| 1 | MELD score,insulin-like growth factor 1 and cytokines on bone density in end-stage liver disease显示文摘AIM:To determine the contributions of insulin-like growth factor 1 (IGF-1),cytokines and liver disease severity to bone mineral density in patients pre-transplantation.METHODS:Serum IGF-1,tumor necrosis factor-α (TNFα) and interleukin 6 (IL-6) were measured and the Model for End-Stage Liver Disease (MELD) score calculated in 121 adult patients referred to a single centre for liver transplantation.Bone mineral density (BMD) of the lumbar spine and femoral neck were assessed via dual energy X-ray absorptiometry.Demographics,liver disease etiology,medication use and relevant biochemistry were recorded.RESULTS:A total of 117 subjects were included,with low BMD seen in 68.6%,irrespective of disease etiol-ogy.In multivariable analysis,low body mass index (BMI),increased bone turnover and low IGF-1 were independent predictors of low spinal bone density.At the hip,BMI,IGF-1 and vitamin D status were predictive.Despite prevalent elevations of TNFα and IL-6,levels did not correlate with degree of bone loss.The MELD score failed to predict low BMD in this pre-transplant population.CONCLUSION:Osteopenia/osteoporosis is common in advanced liver disease.Low serum IGF-1 is weakly predictive but serum cytokine and MELD score fail to predict the severity of bone disease. | Rebecca Mitchell Jill McDermid Mang M Ma Constance L Chik | 2011 | World Journal of Hepatology2011，3，6： | 5 |
| 2 | Gut microbiome in primary sclerosing cholangitis:A review显示文摘Primary sclerosing cholangitis(PSC)is a chronic cholestatic liver disease characterized by biliary inflammation and stricturing.Exploration of the pathogenesis of PSC in light of its association with inflammatory bowel disease(IBD)and the“gut-liver”axis is an emerging area of interest.A growing number of studies have begun to elucidate the role of the gut microbiota,its metabolites and its influence on host immune responses in the development of PSC and PSCIBD.Studies of the fecal microbiota have highlighted enriched levels of certain species,including Veillonella,Streptococcus and Enterococcus,among others.A heightened immune response to enteric dysbiosis and bacterial translocation have also been implicated.For example,Klebsiella pneumoniae strains derived from gnotobiotic mice transplanted with PSC-IBD microbiota were found to induce pore formation in human intestinal epithelial cells and enhanced Th17 responses.Gut microbes have additionally been hypothesized to be implicated in PSC pathogenesis through their role in the synthesis of various metabolites,including bile acids(BAs),which function as signaling molecules with important gut and hepatic effects.An expanded knowledge of the gut microbiome as it relates to PSC offers critical insight into the development of microbe-altering therapeutic interventions,such as antibiotics,nutritional interventions and fecal microbial transplantation.Some of these have already shown some preliminary evidence of benefit.Despite exciting progress in the field,much work remains to be done;areas that are particularly lacking include functional characterization of the microbiome and examination of pediatric populations.In this review,we summarize studies that have investigated the microbiome in PSC and PSC-IBD as well as putative mechanisms,including the potential role of metabolites,such as BAs.We then briefly review the evidence for interventions with microbe-altering properties for treating PSC. | Rebecca Little Eytan Wine Binita M Kamath Anne M Griffiths Amanda Ricciuto | 2020 | World Journal of Gastroenterology2020，26，21： | 5 |

| 5 | Pediatric Nd:YAG laser capsulotomy in the operating room: review of 87 cases显示文摘AIM: To report a large series of children having Nd:YAG laser capsulotomy in the operating room using the lateral decubitus position. METHODS: Medical records of children who underwent Nd:YAG laser capsulotomy in the operating room at Ann & Robert H. Lurie Children's Hospital of Chicago between September 2008 and April 2017 were reviewed. Induction of general anesthesia and intubation was performed in the supine position after which the patient was placed in lateral decubitus position. The Nd:YAG laser capsulotomy was performed using a standard protocol. At the completion of the procedure, the patient was turned back into the supine position and extubated. RESULTS: This study included 87 eyes of 60 patients. Patient's age ranged from 1 to 18 y(mean 6.4±4.1 y). In most cases(84/87, 97%), the procedure was performed under general anesthesia. In all cases, good focus on the membrane was achieved, and the procedure was performed successfully. There were no intraoperative ocular or anesthesia-related complications. | 6 | Gastrointestinal and liver disease in patients with schizophrenia:A narrative review显示文摘Schizophrenia is a severe mental illness which can have a devastating impact onan individual’s quality of life. Comorbidities are high amongst patients and lifeexpectancy is approximately 15 years less than the general population. Despite thewell-known increased mortality, little is known about the impact of gastrointestinaland liver disease on patients with schizophrenia. We aimed to reviewthe literature and to make recommendations regarding future care. Literaturesearches were performed on PubMed to identify studies related to gastrointestinaland liver disease in patients with schizophrenia. High rates of chronic liverdisease were reported, with Non-Alcoholic Fatty Liver Disease being of particularconcern;antipsychotics and metabolic syndrome were contributing factors. Ratesof acute liver failure were low but have been associated with antipsychotic useand paracetamol overdose. Coeliac disease has historically been linked to schizophrenia;however, recent research suggests that a causal link is yet to be proven.Evidence is emerging regarding the relationships between schizophrenia andpeptic ulcer disease, inflammatory bowel disease and irritable bowel syndrome;clinical vigilance regarding these conditions should be high. Patients with schizophreniapoorly engage with bowel cancer screening programmes, leading to latediagnosis and increased mortality. Clozapine induced constipation is a significantissue for many patients and requires close monitoring.
