| 6 | Short-term entecavir versus lamivudine therapy for HBeAg-negative patients with acute-on-chronic hepatitis B liver failure显示文摘BACKGROUND: Selection of drugs for antiviral therapy of patients with hepatitis B virus (HBV)-related acute-on-chronic liver failure (ACLF) remains difficult. This study was undertaken to evaluate the short-term efficacy of entecavir versus lamivudine on hepatitis B e antigen (HBeAg)-negative patients with ACLF. METHODS: The data of 182 HBeAg-negative patients with ACLF were retrospectively collected from patient profiles of the hospital. In these patients, 93 HBeAg-negative patients with ACLF were treated orally with 0.5 mg of entecavir and 89 were treated orally with 100 mg of lamivudine every day. The gender and age were matched between the two groups. Biochemical items, the model for end-stage liver disease (MELD) score, and HBV DNA level were matched at baseline between the two groups and monitored during treatment. The 3-month mortalities of the two groups were compared. RESULTS: No significant differences were found in biochemical items, MELD score, and HBV DNA level at baseline (P>0.05). HBV DNA level decreased within 3 months in both groups (P<0.05), regardless of the pretreatment MELD score. In patients with the same range of pretreatment MELD scores, treatment duration, posttreatment HBV DNA levels, percentage of HBV DNA level <2.7 lg copies/mL, biochemical items, MELD scores and 3-month mortality were similar in the two groups (all P>0.05). Pretreatment MELD score was not related to posttreatment HBV DNA levels (P>0.05), but related to a 3-month mortality in both groups (both P<0.001).CONCLUSIONS: In HBeAg-negative patients with ACLF, the short-term efficacy of entecavir versus lamivudine was similar. | 8 | Early filiform needle acupuncture for poststroke depression: a meta-analysis of 17 randomized controlled clinical trials显示文摘OBJECTIVE: To evaluate the effectiveness and safety of filiform needle acupuncture for poststroke depression, and to compare acupuncture with the therapeutic efficacy of antidepressant drugs. DATA RETRIEVAL: We retrieved data from the Chinese National Knowledge Infrastructure(1979–2012), Wanfang(1980–2012), VIP(1989–2012), Chinese Biomedical Literature(1975–2012), PubMed(1966–2012), Ovid Lww(–2012), and Cochrane Library(–2012) Database using the internet. SELECTION CRITERIA: Randomized controlled trials on filiform needle acupuncture versus antidepressant drugs for treatment of poststroke depression were included. Moreover, the included articles scored at least 4 points on the Jadad scale. Exclusion criteria: other acupuncture therapies as treatment group, not stroke-induced depression patients, score < 4 points, non-randomized controlled trials, or animal trials. MAIN OUTCOME MEASURES: These were the Hamilton Depression Scale scores, clinical effective rate, Self-Rating Depression Scale scores, Side Effect Rating Scale scores, and incidence of adverse reaction and events. RESULTS: A total of 17 randomized controlled clinical trials were included. Meta-analysis results displayed that after 4 weeks of treatment, clinical effective rate was better in patients treated with filiform needle acupuncture than those treated with simple antidepressant drugs [relative risk = 1.11, 95% confidence interval(CI): 1.03–1.21, P = 0.01]. At 6 weeks, clinical effective rate was similar between filiform needle acupuncture and antidepressant drug groups. At 2 weeks after filiform needle acupuncture, Hamilton Depression Scale(17 items) scores were lower than in the antidepressant drug group(mean difference =-2.34, 95%CI:-3.46 to-1.22, P < 0.000,1). At 4 weeks, Hamilton Depression Scale(24 items) scores were similar between filiform needle acupuncture and antidepressant drug groups. Self-Rating Depression Scale scores were lower in filiform needle acupuncture group than in the antidepressant drug group. Side Effect Rating Scale was used in only two articles, and no meta-analysis was conducted. Safety evaluation of the 17 articles showed that gastrointestinal tract reactions such as nausea and vomiting were very common in the antidepressant drug group. Incidence of adverse reaction and events was very low in the filiform needle acupuncture group. CONCLUSION: Early filiform needle acupuncture for poststroke depression can perfectly control depression. Filiform needle acupuncture is safe and reliable. | 9 | Science Letters:IGFBP7 plays a potential tumor suppressor role against colorectal carcinogenesis with its expression associated with DNA hypomethylation of exon 1显示文摘Insulin-like growth factor binding-protein-7 (IGFBP7) was obtained from our previous colonic adenocarcinoma (CRC) and normal mucosa suppression subtraction hybridization (SSH) cDNA libraries. By RT-PCR and immunohistochemistry, we found that IGFBP7 was overexpressed in CRC tissue compared to normal tissue. However, our in vitro experiments performed in 10 CRC cell lines showed that IGFBP7 expressed only in SW480 and Caco2 cell lines, which implied an underlying reversible regulatory mechanism. Using methylation-specific PCR (MSP) and bisulfite sodium PCR (BSP), we found that its expression was associated with DNA hypomethylation of exon1. This was further supported by the in vitro study which showed restored IGFBP7 expression after demethylation agent 5-aza-2’-deoxycytidine treatment. Correlation analysis between IGFBP7 expression and prognosis indicated that overexpression of IGFBP7 in CRC tissue correlated with favourable survival. Investigation of the func-tional role of IGFBP7 through transfection studies showed that IGFBP7 protein could inhibit growth rate, decrease colony for-mation activity, and induce apoptosis in RKO and SW620 cells, suggesting it a potential tumor suppressor protein in colorectal carcinogenesis. | 11 | Thermodynamic conditions of framework grain dissolution of clastic rocks and its application in Kela 2 gas field显示文摘Feldspar and clastic debris are the most important constituent framework grains of sedimentary clastic rocks and their chemical dissolution plays an essential role in the formation and evolution of the secondary pore in the reservoir rocks. On the basis of thermodynamic phase equilibrium, this study investigates the chemical equilibrium relationships between fluid and various plagioclase and K-feldspar in diagenesis of the sediments, particularly, the impact of temperature and fluid compositions (pH, activity of K+, Na+, Ca2+ and so on) on precipitation and dissolution equilibria of feldspars. Feldspar is extremely easily dissolved in the acid pore water with a low salinity when temperature decreases. The dissolution of anorthite end-member of plagioclase is related to the Ca content of the mineral and the fluid, higher Ca either in the mineral or in the fluid, easier dissolution of the feldspar. Moreover, the dissolution of albite end-member of plagioclase is related to Na of both the mineral and fluid, lower Na out of both the mineral and fluid, easier dissolution of the mineral. Similarly, lower-K fluid is more powerful to dissolve K-feldspar than the higher. The anorthite component of plagioclase is most easily dissolved in ground water-rock system, the albite is the secondary, and K-feldspar is the most stable. Selective dissolution of plagioclase occurs in diagenesis because of the plagioclase solid solution that is mainly composed of albite and anorthite end-members, Ca-rich part of which is preferentially dissolved by the pore water, in contrast to the Na-rich parts. Based on investigation of reservoir quality, secondary pore, dissolution structures of feldspar, clay minerals and ground water chemistry of the Kela 2 gas field of Kuqa Depression in the Tarim basin of Western China, we discovered that the secondary pore is very well developed in the highest quality section of the reservoir, and the plagioclase of which was obviously selectively dissolved, in contrast to the overgrowth of K-feldspar. Chemistry of the ground water out of the highest quality section of the reservoir is consistent with the complete dissolution field of anorthite, partial dissolution field of albite component of plagioclase, and the precipitation of K-feldspar on the temperature-logK phase equilibrium diagram drawn from this study. The thermodynamic modeling calculation gave us an insight into formation of the secondary pore within the reservoir, and the dissolution of plagioclase an important cause of the secondary pore. | 13 | Safety evaluation for railway vehicles using an improved indirect measurement method of wheel–rail forces显示文摘The wheel–rail force measurement is of great importance to the condition monitoring and safety evaluation of railway vehicles. In this paper, an improved indirect method for wheel–rail force measurement is proposed to evaluate the running safety of railway vehicles. In this method, the equilibrium equations of a suspended wheelset are derived and the wheel–rail forces are then be obtained from measured suspension and inertia forces. This indirect method avoids structural modifications to the wheelset and is applicable to the long-term operation of railway vehicles.As the wheel–rail lateral forces at two sides of the wheelset are difficult to separate, a new derailment criterion by combined use of wheelset derailment coefficient and wheel unloading ratio is proposed. To illustrate its effectiveness,the indirect method is applied to safety evaluation of railway vehicles in different scenarios, such as the cross wind safety of a high-speed train and the safety of a metro vehicle with hunting motions. Then, the feasibility of using this method to identify wheel–rail forces for low-floor light rail vehicles with resilient wheels is discussed. The values identified by this method is compared with that by Simpack simulation for the same low-floor vehicle, which shows a good coincidence between them in the time domain of the wheelset lateral force and the wheel–rail vertical force. | 15 | 15-PGDH is reduced and induces apoptosis and cell cycle arrest in gastric carcinoma显示文摘AIM: To investigate the expression of 15-hydroxyprostaglandin dehydrogenase (15-PGDH) in human gastric cancer and it's mechanism in apoptosis and cell cycle arrest. METHODS: Expression of 15-PGDH mRNA and protein was examined by immunohistochemistry, immunocytochemistry, reverse transcriptase polymerase chain reaction (RT-PCR) and Western blotting in tissue from human gastric cancer, gastric precancerous state (gastric polyps and atrophic gastritis), normal stomach, and gastric cancer cell lines. The relationship between gastric cancer, gastric precancerous state and 15-PGDH expression was determined. The association between expression of 15-PGDH and various clinicopathological parameters in gastric cancer was evaluated. Human gastric cancer cell line SGC-7901 was transfected with 15-PGDH expression plasmids. The effect of 15-PGDH on the cell cycle was examined by flow cytometry. The effect of 15-PGDH on apoptosis was examined by transmission electron microscopy, flow cytometry and transferasemediated nick end labeling (TUNEL) assay. Expression of cell cycle (p21, p27, p16 and p53) and apoptosis (Survivin, BCL-2, BCL-XL, BAK and BAX) genes was analyzed by RT-PCR. RESULTS: Expression of 15-PGDH mRNA and protein in human gastric cancer tissues was significantly lower than in normal gastric tissues (P < 0.01). Expression in human gastric cancer cell lines MKN-28 and MKN-45 was reduced, and absent in SGC-7901 cells (P < 0.05). Reduction of 15-PGDH expression was also found in precancerous tissues, such as gastric polyps and atrophic gastritis (P < 0.01). There was a significant difference in expression of 15-PGDH among various gastric cancer pathological types (P < 0.05), with or without distant metastasis (P < 0.05) and different TNM stage (P < 0.01). Flow cytometry demonstrated a significant increase in apoptotic cells in SGC-7901 cells transfected with pcDNA3/15-PGDH plasmid for 24 and 48 h (P < 0.01), and an increased fraction of sub-G1 phase after transfection (P < 0.05). TUNEL assay showed an increased Apoptotic Index in cells overexpressing 15-PGDH (P < 0.01). After transfection, expression of proapoptotic genes, such as BAK (P < 0.05), BAX and p53 (P < 0.01), was increased. Expression of antiapoptotic genes was decreased, such as Survivin, BCL-2 and BCL-XL (P < 0.01). Expression of cyclin-dependent kinase inhibitors p21 and p16 (P < 0.01) was significantly upregulated in cells overexpressing 15-PGDH. | 17 | Defective Kernel 39 encodes a PPR protein required for seed development in maize显示文摘RNA editing is a posttranscriptional process that is important in mitochondria and plastids of higher plants. All RNA editing-specific trans-factors reported so far belong to PLS-class of pentatricopeptide repeat(PPR)proteins. Here, we report the map-based cloning and molecular characterization of a defective kernel mutant dek39 in maize. Loss of Dek39 function leads to delayed embryogenesis and endosperm development, reduced kernel size, and seedling lethality. Dek39 encodes an E subclass PPR protein that targets to both mitochondria and chloroplasts, and is involved in RNA editing in mitochondrial NADH dehydrogenase3(nad3) at nad3-247 and nad3-275. C-to-U editing of nad3-275 is not conserved and even lost in Arabidopsis, consistent with the idea that no close DEK39 homologs are present in Arabidopsis. However, the amino acids generated by editing nad3-247 and nad3-275 are highly conserved in many other plant species, and the reductions of editing at these two sites decrease the activity of mitochondria NADH dehydrogenase complex I,indicating that the alteration of amino acid sequence is necessary for Nad3 function. | 18 | Validity assessment and determination of the cutoff value for the Index of Complexity, Outcome and Need among 12-13 year-olds in Southern Chinese显示文摘To validate the use of the Index of Complexity,Outcome and Need (ICON) in assessing orthodontic treatment need among 12-13 year-olds in southern China,we determined the threshold value of ICON based on Chinese orthodontists' judgments. The samples consisted of 335 students in grade 7 from 16 randomly selected middle schools in Chengdu,China. Three associate professors provided ICON scores for each participant and the results were compared with the gold standard judgments from 25 experts on treatment needs. Based on the gold standard,195 casts belonged to the treatment category,while the rest 140 belonged to the no-treatment category. With the international cutoff point of 43,the sensitivity and specificity of the ICON score were 0.29 and 0.98. The best compromise between sensitivity and specificity in Chengdu,compared with the gold standard,was found at a cutoff point of 29,and the sensitivity and specificity were 0.88 and 0.83. | 19 | Polymyxin for the treatment of intracranial infections of extensively drug-resistant bacteria in children after neurosurgical operation显示文摘Background Increased meningitis caused by extensively drug-resistant bacillary presents a significant challenge in antibiotic selection.The aim of our study was to evaluate the efficacy and safety of polymyxin in the treatment of post-neurosurgical meningitis due to the extensively drug-resistant bacillary in children.Methods We performed a retrospective study on post-neurosurgical meningitis caused by the extensively drug-resistant bacillary in children,who were treated with polymyxin for≥3 days.Results Among five post-neurosurgical meningitis cases that were included,the children were infected by Acinetobacter baumannii(n=3),Klebsiella pneumonia(n=l),and Pseudomonas aeruginosa(n=1).The drug susceptibility test showed that they were extensively drug-resistant bacillary.Two patients received intravenous polymyxin E.Three children received intravenous combined with intraventricular injection of polymyxin B.One patient infected by Klebsiella pneumonia eventually died of septic shock.No serious adverse effects of polymyxin were observed.Conclusions Polymyxin is a safe and effective therapy for post-neurosurgical,multidrug-resistant bacillary meningitis in children. | Jing Ye Lin-Hua Tan Zhi-Peng Shen Yun-Song Yu Deng-Ming Lai Jie Fan Qiang Shu | 2020 | World Journal of Pediatrics2020,16,5: | 5 |
