| 2 | Early enteral nutrition vs parenteral nutrition following pancreaticoduodenectomy: Experience from a single center显示文摘AIM: To analyze and compare postoperative morbidity between patients receiving total parenteral nutrition(TPN) and early enteral nutrition supplemented with parenteral nutrition(EEN + PN).METHODS: Three hundred and forty patients receiving pancreaticoduodenectomy(PD) from 2009 to 2013 at our center were enrolled retrospectively. Patients were divided into two groups depending on postoperative nutrition support scheme: an EEN + PN group(n = 87) and a TPN group(n = 253). Demographic characteristics, comorbidities, preoperative biochemicalparameters, pathological diagnosis, intraoperative information, and postoperative complications of the two groups were analyzed. RESULTS: The two groups did not differ in demographic characteristics, preoperative comorbidities, preoperative biochemical parameters or pathological findings(P > 0.05 for all). However, patients with EEN + PN following PD had a higher incidence of delayed gastric emptying(16.1% vs 6.7%, P = 0.016), pulmonary infection(10.3% vs 3.6%, P = 0.024), and probably intraperitoneal infection(18.4% vs 10.3%, P = 0.059), which might account for their longer nasogastric tube retention time(9 d vs 5 d, P = 0.006), postoperative hospital stay(25 d vs 20 d, P = 0.055) and higher hospitalization expenses(USD10397 vs USD8663.9, P = 0.008), compared to those with TPN.CONCLUSION: Our study suggests that TPN might be safe and sufficient for patient recovery after PD. | 5 | Outcome of multimodal MRI-guided intravenous thrombolysis in patients with stroke with unknown time of onset显示文摘Objective Intravenous tissue plasminogen activator(tPA)is the standard therapy for patients with acute ischaemic stroke(AIS)within 4.5 hours of onset.Recent trials have expanded the endovascular treatment window to 24 hours.We investigated the efficacy and safety of using multimodal MRI to guide intravenous tPA treatment for patients with AIS of unknown time of onset(UTO).Methods Data on patients with AIS with UTO and within 4.5 hours of onset were reviewed.Data elements collected and analysed included:demographics,National Institutes of Health Stroke Scale(NIHSS)score at baseline and 2 hours,24 hours,7 days after thrombolysis and before discharge,the modified Rankin Scale(mRS)score at 3 months after discharge,imaging findings and any adverse event.results Forty-two patients with UTO and 62 in control group treated within 4.5 hours of onset were treated with intravenous tPA.The NIHSS scores after thrombolysis and/or before discharge in UTO group were significantly improved compared with the baseline(p<0.05).Between the two groups,no significant differences in NIHSS score were observed(p>0.05).Utilising the non-inferiority test,to compare mRS scores(0-2)at 3 months between the two groups,the difference was 5.2%(92%CI,OR 0.196).Patients in the UTO group had mRS scores of 0-2,which were non-inferior to the control group.Their incidence of adverse events was similar.Conclusions Utilising multimodal MRI to guide intravenous only thrombolysis for patients with AIS with UTO was safe and effective.In those patients with AIS between 6 and 24 hours of time of onset but without large arterial occlusion,intravenous thrombolysis could be considered an option. | Jie Zhao Hongmei Zhao Runtao Li Jiangtao Li Chang Liu Juan Lv Yanan Li Wei Liu Dongpu Ma Huaihai Hao Xinguang Xiao Junzhong Liu Yongfeng Yin Rongli Liu Qiaoyan Yu Yingjie Wei Pengyan Li Yue Wang Runqing Wang | 2019 | Stroke & Vascular Neurology2019,4,1: | 8 |
| 6 | Laparoscopic lateral lymph node dissection in two fascial spaces for locally advanced lower rectal cancer显示文摘BACKGROUND The procedure for lateral lymph node(LLN)dissection(LLND)is complicated and can result in complications.We developed a technique for laparoscopic LLND based on two fascial spaces to simplify the procedure.AIM To clarify the anatomical basis of laparoscopic LLND in two fascial spaces and to evaluate its efficacy and safety in treating locally advanced low rectal cancer(LALRC).METHODS Cadaveric dissection was performed on 24 pelvises,and the fascial composition related to LLND was observed and described.Three dimensional-laparoscopic total mesorectal excision with LLND was performed in 20 patients with LALRC,and their clinical data were analyzed.RESULTS The cadaver study showed that the fascia propria of the rectum,urogenital fascia,vesicohypogastric fascia and parietal fascia lie side by side in a medial-lateral direction constituting the dissection plane for curative rectal cancer surgery,and the last three fasciae formed two spaces(Latzko's pararectal space and paravesical space)which were the surgical area for LLND.Laparoscopic LLND in two fascial spaces was performed successfully in all 20 patients.The median operating time,blood loss and postoperative hospitalization were 178(152-243)min,55(25-150)mL and 10(7-20)d,respectively.The median number of harvested LLNs was 8.6(6-12),and pathologically positive LLN metastasis was confirmed in 7(35.0%)cases.Postoperative complications included lower limb pain in 1 case and lymph leakage in 1 case.CONCLUSION Our preliminary surgical experience suggests that laparoscopic LLND based on fascial spaces is a feasible,effective and safe procedure for treating LALRC. | Hui-Hong Jiang Hai-Long Liu A-Jian Li Wen-Chao Wang Liang Lv Jian Peng Zhi-Hui Pan Yi Chang Mou-Bin Lin | 2021 | World Journal of Gastroenterology2021,27,24: | 8 |
| 7 | Everyone has a donor:contribution of the Chinese experience to global practice of haploidentical hematopoietic stem cell transplantation显示文摘Human leukocyte antigen (HLA)-matched donors for hematopoietic stem cell transplantation (HSCT) have long been scarce in China. Haploidentical (haplo) donors are available for the vast majority of patients, but toxicity has limited this approach. Three new approaches for haplo-HSCT originated from Italy, China, and USA in 1990 and have been developed to world-renowned system up to now. The Chinese approach have been greatly improved by implementing new individualized conditioning regimens, donor selection based on non-HLA systems, risk-directed strategies for graft-versus-host disease and relapse, and infection management. Haplo-HSCT has exhibited similar efficacy to HLA-matched HSCT and has gradually become the predominant donor source and the first alternative donor choice for allo-HSCT in China. | 11 | Overexpression of metastasis-associated in colon cancer 1 predicts a poor outcome of hepatitis B virus-related hepatocellular carcinoma显示文摘AIM: To investigate the intratumoral expression of metastasis-associated in colon cancer 1 (MACC1) and c-Met and determine their clinical values associated with hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC). METHODS: A retrospective study admitted three hundred fifty-four patients with HBV-related HCC. The expression and distribution of MACC1 and c-Met were assessed by quantitative real-time polymerase chain reaction and immunohistochemistry staining. Prognostic factors influencing survival, metastasis and recurrence were assessed. RESULTS: Intratumoral MACC1 level was found to be associated with HCC disease progression. Both median tumor-free survival (TFS) and overall survival (OS) were significantly shorter in the postoperative HCC patients with high intratumoral MACC1 expression, as compared to those with low intratumoral MACC1 levels (TFS: 34 mo vs 48.0 mo, P < 0.001; OS: 40 mo vs 48 mo, P < 0.01). Multivariable analysis indicated that high MACC1 expression or co-expression with c-Met were independent predictors for HCC clinic outcome (P < 0.001). CONCLUSION: High intratumoral MACC1 expression can be associated with enhanced tumor progression and poor outcome of HBV-related HCC. | 12 | Effects of interactions between environmental factors and KIF1B genetic variants on the risk of hepatocellular carcinoma in a Chinese cohort显示文摘AIM: To examine the effect of the potential interaction between KIF1 B variants(rs17401966 and rs3748578) and environmental factors on the risk of hepatocellular carcinoma(HCC) in a high-risk region in China.METHODS: Three hundred and six patients with HCC and 306 hospital-based control participants residing in the Shunde region of Guangdong Province, China were enrolled. Clinical characteristics were collected by reviewing the complete medical histories from the patient archives, and epidemiological data were collected using a questionnaire and clinical examination. Two single nucleotide polymorphisms(SNPs) of KIF1B(rs17401966 and rs3748578) were chosen for the current study. All subjects were genotypedusing a Taq Man real-time polymerase chain reaction. Multiplicative and additive logistic regression models were used to evaluate various gene-environment interactions.RESULTS: Smoking, frequent consumption of raw freshwater fish, hepatitis B virus(HBV) infection, and a family history of HCC were important risk factors for HCC in this population. Chronic infection with HBV was the most important environmental risk factor for HCC [odds ratio(OR) = 12.02; 95% confidence interval(95%CI): 6.02-24.00]. No significant association was found between the KIF1 B variants alone and the risk of HCC. Nevertheless, a significant additive effect modification was observed between rs17401966 and alcohol consumption(P for additive interaction = 0.0382). | 13 | Towards context consistency by concurrent checking for Internetware applications显示文摘Internetware applications are emerging and being widely used.They can adapt their behavior based on environmental contexts and deliver smart services.These contexts can be subject to various noises,which cause them to be inaccurate,incomplete,or even to conflict with each other.This is known as context inconsistency problem.Context inconsistency can trigger unexpected behavior to applications,and therefore should be prevented.One promising approach is to check contexts against consistency constraints so as to detect the occurrences of context inconsistency at runtime.Existing techniques have attempted different ways to improve the checking efficiency or effectiveness with different trade-offs in space overhead or communication cost.However,none of them has exploited multi-core computing capability to systematically improve the checking efficiency.In this paper,we propose a novel concurrent checking technique Con-C to efficiently detect inconsistencies in huge volumes of dynamic contexts.Con-C derives checking subtasks for each consistency constraint based on its structure and semantics.It achieves this in a fully automated way,and at the same time can guarantee its derived checking subtasks to be persistently balanced.We evaluated Con-C by controlled experiments through a large-scale real-world application.It reported promising results that Con-C improved the checking efficiency by extra 57.0%,in addition to what had been gained by incremental checking. | XU Chang LIU YePang CHEUNG S.C. | 14 | Injectable dual glucose-responsive hydrogel-micelle composite for mimicking physiological basal and prandial insulin delivery显示文摘For type 1 and advanced type 2 diabetic patients, insulin replacement therapy with simulating on-demand prandial and basal insulin secretion is the best option for optimal glycemic control. However, there is no insulin delivery system yet could mimic both controlled basal insulin release and rapid prandial insulin release in response to real-time blood glucose changes. Here we reported an artificial insulin delivery system, mimicking physiological basal and prandial insulin secretion, to achieve real-time glycemic control and reduce risk of hypoglycemia. A phenylboronic acid(PBA)/galactosyl-based glucose-responsive insulin delivery system was prepared with insulin-loaded micelles embedded in hydrogel matrix. At the hyperglycemic state, both the hydrogel and micelles could swell and achieve rapid glucose-responsive release of insulin, mimicking prandial insulin secretion.When the glucose level returned to the normal state, only the micelles partially responded to glucose and still released insulin gradually. The hydrogel with increased crosslinking density could slow down the diffusion speed of insulin inside, resulting in controlled release of insulin and simulating physiological basal insulin secretion. This hydrogel-micelle composite insulin delivery system could quickly reduce the blood glucose level in a mouse model of type 1 diabetes, and maintain normal blood glucose level without hypoglycemia for about 24 h. | 16 | Reduced Popdc3 expression correlates with high risk and poor survival in patients with gastric cancer显示文摘AIM:To investigate the expression of Popeye domain containing 3(Popdc3) and its correlation with clinicopathological features and prognosis of gastric cancer.METHODS:The method of immunohistochemistry was used to investigate the expression of Popdc3 in 306 cases of human gastric cancer and 84 noncancerous gastric tissues.Simultaneously,the relationship between Popdc3 expression and the survival of the patients was retrospectively analyzed.RESULTS:Popdc3 was detected in 72(85.71%) of 84 human nontumor mucosa.High expression of Popdc3 protein was detected in 78(25.49%) of 306 human gastric cancer cases,and low expression was detected in 228(74.51%).Low expression of Popdc3 correlated with depth of invasion(P < 0.0001),regional lymph nodes(P < 0.0001) and distant metastasis(P = 0.02),and tumor,nodes,metastasis(TNM) stages(P < 0.0001).On multivariate analysis,only the patient's gender,regional lymph node metastasis,distant metastasis,TNM stages,and the expression of Popdc3 were independent prognostic factors in patients with gastric cancer.The Kaplan-Meier plot showed that low Popdc3 expression had a much more significant effect on the survival of those patients with early-stage tumors(χ 2 = 104.741,P < 0.0001),with a > 51.9% reduction in the three-year survival compared with high Popdc3 expression.In late stages,the difference was also significant(χ 2 = 5.930,P = 0.015),with a 32.6% reduction in the three-year survival.CONCLUSION:Reduced expression of Popdc3 may play a significant role in the carcinogenesis and progression of gastric cancer.Popdc3 may be an independent prognostic factor. | Deng Luo Ming-Liang Lu Gong-Fang Zhao Hua Huang Meng-Yao Zheng Jiang Chang Lin Lv Jin-Bo Luo | 2012 | World Journal of Gastroenterology2012,18,19: | 4 |

| 18 | Oligolayered Ti3C2Tx MXene towards high performance lithium/sodium storage显示文摘Nano Research volume 13,pages1659–1667(2020)Cite this article 232 Accesses 3 Citations Metrics details Abstract 2D MXenes are highly attractive for achieving ultrafast and stable lithium/sodium storage due to their good electric conductivity and abundant redox active sites.While,effective strategies for scalable preparation of oligolayered MXenes are still under exploration.Herein,oligolayered Ti3C2Tx MXene is successfully obtained after conventional synthesis of multilayered Ti3C2 and subsequent delamination process via an organic solvent of tetramethyl-ammonium hydroxide(TMAOH).Comprehensive electrochemical study reveals that surface-controlled redox reaction dominated the charge storage behavior of oligolayered Ti3C2Tx with fast reaction kinetics.Impressively,the obtained oligolayered Ti3C2Tx exhibits excellent lithium/sodium storage performance,featured for a high specific capacity of 330 mAhg^−1 at 1.0 Ag^−1 after 800 cycles for lithium storage and 280 mAhg^−1 at 0.5 Ag^−1 after 500 cycles for sodium storage.Such impressive performance will advance the development of oligolayered Ti3C2Tx based materials for lithium/sodium storage and even broaden their application into energy storage. | Xiaolan Song Hui Wang Shengming Jin Miao Lv Ying Zhang Xiaodong Kong Hongmei Xu Ting Ma Xinyuan Luo Hengfeng Tan Dong Hu Chaoyong Deng Xinghua Chang Jianlong Xu | 2020 | Nano Research2020,13,6: | 4 |
