| 1 | Randomized controlled trial of bipolar diathermy vs ultrasonic scalpel for closed hemorrhoidectomy显示文摘AIM:To compare hemorrhoidectomy with a bipolar electrothermal device or hemorrhoidectomy using an ultrasonically activated scalpel.METHODS:Sixty patients with grade Ⅲ or Ⅳ hemorrhoids were prospectively randomized to undergo closed hemorrhoidectomy assisted by bipolar diathermy(group 1) or hemorrhoidectomy with the ultrasonic scalpel(group 2).Operative data were recorded,and patients were followed at 1,3,and 6 wk to evaluate complications.Independent assessors were assigned to obtain postoperative pain scores,oral analgesic requirement and satisfaction scores.RESULTS:Reduced intraoperative blood loss median 0.9 mL(95% CI:0.8-3.7) vs 4.6 mL(95% CI:3.8-7.0),P = 0.001 and a short operating time median 16(95% CI:14.6-18.2) min vs 31(95% CI:28.1-35.3) min,P < 0.0001 was observed in group 1 compared with group 2.There was a trend towards lower postoperative pain scores on day 1 group 1 median 2(95% CI:1.8-3.5) vs group 2 median 3(95% CI:2.6-4.2),P = 0.135.Reduced oral analgesic requirement during postoperative 24 h after operation median 1(95% CI:0.4-0.9) tablet vs 1(95% CI:0.9-1.3) tablet,P = 0.006 was observed in group 1 compared with group 2.There was no difference between the two groups in the degree of patient satisfaction or number of postoperative complications.CONCLUSION:Bipolar diathermy hemorrhoidectomy is quick and bloodless and,although as painful as closed hemorrhoidectomy with the ultrasonic scalpel,is associated with a reduced analgesic requirement immediately after operation. | Akira Tsunoda Haruki Sada Takuya Sugimoto Nobuyasu Kano Mariko Kawana Tadanori Sasaki Hideki Hashimoto | 2011 | World Journal of Gastrointestinal Surgery2011，3，10： | 6 |

| 15 | Clinical characteristics of gastrointestinal immune-related adverse events of immune checkpoint inhibitors and their association with survival显示文摘BACKGROUND Despite the popularity of immune checkpoint inhibitors(ICIs)in the treatment of advanced cancer,patients often develop gastrointestinal(GI)and non-GI immune-related adverse events(irAEs).The clinical characteristics and survival outcomes of GI-irAEs have not been fully elucidated in previous reports.This necessitates the evaluation of the impact of GI-irAEs on patients receiving ICI treatment.AIM To evaluate the clinical characteristics of GI-irAEs and their impact on survival in patients treated with ICIs.METHODS In this single-center,retrospective,observational study,we reviewed the records of 661 patients who received ICIs for various cancers at Nagoya University Hospital from September 2014 to August 2020.We analyzed the clinical characteristics of patients who received ICI treatment.We also evaluated the correlation between GI-irAE development and prognosis in non-small cell lung cancer(LC)and malignant melanoma(MM).Kaplan-Meier analysis was used to compare the median overall survival(OS).Multivariate Cox proportional hazards models were used to identify prognostic factors.A P value<0.05 was considered statistically significant.RESULTS GI-irAEs occurred in 34 of 605 patients(5.6%)treated with an anti-programmed cell death-1/programmed death-ligand 1(anti-PD-1/PD-L1)antibody alone and in nine of 56 patients(16.1%)treated with an anti-cytotoxic T-lymphocyte antigen 4(CTLA-4)antibody alone or a combination of anti-PD-1 and anti-CTLA-4 antibodies.The cumulative incidence and median daily diarrhea frequency were significantly higher in patients receiving anti-CTLA-4 antibodies(P<0.05).In 130 patients with MM,OS was significantly prolonged in the group that continued ICI treatment despite the development of GI-irAEs compared to the group that did not experience GI-irAEs(P=0.035).In contrast,in 209 patients with non-small cell LC,there was no significant difference in OS between the groups.The multivariate analyses showed that a performance status of 2-3(hazard ratio:2.406;95%confidence interval:1.125–5.147;P=0.024)was an independent predictive factor for OS in patients with MM.CONCLUSION Patients receiving anti-CTLA-4 antibodies develop GI-irAEs more frequently and with higher severity than those receiving anti-PD-1/PD-L1 antibodies.Continuing ICI treatment in patients with MM with GI-irAEs have better OS. | Kentaro Yamada Tsunaki Sawada Masanao Nakamura Takeshi Yamamura Keiko Maeda Eri Ishikawa Tadashi Iida Yasuyuki Mizutani Naomi Kakushima Takuya Ishikawa Kazuhiro Furukawa Eizaburo Ohno Takashi Honda Hiroki Kawashima Masatoshi Ishigami Satoshi Furune Tetsunari Hase Kenji Yokota Osamu Maeda Naozumi Hashimoto Masashi Akiyama Yuichi Ando Mitsuhiro Fujishiro | 2021 | World Journal of Gastroenterology2021，27，41： | 0 |
| 16 | Decline in subarachnoid haemorrhage volumes associated with the first wave of the COVID- 19 pandemic显示文摘Background During the COVID-19 pandemic,decreased volumes of stroke admissions and mechanical thrombectomy were reported.The study’s objective was to examine whether subarachnoid haemorrhage(SAH)hospitalisations and ruptured aneurysm coiling interventions demonstrated similar declines.Methods We conducted a cross-sectional,retrospective,observational study across 6 continents,37 countries and 140 comprehensive stroke centres.Patients with the diagnosis of SAH,aneurysmal SAH,ruptured aneurysm coiling interventions and COVID-19 were identified by prospective aneurysm databases or by International Classification of Diseases,10th Revision,codes.The 3-month cumulative volume,monthly volumes for SAH hospitalisations and ruptured aneurysm coiling procedures were compared for the period before(1 year and immediately before)and during the pandemic,defined as 1 March-31 May 2020.The prior 1-year control period(1 March-31 May 2019)was obtained to account for seasonal variation.Findings There was a significant decline in SAH hospitalisations,with 2044 admissions in the 3 months immediately before and 1585 admissions during the pandemic,representing a relative decline of 22.5%(95%CI−24.3%to−20.7%,p<0.0001).Embolisation of ruptured aneurysms declined with 1170-1035 procedures,respectively,representing an 11.5%(95%CI−13.5%to−9.8%,p=0.002)relative drop.Subgroup analysis was noted for aneurysmal SAH hospitalisation decline from 834 to 626 hospitalisations,a 24.9%relative decline(95%CI−28.0%to−22.1%,p<0.0001).A relative increase in ruptured aneurysm coiling was noted in low coiling volume hospitals of 41.1%(95%CI 32.3%to 50.6%,p=0.008)despite a decrease in SAH admissions in this tertile.Interpretation There was a relative decrease in the volume of SAH hospitalisations,aneurysmal SAH hospitalisations and ruptured aneurysm embolisations during the | 17 | Stereotactic body radiation therapy for primary liver tumors with adverse factors显示文摘Aim:To test the efficacy and safety of liver stereotactic body radiation therapy(SBRT)in patients who harbor adverse factors.Methods:We retrospectively evaluated the outcomes of liver SBRT in a single cancer center.We invented criteria consisting of two physical factors and two tumor factors to measure the treatment difficulty in each case.The clinical outcomes and toxicity were evaluated by stratification of the harboring factors.Results:A total of 24(23 hepatocellular carcinoma and 1 intrahepatic cholangiocarcinoma)patients were eligible for this study,with a median follow-up duration of 18 months.Of all eligible patients,21 patients(88%)had one or more factors.The local control,progression-free survival,and overall survival rates for all patients at 2 years were 89%,42%,and 76%respectively.In the patients with physical and tumor adverse factors,local control/progression-free survival/overall survival rates at 2 years were 100%/42%/69%and 80%/23%/78%,respectively.The subgroup of 11 patients with 2 or more factors showed comparable local control rate at 2 years to the subgroup of 13 patients with 0 to 1 factors(100%vs.86%,P=0.59).One patient(4.2%)experienced a decline in the Child-Pugh score by 2 points at 3 months after the treatment.Grade 2 to 3 gastrointestinal toxicity was observed in three patients.Conclusion:SBRT showed a high local control rate with acceptable toxicity for the group of liver cancer patients harboring both physical and tumor adverse factors as long as conducted following patient selection and dose constraints that were used in this study. | Takuya Shimizuguchi Jun Imamura Simpei Hashimoto Katsuyuki Karasawa | 2020 | Hepatoma Research2020，6，10： | 0 |
| 18 | Near-infrared polarimetry of the GG Tauri A binary system显示文摘A high angular resolution near-infrared image that shows the intensity of polarization for the GG Tau A binary system was obtained with the Subaru Telescope.The image shows a circumbinary disk scattering the light from the central binary. The azimuthal profile of the intensity of polarization for the circumbinary disk is roughly reproduced by a simple disk model with the Henyey-Greenstein phase function and the Rayleigh function, indicating there are small dust grains at the surface of the disk.Combined with a previous observation of the circumbinary disk, our image indicates that the gap structure in the circumbinary disk orbits counterclockwise, but material in the disk orbits clockwise. We propose that there is a shadow caused by material located between the central binary and the circumbinary disk.
