| 2 | Lp-PLA2 inhibition prevents Ang Ⅱ-induced cardiac inflammation and fibrosis by blocking macrophage NLRP3 inflammasome activation显示文摘Macrophage-mediated inflammation plays an important role in hypertensive cardiac remodeling,whereas effective pharmacological treatments targeting cardiac inflammation remain unclear.Lipoprotein-associated phospholipase A2(Lp-PLA2)contributes to vascular inflammation-related diseases by mediating macrophage migration and activation.Darapladib,the most advanced Lp-PLA2 inhibitor,has been evaluated in phase Ⅲ trials in atherosclerosis patients.However,the role of darapladib in inhibiting hypertensive cardiac fibrosis remains unknown.Using a murine angiotensin Ⅱ(Ang Ⅱ)infusion-induced hypertension model,we found that Pla2g7(the gene of Lp-PLA2)was the only upregulated PLA2 gene detected in hypertensive cardiac tissue,and it was primarily localized in heart-infiltrating macrophages.As expected,darapladib significantly prevented Ang Ⅱ-induced cardiac fibrosis,ventricular hypertrophy,and cardiac dysfunction,with potent abatement of macrophage infiltration and inflammatory response.RNA sequencing revealed that darapladib strongly downregulated the expression of genes and signaling pathways related to inflammation,extracellular matrix,and proliferation.Moreover,darapladib substantially reduced the Ang Ⅱ infusion-induced expression of nucleotide-binding oligomerization domain-like receptor with pyrin domain 3(NLRP3)and interleukin(IL)-1β and markedly attenuated caspase-1 activation in cardiac tissues.Furthermore,darapladib ameliorated Ang Ⅱ-stimulated macrophage migration and IL-1β secretion in macrophages by blocking NLRP3 inflammasome activation.Darapladib also effectively blocked macrophage-mediated transformation of fibroblasts into myofibroblasts by inhibiting the activation of the NLRP3 inflammasome in macrophages.Overall,our study identifies a novel anti-inflammatory and anti-cardiac fibrosis role of darapladib in Lp-PLA2 inhibition,elucidating the protective effects of suppressing NLRP3 inflammasome activation.Lp-PLA2 inhibition by darapladib represents a novel therapeutic strategy for hypertensive cardiac damage treatment. | Si-lin Lv Zi-fan Zeng Wen-qiang Gan Wei-qi Wang Tie-gang Li Yu-fang Hou Zheng Yan Ri-xin Zhang Min Yang | 2021 | Acta Pharmacologica Sinica2021,42,12: | 21 |
| 3 | Daclatasvir plus asunaprevir in treatment-na?ve patients with hepatitis C virus genotype 1b infection显示文摘AIM To assess daclatasvir plus asunaprevir(d UAL) in treatment-na?ve patients from China's Mainland, Russia and South Korea with hepatitis C virus(HCV) genotype 1 b infection. METHODS Patients were randomly assigned(3:1) to receive 24 wk of treatment with d UAL(daclatasvir 60 mg once daily and asunaprevir 100 mg twice daily) beginning on day 1 of the treatment period(immediate treatment arm) or following 12 wk of matching placebo(placebodeferred treatment arm). The primary endpoint was a comparison of sustained virologic response at posttreatment week 12(SVR12) compared with the historical SVR rate for peg-interferon plus ribavirin(70%) among patients in the immediate treatment arm. The first 12 wk of the study were blinded. Safety was assessed in d UAL-treated patients compared with placebo patients during the first 12 wk(doubleblind phase), and during 24 wk of d UAL in both arms combined.RESULTS In total, 207 patients were randomly assigned to immediate(n = 155) or placebo-deferred(n = 52) treatment. Most patients were Asian(86%), female(59%) and aged < 65 years(90%). Among them, 13% had cirrhosis, 32% had IL28 B non-CC genotypes and 53% had baseline HCV RNA levels of ≥ 6 million IU/m L. Among patients in the immediate treatment arm, SVR12 was achieved by 92%(95% confidence interval: 87.2-96.0), which was significantly higher than the historical comparator rate(70%). SVR12 was largely unaffected by cirrhosis(89%), age ≥ 65 years(92%), male sex(90%), baseline HCV RNA ≥ 6 million(89%) or IL28 B non-CC genotypes(96%), although SVR12 was higher among patients without(96%) than among those with(53%) baseline NS5 A resistanceassociated polymorphisms(at L31 or Y93 H). during the double-blind phase, aminotransferase elevations were more common among placebo recipients than among patients receiving d UAL. during 24 wk of d UAL therapy(combined arms), the most common adverse events(≥ 10%) were elevated alanine aminotransferase and upper respiratory tract infection; emergent grade 3-4 laboratory abnormalities were infrequently observed, and all grade 3-4 aminotransferase abnormalities(alanine aminotransferase, n = 9; aspartate transaminase, n = 6) reversed within 8-11 d. Two patients discontinued d UAL treatment; one due to aminotransferase elevations, nausea, and jaundice and the other due to a fatal adverse event unrelated to treatment. | 4 | Blocking FSH inhibits hepatic cholesterol biosynthesis and reduces serum cholesterol显示文摘Menopause is associated with dyslipidemia and an increased risk of cardio-cerebrovascular disease.The classic view assumes that the underlying mechanism of dyslipidemia is attributed to an insufficiency of estrogen.In addition to a decrease in estrogen, circulating follicle-stimulating hormone (FSH)levels become elevated at menopause.In this study,we find that blocking FSH reduces serum cholesterol via inhibiting hepatic cholesterol biosynthesis.First,epidemiological results show that the serum FSH levels are positively correlated with the serum total cholesterol levels,even after adjustment by considering the effects of serum estrogen,in addition,the prevalence of hypercholesterolemia is significantly higher in peri-menopausal women than that in premenopausal women.Furthermore,we generated a mouse model of FSH elevation by intraperitoneally injecting exogenous FSH into ovariectomized (OVX)mice,in which a normal level of estrogen (E2)was maintained by exogenous supplementation. Consistently,the results indicate that FSH,independent of estrogen,increases the serum cholesterol level in this mouse model. | 8 | Peginterferon alfa-2a for the treatment of chronic hepatitis C in the era of direct-acting antivirals显示文摘BACKGROUND: The availability of novel direct-acting antivirals(DAAs) represents a new era of curative hepatitis C virus(HCV) treatment, with over 95% of patients infected with HCV genotype 1 achieving sustained virological response(SVR). Nevertheless, the majority of patients globally are unable to access these treatments because of cost and infrastructure constraints and, thus, remain untreated and uncured.DATA SOURCE: Relevant articles of peginterferon(Peg IFN)-based treatments in HCV and sofosbuvir-based treatments, simeprevir, daclatasvir/asunaprevir, ritonavir-boosted paritaprevir/ombitasvir/dasabuvir, and grazoprevir/elbasvir, were searched in Pub Med database, including general population and special population.RESULTS: Peg IFN in combination with ribavirin remains an important and relevant option for some patients, achieving SVR rates of up to 79% in genotype 1 and 89% in genotype 2 or 3 infections, which increases for patients with favorable IL28 B genotypes. Triple therapy of DAA plus Peg IFN/ribavirin is effective in treating difficult-to-cure patients infected with HCV genotype 3 or with resistance-associated variants. Owing to its long history in HCV management, the efficacy, tolerability and long-term outcomes associated with Peg IFN alfa-2a are well established and have been validated in largescale studies and in clinical practice for many populations. Furthermore, emerging data show that IFN-induced SVR is associated with lower incidences of hepatocellular carcinoma compared with DAAs. | 9 | Chemical characteristics of size-resolved aerosols in winter in Beijing显示文摘Size-resolved aerosols were continuously collected by a Nano Sampler for 13 days at an urban site in Beijing during winter 2012 to measure the chemical composition of ambient aerosol particles. Data collected by the Nano Sampler and an ACSM(Aerodyne Aerosol Chemical Speciation Monitor) were compared. Between the data sets,similar trends and strong correlations were observed,demonstrating the validity of the Nano Sampler. PM10 and PM2.5concentrations during the measurement were 150.5 ± 96.0 μg/m3(mean ± standard variation)and 106.9 ± 71.6 μg/m3,respectively. The PM2.5/PM10 ratio was 0.70 ± 0.10,indicating that PM2.5dominated PM10. The aerosol size distributions showed that three size bins of 0.5–1,1–2.5 and 2.5–10 μm contributed 21.8%,23.3% and 26.0% to the total mass concentration(TMC),respectively. OM(organic matter) and SIA(secondary ionic aerosol,mainly SO42-,NO3-and NH4+) were major components of PM2.5. Secondary compounds(SIA and secondary organic carbon) accounted for half of TMC(about 49.8%) in PM2.5,and suggested that secondary aerosols significantly contributed to the serious particulate matter pollution observed in winter. Coal burning,biomass combustion,vehicle emissions and SIA were found to be the main sources of PM2.5. | 10 | A Confocal Endoscope for Cellular Imaging显示文摘Since its inception, endoscopy has aimed to establish an immediate diagnosis that is virtually consistent with a histologic diagnosis. In the past decade, confocal laser scanning microscopy has been brought into endoscopy, thus enabling in vivo microscopic tissue visualization with a magnification and resolution comparable to that obtained with the ex vivo microscopy of histological specimens. The major challenge in the development of instrumentation lies in the miniaturization of a fiber-optic probe for microscopic imaging with micron-scale resolution. Here, we present the design and construction of a confocal endoscope based on a fiber bundle with 1.4-μm lateral resolution and 8-frames per second(fps) imaging speed. The fiber-optic probe has a diameter of 2.6 mm that is compatible with the biopsy channel of a conventional endoscope. The prototype of a confocal endoscope has been used to observe epithelial cells of the gastrointestinal tracts of mice and will be further demonstrated in clinical trials. | 11 | Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. | 12 | Estrogenic activity of osthole and imperatorin in MCF-7 cells and their osteoblastic effects in Saos-2 cells显示文摘There is an increasing interest in phytoestrogens due to their potential medical usage in hormone replacement therapy(HRT). The present study was designed to investigate the in vitro effects of estrogen-like activities of two widespread coumarins, osthole and imperatorin, using the MCF-7 cell proliferation assay and their alkaline phosphatase(ALP) activities in osteoblasts Saos-2 cells. The two compounds were found to strongly stimulate the proliferation of MCF-7 cells. The estrogen receptor-regulated ERα, progesterone receptor(PR) and PS2 m RNA levels were increased by treatment with osthole and imperatorin. All these effects were significantly inhibited by the specific estrogen receptor antagonist ICI182, 780. Cell cycle analysis revealed that their proliferation stimulatory effect was associated with a marked increase in the number of MCF-7 cells in S phase, which was similar to that observed with estradiol. It was also observed that they significantly increased ALP activity, which was reversed by ICI182,780. These results suggested that osthole and imperatorin could stimulate osteoblastic activity by displaying estrogenic properties or through the ER pathway. Methods 250 PCa patients and 500 controls were enrolled in this case-control study. Information was collected and logistic regression analysis was used to estimate the odds ratios(OR) and 95% confidence intervals(95% CI) for relationship between lifestyle, eating habits and psycho-social factors with PCa risk. Results Green vegetables and green tea were associated with a decreased risk of PCa(OR=0.39, 95% CI: 0.28-0.53; OR=0.59, 95% CI: 0.40-0.87, respectively). Family history of PCa(OR=7.16, 95% CI: 2.01-25.49), history of prostate diseases(OR=2.28, 95% CI: 1.53-3.41), alcohol consumption(OR=1.97, 95% CI: 1.33-2.90), red meat consumption(OR=1.74, 95% CI: 1.20-2.52), barbecued(OR=2.29, 95% CI: 1.11-4.73) or fried(OR=2.35, 95% CI: 1.24-4.43) foods were related with increased PCa risk. Negative psycho-social factors including occupational setbacks(OR=1.61, 95% CI: 1.00-2.59), marital separation(OR=1.94, 95% CI: 1.29-2.91), self-contained suffering(OR=2.37, 95% CI: 1.58-3.55), and high sensitivity to the personal comments(OR=1.73, 95% CI: 1.18-2.54) were related to PCa. However,erythrocytes continuously suffer from reactive oxygen species(ROS)-induced oxidative stress and apoptosis. Thus, it is essential to expand our knowledge of how to protect erythrocytes against ROS-induced oxidative stress and apoptosis in fish. In this study, we explored the cytotoxicity and the effects of butylated hydroxyanisole(BHA), ethyl ether extracts, ethyl acetate extracts, acetone extracts(AE), ethanol extracts, and aqueous extracts of Astragalus membranaceus(EAm) on hydroxyl radical(·OH)-induced apoptosis in carp erythrocytes. The rat hepatocytes and carp erythrocytes were incubated with different concentrations of BHA or EAm(0.125 to 1 mg/mL). The toxicity in rat hepatocytes and carp erythrocytes was then measured using a 3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide(MTT) assay and a haemolysis assay,respectively. The carp erythrocytes were treated with BHA or EAm in the presence of 40 μmol/L FeSO4 and20 μmol/L H_2 O_2 at 37℃, except for the control group. Oxidative stress and apoptosis parameters in the carp erythrocytes were then evaluated using the commercial kit. The results indicated that at high concentrations, BHA and EAm could induce toxicity in rat hepatocytes and fish erythrocytes. However, BHA was more toxic than EAm at the same concentrations. Moreover, the toxicity order of BHA and EAm in the fish erythrocytes approximately agreed with that for the rat hepatocytes. Butylated hydroxyanisole and EAm suppressed the ·OH-induced phosphatidylserine exposure and DNA fragmentation(the biomarkers of apoptosis) by decreasing the generation of ROS, inhibiting the oxidation of cellular components, and restoring the activities of antioxidants in carp erythrocytes. Of all of the examined EAm, the AE showed the strongest effects. The effects of AE on superoxide anion, H_2 O_2, met-haemoglobin and reduced glutathione levels, as well as glutathione reductase activity and apoptosis were equivalent to or stronger than those of BHA.
