1Contrast-enhanced ultrasound:Improving the preoperative staging of hepatocellular carcinoma and guiding individual treatment显示文摘AIM:To investigate the clinical role of contrast-enhanced ultrasound(CEUS) combined with contrast-enhanced computed tomography(CE-CT) or magnetic resonance imaging to improve the preoperative staging of hepatocellular carcinoma(HCC) and guide surgical decisionmaking.METHODS:Sixty-nine patients who underwent liver resection for HCC in our center were enrolled prospectively in the study.CEUS and CE-CT/MRI were performed before surgery.Intraoperative ultrasound(IOUS)wascarried out after liver mobilization.Lesions depicted by each imaging modality were counted and mapped.To investigate the impact of tumor size on the study,we divided the patients into two groups,the'Smaller group'(S-group,≤5 cm in diameter)and the'Largergroup'(L-group,>5 cm in diameter).The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of CE-CT/MRI,CEUS,IOUS,CEUS+CE-CT/MRI and the tumor node metastasis staging of tumors were calculated and compared.Changes in the surgical strategy as a result of CEUS and IOUS were analyzed.RESULTS:One hundred and twenty-seven nodules,comprising 94 HCCs confirmed by histopathology and33 benign lesions confirmed by histopathology and follow-up,were identified in 69 patients.The overall diagnostic sensitivity rates of CE-CT/MRI,CEUS,IOUS and CEUS+CE-CT/MRI were 78.7%,89.4%,89.4%and 89.4%,respectively.There was a significant difference between CEUS+CE-CT/MRI and CE-CT/MRI(P=0.046).Combining CEUS with CT or MRI increased,the diagnostic specificity compared with CT/MRI,CEUS and IOUS,and this difference was statistically significant(100%,72.7%,97.0%,and 69.7%,P=0.004,P=0.002,P=0.002,respectively).The diagnostic accuracy was significantly higher for CEUS+CT/MRI compared with CT/MRI(92.1%vs 77.2%,P=0.001).The TNM staging of tumors based on CEUS+CE-CT/MRI approximated to the final pathological TNM staging(P=0.977).There was a significant difference in the accuracy of TNM staging when comparing CEUS+CECT/MRI with CE-CT/MRI(P=0.002).Before surgery,strategies were changed in 15.9%(11/69)of patients as a result of CEUS.Finally,only 5.7%(4/69)of surgical strategies were changed because of IOUS findings.In the S-group,CEUS revealed 12 false positive lesions,including seven false positive lesions that were diagnosedby preoperative imaging examinations and five by IOUS.In contrast,in the L-group,IUOS revealed eight new malignant lesions;six of these lesions were true HCCs that were also identified by preoperative CEUS.CONCLUSION:CEUS combined with CT or MRI improves the accuracy of preoperative staging for hepatocellular carcinoma and may help to guide individualized treatment for patients with HCC.CEUS may better identify non-malignant lesions in patients with small tumors and discover new malignant lesions in patients with large tumors.Xiao-Yun Zhang Yan Luo Tian-Fu Wen Li Jiang Chuan Li Xiao-Fei Zhong Jing-Yi Zhang Wen-Wu Ling Lu-Nan Yan Yong Zeng Hong Wu 2014World Journal of Gastroenterology2014,20,35:16
2Insight into fouling behavior of poly(vinylidene fluoride)(PVDF)hollow fiber membranes caused by dextran with different pore size distributions显示文摘Membrane fouling is the key problem that occurs in membrane process for water treatment. However, how membrane microstructure influences the fouling behavior is still not clear. In this study, fouling behavior caused by dextran was deeply and systematically investigated by employing four poly(vinylidene fluoride)(PVDF)membranes with different pore sizes, ranging from 24 to 94 nm. The extent of fouling by dextran was accurately characterized by pore reduction, flux decline, and the change of critical flux. The result shows that membrane with the smallest pore size of 24 nm experienced the smallest fouling rate and the lowest fouling extent. As the membrane pore size increased, the critical flux ranges were 105-114, 63-73, 38-44 and 34-43 L·m^(-2)·h^(-1),respectively. The critical flux and fouling resistances indicated that the fouling propensity increases with the increase of membrane pore size. Two pilot membrane modules with mean pore size of 25 nm and 60 nm were applied in membrane filtration of surface water treatment. 5Changes in Soil Organic Carbon Dynamics in a Native C4 Plant-Dominated Tidal Marsh Following Spartina alterniflora Invasion显示文摘Invasion of an exotic C_4 plant Spartina alterniflora has been shown to increase soil organic carbon(SOC) concentrations in native C_3 plant-dominated coastal wetlands of China. However, little is known about the effects of S. alterniflora invasion on SOC concentrations and fractions in tidal marshes dominated by native C_4 plants. In this study, a field experiment was conducted in a tidal marsh dominated by the native C_4 plant Cyperus malaccensis in the Minjiang River estuary, China. Concentrations of SOC and liable SOC fractions, dissolved organic carbon(DOC), microbial biomass carbon(MBC), and easily oxidizable carbon(EOC),were measured in the top 50-cm soils of the C. malaccensis community, as well as those of three S. alterniflora communities with an invasion duration of 0–4 years(SA-4), 4–8 years(SA-8), and 8–12 years(SA-12), respectively. Results showed that both SOC stocks in the 50-cm soils and mean SOC concentrations in the surface soils(0–10 cm) of the C. malaccensis community increased with the duration of S. alterniflora invasion, whereas SOC concentrations in the 10–50-cm soils decreased slightly during the initial period of S. alterniflora invasion, before increasing again. The pattern of changes in labile SOC fractions(DOC, MBC, and EOC) with invasion duration was generally similar to that of SOC, while the ratios of labile SOC fractions to total SOC(DOC:SOC, MBC:SOC, and EOC:SOC) decreased significantly with the duration of S. alterniflora invasion. 6Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. 8Glacial Refugia of Ginkgo biloba and Human Impact on Its Genetic Diversity:Evidence from Chloroplast DNA显示文摘Variations in the trnK region of chloroplast DNA were investigated in the present study using polymerase chain reaction- restriction fragment length polymorphism to detect the genetic structure and to infer the possible glacial refugia of Ginkgo biloba L. in China. In total, 220 individuals from 12 populations in China and three populations outside China were analyzed, representing the largest number of populations studied by molecular markers to date. Nineteen haplotypes were produced and haplotype A was found in all populations. Populations in south-western China, including WC, JF, PX, and SP, contained 14 of the 19 haplotypes and their genetic diversity ranged from 0.771 4 to 0.867 6. The TM population from China also showed a high genetic diversity (H = 0.848 5). Most of the genetic variation existed within populations and the differentiation among populations was low (GST = 0.2). According to haplotype distribution and the historical record, we suggest that populations of G. biloba have been subjected to extensive human impact, which has compounded our attempt to infer glacial refugia for Ginkgo. 9Pharmacokinetics-based identification of pseudoaldosterogenic compounds originating from Glycyrrhiza uralensis roots(Gancao)after dosing LianhuaQingwen capsule显示文摘LianhuaQingwen capsule,prepared from an herbal combination,is officially recommended as treatment for COVID-19 in China.Of the serial pharmacokinetic investigations we designed to facilitate identifying LianhuaQingwen compounds that are likely to be therapeutically important,the current investigation focused on the component Glycyrrhiza uralensis roots(Gancao).Besides its function in COVID-19 treatment,Gancao is able to induce pseudoaldosteronism by inhibiting renal 11β-HSD2.Systemic and colon-luminal exposure to Gancao compounds were characterized in volunteers receiving LianhuaQingwen and by in vitro metabolism studies.Access of Gancao compounds to 11β-HSD2 was characterized using human/rat,in vitro transport,and plasma protein binding studies,while 11β-HSD2 inhibition was assessed using human kidney microsomes.LianhuaQingwen contained a total of 41 Gancao constituents(0.01-8.56μmol/day).Although glycyrrhizin(1),licorice saponin G2(2),and liquiritin/liquiritin apioside(21/22)were the major Gancao constituents in LianhuaQingwen,their poor intestinal absorption and access to colonic microbiota resulted in significant levels of their respective deglycosylated metabolites glycyrrhetic acid(8),24-hydroxyglycyrrhetic acid(M2_(D);a new Gancao metabolite),and liquiritigenin(27)in human plasma and feces after dosing.These circulating metabolites were glucuronized/sulfated in the liver and then excreted into bile.Hepatic oxidation of 8 also yielded M2_(D).Circulating 8 and M2D,having good membrane permeability,could access(via passive tubular reabsorption)and inhibit renal 11β-HSD2.Collectively,1 and 2 were metabolically activated to the pseudoaldosterogenic compounds 8 and M2_(D).This investigation,together with such investigations of other components,has implications for precisely defining therapeutic benefit of LianhuaQingwen and conditions for its safe use.Xiao-fang Lan Olajide EOlaleye Jun-lan Lu Wei Yang Fei-fei Du Jun-ling Yang Chen Cheng Yan-hong Shi Feng-qing Wang Xue-shan Zeng Nan-nan Tian Pei-wei Liao Xuan Yu Fang Xu Ying-fei Li Hong-tao Wang Nai-xia Zhang Wei-wei Jia Chuan Li 2021Acta Pharmacologica Sinica2021,42,12:6
10Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Gastric Cancer Patients显示文摘Purpose Malnutrition is severe among gastric cancer patients in China. While the Patient-Generated Subjective Global Assessment (PG-SGA) is a widely used nutritional assessment tool, the validation for Chinese patients with gastric cancer has not been performed yet. The aim of this research is to validate the Chinese version of the PGSGA in Chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in China. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P < 0.001) were significant in the majority of tests. 12Detecting P2P bots by mining the regional periodicity显示文摘Peer-to-peer (P2P) botnets outperform the traditional Internet relay chat (IRC) botnets in evading detection and they have become a prevailing type of threat to the Internet nowadays.Current methods for detecting P2P botnets,such as similarity analysis of network behavior and machine-learning based classification,cannot handle the challenges brought about by different network scenarios and botnet variants.We noticed that one important but neglected characteristic of P2P bots is that they periodically send requests to update their peer lists or receive commands from botmasters in the command-and-control (C&C) phase.In this paper,we propose a novel detection model named detection by mining regional periodicity (DMRP),including capturing the event time series,mining the hidden periodicity of host behaviors,and evaluating the mined periodic patterns to identify P2P bot traffic.As our detection model is built based on the basic properties of P2P protocols,it is difficult for P2P bots to avoid being detected as long as P2P protocols are employed in their C&C.For hidden periodicity mining,we introduce the so-called regional periodic pattern mining in a time series and present our algorithms to solve the mining problem.The experimental evaluation on public datasets demonstrates that the algorithms are promising for efficient P2P bot detection in the C&C phase.Yong QIAO Yue-xiang YANG Jie HE Chuan TANG Ying-zhi ZENG 2013Journal of Zhejiang University-Science C(Computers and Electronics)2013,14,9:3

14Segmental ureteroileal conduit resection for the treatment of distal upper urinary tract recurrence of bladder cancer fol lowing cystectomy显示文摘Segmental ureterectomy is less invasive than radical nephroureterectomy and results in nephron preservation and satisfactory tumor control.This study was to determine the feasibility of segmental ureteroileal conduit resection(SUICR) for patients with distal upper urinary tract recurrence of bladder cancer following radical cystectomy.Four patients with high-grade distal upper urinary tract recurrence underwent SUICR 15-108 months after radical cystectomy.The surgical technique details of SUICR,operative results,and follow-up outcomes are reported.The median operation time was 280 min,and estimated blood loss was less than 100 ml One patient suffered from ileus 5 days after surgery and was managed conservatively.Histopathologic evaluation showed high-grade stages pTa-pT1 diseases for these patients,and ureteral margins were all negative.No patient suffered from tumor recurrence,with a median follow-up of 39 months.SUICR preserved the ipsilateral renal unit and conformed to oncological principles during surgery.The oncological outcome was satisfactory for these properly selected patients.This technique provides a valid alternative to nephroureterectomy for patients with imperative indications and high-grade upper urinary tract recurrence of bladder cancer following radical cystectomy.Shu‑Xiong Zeng Xin Lu Wei‑Dong Xu Zhen‑Sheng Zhang Hai‑Hang Li Ying‑Hao Sun Chuan‑Liang Xu 2016Chinese Journal of Cancer2016,35,3:3

19Equivalency and unbiasedness of grey prediction models显示文摘In order to deeply research the structure discrepancy and modeling mechanism among different grey prediction models, the equivalence and unbiasedness of grey prediction models are analyzed and verified. The results show that all the grey prediction models that are strictly derived from x(0)(k) +az(1)(k) = b have the identical model structure and simulation precision. Moreover, the unbiased simulation for the homogeneous exponential sequence can be accomplished. However, the models derived from dx(1)/dt + ax(1)= b are only close to those derived from x(0)(k) + az(1)(k) = b provided that |a| has to satisfy|a| < 0.1; neither could the unbiased simulation for the homogeneous exponential sequence be achieved. 20A graphene-laminated electrode with high glucose oxidase loading for highly-sensitive glucose detection显示文摘Graphene oxide(GO) has received considerable attention for glucose detection because of high surface area, abundant functional groups, and good biocompatibility. Defects and functional groups of the GO are beneficial to immobilization of glucose oxidase(GOD), but sacrificing electron-transfer capability for highly-sensitive detection. In order to obtain high GOD loading and highly-sensitive detection of biosensors, we first designed and fabricated a graphene-laminated electrode by combining GO and edgefunctionalized graphene(FG) layers together onto glassy-carbon electrode. The graphene-laminated electrodes exhibited faster electron transfer rate, higher GOD loading of 3.80 × 10^(-9) mol·cm^(-2), and higher detection sensitivity of 46.71 μA·mM^(-1)·cm^(-2) than other graphene-based biosensors reported in literature.
