| 3 | Clinical significance of loss of heterozygosity for M6P/IGF2R in patients with primary hepatocellular carcinoma显示文摘AIM:To investigate the relationship between loss of heterozygosity (LOH) for mannose 6-phosphate/insulin- like growth factor 2 receptor (M6P/IGF2R) and the outcomes for primary HCC patients treated with partial hepatectomy. METHODS: The LOH for M6P/IGF2R in primary HCC patients was assessed using six different gene-specific nucleotide polymorphisms. The patients studied were enrolled to undergo partial hepatectomy. RESULTS: M6P/IGF2R was found to be polymorphic in 73.3% (22/30) of the patients, and of these patients, 50.0% (11/22) had tumors showing LOH in M6P/IGF2R. Loss of heterozygosity in M6P/IGF2R was associated with significant reductions in the two year overall survival rate (24.9% vs 65.5%; P = 0.04) and the disease-free survival rate (17.8% vs 59.3%; P = 0.03). | 9 | Individual having a parent with early-onset gastric cancer may need screening at younger age显示文摘AIM: To evaluate whether individuals with gastric cancer(GC) are diagnosed earlier if they have firstdegree relatives with GC.METHODS: A total of 4282 patients diagnosed with GC at National Cancer Center Hospital from 2002 to 2012 were enrolled in this retrospective study. We classified the patients according to presence or absence of first-degree family history of GC and compared age at diagnosis and clinicopathologic characteristics. In addition, we further classified patients according to specific family member with GC(father, mother, sibling, or offspring) and compared age at GC diagnosis among these patient groups. Baseline characteristics were obtained from a prospectively collected database. Information about the family member's age at GC diagnosis was obtained by questionnaire.RESULTS: A total of 924 patients(21.6%) had a firstdegree family history of GC. The mean age at GC diagnosis in patients having paternal history of GC was 54.4 ± 10.4 years and was significantly younger than in those without a first-degree family history(58.1 ± 12.0 years, P < 0.001). However, this finding was not observed in patients who had an affected mother(57.2 ± 10.0 years) or sibling(62.2 ± 9.8 years). Among patients with family member having early-onset GC(< 50 years old), mean age at diagnosis was 47.7 ± 10.3 years for those with an affected father, 48.6 ± 10.4 years for those with an affected mother, and 57.4 ± 11.5 years for those with an affected sibling. Thus, patients with a parent diagnosed before 50 years of age developed GC 10.4 or 9.5 years earlier than individuals without a family history of GC(both P <0.001).CONCLUSION: Early-onset GC before age of 50 was associated with parental history of early-onset of GC. Understanding the nonlinear relationships among the strain,strain rate, temperature, and flow stress of this alloy is essential to optimize the hot working process.In this study, a deep neural network(DNN) model was developed to correlate flow stress with a wide range of strains(0.025–0.6), strain rates(0.01–10 s^-1) and temperatures(750–1000℃). The model, which was tested with 96 unseen datasets, showed better performance than existing models, with a correlation coefficient of 0.999. The processing map constructed using the DNN model was effective in predicting the microstructural evolution of the alloy. | 12 | The clinical association of programmed cell death protein 4(PDCD4) with solid tumors and its prognostic significance:a meta-analysis显示文摘Background: Programmed cell death protein 4(PDCD4) is a novel tumor suppressor protein involved in pro?grammed cell death. Its association with cancer progression has been observed in multiple tumor models, but evidence supporting its association with solid tumors in humans remains controversial. This study aimed to determine the clinical signiicance and prognostic value of PDCD4 in solid tumors.Methods: A systematic literature review was performed to retrieve publications with available clinical informa?tion and survival data. The eligibility of the selected articles was based on the criteria of the Dutch Cochrane Centre proposed by the Meta?analysis Of Observational Studies in Epidemiology group. Pooled odds ratios(ORs), hazard ratios(HRs), and 95% conidence intervals(CIs) for survival analysis were calculated. Publication bias was examined by Begg's and Egger's tests.Results: Clinical data of 2227 cancer patients with solid tumors from 23 studies were evaluated. PDCD4 expression was signiicantly associated with the diferentiation status of head and neck cancer(OR 4.25, 95% CI 1.87–9.66) and digestive system cancer(OR 2.87, 95% CI 1.84–4.48). Down?regulation of PDCD4 was signiicantly associated with short overall survival of patients with head and neck(HR: 3.44, 95% CI 2.38–4.98), breast(HR: 1.86, 95% CI 1.36–2.54), digestive system(HR: 2.12, 95% CI 1.75–2.56), and urinary system cancers(HR: 3.16, 95% CI 1.06–9.41).Conclusions: The current evidence suggests that PDCD4 down?regulation is involved in the progression of several types of solid tumor and is a potential marker for solid tumor prognoses. | 14 | Characteristics of vertical ozone distribution in the lower troposphere in the Yangtze River Delta at Lin'an in the spring of 2001显示文摘We analyzed vertical distributions of ozone (O3) in the lower troposphere (< 5 km above ground) at Lin’an (119.75°E, 30.30°N), Zhejiang Province using electrochemical concen- tration cell (ECC) ozonesonde data obtained from February 21 to April 13, 2001. The results showed that the vertical O3 distributions are controlled by metrological conditions and the char- acteristics of O3 profiles are related to those of wet bulb potential temperature and wind field. O3 below 2 km showed that the strongest variability and enhanced O3 mixing ratios were associated with easterly winds that blow pollutants from the upwind source region of the Yangtze River Delta (YRD) region. Vertical O3 profiles below 5 km can be grouped into 5 categories: (1) peak mixing ratio type, (2) well-mixed type, (3) layered-structure type, (4) episodic pollution type and (5) alti- tudinal increasing type. Vertical distributions of O3 affected by regional transport of polluted air masses were investigated. Transport of polluted air from high latitudes of northern China, ac- companying subsiding motion of air and stagnant atmospheric conditions are important factors that lead to high mixing ratios of O3 at Lin’an. The stagnant atmospheric conditions associated with a continental high pressure system and pollution plume transported from the YRD and cen- tral-eastern China also lead to regional accumulation of O3 and high O3 mixing ratio at Lin’an. | 16 | Current status and challenges in sentinel node navigation surgery for early gastric cancer显示文摘Although a number of feasibility studies for sentinel node(SN) concepts in gastric cancer have been conducted since 2000, there remains a debate regarding detailed detection techniques and oncological safety. Two important multicenter phase II clinical trials were performed in Japan that used different methods and reached different conclusions; one confirmed acceptable results with a false-negative rate of 7%, and the other showed an unacceptably high false-negative rate of 46.4%. The Sentinel Node Oriented Tailored Approach(SENORITA)trial is a multicenter randomized controlled phase III trial being performed in Korea. Patient enrollment is now complete and the long-term results are currently awaited. Recently, an image-guided SN mapping technique using infrared ray/fluorescence was introduced. This method might be a promising technology because it allows the clear visualization of SNs. With regard to the primary tumor, the non-exposed endoscopic wall-inversion surgery technique and non-exposure endolaparoscopic full-thickness resection with simple suturing technique have been reported. These methods prevent abdominal infection and tumor seeding and can be good alternatives to conventional laparoscopic gastric wedge resection. For indications, SN navigation surgery can be extended to patients who underwent non-curative endoscopic resection. Although a few studies have been performed on these patients, sentinel concepts may be beneficial to patients as they omit the need for additional gastrectomy.
