| 18 | Slope stability analysis of Balia Nala landslide, Kumaun Lesser Himalaya, Nainital, Uttarakhand, India显示文摘Balia Nala is the outlet of the Nainital lake, flowing towards southeast direction. Presence of Nainital habitation at its right bank has high socio-economic importance. This study presents the stability analysis of a ravine/valley along Balia Nala. Variegated slates(lower Krol and upper Blaini formations) are the main rock types, wherever the outcrop does exist and rest of the area is covered by slope wash and river borne materials. Three sets of joints are presented in the area, but 4 sets of joints also exist at some locations. Nainital lake fault intersected by Manora fault from southwest direction passes through eastern side of the study area, and some small faults, which are sub-branches of Nainital lake fault, are observed(with 10 m offset) and promote the landslide in the area. This study shows that different kinds of discontinuities(joints, faults and shear zones) and rapid down cutting by the stream due to neotectonic activity affect the stability of the slope. The fragile lithology and deep V-shaped valley further accelerate the mass movement in the study area. In addition, rock mass rating(RMR), factor of safety(FOS) and graphical analysis of the joints indicate the study area as landslide-prone zone. | 19 | Field cancerisation in colorectal cancer:A new frontier or pastures past?显示文摘Despite considerable advances in our understanding of cancer biology, early diagnosis of colorectal cancer remains elusive. Based on the adenoma-carcinoma sequence, cancer develops through the progressive accumulation of mutations in key genes that regulate cell growth. However, recent mathematical modelling suggests that some of these genetic events occur prior to the development of any discernible histological abnormality. Cells acquire pro-tumourigenic mutations that are not able to produce morphological change but predispose to cancer formation. These cells can grow to form large patches of mucosa from which a cancer arises. This process has been termed 'field cancerisation'. It has received little attention in the scientific literature until recently. Several studies have now demonstrated cellular, genetic and epigenetic alterations in the macroscopically normal mucosa of colorectal cancer patients. In some reports, these changes were effectively utilised to identify patients with a neoplastic lesion suggesting potential application in the clinical setting. In this article, we present the scientific evidence to support field cancerisation in colorectal cancer and discuss important limitations that require further investigation. Characterisation of the field defect is necessary to enable early diagnosis of colorectal cancer and identify molecular targets for chemoprevention. | 20 | Amebic liver abscess: An update显示文摘Amebic liver abscess(ALA)is still a common problem in the tropical world,where it affects over three-quarters of patients with liver abscess.It is caused by an anaerobic protozoan Entamoeba hystolytica,which primarily colonises the cecum.It is a non-suppurative infection of the liver consisting primarily of dead hepatocytes and cellular debris.People of the male gender,during their reproductive years,are most prone to ALA,and this appears to be due to a poorly mounted immune response linked to serum testosterone levels.ALA is more common in the right lobe of the liver,is strongly associated with alcohol consumption,and can heal without the need for drainage.While majority of ALA patients have an uncomplicated course,a number of complications have been described,including rupture into abdomino-thoracic structures,biliary fistula,vascular thrombosis,bilio-vascular compression,and secondary bacterial infection.Based on clinico-radiological findings,a classification system for ALA has emerged recently,which can assist clinicians in making treatment decisions.Recent research has revealed the role of venous thrombosis-related ischemia in the severity of ALA.Recent years have seen the development and refinement of newer molecular diagnostic techniques that can greatly aid in overcoming the diagnostic challenge in endemic area where serology-based tests have limited accuracy.Metronidazole has been the drug of choice for ALA patients for many years.However,concerns over the resistance and adverse effects necessitate the creation of new,safe,and potent antiamebic medications.Although the indication of the drainage of uncomplicated ALA has become more clear,high-quality randomised trials are still necessary for robust conclusions.Percutaneous drainage appears to be a viable option for patients with ruptured ALA and diffuse peritonitis,for whom surgery represents a significant risk of mortality.With regard to all of the aforementioned issues,this article intends to present an updated review of ALA. | Ramesh Kumar Rishabh Patel Rajeev Nayan Priyadarshi Ruchika Narayan Tanmoy Maji Utpal Anand Jinit R Soni | 2024 | World Journal of Hepatology2024,16,3: | 0 |
