| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 4 | Diagnosis and treatment of polypoid lesions of the gallbladder:report of 194 cases显示文摘BACKGROUND: With the wide use of B-ultrasonography in recent years, the polypoid lesion of the gallbladde ( PLG) has been one of the most common diseases detected in biliary surgery. This study was to investigate the diag nostic method and operative indications of PLG. METHODS: The clinical and pathological data of 194 pa tients with PLG who had received operation at our hospita from January 1994 to September 2002 were analyzed retro spectively. Categorized data were analyzed by the chi square test. RESULTS: All the patients received preoperative B-ultra- sonography. 185 of the 194 PLG patients were diagnosed as having cholecystic polyp, and 9 adenomas. Among the 42 patients who received CT, 6 showed early gallbladder can cer. Pathologically, cholesterol polyps were mostly multi ple lesions (64.7%) with a mean diameter of3.86±2.2 mm in 136 patients. Of 16 patients with adenomas, 10 had a tumor diameter of more than 10 mm (62.5%). In 11 pa tients with gallbladder carcinoma, 7 were accompanied with gallbladder stone (63.6%). In addition, inflammatory polyps and adenomyomas were found in 25 and 6 patients respectively. CONCLUSIONS: B-ultrasonography is the most effective diagnostic method for detecting PLG. When large or irreg- ular lesions are found, CT should be performed in order to avoid missing of gallbladder carcinoma. | 5 | AtbHLH29 of Arabidopsis thaliana is a functional ortholog of tomato FER involved in controlling iron acquisition in strategy I plants显示文摘AtbHLH29 of Arabidopsis, encoding a bHLH protein, reveals a high similarity to the tomato FER which is proposed as a transcriptional regulator involved in controlling the iron deficiency responses and the iron uptake in tomato. For identification of its biological functions, AtbHLH29 was introduced into the genome of the tomato FER mutant T3238fer mediated by Agrobacterium tumefaciencs. Transgenic plants were regenerated and the stable integration of AtbHLH29 into their genomes was confirmed by Southern hybridization. Molecular analysis demonstrated that expression of the exogenous AtbHLH29 of Arabidopsis in roots of the FER mutant T3238fer enabled to complement the defect functions of FER. The transgenic plants regained the ability to activate the whole iron deficiency responses and showed normal growth as the wild type under iron-limiting stress. Our transformation data demonstrate that AtbHLH29 is a functional ortholog of the tomato FER and can completely replace FER in controlling the effective iron acquisition in tomato. Except of iron, FER protein was directly or indirectly involved in manganese homeostasis due to that loss functions of FER in T3238fer resulted in strong reduction of Mn content in leaves and the defect function on Mn accumulation in leaves was complemented by expression of AtbHLH29 in the transgenic plants. | 8 | A novel quantified bitterness evaluation model for traditional Chinese herbs based on an animal ethology principle显示文摘Traditional Chinese herbs(TCH) are currently gaining attention in disease prevention and health care plans. However, their general bitter taste hinders their use. Despite the development of a variety of taste evaluation methods, it is still a major challenge to establish a quantitative detection technique that is objective, authentic and sensitive. Based on the two-bottle preference test(TBP), we proposed a novel quantitative strategy using a standardized animal test and a unified quantitative benchmark. To reduce the difference of results, the methodology of TBP was optimized. The relationship between the concentration of quinine and animal preference index(PI) was obtained. Then the PI of TCH was measured through TBP, and bitterness results were converted into a unified numerical system using the relationship of concentration and PI. To verify the authenticity and sensitivity of quantified results,human sensory testing and electronic tongue testing were applied. The quantified results showed a good discrimination ability. For example, the bitterness of Coptidis Rhizoma was equal to 0.0579 mg/mL quinine, and Nelumbinis Folium was equal to 0.0001 mg/mL. The validation results proved that the newassessment method for TCH was objective and reliable. | 9 | Coping as a Mechanism Linking Stressful Life Events and Mental Health Problems in Adolescents显示文摘Objective Although stressful life events represent an etiologic factor of mental health problems in adolescents,few studies have been conducted to address mechanisms linking the stress‐psychopathology relation.The present study was designed to examine coping as a mediate factor on the relationship between stressful life events and symptoms of anxiety and depression.Methods The participants were 13 512 students from eight cities of China,who participated in a school‐based survey.Data were collected by a questionnaire comprising coping,stressful life events,anxiety,and depressive symptoms.As a model,a series of regression equations were used to examine whether coping mediated the association between stressful life events and symptoms of anxiety and depression.Results Each dimension of stressful life events showed significant correlation with anxiety,depression and coping (all P<0.001).In the model to analyze mediate effects,all standardized coefficients (β) were significant (all P<0.01),indicating marked mediator effects.Furthermore,negative coping might account for more mediate effects than positive coping on this relationship.Conclusion Coping partially mediated the relationship between stressful life events and mental health during adolescence.This study highlighted an important public health priority for preventive interventions targeting stress‐related psychopathology,and for further promoting adolescents'mental health. | MENG Xiu Hong TAO Fang Biao WAN Yu Hui HU Yan WANG Ren Xi | 2011 | Biomedical and Environmental Sciences2011,24,6: | 16 |
| 10 | Evaluation of liver functional reserve by combining D-sorbitol clearance rate and CT measured liver volume显示文摘AIM: Our research attempted to evaluate the overall functional reserve of cirrhotic liver by combination of hepatic functional blood flow, liver volume, and ChildPugh′s classification, and to discuss its value of clinical application.METHODS: Ninety two patients with portal hypertension due to hepatic cirrhosis were investigated. All had a historyof haematemesis and hematochezia, esophageal and gastric fundus varices, splenomegaly and hypersplenia.A 2-year follow-up was routinely performed and no one was lost. Twenty two healthy volunteers were used as control group. Blood and urine samples were collected 4times before and after intravenous D-sorbitol infusion.The hepatic clearance (CLH) of D-sorbitol was then calculated according to enzymatic spectrophotometric method while the total blood flow (QToTAL) and intrahepatic shunt (RINs) were detected by multicolor Doppler ultrasound, and the liver volume was measured by spiral CT. Data were estimated by t-test, variance calculation and chi-squared test. The relationships between all these parameters and different groups were investigated according to Child-Pugh classification and postoperative complications respectively.RESULTS: Steady blood concentration was achieved 120 mins after D-sorbitol intravenous infusion, which was (0.358±0.064) mmoⅠ@L-1 in cirrhotic group and (0.189±0.05)mmol@L-1 in control group (P<0.01). CLH=(812.7±112.4) ml@min-1,QTOTAL=(1280.6±131.4) ml@min-1, and RINS=(36.54±10.65)%in cirrhotic group and CLH=(1248.3±210.5) ml.min-1, QTOTAL=(1362.4-±126.9) ml@min-1, and RINS=(8.37±3.32) % in control group (P<0.01). The liver volume of cirrhotic group was 1057±249 cm3, 851±148 cm3 and 663±77 cm3 in Child A, B and C group respectively with significant difference (P<0.001).The average volume of cirrhotic liver in Child B, C group was significantly reduced in comparison with that in control group (P<0.001). The patient, whose liver volume decreased by 40 % with the CLH below 600 ml@min-1, would have a higher incidence of postoperative complications. There was no strict correspondent relationship between CLH, liver volume and Child-Pugh′s classification.CONCLUSION: The hepatic clearance of D-sorbitol, CT measured liver volume can be reliably used for the evaluation of hepatic functional blood flow and liver metabolic volume. | 13 | Effect of Qingfei Mixture (清肺合剂) on Pediatric Mycoplasma Pneumoniae Pneumonia with Phlegm Heat Obstructing Fei (Lung) Syndrome显示文摘Objective: To explore the effect and mechanism of Qingfei Mixture(清肺合剂), a Chinese medicine, in treating mycoplasma pneumonia(MP) in MP patients and rat model. Methods: A total of 46 MP children with phlegm heat obstructing Fei(Lung) syndrome were randomly assigned to two groups by the method of random number table, with 23 children in each group. The control group was treated with intravenous infusion of azithromycin; the treatment group received intravenous infusion of azithromycin and oral administration of Qingfei Mixture. The treatment course was 7 days. Major symptoms and minor symptoms were observed and scored before and after treatments. A rat model of MP was also established. A total of 120 wistar rats were randomly divided into 5 groups: a normal group, infection group, Qingfei Mixture treatment group, azithromycin treatment group, and Qingfei Mixture + azithromycin treatment group. Each group contained 24 rats, from which every 6 were euthanatized 1, 3, 7 and 14 days after infection. MP DNA in pulmonary tissue homogenates was detected using real-time fluorescence quantitative polymerase chain reaction. Pathology was assessed after hematoxylin(HE) staining and lung tissue pathology scores were determined in pulmonary tissue. Transmission electron microscopic detection and electronic image analysis were performed on lung tissue 3 days after infection. Interleukin(IL)-17 was detected in serum using enzymelinked immunosorbent assay(ELISA) 7 days after infection. Results: In the clinical study, both control and the treatment group showed improved results on removing symptoms of phlegm heat syndrome compared to the control group(P<0.05). In animal experiments, On the 7th day after MP infection, as detected by electron microscopy, the pulmonary capillary basement membranes of the azithromycin + Qingfei Mixture treatment group were much thinner than those of the azithromycin or Qingfei mixture treatment groups(P<0.05). The level of serum IL-17 in the azithromycin + Qingfei Mixture treatment group was lower than that in the azithromycin or Qingfei Mixture groups(P<0.01). But the existing resource allocation algorithm designed for such system is always with high complexity thus hard to be realized. To solve such problem with the constraints of spectrum efficiency and buffer state,a novel cross-layer resource allocation algorithm( RAA) is proposed in this paper. The goal of our RAA is to maximize the system throughput while satisfying several practical constraints,such as fairness among services,head of line( Ho L) delay and diverse quality of service( Qo S) requirements. Due to these constraints,finding the optimal solution becomes a NPhard problem. Therefore in this paper a novel method to solve such problem with acceptable complexity is proposed within following steps: firstly,based on the link state we formulate the ideal subchannel allocation strategy as a convex optimization problem,which can be efficiently solved by our proposed lagrange multiplier technique subchannel allocation( LMTSA) algorithm; secondly,according to the obtained channel allocation matrix,a power allocation algorithm based on the water-filling power allocation( WPA) idea is deployed to get the optimal power allocation matrix combining with adaptive modulation and coding( AMC); finally,through a greedy algorithm,the ultimate subchannel and power allocation matrix can be obtained based on iterative method. | 20 | miRNA-133a-UCP2 pathway regulates inflammatory bowel disease progress by influencing inflammation, oxidative stress and energy metabolism显示文摘AIM To investigate the role of the mi R-133a-UCP2 pathway in the pathogenesis of inflammatory bowel disease(IBD)and to explore the potential downstream mechanisms with respect to inflammation,oxidative stress and energy metabolism.METHODS C57BL/6 mice were fed dextran sulfate sodium(DSS)liquid for 7 consecutive days,followed by the administration of saline to the DSS group,UCP2 si RNA to the UCP2 group and a mi R-133a mimic to the mi R-133a group on days 8 and 11.Body weight,stool consistencyand rectal bleeding were recorded daily,and these composed the disease activity index(DAI)score for the assessment of disease severity.After cervical dislocation was performed on day 14,the length of the colon in each mouse was measured,and colonic tissue was collected for further study,which included the following:haematoxylin and eosin staining,UCP2 and mi R-133a detection by immunohistochemical staining,western blot and quantitative real-time PCR,measurement of apoptosis by TUNEL assay,and the assessment of inflammation(TNF-α,IL-1β,IL-6 and MCP1),oxidative stress(H2O2 and MDA)and metabolic parameters(ATP)by ELISA and colorimetric methods.RESULTS An animal model of IBD was successfully established,as shown by an increased DAI score,shortened colon length and specific pathologic changes,along with significantly increased UCP2 and decreased mi R-133a levels.Compared with the DSS group,the severity of IBD was alleviated in the UCP2 and the mi R-133a groups after successful UCP2 knockdown and mi R-133a overexpression.The extent of apoptosis,as well as the levels of TNF-α,IL-1β,MDA and ATP,were significantly increased in both the UCP2 and mi R-133a groups compared with the DSS group.CONCLUSION The mi R-133a-UCP2 pathway participates in IBD by altering downstream inflammation,oxidative stress and markers of energy metabolism,which provides novel clues and potential therapeutic targets for IBD. | Xi Jin Dong Chen Ruo-Heng Zheng Hong Zhang Yi-Peng Chen Zun Xiang | 2017 | World Journal of Gastroenterology2017,23,1: | 11 |
