adjustment of antibiotic therapy,or medical costs,despite a trend toward improvement. | Ziqi Guo Bo Guo Shanmei Wang Huifeng Zhang Wenxiao Zhang Bingyu Qin Huanzhang Shao | 2022 | Journal of Intensive Medicine2022,2,1: | 0 |

| 15 | Clinical Application of PES Membrane Dialyzer显示文摘Objective The dialysate membrane can be classified into acetate membrane and synthetic membrane,while the latter can be further divided into polysulfone,polyacrylonitrile,etc.Polyethersulfone membrane is high molecular membrane material with excellent biocompatibility,stable mechanical properties,high temperature and pressure resistance,and good cleaning performance,which has been applied for more than 10 years.This trial is to assess the safety and efficacy of hollow polysulfone membrane fiber dialyzer with pattern number Enttex^(TM)-16LF(E60)for hemodialysis in patients with acute or chronic renal failure due to multiple factors.Methods Totally 76 patients receiving maintenance hemodialysis were included in a two-site,random,open label,two-stage,positive crossover and non inferiority validation study.The clinical efficacy and safety of the test device were observed,which was a hollow fiber dialyzer produced by Guangzhou Enttex Medical Products Co.,Ltd.,with polyethersulfone membrane imported from Germany.It was a low flux dialyzer with the pattern number of EnttexTM-16LF(E60).Inclusion criteria:①patients with acute or chronic renal failure due to various reasons who needed hemodialysis;②18 to 80 years old,gender unlimited;③patients who received stable dialysis for more than 3 months,regular hemodialysis 3 times a week,4 h at each time,blood flow rate of 200~350ml/min;④using bicarbonate dialysate at a flow rate of 500ml/min;⑤patients with arteriovenous fistula or who received artificial blood vessel puncture dialysis;received anticoagulation by heparin or low molecular weight heparin.Exclusion criteria:①patients needing heparin free dialysis for severe anemia,tumor and active bleeding;②patients with severe cardiac(gradeⅢcardiac function),hepatic(severe hepatitis and cirrhosis)and pulmonary diseases(severe respiratory failure).Results Comparison of parameters,including the changes of Kt/V,general creatinine and urea clearance rates and URR(urea reduction rate)shows no statistical significance in differences between the two devices,indicating that the test and control devices can equally and effectively eliminate toxins including creatinine and urea,etc.PPP and MITTP results are consistent.Comparison of ultrafiltration rates between two groups of patients shows no statistical difference.PPP and MITTP results are consistent,indicating that both the test device and control device can effectively eliminate water.In MITTP,electrolytes including K^+,Na^+,Cl^-,Ca^2+,HCO3^-,etc.show no statistical significance in difference between the test and control devices before and after dialysis(P>0.05).After treating by using test and control devices,body weight,serum potassium and phos-phorus all significantly decrease(P<0.001);serum Ca^2+,PO2,PCO2and HCO3^-all significantly increase(P<0.001).These results indicate that both the test and control devices can effectively eliminate serum potassium and phosphorus,and eliminate excess water in body,maintain the balance between liquid and electrolytes in patients.PPP and MITTP results are consistent.Conclusion This clinical trial confirms that hollow polyethersulfone membrane fiber dialyzer can be safely and effectively used in hemodialysis therapy. | YUN Chen YANG Peidian LIANG Wenxue CHEN Yu CHEN Gangyi LIU Hongping OUYANG Zihua SONG Huifeng LI Yunyi YU Zongchao GUAN Baozhang DONG Xiangnan LIU Huanhuan Cao Rui ZHANG Zeyu FAN Lijing HU Bo HE Yongcheng SHI Mingjie XU Jun Yang Lifeng TANG Shuifu | 2019 | 临床医学工程2019,26,S01: | 0 |
