3Stool DNA methylation assays in colorectal cancer screening显示文摘Colorectal cancer(CRC) is fourth most common cancer in men and third in women worldwide. Developing a diagnostic panel of sensitive and specific biomarkers for the early detection of CRC is recognised as to be crucial for early initial diagnosis, which in turn leads to better long term survival. Most of the research on novel potential CRC biomarkers in the last 2 decades has been focussed on stool DNA analysis. In this paper, we describe the recent advances in non-invasive CRC screening and more specifically in molecular assays for aberrantly methylated BMP3 and NDRG4 promoter regions. In several research papers these markers showed superior rates for sensitivity and specificity in comparison to previously described assays. These tests detected the majority of adenomas ≥ 1 cm in size and the detection rates progressively increased with larger adenomas. The methylation status of the BMP3 and NDRG4 promoters demonstrated effective detection of neoplasms at all sites throughout the colon and was not affected by common clinical variables. Recently, a multitarget stool DNA test consisting of molecular assays for aberrantly methylated BMP3 and NDRG4 promoter regions, mutant KRAS and immunochemical assay for human haemoglobin has been made commercially available and is currently reimbursed in the United States. Although this is the most sensitive noninvasive CRC screening test, there is the need for further research in several areas- establishment of the best timeframe for repeated DNA stool testing; validation of the results in populations outside of North America; usefulness for surveillance and prognosis of patients; cost-effectiveness of DNA stool testing in real-life populations.Tanya Kadiyska Alexander Nossikoff 2015World Journal of Gastroenterology2015,21,35:9

6Curcumin reverses T cell-mediated adaptive immune dysfunctions in tumor-bearing hosts显示文摘Immune dysfunction is well documented during tumor progression and likely contributes to tumor immune evasion.CD81 cytotoxic T lymphocytes(CTLs)are involved in antigen-specific tumor destruction and CD41 T cells are essential for helping this CD81 T cell-dependent tumor eradication.Tumors often target and inhibit T-cell function to escape from immune surveillance.This dysfunction includes loss of effector and memory T cells,bias towards type 2 cytokines and expansion of T regulatory(Treg)cells.Curcumin has previously been shown to have antitumor activity and some research has addressed the immunoprotective potential of this plant-derived polyphenol in tumor-bearing hosts.Here we examined the role of curcumin in the prevention of tumor-induced dysfunction of T cell-based immune responses.We observed severe loss of both effector and memory T-cell populations,downregulation of type 1 and upregulation of type 2 immune responses and decreased proliferation of effector T cells in the presence of tumors.Curcumin,in turn,prevented this loss of T cells,expanded central memory T cell(TCM)/effector memory T cell(TEM)populations,reversed the type 2 immune bias and attenuated the tumor-induced inhibition of T-cell proliferation in tumor-bearing hosts.Further investigation revealed that tumor burden upregulated Treg cell populations and stimulated the production of the immunosuppressive cytokines transforming growth factor(TGF)-b and IL-10 in these cells.Curcumin,however,inhibited the suppressive activity of Treg cells by downregulating the production of TGF-b and IL-10 in these cells.More importantly,curcumin treatment enhanced the ability of effector T cells to kill cancer cells.Overall,our observations suggest that the unique properties of curcumin may be exploited for successful attenuation of tumor-induced suppression of cell-mediated immune responses.Sankar Bhattacharyya Dewan Md Sakib Hossain Suchismita Mohanty Gouri Sankar Sen Sreya Chattopadhyay Shuvomoy Banerjee Juni Chakraborty Kaushik Das Diptendra Sarkar Tanya Das Gaurisankar Sa 2010Cellular & Molecular Immunology2010,7,4:7
7诺西那生治疗脊髓性肌萎缩显示文摘目的观察诺西那生(Nusinersen)治疗脊髓性肌萎缩(SMA)患者的疗效和不良反应。方法回顾性分析2017年9月至2018年3月以色列施耐德儿童医学中心神经内科就诊的12例SMA患儿的临床资料。所有患儿均经基因检测证实为运动神经元存活基因(SMN)1纯合子7号外显子缺失,无SMN1杂合子突变。应用诺西那生12mg腰椎穿刺鞘内注射治疗,前4次鞘内给药为负荷剂量12mg,分别于1、15、30、60d给药,以后每4个月给予1次维持剂量12mg。于181d运用费城儿童医院神经肌肉障碍婴儿测试量表(CHOP-INTEND)和Hammersmith运动功能评分量表(HFMSE)量表评价患儿运动功能改善情况。结果治疗181d评价,早发型(SMA-Ⅰ)4例中2例CHOP-INTEND评分提高3分和5分,另2例肌张力有明显改善;晚发型(SMA-Ⅱ/SMA-Ⅲ/SMA-Ⅳ)8例HFMSE评分均有不同程度改善,最低提高3.1分,最高提高5.2分,平均提高4.1分。不良反应少且轻微。结论诺西那生治疗SMA的疗效肯定,安全性较好。陈峥 Yoram Nevo Tanya Moss 2019中国临床研究2019,32,6:7

9Ammonia-oxidizing archaea and bacteria are structured by geography in biological soil crusts across North American arid lands显示文摘Introduction:Biological soil crusts(BSCs)can dominate surface cover in dry lands worldwide,playing an integral role in arid land biogeochemistry,particularly in N fertilization through fixation and cycling.Nitrification is a characteristic and universal N transformation in BSCs that becomes important for the export of N beyond the microscopic bounds of the crust itself.The contribution of ammonia-oxidizing bacteria(AOB)in BSCs has been shown,but the role and extent of the recently discovered ammonia-oxidizing archaea(AOA)have not.Methods:We sampled various types of crusts in four desert regions across the western United States and characterized the composition and size of ammonia-oxidizing communities using clone libraries and quantitative PCR targeting the amoA gene,which codes for the ammonia monooxygenase enzyme,universally present in ammonia-oxidizing microbes.Results:All archaeal amoA sequences retrieved from BSCs belonged to the Thaumarchaeota(Nitrososphaera associated Group I.1b).Sequences from the Sonoran Desert,Colorado Plateau,and Great Basin were indistinguishable from each other but distinct from those of the Chihuahuan Desert.Based on amoA gene abundances,archaeal and bacterial ammonia oxidizers were ubiquitous in our survey,but the ratios of archaeal to bacterial ammonia oxidizers shifted from bacterially dominated in northern,cooler deserts to archaeally dominated in southern,warmer deserts.Conclusions:Archaea are shown to be potentially important biogeochemical agents of biological soil crust N cycling.Conditions associated with different types of BSCs and biogeographical factors reveal a niche differentiation between AOA and AOB,possibly driven by temperature.Yevgeniy Marusenko Scott T Bates Ian Anderson Shannon L Johnson Tanya Soule Ferran Garcia-Pichel 2013Ecological Processes2013,2,1:4

14Budd-Chiari syndrome:A single-center experience显示文摘AIM:To investigate challenges,risk factors,prognostic indicators,and treatment outcomes associated with Budd-Chiari syndrome(BCS)at a tertiary care center.METHODS:A retrospective cohort study was conducted at the University of Pennsylvania in patients with a diagnosis of BCS or hepatic vein thrombosis.All patients receiving care at the University of Pennsylvania,and who had at least 2 clinical encounters in the University of Pennsylvania Health system from January1,2008 to September 10,2013 were eligible for study inclusion.Data were extracted from the electronic medical record of each patient,and recorded in a secure Research Electronic Data Capture database.Logistic regression analyses were applied to identify predictors of outcome of liver transplant(LT)or death.RESULTS:Between January 1,2008 and September10,2013,forty-seven patients were identified.Median age was 42.4 years.Thirty-one(66.0%)were women.A majority were Caucasian(68.1%).At diagnosis,43(91.5%)patients had ascites,27(57.4%)patients had a hematologic disorder associated with a hypercoagulable state and 26(55.3%)had cirrhosis.Forty(85.1%)patients were on anticoagulation(AC),30(63.8%)of whom were maintained on warfarin.Two patients(4.3%)underwent thrombolytic therapy.A transjugular intrahepatic portosystemic shunt(TIPS)was placed in21(44.7%)patients,19(90.5%)of whom were also on AC.Twenty-one(44.7%)received AC alone.Over a median of 974 d,8(17.0%)patients received LT,and10(21.3%)died.The median time from listing to death was 26 mo[interquartile range(IQR)=16,65)].TIPS with AC was utilized more frequently in younger patients(P=0.02).Age,cirrhosis and chronic kidney disease(CKD)were significant predictors of LT or death.CONCLUSION:AC alone was employed as frequently as TIPS with AC,though the latter was used more frequently in younger patients with polycythemia vera.There were no significant differences in treatment outcome regardless of the therapeutic intervention employed.Significant predictors of poor prognosis included age,cirrhosis and CKD.Tanya M Pavri Alan Herbst Rajender Reddy Kimberly A Forde 2014World Journal of Gastroenterology2014,20,43:2

16White matter hyperintensity progression is associated with incident probable dementia or mild cognitive impairment显示文摘Background White matter hyperintensity(WMH)on brain MRI is associated with developing dementia or mild cognitive impairment(MCI),but WMH progression over time has not been fully investigated as an independent risk factor.Methods We performed a post hoc analysis of the Systolic Blood Pressure Intervention Trial-Memory and Cognition in Decreased Hypertension(SPRINT MIND)trial.The primary outcome was incident probable dementia or MCI(dementia/MCI)before the follow-up MRI at 48 months from enrolment.The primary predictor was WMH progression,defined as the Z score difference between the follow-up and baseline WMH volumes.The secondary predictor was a binary WMH progression threshold(≥1.4 mL vs<1.4 mL).Results Among the 433 included patients,33(7.6%)developed dementia/MCI.There were 156(36.0%)patients who met the WMH progression threshold of≥1.4 mL,in whom the rate of dementia/MCI was 12.8%(20/156)vs 4.7%(13/277)of patients with<1.4 mL WMH progression(p=0.002).In multivariable logistic regression,the Z score of WMH progression was associated with dementia/MCI(OR 1.51,95%CI 1.12 to 2.04,p=0.007)as was the WMH progression threshold of≥1.4 mL(OR 2.89,95%CI 1.23 to 6.81,p=0.015).Conclusions In this post hoc analysis of SPRINT MIND,WMH progression over 48 months was associated with the development of probable dementia or MCI.Adam de Havenon Kevin N Sheth Sharon D Yeatts Tanya N Turan Shyam Prabhakaran 2022Stroke & Vascular Neurology2022,7,4:2

19Role of TNFSF15 in the intestinal inflammatory response显示文摘Gastrointestinal diseases, specifically Crohn's disease, ulcerative colitis, diverticular disease, and primary biliary cirrhosis are all characterized by complicated inflammation of the digestive tract. Their pathology is multifactorial, and risk factors encompass both genetic and environmental factors. Recent advances in the genetic component of inflammatory bowel diseases(IBDs) have revealed that the tumor necrosis factor superfamily member 15(TNFSF15) contains a number of risk alleles associated not only with IBD but also with other diseases such as diverticular disease and primary biliary cirrhosis. These risk alleles in TNFSF15 and the altered expression of its gene product can serve as the common ground between these disorders by explaining at least some of the underlying processes that lead to a dysregulated immune response and subsequent chronic inflammation. Here, we aim to outline how the TNFSF15 gene is involved in the proliferation and cell fate of different populations of T cells and subsequently in the control of both pro-and anti-inflammatory cytokines.
