2Nonintubated uniportal video-assisted thoracoscopic surgery for primary spontaneous pneumothorax显示文摘Objective:The objective of the current study was to evaluate the feasibility and safety of nonintubated uniportal video-assisted thoracoscopic surgery(VATS) for the management of primary spontaneous pneumothorax(PSP).Methods:From November 2011 to June 2013,32 consecutive patients with PSP were treated by nonintubated uniportal thoracoscopic bullectomy using epidural anaesthesia and sedation without endotracheal intubation.An incision 2 cm in length was made at the 6^(th) intercostal space in the median axillary line.The pleural space was entered by blunt dissection for placement of a soft incision protector.Instruments were then inserted through the incision protector to perform thoracoscopic bullectomy.Data were collected within a minimum follow-up period of 10 months.Results:The average time of surgery was 49.0 min(range,33-65 min).No complications were recorded.The postoperative feeding time was 6 h.The mean postoperative chest tube drainage and hospital stay were19.3 h and 41.6 h,respectively.The postoperative pain was mild for 30 patients(93.75%) and moderate for two patients(6.25%).No recurrences of pneumothorax were observed at follow-up.Conclusions:The initial results indicated that nonintubated uniportal video-assisted thoracoscopic operations are not only technically feasible,but may also be a safe and less invasive alternative for select patients in the management of PSP.This is the first report to include the use of a nonintubated uniportal technique in VATS for such a large number of PSP cases.Further work and development of instruments are needed to define the applications and advantages of this technique.Shuben Li Fei Cui Jun Liu Xin Xu Wenlong Shao Weiqiang Yin Hanzhang Chen Jianxing He 2015Chinese Journal of Cancer Research2015,27,2:15
3Safety and feasibility of video-assisted thoracoscopic surgery for stage IIIA lung cancer显示文摘Objective: The current study was prospectively designed to explore the application of video-assisted thoracoscopic surgery(VATS) radical treatment for patients with stage IIIA lung cancer, with the primary endpoints being the safety and feasibility of this operation and the second endpoints being the survival and complications after the surgery.Methods: A total of 51 patients with radiologically or mediastinoscopically confirmed stage IIIA lung cancer underwent VATS radical treatment, during which the standard pulmonary lobectomy and mediastinal lymph node dissection were performed after pre-operative assessment. The operative time, intraoperative blood loss/complications, postoperative recovery, postoperative complications, and lymph node dissection were recorded and analyzed. This study was regarded as successful if the surgical success rate reached 90% or higher.Results: A total of 51 patients with non-small cell lung cancer(NSCLC) were enrolled in this study from March 2009 to February 2010. The median post-operative follow-up duration was 50.5 months. Of these 51 patients, 41(80.4%) had N2 lymph node metastases. All patients underwent the thoracoscopic surgeries, among whom 50(98%) received pulmonary lobectomy and mediastinal lymph node dissection completely under the thoracoscope, 6 had their incisions extended to about 6 cm due to larger tumor sizes, and 1 had his surgery performed using a 12 cm small incision for handling the adhesions between lymph nodes and blood vessels. No patient was converted to conventional open thoracotomy. No perioperative death was noted. One patient received a second surgery on the second post-operative day due to large drainage(>1,000 mL), and the postoperative recovery was satisfactory. 4Long-term survival outcomes of video-assisted thoracic surgery for patients with non-small cell lung cancer显示文摘Background: Video-assisted thoracic surgery(VATS) has been shown to be a safe alternative to conventional thoracotomy for patients with non-small cell lung cancer(NSCLC). However, popularization of this relatively novel technique has been slow, partly due to concerns about its long-term outcomes. The present study aimed to evaluate the long-term survival outcomes of patients with NSCLC after VATS, and to determine the significant prognostic factors on overall survival.Methods: Consecutive patients diagnosed with NSCLC referred to one institution for VATS were identified from a central database. Patients were treated by either complete-VATS or assisted-VATS, as described in previous studies. A number of baseline patient characteristics, clinicopathologic data and treatment-related factors were analyzed as potential prognostic factors on overall survival.Results: Between January 2000 and December 2007, 1,139 patients with NSCLC who underwent VATS and fulfilled a set of predetermined inclusion criteria were included for analysis. The median age of the entire group was 60 years, with 791 male patients(69%). The median 5-year overall survival for Stage I, II, III and IV disease according to the recently updated TNM classification system were 72.2%, 47.5%, 29.8% and 28.6%, respectively. 5Fracture of two three-dimensional parallel internal cracks in brittle solid under ultrasonic fracturing显示文摘Similar to hydraulic fracturing(HF), the coalescence and fracture of cracks are induced within a rock under the action of an ultrasonic field, known as ultrasonic fracturing(UF). Investigating UF is important in both hard rock drilling and oil and gas recovery. A three-dimensional internal laser-engraved crack(3D-ILC) method was introduced to prefabricate two parallel internal cracks within the samples without any damage to the surface. The samples were subjected to UF. The mechanism of UF was elucidated by analyzing the characteristics of fracture surfaces. The crack propagation path under different ultrasonic parameters was obtained by numerical simulation based on the Paris fatigue model and compared to the experimental results of UF. The results show that the 3D-ILC method is a powerful tool for UF research.Under the action of an ultrasonic field, the fracture surface shows the characteristics of beach marks and contains powder locally, indicating that the UF mechanism includes high-cycle fatigue fracture, shear and friction, and temperature load. The two internal cracks become close under UF. The numerical result obtained by the Paris fatigue model also shows the attraction of the two cracks, consistent with the test results. The 3D-ILC method provides a new tool for the experimental study of UF. 7A Trajectory Prediction Based Intelligent Handover Control Method in UAV Cellular Networks显示文摘The airborne base station(ABS) can provide wireless coverage to the ground in unmanned aerial vehicle(UAV) cellular networks.When mobile users move among adjacent ABSs,the measurement information reported by a single mobile user is used to trigger the handover mechanism.This handover mechanism lacks the consideration of movement state of mobile users and the location relationship between mobile users,which may lead to handover misjudgments and even communication interrupts.In this paper,we propose an intelligent handover control method in UAV cellular networks.Firstly,we introduce a deep learning model to predict the user trajectories.This prediction model learns the movement behavior of mobile users from the measurement information and analyzes the positional relations between mobile users such as avoiding collision and accommodating fellow pedestrians.Secondly,we propose a handover decision method,which can calculate the users' corresponding receiving power based on the predicted location and the characteristic of air-to-ground channel,to make handover decisions accurately.Finally,we use realistic data sets with thousands of non-linear trajectories to verify the basic functions and performance of our proposed intelligent handover controlmethod.The simulation results show that the handover success rate of the proposed method is 8% higher than existing methods.Bo Hu Hanzhang Yang Lei Wang Shanzhi Chen 2019China Communications2019,16,1:3
8Emergency medical services use and its association with acute ischaemic stroke evaluation and treatment in Singapore显示文摘Background Emergency medical services(EMS)is a critical link in the chain of stroke survival.We aimed to assess EMS use for stroke in Singapore,identify characteristics associated with EMS use and the association of EMS use with stroke evaluation and treatment.Methods The Singapore Stroke Registry combines nationwide EMS and public hospital data for stroke cases in Singapore.Multivariate regressions with the generalised estimating equations were performed to examine the association between EMS use and timely stroke evaluation and treatment.results Of 3555 acute ischaemic patients with symptom onset within 24 hours admitted to all five public hospitals between 2015 and 2016,68%arrived via EMS.Patients who used EMS were older,were less likely to be female,had higher stroke severity by National Institute of Health Stroke Scale and had a higher prevalence of atrial fibrillation or peripheral arterial disease.Patients transported by EMS were more likely to receive rapid evaluation(door-to imaging time≤25 min 34.3%vs 11.1%,OR=2.74(95%CI 1.40 to 5.38))and were more likely to receive intravenous tissue plasminogen activator(tPA,22.8%vs 4.6%,OR=4.61(95%CI 3.52 to 6.03)).Among patients treated with tPA,patients who arrived via EMS were more likely to receive timely treatment than self-transported patients(door-to needle time≤60 min 52.6%vs 29.4%,OR=2.58(95%CI 1.35 to 4.92)).Conclusions EMS use is associated with timely stroke evaluation and treatment in Singapore.Seamless EMS-Hospital stroke pathways and targeted public campaigns to advocate for appropriate EMS use have the potential to improve acute stroke care.Hanzhang Xu Ying Xian Fung Peng Woon Janet Prvu Bettger Daniel T Laskowitz Yih Yng Ng Marcus Eng Hock Ong David Bruce Matchar Deidre Anne De Silva 2020Stroke & Vascular Neurology2020,5,2:2

10Fracturing behavior of brittle solids containing 3D internal crack of different depths under ultrasonic fracturing显示文摘Ultrasonic fracturing(UF)can be used to form crack networks for oil or gas in the mining industry and hard rock excavation.In this paper,the three-dimensional internal laser-engraved crack method(3DILC)is introduced to make penny-shaped internal crack within the samples without any damage to the surfaces.Physical experiments were performed on the transparent samples with internal cracks at different depths.The macro-and micro-fracture fractography characteristics were focused on and analyzed in details.Based on fractography,the mechanisms of the UF were revealed.The propagation paths of cracks of different depths were simulated based on the Paris fatigue model and compared with experimental results.The results show that the 3D-ILC method is a powerful tool for the experimental research on the internal crack propagation in the UF.Under the action of ultrasonic field,fractography shows the characteristics of beach marks,lance-like pattern,rime-like pattern and glass powder.It can be judged that the mechanisms of UF include high cycle fatigue fracture,shear fracture,and friction.The results of the numerical simulation using the Paris fatigue model are consistent with the experimental results.Haijun Wang Hanzhang Li Lei Tang Jianchun Li Xuhua Ren 2022International Journal of Mining Science and Technology2022,32,6:1

14A Simple and High Quality Method for Isolation and Extraction of Total RNA of Pholiota adipose显示文摘Analyzing functional values of RNA using RNA-seq technology is a hot spot of researches. In order to study the medicinal value of Pholiota adipose from the transcriptome level,it is necessary to extract and isolate RNA samples of high purity and high quality P. adipose. In this study,liquid nitrogen grinding Trizol one-step method was used to extract the total RNA of P. adipose. Quality test and statistical comparative analysis were carried out for RNA extract of liquid nitrogen grinding treated and untreated P. adipose. The results showed that the concentration of RNA in the samples treated with liquid nitrogen was much higher than that of the samples without grinding treatment. The OD260/280 of both was about 2,indicating that the purity of RNA was very high. Besides,the ratio of fluorescence intensity of 25 S and 18 S subunit strips of three replicate samples was 1. 8,1. 9,and 1. 9,close to 2,indicating RNA integrity is good. RIN test results of Agilent 2100 were 9. 1,8. 7,and 9. 3,higher than the standard value 6. 8,further proving the integrity. 17Ubiquitin-specific protease 47 regulates intestinal inflammation through deubiquitination of TRAF6 in epithelial cells显示文摘Deubiquitinates(DUBs) alter the stabilities, localizations or activities of substrates by removing their ubiquitin conjugates,which are closely related to the development of inflammatory response. Here, we show that ubiquitin-specific protease 47(USP47) prevents inflammation development in inflammatory bowel disease(IBD). Compared with wild-type mice, Usp47 knockout mice are more susceptible to dextran sodium sulfate(DSS)-induced acute and chronic colitis with higher inflammatory cytokines expression and severe intestinal tissue damage. Chimeric mouse experiments suggest that non-hematopoietic cells mainly contribute to the phenotype. And, DSS-induced colitis of the Usp47 knockout mice depends on commensal bacteria.Mechanistically, down-regulation of USP47 aggravates the activation of NF-κB signaling pathway by increasing the K63-linked poly-ubiquitination of tumor necrosis factor receptor-associated factor 6(TRAF6) in intestinal epithelial cells. Furthermore, the expression of USP47, negatively correlated with the degree of inflammation, is lower at colonic inflammatory lesions than that non-inflammatory sites from the intestine from ulcerative colitis(UC) and Crohn's disease(CD) patients.
