| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 5 | A Review on Cable-driven Parallel Robots显示文摘Cable-driven parallel robots(CDPRs) are categorized as a type of parallel manipulators. In CDPRs, flexible cables are used to take the place of rigid links. The particular property of cables provides CDPRs several advantages, including larger workspaces, higher payload-to-weight ratio and lower manufacturing costs rather than rigid-link robots. In this paper, the history of the development of CDPRs is introduced and several successful latest application cases of CDPRs are presented. The theory development of CDPRs is introduced focusing on design, performance analysis and control theory. Research on CDPRs gains wide attention and is highly motivated by the modern engineering demand for large load capacity and workspace. A number of exciting advances in CDPRs are summarized in this paper since it is proposed in the 1980 s, which points to a fruitful future both in theory and application. | 6 | Genotypic Characterization of Methicillin-resistant Staphylococcus aureus Isolated from Pigs and Retail Foods in China显示文摘Objective To investigate the genotypic diversity of Methicillin-resistant Staphylococcus aureus(MRSA) isolated from pigs and retail foods from different geographical areas in China and further to study the routes and rates of transmission of this pathogen from animals to food. Methods Seventy-one MRSA isolates were obtained from pigs and retail foods and then characterized by multi-locus sequencing typing(MLST), spa typing, multiple-locus variable number of tandem repeat analysis(MLVA), pulsed-field gel electrophoresis(PFGE), and antimicrobial susceptibility testing. Results All isolated MRSA exhibited multi-drug resistance(MDR). Greater diversity was found in food-associated MRSA(7 STs, 8 spa types, and 10 MLVA patterns) compared to pig-associated MRSA(3 STs, 1 spa type, and 6 MLVA patterns). PFGE patterns were more diverse for pig-associated MRSA than those of food-associated isolates(40 vs. 11 pulse types). Among the pig-associated isolates, CC9-ST9-t899-MC2236 was the most prevalent clone(96.4%), and CC9-ST9-t437-MC621(20.0%) was the predominant clone among the food-associated isolates. The CC9-ST9 isolates showed significantly higher antimicrobial resistance than other clones. Interestingly, CC398-ST398-t034 clone was identified from both pig-and food-associated isolates. Of note, some community-and hospital-associated MRSA strains(t030, t172, t1244, and t4549) were also identified as food-associated isolates. Conclusion CC9-ST9-t899-MC2236-MDR was the most predominant clone in pigs, but significant genetic diversity was observed in food-associated MRSA. | 9 | Handgrip Strength as a Predictor of Nutritional Status in Chinese Elderly Inpatients at Hospital Admission显示文摘Objective To assess nutritional status and define gender-and age-specific handgrip strength(HGS) cut-point values for malnutrition or nutritional risk in elderly inpatients. Methods A cross-sectional study of 1,343 elderly inpatients was conducted in the Chinese PLA General Hospital. Nutrition Risk Screening(NRS 2002) and Subjective Global Assessment(SGA) were administered. Anthropometric measurements and blood biochemical indicators were obtained using standard techniques. The gender-and age-specific receiver operating characteristic(ROC) curves were constructed to evaluate the HGS for nutritional status by SGA and NRS 2002. Sensitivity, specificity, and areas under the curves(AUCs) were calculated. Results According to NRS 2002 and SGA, 63.81% of elderly inpatients were at nutritional risk and 28.22% were malnourished. Patients with higher HGS had an independently decreased risk of malnutrition and nutritional risk. The AUCs varied between 0.670 and 0.761. According to NRS 2002, the optimal HGS cut-points were 27.5 kg(65-74 years) and 21.0 kg(75-90 years) for men and 17.0 kg(65-74 years) and 14.6 kg(75-90 years) for women. According to SGA, the optimal HGS cut-points were 24.9 kg(65-74 years) and 20.8 kg(75-90 years) for men and 15.2 kg(65-74 years) and 13.5 kg(75-90 years) for women. Conclusion Elderly inpatients had increased incidence of malnutrition or nutritional risk. | 12 | Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. | 15 | Comparison of Histological, Microbiological, and Molecular Methods in Diagnosis of Patients with TBLN Having Different Anti-TB Treatment Background显示文摘Objective The influence of anti-tuberculosis(TB) treatment history on tuberculous lymphadenitis(TBLN) diagnosis is unclear. Therefore, this study aims to evaluate the diagnostic methods, including histology, microbiology, and molecular tests, used for TBLN. Methods In this study, suspected patients with TBLN and having different anti-TB treatment background were enrolled. All the samples were tested simultaneously by histology, Ziehl-Neelsen(ZN) staining, mycobacterial culture(culture), Xpert MTB/RIF(xpert), real-time PCR, and high-resolution melting curve PCR(HRM). Thereafter, the performance of these methods on samples with different anti-TB treatment background was assessed. Results In our study, 89 patients were prospectively included 82 patients with TBLN and 7 with other diseases. The overall sensitivities of Xpert, real-time PCR, histology, ZN staining, and culture were 86.6%, 69.5%, 58.5%, 43.9%, and 22.0%, respectively. The anti-TB treatment history revealed dramatic influences on the sensitivity of culture(P < 0.0001). In fact, the treatment that lasted over 3 months also influenced the sensitivity of Xpert(P < 0.05). However, the treatment history did not affect the performance of remaining tests(P > 0.05). For rifampicin drug susceptibility test(DST), the anti-TB treatment showed only significant influence on the success rate of culture DST(P = 0.001), but not on those of Xpert and HRM tests(P > 0.05). | 16 | Association between socioeconomic status and chronic obstructive pulmonary disease in Jiangsu province, China: a population-based study显示文摘Background:Chronic obstructive pulmonary disease(COPD)is a common public health problem worldwide.Recent studies have reported that socioeconomic status(SES)is related to the incidence of COPD.This study aimed to investigate the association between SES and COPD among adults in Jiangsu province,China,and to determine the possible direct and indirect effects of SES on the morbidity of COPD.Methods:A cross-sectional study was conducted among adults aged 40 years and above between May and December of 2015 in Jiangsu province,China.Participants were selected using a multistage sampling approach.COPD,the outcome variable,was diagnosed by physicians based on spirometry,respiratory symptoms,and risk factors.Education,occupation,and monthly family average income(FAI)were used to separately indicate SES as the explanatory variable.Mixed-effects logistic regression models were introduced to calculate odds ratios(ORs)and 95%confidence intervals(CIs)for examining the SES-COPD relationship.A pathway analysis was conducted to further explore the pulmonary function impairment of patients with different SES.Results:The mean age of the 2421 participants was 56.63±9.62 years.The prevalence of COPD was 11.8%(95%CI:10.5%–13.1%)among the overall sample population.After adjustment for age,gender,residence,outdoor and indoor air pollution,body weight status,cigarette smoking,and potential study area-level clustering effects,educational attainment was negatively associated with COPD prevalence in men;white collars were at lower risk(OR:0.60,95%CI:0.43–0.83)of experiencing COPD than blue collars;compared with those within the lower FAI subgroup,participants in the upper(OR:0.68,95%CI:0.49–0.97)tertiles were less likely to experience COPD.Such negative associations between all these three SES indicators and COPD were significant among men only.Education,FAI,and occupation had direct or indirect effects on pulmonary function including post-bronchodilator forced expiratory volume in 1 s/forced vital capacity(FEV1/FVC),FEV1,FVC,and FEV1 percentage of predicted.Education,FAI,and occupation had indirect effects on pulmonary function indices of all participants mainly through smoking status,indoor air pollution,and outdoor air pollution.We also found that occupation could affect post-bronchodilator FEV1/FVC through body mass index.Conclusions:Education,occupation,and FAI had an adverse relationship with COPD prevalence in Jiangsu province,China.SES has both direct and indirect associations with pulmonary function impairment.SES is of great significance for COPD morbidity.It is important that population-based COPD prevention strategies should be tailored for people with different SES. | Dan-Dan Zhang Jian-Nan Liu Qing Ye Zi Chen Ling Wu Xue-Qing Peng Gan Lu Jin-Yi Zhou Ran Tao Zhen Ding Fei Xu Linfu Zhou | 2021 | Chinese Medical Journal2021,,13: | 5 |

| 20 | Exposure Effects of Terahertz Waves on Primary Neurons and Neuron-like Cells Under Nonthermal Conditions显示文摘Objective This study aimed to explore the potential effects of terahertz(THz) waves on primary cultured neurons from 4 rat brain regions(hippocampus, cerebral cortex, cerebellum, and brainstem)and 3 kinds of neuron-like cells(MN9 D, PC12, and HT22 cells) under nonthermal conditions.Methods THz waves with an output power of 50(0.16 THz) and 10(0.17 THz) m W with exposure times of 6 and 60 min were used in this study. Analysis of temperature change, neurite growth, cell membrane roughness, micromorphology, neurotransmitters and synaptic-related proteins(SYN and PSD95) was used to evaluate the potential effects.Results Temperature increase caused by the THz wave was negligible. THz waves induced significant neurotransmitter changes in primary hippocampal, cerebellar, and brainstem neurons and in MN9 D and PC12 cells. THz wave downregulated SYN expression in primary hippocampal neurons and downregulated PSD95 expression in primary cortical neurons.Conclusion Different types of cells responded differently after THz wave exposure, and primary hippocampal and cortical neurons and MN9 D cells were relatively sensitive to the THz waves.
