| 1 | Pathogenesis and clinical management of Helicobacter pylori gastric infection显示文摘Helicobacter pylori(H.pylori)is a gram-negative bacterium that infects approximately 4.4 billion individuals worldwide.However,its prevalence varies among different geographic areas,and is influenced by several factors.The infection can be acquired by means of oral-oral or fecal-oral transmission,and the pathogen possesses various mechanisms that improve its capacity of mobility,adherence and manipulation of the gastric microenvironment,making possible the colonization of an organ with a highly acidic lumen.In addition,H.pylori presents a large variety of virulence factors that improve its pathogenicity,of which we highlight cytotoxin associated antigen A,vacuolating cytotoxin,duodenal ulcer promoting gene A protein,outer inflammatory protein and gamma-glutamyl transpeptidase.The host immune system,mainly by means of a Th1-polarized response,also plays a crucial role in the infection course.Although most H.pylori-positive individuals remain asymptomatic,the infection predisposes the development of various clinical conditions as peptic ulcers,gastric adenocarcinomas and mucosa-associated lymphoid tissue lymphomas.Invasive and non-invasive diagnostic methods,each of them with their related advantages and limitations,have been applied in H.pylori detection.Moreover,bacterial resistance to antimicrobial therapy is a major challenge in the treatment of this infection,and new therapy alternatives are being tested to improve H.pylori eradication.Last but not least,the development of effective vaccines against H.pylori infection have been the aim of several research studies. | Breno Bittencourt de Brito Filipe Ant?nio Fran?a da Silva Aline Silva Soares Vinícius Afonso Pereira Maria Luísa Cordeiro Santos Mariana Miranda Sampaio Pedro Henrique Moreira Neves Fabrício Freire de Melo | 2019 | World Journal of Gastroenterology2019,25,37: | 84 |

| 3 | Short-term complementary and alternative medicine on quality of life in women with fibromyalgia显示文摘BACKGROUND:Fibromyalgia(FMS)is a syndrome characterized by chronic widespread musculoskeletal pain,whose etiology is not completely understood.Different therapeutic approaches have been used with inconsistent results.This observation does not invalidate the continued search for alternative treatments aimed at improving quality of life(Qo L)in FMS.OBJECTIVE:This study compared three classical traditional Chinese medicine(TCM)therapies:acupuncture(AC),electroacupuncture(EAC)and moxibustion(MX)in the management of pain and promotion of Qo L in FMS patients.DESIGN,SETTING,PARTICIPANTS AND INTERVENTIONS:A preliminary,group-assigned,comparative study enrolled 30 women,mean age(46.90±9.24)years(range 20–60 years),who met the 1990 American College of Rheumatology criteria for FMS diagnosis and a pain-pressure threshold(PPT)< 4 kg/cm^2.The study was conducted in a teaching tertiary-care medical institution from May 2010 through April 2012.AC,EAC and MX were delivered for 30 min,once a week,for 8 weeks,bilaterally at Neiguan(PC6),Hegu(LI4),Yanglingquan(GB34),Sanyinjiao(SP6)and Taichong(LR3)acupoints.MAIN OUTCOME MEASURES:Each week,immediately before treatment and after treatment,subjects were tested for PPTs,Wong-Baker Faces Pain Scale(WBFPS;for pain intensity)and Medical Outcomes Study 36-item Short Form Health Survey(SF-36:for Qo L).RESULTS:There was no signifi cant improvement in pain or reduction of tender points in any of the groups studied,at the end of the 8th session.Signifi cant improvement of Qo L was perceived in vitality(after AC treatment)and in mental health(after EAC and MX treatments).CONCLUSION:TCM therapies(AC,EAC and MX)promoted an improvement in the Qo L in two areas(vitality and mental health)in FMS women.Further large-scale clinical trials are required to confi rm this effect. | Paulo Araujo Dias Andre Brito Bastos Guimaraes Andrea de Oliveira Albuquerque Karoline Lucas de Oliveira Maria Luzete Costa Cavalcante Sergio Botelho Guimaraes | 2016 | Journal of Integrative Medicine2016,14,1: | 7 |

| 11 | Non-pharmacological management of pediatric functional abdominal pain disorders:Current evidence and future perspectives显示文摘Functional abdominal pain disorders(FAPDs) are an important and prevalent cause of functional gastrointestinal disorders among children, encompassing the diagnoses of functional dyspepsia, irritable bowel syndrome, abdominal migraine, and the one not previously present in Rome Ⅲ, functional abdominal pain not otherwise specified. In the absence of sufficiently effective and safe pharmacological treatments for this public problem, non-pharmacological therapies emerge as a viable means of treating these patients, avoiding not only possible side effects, but also unnecessary prescription, since many of the pharmacological treatments prescribed do not have good efficacy when compared to placebo. Thus, the present study provides a review of current and relevant evidence on non-pharmacological management of FAPDs, covering the most commonly indicated treatments, from cognitive behavioral therapy to meditation, acupuncture, yoga, massage, spinal manipulation, moxibustion, and physical activities. In addition, this article also analyzes the quality of publications in the area, assessing whether it is possible to state if non-pharmacological therapies are viable, safe, and sufficiently well-based for an appropriate and effective prescription of these treatments. Finally, it is possible to observe an increase not only in the number of publications on the non-pharmacological treatments for FAPDs in recent years, but also an increase in the quality of these publications. | 18 | α,β-amyrin, a natural triterpenoid ameliorates L-arginine-induced acute pancreatitis in rats显示文摘AIM: To study the benef icial effects of triterpene α,β-amyrin and the underlying mechanisms in an experimental pancreatitis model. METHODS: Acute pancreatitis was induced in five groups of rats (n = 8) by L-arginine (2 × 2.5 g/kg, intraperitoneal, 1 h apart) and 1 h later, they received a single oral dose of α,β-amyrin (10, 30 and 100 mg/kg),methylprednisolone (30 mg/kg) and vehicle (3% Tween 80). A saline (0.9% NaCl) treated group served as a normal control. Efficacy was assessed at 24 h by determination of serum levels of amylase, lipase and proinflammatory cytokines [tumor necrosis factor (TNF)-α and interleukin (IL)-6], pancreatic myeloperoxidase (MPO) activity, lipid peroxidation [thiobarbituric acid reactive substances (TBARS)], nitrate/nitrite levels, and the wet weight/body weight ratio. Tissue histology and the immunoreactivity for TNF-α and inducible nitric oxide synthetase (iNOS) were performed. RESULTS: α,β-amyrin and methylprednisolone treatments significantly (P < 0.05) attenuated the L-arginine-induced increases in pancreatic wet weight/body weight ratio, and decreased the serum levels of amylase and lipase, and TNF-α and IL-6, as compared to the vehicle control. Also, pancreatic levels of MPO activity, TBARS, and nitrate/nitrite were signifi cantly lower. Histological f indings and TNF-α and iNOS immunostaining further confirmed the amelioration of pancreatic injury by α,β-amyrin.
