3A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. 4Clinical characteristics of 24 asymptomatic infections with COVID-19 screened among close contacts in Nanjing,China显示文摘Previous studies have showed clinical characteristics of patients with the 2019 novel coronavirus disease(COVID-19)and the evidence of person-to-person transmission.Limited data are available for asymptomatic infections.This study aims to present the clinical characteristics of 24 cases with asymptomatic infection screened from close contacts and to show the transmission potential of asymptomatic COVID-19 virus carriers.Epidemiological investigations were conducted among all close contacts of COVID-19 patients(or suspected patients)in Nanjing,Jiangsu Province,China,from Jan 28 to Feb 9,2020,both in clinic and in community.Asymptomatic carriers were laboratory-confirmed positive for the COVID-19 virus by testing the nucleic acid of the pharyngeal swab samples.Their clinical records,laboratory assessments,and chest CT scans were reviewed.As a result,none of the 24 asymptomatic cases presented any obvious symptoms while nucleic acid screening.Five cases(20.8%)developed symptoms(fever,cough,fatigue,etc.)during hospitalization.Twelve(50.0%)cases showed typical CT images of ground-glass chest and 5(20.8%)presented stripe shadowing in the lungs.The remaining 7(29.2%)cases showed normal CT image and had no symptoms during hospitalization.These 7 cases were younger(median age:14.0 years;P=0.012)than the rest.None of the 24 cases developed severe COVID-19 pneumonia or died.The median communicable period,defined as the interval from the first day of positive nucleic acid tests to the first day of continuous negative tests,was 9.5 days(up to 21 days among the 24 asymptomatic cases).Through epidemiological investigation,we observed a typical asymptomatic transmission to the cohabiting family members,which even caused severe COVID-19 pneumonia.Overall,the asymptomatic carriers identified from close contacts were prone to be mildly ill during hospitalization.However,the communicable period could be up to three weeks and the communicated patients could develop severe illness.These results highlighted the importance of close contact tracing and longitudinally surveillance via virus nucleic acid tests.Further isolation recommendation and continuous nucleic acid tests may also be recommended to the patients discharged.Zhiliang Hu Ci Song Chuanjun Xu Guangfu Jin Yaling Chen Xin Xu Hongxia Ma Wei Chen Yuan Lin Yishan Zheng Jianming Wang Zhibin Hu Yongxiang Yi Hongbing Shen 2020Science China(Life Sciences)2020,63,5:77
52018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. 6Alzheimer's disease and gut microbiota显示文摘Alzheimer's disease(AD) is a most common neurodegenerative disorder, which associates with impaired cognition. Gut microbiota can modulate host brain function and behavior via microbiota-gut-brain axis, including cognitive behavior.Germ-free animals, antibiotics, probiotics intervention and diet can induce alterations of gut microbiota and gut physiology and also host cognitive behavior, increasing or decreasing risks of AD. The increased permeability of intestine and blood-brain barrier induced by gut microbiota disturbance will increase the incidence of neurodegeneration disorders. Gut microbial metabolites and their effects on host neurochemical changes may increase or decrease the risk of AD. Pathogenic microbes infection will also increase the risk of AD, and meanwhile, the onset of AD support the 'hygiene hypothesis'. All the results suggest that AD may begin in the gut, and is closely related to the imbalance of gut microbiota. 7Discovery of direct inhibitors of Keap1–Nrf2 protein–protein interaction as potential therapeutic and preventive agents显示文摘The Keap1–Nrf2–ARE pathway is an important antioxidant defense mechanism that protects cells from oxidative stress and the Keap1–Nrf2 protein–protein interaction(PPI) has become an important drug target to upregulate the expression of ARE-controlled cytoprotective oxidative stress response enzymes in the development of therapeutic and preventive agents for a number of diseases and conditions. However, most known Nrf2 activators/ARE inducers are indirect inhibitors of Keap1–Nrf2PPI and they are electrophilic species that act by modifying the sulfhydryl groups of Keap1's cysteine residues. The electrophilicity of these indirect inhibitors may cause 'off-target' side effects by reacting with cysteine residues of other important cellular proteins. Efforts have recently been focused on the development of direct inhibitors of Keap1–Nrf2 PPI. This article reviews these recent research efforts including the development of high throughput screening assays, the discovery of peptide and small molecule direct inhibitors, and the biophysical characterization of the binding of these inhibitors to the target Keap1 Kelch domain protein. 8New vacuum solar telescope and observations with high resolution显示文摘The New Vacuum Solar Telescope(NVST) is a one meter vacuum solar telescope that aims to observe fine structures on the Sun. The main goals of NVST are high resolution imaging and spectral observations, including measurements of the solar magnetic field. NVST is the primary ground-based facility used by the Chinese solar research community in this solar cycle. It is located by Fuxian Lake in southwest China, where the seeing is good enough to perform high resolution observations. We first introduce the general conditions at the Fuxian Solar Observatory and the primary science cases of NVST. Then, the basic structures of this telescope and instruments are described in detail. 9Hydrothermal recrystallization of the Lower Ordovician dolomite and its significance to reservoir in northern Tarim Basin显示文摘Discovered in S15 and some other wells, the Lower Ordovician in the northern Tarim Basin consists mainly of brown gray-dark gray very fine-fine crystalline dolomite, with a minor portion of locally light gray-white medium-coarse crystalline dolomite. Silicification can be observed in the medium-coarse dolomite, and some euhedral drusy quartz can also be found in pores and fractures of the dolomite. The homogenization temperature of the fluid inclusions in the medium-coarse dolomite is between 110 and 200°C with maximum between 140 and 190°C, and the salinity is between 10.7 and 18.5 wt.% NaCl Eq. The homogenization temperature and salinity of the fluid inclusions in the medium-coarse dolomite are similar to those in the drusy quartz. Compared with the very fine-fine dolomite, the medium-coarse phase contains relatively high Fe and Mn. The average concentration of FeO and MnO in the medium-coarse dolomite is 1.917% and 0.323%, respectively. The medium-coarse dolomite has a remarkable negative Eu anomaly, consistent with the REE pattern of the intermediate-felsic igneous rocks in the Tarim Basin. The oxygen isotopic composition of the medium-coarse dolomite is relatively lighter than that of the very fine-fine dolomite. The δ18OPDB values of the medium-coarse dolomite are between -10.35‰ and -7.31‰. The δ18OSMOW values of the fluid associated with the medium-coarse dolomite can be calculated according to homogenization temperature and oxygen isotope fractionation factor between dolomite and fluid, and the calculated values are between +4‰ and +10‰, consistent with those of the hydrothermal fluid. The medium-coarse dolomite has relatively high 87Sr/86Sr ratios as well, indi- cating an origin associated with intermediate-felsic igneous rock. The homogenization temperature, element composition, REE pattern, oxygen and strontium isotopes demonstrate that the medium-coarse dolomite is the result of recrystallization of very fine-fine dolomite under hydrothermal environment. The hydrothermal dolomite recrystallization is a special event but exists extensively in the Tarim Basin. 14The Relationship between Glucose Excursion and Cognitive Function in Aged Type 2 Diabetes Patients显示文摘Objective Evidence suggests that type 2 diabetes (T2DM) is associated with an increased risk of dementia and that glucose variability is an independent risk factor for diabetic complications.This study investigated the relationship between glucose excursion and cognitive function in aged T2DM patients.Methods A total of 248 aged T2DM patients wore a continuous glucose monitoring system (CGMS) for 3 days in order to evaluate glucose excursion,including mean amplitude of glycemic excursions (MAGE) and mean of daily difference (MODD).All subjects were evaluated with a number of accepted cognitive function tests,including themini‐mental status examination (MMSE).The relationship between MAGE and MODD and performance on these cognitive tests was assessed.Results The MAGE and MMSE score were negatively correlated,likewise with the correlation between MODD and MMSE.Liner multivariate regression analysis showed that MAGE and MODD were also negatively related to MMSE independent of age,sex,glycemic control,hypertension,smoking,or coronary heart disease history.Conclusion Glucose excursion is related to cognitive function in aged T2DM patients.Elevated glucose excursion decreased the MMSE score,which reflects general cognitive function.Thus,therapy aimed at controlling glucose excursion may be beneficial for maintaining cognitive function in aged T2DM patients.ZHONG Yuan ZHANG Xiao Yan MIAO Ya ZHU Jie Hua YAN Hong WANG Bei Yun JIN Jun HU Ting Jun JIA Wei Ping 2012Biomedical and Environmental Sciences2012,25,1:34

16National Epidemiology and Evolutionary History of Four Hand, Foot and Mouth Disease-Related Enteroviruses in China from 2008 to 2016显示文摘Hand, foot and mouth disease(HFMD) is a major public health concern in China. The most predominant enteroviruses that cause HFMD have traditionally been attributed to enterovirus A71(EVA71) and coxsackievirus A16(CVA16). Since its first large outbreak in 2008, the dominant HFMD pathogens are constantly changing. In 2013 and 2015, CVA6 exceeded both EVA71 and CVA16 to become the leading cause of HFMD in some provinces. However, there still lacks a comprehensive overview on the molecular epidemiology and evolution of HFMD-related enteroviruses at the national level. In this study, we performed systematic epidemiological analyses of HFMD-related enteroviruses using the data of 64 published papers that met the inclusion criteria, and conducted phylogenetic analyses based on 12,080 partial VP1 sequences identified in China before 31 st June 2018. We found that EVA71 prevalence has decreased sharply but other enteroviruses have increased rapidly from 2008 to 2016 and that one subtype of each enterovirus is represented during the epidemic. In addition, four genotypes EVA71_C4, CVA16_B1, CVA6_D and CVA10_C are the most predominant enterovirus strains and collectively they cause over 90% of all HFMD cases in China according to the phylogenetic trees using representative partial VP1 sequences. These four major enterovirus genotypes have different geographical distributions, and they may cocirculate with other genotypes and serotypes.
