| 2 | Gastric submucosa-invasive carcinoma associated with EpsteinBarr virus and endoscopic submucosal dissection: A case report显示文摘BACKGROUND Epstein-Barr virus(EBV)-associated carcinoma is a gastric cancer subtype with a morphology characterized by gastric carcinoma with lymphoid stroma(GCLS).Clinicopathological studies have indicated a better prognosis for GCLS than for common gastric carcinomas.Some previous cases of early gastric cancer associated with EBV had been diagnosed by endoscopic resection.CASE SUMMARY We present two GCLS cases subjected to endoscopic submucosal dissection(ESD)for a definitive diagnosis.A protruded gastric lesion was identified by routine endoscopic examination,but forceps biopsy showed no atypical cells before ESD.The resected specimen showed a poorly differentiated adenocarcinoma with lymphoid cells involving the mucosa and submucosa.The final diagnosis was submucosa-invasive poorly differentiated gastric adenocarcinoma.Accordingly,additional gastrectomy was recommended to obtain a complete cure.One patient underwent additional distal gastrectomy with lymph node dissection,but the other was refused because of cardiovascular complications.Both patients remained in remission for more than half a year.EBV positivity was determined by EBV-encoded RNA in situ hybridization.We also conducted a literature review of cases of early gastric cancer associated with EBV that had been diagnosed by ESD.CONCLUSION Submucosa-invasive GCLS could be dissected using ESD,and EBV positivity should be subsequently assessed to determine whether or not any additional curative surgery is required.Further prospective investigations on the prevalence of lymph node metastasis in EBV-associated carcinoma should be performed to expand the indications for endoscopic resection. | Yu Kobayashi Takehito Kunogi Hiroki Tanabe Yuki Murakami Takuya Iwama Takahiro Sasaki Keitaro Takahashi Katsuyoshi Ando Yoshiki Nomura Nobuhiro Ueno Shin Kashima Kentaro Moriichi Hidehiro Takei Mikihiro Fujiya Toshikatsu Okumura | 2019 | World Journal of Gastrointestinal Oncology2019,11,10: | 2 |
| 3 | Deep-learning-based quality filtering of mechanically exfoliated 2D crystals显示文摘Two-dimensional(2D)crystals are attracting growing interest in various research fields such as engineering,physics,chemistry,pharmacy,and biology owing to their low dimensionality and dramatic change of properties compared to the bulk counter parts.Among the various techniques used to manufacture 2D crystals,mechanical exfoliation has been essential to practical applications and fundamental research.However,mechanically exfoliated crystals on substrates contain relatively thick flakes that must be found and removed manually,limiting high-throughput manufacturing of atomic 2D crystals and van der Waals heterostructures.Here,we present a deep-learning-based method to segment and identify the thickness of atomic layer flakes from optical microscopy images.Through carefully designing a neural network based on U-Net,we found that our neural network based on Unet trained only with the data based on realistically small number of images successfully distinguish monolayer and bilayer MoS2 and graphene with a success rate of 70–80%,which is a practical value in the first screening process for choosing monolayer and bilayer flakes of all flakes on substrates without human eye.The remarkable results highlight the possibility that a large fraction of manual laboratory work can be replaced by AI-based systems,boosting productivity. | Yu Saito Kento Shin Kei Terayama Shaan Desai Masaru Onga Yuji Nakagawa Yuki M.Itahashi Yoshihiro Iwasa Makoto Yamada Koji Tsuda | 2019 | npj Computational Materials2019,,1: | 1 |

| 5 | Surgical resection for pulmonary recurrence of esophageal cancer after curative esophagectomy显示文摘Aim:To clarify the significance of surgical resection for pulmonary recurrence after curative esophagectomy for esophageal cancer.Methods:Clinical details,such as the recurrence site,timing,and contents of therapies for recurrence,and the prognosis,were examined in 14 patients who underwent surgical resection for pulmonary recurrence that developed after curative esophagectomy.Results:The median disease-free interval after esophagectomy was 17.2 months.Two patients underwent pulmonary resection two times,and in one patient,three times.All pulmonary resections were performed when other extra-pulmonary recurrences had been controlled,and R0 resection was achieved.Chemotherapy and/or radiotherapy were additionally performed for pulmonary metastasis in 13 patients.The median survival time after initial pulmonary resection was 45.5 months,and the 1-,3-,and 5-year overall survival rates were 93%,68%,and 43%,respectively.The 5-year overall survival rate after initial pulmonary resection was 13%in patients with Stage III or IV esophageal cancer and 100%in those with Stage I or II disease(P=0.010).The rate was 56%in patients with tumors<20 mm in size,while all 5 patients with lesions≥20 mm in size died within 3 years(P=0.005).Conclusion:Surgical resection along with systemic therapy is a promising treatment strategy for pulmonary recurrence after curative esophagectomy when it is solitary and localized.Clinical factors,such as the esophageal cancer stage and the size of the pulmonary metastasis,are useful for deciding on the surgical indication. | Masaru Morita Manabu Yamamoto Yuichiro Nakashima Keiichi Shiokawa Yuki Shin Yoshiaki Fujimoto Tomonori Nakanoko Hideo Uehara Masahiko Sugiyama Mitsuhiko Ota Yohei Mano Keishi Sugimachi Tatsuro Okamoto Yasushi Toh | 2021 | Journal of Cancer Metastasis and Treatment2021,7,1: | 0 |
| 6 | Superiority of urgent vs early endoscopic hemostasis in patients with upper gastrointestinal bleeding with high-risk stigmata显示文摘Background Guidelines recommend that all patients with upper gastrointestinal bleeding(UGIB)undergo endoscopy within 24 h.It is unclear whether a subgroup may benefit from an urgent intervention.We aimed to evaluate the influence of endoscopic hemostasis and urgent endoscopy on mortality in UGIB patients with high-risk stigmata(HRS).Methods Consecutive patients with suspected UGIB were enrolled in three Japanese hospitals with a policy to perform endoscopy within 24 h.The primary outcome was 30-day mortality.Endoscopic hemostasis and endoscopy timing(urgent,6h;early,>6h)were evaluated in a regression model adjusting for age,systolic pressure,heart rate,hemoglobin,creatinine,and variceal bleeding in multivariate analysis.A propensity score of 1:1 matched sensitivity analysis was also performed.Results HRS were present in 886 of 1966 patients,and 35 of 886(3.95%)patients perished.Median urgent-endoscopy time(n=769)was 3.0h(interquartile range[IQR],2.0–4.0 h)and early endoscopy(n紏117)was 12.0h(IQR,8.5–19.0 h).Successful endoscopic hemostasis and urgent endoscopy were significantly associated with reduced mortality in multivariable analysis(odds ratio[OR],0.22;95%confidence interval[CI],0.09–0.52;P=0.0006,and OR,0.37;95%CI,0.16–0.87;P=0.023,respectively).In a propensity-score-matched analysis of 115 pairs,adjusted comparisons showed significantly lower mortality of urgent vs early endoscopy(2.61%vs 7.83%,P<0.001).Conclusions A subgroup of UGIB patients,namely those harboring HRS,may benefit from endoscopic hemostasis and urgent endoscopy rather than early endoscopy in reducing mortality.Implementing triage scores that predict the presence of such lesions is important. | Masayasu Horibe Eisuke Iwasaki Juntaro Matsuzaki Fateh Bazerbachi Tetsuji Kaneko Kazuhiro Minami Seiichiro Fukuhara Tatsuhiro Masaoka Naoki Hosoe Yuki Ogura Shin Namiki Yasuo Hosoda Haruhiko Ogata Takanori Kanai | 2021 | Gastroenterology Report2021,9,6: | 0 |

| 8 | Decline in subarachnoid haemorrhage volumes associated with the first wave of the COVID- 19 pandemic显示文摘Background During the COVID-19 pandemic,decreased volumes of stroke admissions and mechanical thrombectomy were reported.The study’s objective was to examine whether subarachnoid haemorrhage(SAH)hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.Methods We conducted a cross-sectional,retrospective,observational study across 6 continents,37 countries and 140 comprehensive stroke centres.Patients with the diagnosis of SAH,aneurysmal SAH,ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases,10th Revision,codes.The 3-month cumulative volume,monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before(1 year and immediately before)and during the pandemic,defined as 1 March-31 May 2020.The prior 1-year control period(1 March-31 May 2019)was obtained to account for seasonal variation.Findings There was a significant decline in SAH hospitalisations,with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic,representing a relative decline of 22.5%(95%CI−24.3%to−20.7%,p<0.0001).Embolisation of ruptured aneurysms declined with 1170-1035 procedures,respectively,representing an 11.5%(95%CI−13.5%to−9.8%,p=0.002)relative drop.Subgroup analysis was noted for aneurysmal SAH hospitalisation decline from 834 to 626 hospitalisations,a 24.9%relative decline(95%CI−28.0%to−22.1%,p<0.0001).A relative increase in ruptured aneurysm coiling was noted in low coiling volume hospitals of 41.1%(95%CI 32.3%to 50.6%,p=0.008)despite a decrease in SAH admissions in this tertile.Interpretation There was a relative decrease in the volume of SAH hospitalisations,aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the | 9 | Natural interferon-beta treatment for patients with chronic hepatitis C in Japan显示文摘Chronic hepatitis C virus(HCV) infection can cause liver cirrhosis and hepatocellular carcinoma(HCC). Several studies have demonstrated that the eradication of HCV reduces the occurrence of HCC. In Japan, as many people live to an advanced age, HCV-infected patients are also getting older, and the age at HCC diagnosis has also increased. Although older HCV-infected patients have a risk of developing HCC, the treatment response to peginterferon-alpha plus ribavirin therapy is relatively poor in these patients because of drop-out or discontinuation of this treatment due to adverse events. It is established that the mechanism of action between interferon-alpha and interferon-beta is slightly different. Short-term natural interferon-beta monotherapy is effective for patients with acute hepatitis C and patients infected with HCV genotype 2 and low viral loads. Natural interferon-beta plus ribavirin for 48 wk or for 24 wk are also effective for some patients with HCV genotype 1 or HCV genotype 2. Natural interferon-beta plus ribavirin has been used for certain 'difficult-totreat' HCV-infected patients. In the era of direct-acting anti-virals, natural interferon-beta plus ribavirin may be one of the therapeutic options for special groups of HCV-infected patients.
