1Nonalcoholic fatty liver disease is a novel predictor of cardiovascular disease显示文摘AIM:To clarify whether nonalcoholic fatty liver disease(NAFLD)increases the risk of cardiovascular disease.METHODS:We carried out a prospective observational study with a total of 1637 apparently healthy Japanese men and women who were recruited from a health check-up program.NAFLD was diagnosed by abdominal ultrasonography.The metabolic syndrome(MS)was defined according to the modified National Cholesterol Education Program(NCEP)ATP Ⅲ criteria.Five years after the baseline evaluations,the incidence of cardiovascular disease was assessed by a self-administered questionnaire.RESULTS:Among 1221 participants available for outcome analyses,the incidence of cardiovascular disease was higher in 231 subjects with NAFLD at baseline(5 coronary heart disease,6 ischemic stroke,and 1 cerebral hemorrhage)than 990 subjects without NAFLD(3 coronary heart disease,6 ischemic stroke,and 1 cerebral hemorrhage).Multivariate analyses indicated that NAFLD was a predictor of cardiovascular disease independent of conventional risk factors(odds ratio 4.12,95% CI,1.58 to 10.75,P = 0.004).MS was alsoindependently associated with cardiovascular events.But simultaneous inclusion of NAFLD and MS in a multivariate model revealed that NAFLD but not MS retained a statistically significant correlation with cardiovascular disease.CONCLUSION:Although both of them were predictors of cardiovascular disease,NAFLD but not MS retained a statistically significant correlation with cardiovascular disease in a multivariate model.NAFLD is a strong predictor of cardiovascular disease and may play a central role in the cardiovascular risk of MS.Masahide Hamaguchi Takao Kojima Noriyuki Takeda Chisato Nagata Jun Takeda Hiroshi Sarui Yutaka Kawahito Naohisa Yoshida Atsushi Suetsugu Takahiro Kato Junichi Okuda Kazunori Ida Toshikazu Yoshikawa 2007World Journal of Gastroenterology2007,13,10:55
2Branched-chain amino acid treatment before transcatheter arterial chemoembolization for hepatocellular carcinoma显示文摘AIM:To examine the significance of branched-chain amino acid (BCAA) treatment before transcatheter arterial chemoembolization (TACE) for hepatocellular carcinoma (HCC).METHODS:This study included 99 patients who underwent TACE therapy for HCC at our hospital and were followed up without treatment for at least 6 mo between January 2004 and January 2010.They were divided into 2 groups:those receiving BCAA granules (n=40) or regular diet (n=59,control).Data obtained were retrospectively analyzed (prior to TACE,and 1 wk,1,3,and 6 mo after TACE) in terms of nutritional condition and clinical laboratory parameters (serum albumin level and Child-Pugh score),both of which are determinants of hepatic functional reserve.RESULTS:The BCAA group comprised 27 males and 13 females with a mean age of 69.9 ± 8.8 years.The patients of the BCAA group were classified as follows:Child-Pugh A/B/C in 22/15/3 patients,and Stage Ⅱ/Ⅲ /ⅣA HCC in 12/23/5 patients,respectively.The controlgroup comprised 32 males and 27 females with a mean age of 73.2 ± 10.1 years.In the control group,9 patients had chronic hepatitis,Child-Pugh A/B/C in 39/10/1 patients,and StageⅠ/Ⅱ/Ⅲ/ⅣA HCC in 1/11/35/12 patients,respectively.Overall,both serum albumin level and Child-Pugh score improved significantly in the BCAA group as compared with the control 3 and 6 mo after TACE (P < 0.05).Further analysis was performed by the following categorization:(1) child-Pugh classification;(2) liver cirrhosis subgroup with a serum albumin level > 3.5 g/dL;and (3) epirubicin dose.A similar trend indicating a significant improvement of all variables in the BCAA group was noted (P < 0.05).CONCLUSION:Treatment with BCAA granules in patients who have undergone TACE for HCC is considered useful to maintain their hepatic functional reserve.Hiroki Nishikawa Yukio Osaki Tadashi Inuzuka Haruhiko Takeda Jun Nakajima Fumihiro Matsuda Shinichiro Henmi Azusa Sakamoto Tetsuro Ishikawa Sumio Saito Ryuichi Kita Toru Kimura 2012World Journal of Gastroenterology2012,18,12:16
3Identification of individuals with non-alcoholic fatty liver disease by the diagnostic criteria for the metabolic syndrome显示文摘AIM:To clarify the efficiency of the criterion of metabolic syndrome to detecting non-alcoholic fatty liver disease(NAFLD).METHODS:Authors performed a cross-sectional study involving participants of a medical health checkup program including abdominal ultrasonography.This study involved 11 714 apparently healthy Japanese men and women,18 to 83 years of age.NAFLD was defined by abdominal ultrasonography without an alcohol intake of more than 20 g/d,known liver disease,or current use of medication.The revised criteria of the National Cholesterol Education Program Adult Treatment PanelⅢ were used to characterize the metabolic syndrome.RESULTS:NAFLD was detected in 32.2%(95%CI:31.0%-33.5%)of men(n=1874 of 5811)and in 8.7%(95%CI:8.0%-9.5%)of women(n=514 of 5903).Among obese people,the prevalence of NAFLD was as high as 67.3%(95%CI:64.8%-69.7%)in men and 45.8%(95%CI:41.7%-50.0%)in women.Although NAFLD was thought of as being the liver phenotype of metabolic syndrome,the prevalence of the metabolic syndrome among subjects with NAFLD was low both in men and women.66.8%of men and 70.4%of women with NAFLD were not diagnosed with the metabolic syndrome.48.2%of men with NAFLD and 49.8%of women with NAFLD weren't overweight[body mass index(BMI)≥25 kg/m2].In the same way,68.6%of men with NAFLD and 37.9%of women with NAFLD weren't satisfied with abdominal classification(≥90 cm for men and≥80 cm for women).Next,authors defined it as positive at screening for NAFLD when participants satisfied at least one criterion of metabolic syndrome.The sensitivity of the definition'at least 1 criterion'was as good as 84.8%in men and 86.6%in women.Separating subjects by BMI,the sensitivity was higher in obese men and women than in non-obese men and women(92.3%vs 76.8%in men,96.1%vs 77.0%in women,respectively).CONCLUSION:Authors could determine NAFLD effectively in epidemiological study by modifying the usage of the criteria for metabolic syndrome.Masahide Hamaguchi Noriyuki Takeda Takao Kojima Akihiro Ohbora Takahiro Kato Hiroshi Sarui Michiaki Fukui Chisato Nagata Jun Takeda 2012World Journal of Gastroenterology2012,18,13:13

5Analysis of stress fractures in athletes based on our clinical experience显示文摘AIM:To analyze stress fractures in athletes based on experience from our sports medicine clinic.METHODS:We investigated the association between stress fractures and age,sex,sports level,sports activity,and skeletal site in athletes seen at our sports medicine clinic between September 1991 and April 2009.Stress fractures of the pars interarticularis were excluded from this analysis.RESULTS:During this period(18 years and 8 mo),14276 patients(9215 males and 5061 females)consulted our clinic because of sports-related injuries,and 263 patients(1.8%)[171 males(1.9%)and 92 females(1.8%)]sustained stress fractures.The average age of the patients with stress fractures was 20.2 years(range 10-46 years);112 patients(42.6%)were 15-19 years of age and 90(34.2%)were 20-24 years of age.Altogether,90 patients(34.2%)were active at a high recreational level and 173(65.8%)at a competitive level.The highest proportion of stress fractures was seen in basketball athletes(21.3%),followed by baseball(13.7%),track and field(11.4%),rowing(9.5%),soccer(8.4%),aerobics(5.3%),and classical ballet(4.9%).The most common sites of stress fractures in these patients were the tibia(44.1%),followed by the rib(14.1%),metatarsal bone(12.9%),ulnar olecranon(8.7%)and pelvis(8.4%).The sites of the stress fractures varied from sport to sport.The ulnar olecranon was the most common stress fracture site in baseball players,and the rib was the most common in rowers.Basketball and classical ballet athletes predominantly sustained stress fractures of the tibia and metatarsal bone.Track and field and soccer athletes predominantly sustained stress fractures of the tibia and pubic bone.Aerobics athletes predominantly sustained stress fractures of the tibia.Middle and long distance female runners who sustained multiple stress fractures had the female athlete triad.CONCLUSION:The results of this analysis showed that stress fractures were seen in high-level young athletes,with similar proportions for males and females,and that particular sports were associated with specific sites for stress fractures.Middle and long distance female runners who suffered from multiple stress fractures had the female athlete triad.Jun Iwamoto Yoshihiro Sato Tsuyoshi Takeda Hideo Matsumoto 2011World Journal of Orthopedics2011,2,1:4
6Polaprezinc protects human colon cells from oxidative injury induced by hydrogen peroxide:Relevant to cytoprotective heat shock proteins显示文摘AIM: To investigate the effect of polaprezinc on cellular damage induced by hydrogen peroxide (H2O2) in human colon CaCo2 cells. METHODS: CaCo2 cells were treated with polaprezinc (10-100 μmol/L) for 6 h. After polaprezinc treatment, the cells were incubated with H2O2 (20 μmol/L) for 1 h. Cell viability was measured by MTT assay. Western blot analysis for heat shock protein (HSP) 27 and HSP72 in the cells was performed. Moreover, cells were pretreated with quercetin (200 μmol/L), an inhibitor of HSP synthe- sis, 2 h before polaprezinc treatment, and cell viability and the expression of HSP27 and 72 were assessed in these cells. RESULTS: Polaprezinc significantly protected CaCo2 cells from cell damage induced by H2O2, and up-regulated the expressions of HSP27 and HSP72 in the cells (10, 30 and 100 μmol/L of polaprezinc; 35.0% ± 7.7%, 58.3% ± 14.6% and 64.2% ± 8.2%, respectively. P < 0.01 versus polaprezinc-nontreated cells; 6.0% ± 4.4%). Quercetin inhibited the up-regulation of HSP27 and HSP72 by pola- prezinc and diminished the protective effect of polaprez- inc against H2O2-caused injury in the cells.
