| 1 | Distinctive inflammatory bowel disease phenotype in primary sclerosing cholangitis显示文摘AIM:To review the current literature for the specificclinical characteristics of inflammatory bowel disease(IBD)associated with primary sclerosing cholangitis(PSC).METHODS:A systematical review for clinical characteristics of IBD in PSC was performed by conducting a broad search for'primary sclerosing cholangitis'in Pubmed.'Clinical characteristics'were specified into five predefined subthemes:epidemiology of IBD in PSC,characteristics of IBD in PSC(i.e.,location,disease behavior),risk of colorectal cancer development,IBD recurrence and de novo disease after liver transplantation for PSC,and safety and complications after proctocolectomy with ileal pouchanal anastomosis.Papers were selected for inclusion based on their relevance to the subthemes,and were reviewed by two independent reviewers.Only full papers relevant to PSC-IBD were included.Additionally the references of recent reviews for PSC(<5 years old)were scrutinized for relevant articles.RESULTS:Initial literature search for PSC yielded 4704results.After careful review 65 papers,comprising a total of 11406 PSC-IBD patients,were selected and divided according to subtheme.Four manuscripts overlapped and were included in two subthemes.Prevalence of IBD in PSC shows a large variance,ranging from 46.5%to 98.7%with ulcerative colitis(UC)being the most common type(>75%).The highest IBD rates in PSC are found in papers reviewing both endoscopic and histological data for IBD diagnosis.Although IBD in PSC is found to be a quiescent disease,pancolitis occurs often,with rates varying from 35%to 95%.Both backwash ileitis and rectal sparing are observed infrequently.The development of dysplasia or colorectal carcinoma is increased in PSC-IBD;the cumulative 10 years risk varying between 0%and11%.Exacerbation of IBD is common after liver transplantation for PSC and de novo disease is seen in1.3%to 31.3%of PSC-IBD patients.The risk for development of pouchitis in PSC-IBD is found to besignificant,affecting 13.8%to 90%of the patients after proctocolectomy with ileo anal-pouch anastomosis.CONCLUSION:IBD in primary sclerosing cholangitis represents a distinct phenotype that differs from UC and Crohn’s disease and therefore requires specialized management. | A Boudewijn de Vries Marcel Janse Hans Blokzijl Rinse K Weersma | 2015 | World Journal of Gastroenterology2015，21，6： | 17 |
| 2 | European cooperation to tackle the legacies of hexachlorocyclohexane(HCH)and lindane显示文摘Hexachlorocyclohexane(HCH)waste isomers from lindane production are the largest single POPs legacy,with an estimated 4.8 to 7.4 million tonnes of disposed waste.The largest part of this waste e 1.8 to 3 million tonnes e was disposed in Europe,where most producers were located.This paper provides a short overview of projects supported by the European Union(EU)to address this waste legacy and to implement the Stockholm Convention for this group of POPs with associated protection of soil,ecosystems and human health.We report here particularly on the results of a project financed by the EU called the“HCH in EU project”,which aimed to develop a systematic inventory of sites where HCH was handled and potentially resulted in contamination.The compiled information provide guidance for competent authorities to further assess their national HCH inventory and to further develop a strategy to address this large POP legacy in future.The systematic inventory revealed that there were at least 299 sites where HCH was handled.These sites include 54 former production sites,76 pesticide processing plants that used lindane,59 uncontrolled HCH waste isomer deposits,29 landfills with HCH waste,34 former or current storage sites for stocks of obsolete pesticides including technical HCH or lindane,and 16 HCH treatment or disposal sites.Additionally,at 31 of the sites lindane/technical HCH was used in applications with significant risk of soil pollution,such as wood treatment.The number of sites in this latter category is likely higher and will need further assessment.In addition to this inventory,the“HCH in EU project”produced detailed country reports,a guidance document for how to find potentially HCHimpacted sites,and a strategy document for implementing the sustainable management of these sites EU-wide,with proposed actions at the EU,country,and site level.Furthermore,the project has facilitated information exchange and e together with other related EU projects e has led to sharing information and best practices among member states and to establishing a network of authorities and other stakeholders working on the lindane/HCH waste legacy.This collaboration will facilitate a more systematic and better coordinated process to further assess,secure,and remediate the large HCH waste legacy and reduce and control lindane/HCH releases in the EU and possibly beyond.Such a coordinated effort and exchange of information for inventorying and managing contaminated sites might also be useful for other POPs such as PFOS/PFOA or dioxins. | John Vijgen Boudewijn Fokke Guido van de Coterlet Katja Amstaetter Javier Sancho Carlo Bensaïah Roland Weber | 2022 | Emerging Contaminants2022，8，1： | 4 |
| 3 | Diagnostic value of drain amylase for detecting intrathoracic leakage after esophagectomy显示文摘AIM:To investigate the value of elevated drain amylase concentrations for detecting anastomotic leakage(AL) after minimally invasive Ivor-Lewis esophagectomy(MIILE).METHODS:This was a retrospective analysis of prospectively collected data in two hospitals in the Netherlands. Consecutive patients undergoing MI-ILE were included. A Jackson-Pratt drain next to the dorsal side of the anastomosis and bilateral chest drains were placed at the end of the thoracoscopic procedure. Amylase levels in drain fluid were determined in all patients during at least the first four postoperative days. Contrast computed tomography scans and/or endoscopic imaging were performed in cases of a clinically suspected AL. Anastomotic leakage was defined as any sign of leakage of the esophago-gastric anastomosis on endoscopy,re-operation,radiographic investigations,post mortal examination or when gastro-intestinal contents were found in drain fluid. Receiver operator characteristic curves were used to determine the cut-off values. Sensitivity,specificity,positive predictive value,negative predictive value,risk ratio and overall test accuracy were calculated for elevated drain amylase concentrations.RESULTS:A t o t a l o f 8 9 p a t ie n t s w e re in c lu d e d between March 2013 and August 2014. No differences in group characteristics were observed between patients with and without AL,except for age. Patients with AL were older than were patients without AL(P = 0.01). One patient(1.1%) without AL died within 30 d after surgery due to pneumonia and acute respiratory distress syndrome. Anastomotic leakage that required any intervention occurred in 15 patients(16.9%). Patients with proven anastomotic leakage had higher drain amylase levels than patients without anastomotic leakage [median 384 IU/L(IQR 34-6263) vs median 37 IU/L(IQR 26-66),P = 0.003]. Optimal cut-off values on postoperative days 1,2,and 3 were 350 IU/L,200 IU/L and 160 IU/L,respectively. An elevated amylase level was found in 9 of the 15 patients with AL. Five of these 9 patients had early elevations of their amylase levels,with a median of 2 d(IQR 2-5) before signs and symptoms occurred.CONCLUSION:Measurement of drain amylase levels is an inexpensive and easy tool that may be used to screen for anastomotic leakage soon after MI-ILE.
