| 3 | Emergency department procedural sedation for primary electrical cardioversion — a comparison with procedural sedations for other reasons显示文摘BACKGROUND: Atrial fibrillation(AF) is the most common arrhythmia treated in the emergency department(ED), with primary electrical cardioversion(PEC) the preferred method of rhythm control. Anecdotally, patients undergoing ED procedural sedation(EDPS) for PEC differ from those requiring EDPS for other procedures: they are at higher risk of adverse events, and require fewer drugs and lower doses. We attempt to verify this using an EDPS registry at a Canadian, tertiary care teaching hospital.METHODS: This is a retrospective review of patients that underwent EDPS for the period of June 2006 to September 2014. We compared demographics, medication use and intra-procedural adverse events between those receiving EDPS for PEC for AF compared to that for other indications. We report the asssociation between AEs and predictors using logistic regression.RESULTS: A total of 4 867 patients were included, 714 for PEC for AF and 4 153 for other indications. PEC patients were more likely male(58.5% vs. 47.1%), older(59.5 years vs. 48.1 years), and less likely to be ASA I(46.6% vs. 69.0%). PEC patients received smaller doses of propofol and less likely to receive adjuvant analgesic therapy(11.5% vs. 78.2%). PEC patients were more likely to experience hypotension(27.6% vs. 16.5%) but respiratory AEs(apnea, hypoxia and airway intervention) were not different.CONCLUSION: EDPS for PEC differs from that conducted for other purposes: patients tend to be less healthy, receive smaller doses of medication and more likely to suffer hypotension without an increase in respiratory AEs. | 5 | Is analysis of lower esophageal sphincter vector volumes of value in diagnosing gastroesophageal reflux disease?显示文摘AIM: With successful surgical treatment of gastroesophagealreflux disease (GERD), there is interest in understandingthe anti-reflux barrier and its mechanisms of failure. To date,the potential use of vector volumes to predict the DeMeesterscore has not been adequately explored.METHODS: 627 patients in the referral database receivedesophageal manometry and ambulatory 24-hour pHmonitoring. Study data included LES resting pressure (LESP),overall LES length (OL) and abdominal length (AL), totalvector volume (TW) and intrabdominal vector volume (IVV).RESULTS: In cases where LESP, TVV or IVV were all belownormal, there was an 81.4 % probability of a positiveDeMeester score. In cases where all three were normal,there was an 86.9 % probability that the DeMeester scorewould be negative. Receiver-operating characteristics (ROC)for LESP, TW and IW were nearly identical and indicatedno useful cut-off values. | 10 | Genetic and environmental control of fruit maturation,dry matter and firmness in apple(Malus×domestica Borkh.)显示文摘For any given genotype,the environment in which an apple is grown can influence the properties of the fruit considerably.While there has been extensive research on the mechanism of the genetic control of fruit quality traits,less effort has been made to investigate the way that these genetic mechanisms interact with the environment.To address this issue,we employed a large‘Royal Gala’בBraeburn’population of 572 seedlings replicated over sites in three climatically diverse apple-growing regions in New Zealand.Phenotyping for traits including fruit maturation timing,firmness and dry matter content was performed at each of these three sites for a single growing season(2011),and at two sites(Motueka and Hawke’s Bay)for two seasons(2009 and 2010).The phenotype data collected over 2 years at two sites enabled the detection of 190 quantitative trait loci(QTL)that controlled these traits regardless of year or growing location,as well as some chromosomal loci that influenced the traits in a single given environment or year.For those loci that were environmentally stable over three sites,there was an interdependency of fruit maturation date,dry matter content and storage potential within this population,with two regions on Linkage Groups(LGs)10 and 16 strongly contributing.If these loci were used in a marker-assisted selection programme to select for progeny bearing firmer fruit,this would have the unintentional consequence of selecting,high dry matter content,later maturing apples.In addition,a further 113 new QTLs with a smaller effect were identified,some of which were exhibited only in a single growing environment,demonstrating the underlying complexity of control of traits determining fruit quality,in addition to the need for being aware of environmental effects when developing new apple varieties. | David Chagne Daya Dayatilake Robert Diack Murray Oliver Hilary Ireland Amy Watson Susan E Gardiner Jason W Johnston Robert J Schaffer Stuart Tustin | 2014 | Horticulture Research2014，1，1： | 2 |
