1Atrial fibrillation in obstructive sleep apnea显示文摘Atrial fibrillation(AF) is a common arrhythmia with rising incidence.Obstructive sleep apnea(OSA) is prevalent among patients with AF.This observation has prompted significant research in understanding the relationship between OSA and AF.Multiple studies support a role of OSA in the initiation and progression of AF.This association has been independent of obesity,body mass index and hypertension.Instability of autonomic tone and wide swings in intrathoracic pressure are seen in OSA.These have been mechanistically linked to initiation of AF in OSA patients by lowering atrial effective refractory period,promoting pulmonary vein discharges and atrial dilation.OSA not only promotes initiation of AF but also makes management of AF difficult.Drug therapy and electrical cardioversion for AF are less successful in presence of OSA.There has been higher rate of early and overall recurrence after catheter ablation of AF in patients with OSA.Treatment of OSA with continuous positive airway pressure has been shown to improve control of AF.However,additional studies are needed to establish a stronger relationship between OSA treatment and success ofAF therapies.There should be heightened suspicion of OSA in patients with AF.There is a need for guidelines to screen for OSA as a part of AF management.Sandeep K Goyal Abhishek Sharma 2013World Journal of Cardiology2013,5,6:16
2Peripheral primitive neuroectodermal tumor of the kidney presenting with pulmonary tumor embolism: A case report显示文摘Peripheral primitive neuroectodermal tumor(PNET) of the kidney is a rare, aggressive tumor known for its recurrence and metastatic potential. Despite the frequency of venous extension to the renal veins and inferior vena cava, pulmonary tumor embolism at the initial presentation is not common. We report a case of 22-year-old female with PNET of the kidney who presented with tumor embolism in the inferior vena cava(IVC) and bilateral pulmonary artery. The patient underwent surgical resection and histopathological analysis confirmed the presence of tumor within the IVC and pulmonary arteries. 3Improved simultaneous quantitation of candesartan and hydrochlorthiazide in human plasma by UPLC–MS/MS and its application in bioequivalence studies显示文摘A validated ultra-performance liquid chromatography mass spectrometric method(UPLC–MS/MS) was used for the simultaneous quantitation of candesartan(CN) and hydrochlorothiazide(HCT) in human plasma. The analysis was performed on UPLC–MS/MS system using turbo ion spray interface.Negative ions were measured in multiple reaction monitoring(MRM) mode. The analytes were extracted using a liquid–liquid extraction(LLE) method by using 0.1 mL of plasma volume. The lower limit of quantitation for CN and HCT was 1.00 ng/mL whereas the upper limit of quantitation was 499.15 ng/mL and601.61 ng/mL for CN and HCT respectively. CN d4 and HCT-13Cd2 were used as the internal standards for CN and HCT respectively. The chromatography was achieved within 2.0 min run time using a C18 Phenomenex, Gemini NX(100 mm 4.6 mm, 5 mm) column with organic mixture:buffer solution(80:20, v/v) at a flow rate of 0.800 mL/min. 4Impacts of tourism development on the physical environment of Mussoorie,a hill station in the lower Himalayan range of India显示文摘Tourism in mountainous regions is a significant source of revenue generation. However, it has also been associated with many adverse environmental consequences. This study aims at assessing the negative impacts of the incessant upsurge in tourism development on the physical environment of Mussoorie, a well-known mountain tourist destination in India. The impact indicators for the region were identified and assessed by qualitative and quantitative analysis of field observations. The observations indicated the aggravation of traffic congestion, atmospheric pollution, undisposed solid waste, water scarcity and infrastructure unavailability as the prevalent issues, especially during the peak tourist months. The extent of the consequential damage to the environment was evaluated by conducting an assessment of tourism-induced human disturbance on the natural landscape of the town. Slope, slope aspect, vegetation cover, road network and drainage network were incorporated as thedetermining landscape attributes to prepare thematic maps of landscape quality(perceivable intrinsic properties) and landscape fragility(vulnerability to anthropogenic disturbances) using GIS technique. An absorption capacity map was finally prepared to characterize the study area into regions of different conservation needs. The results identified the need for planning appropriate preservation strategies for different tourist places in the town. 5Site-Specific Nitrogen Management in Dry Direct-Seeded Rice Using Chlorophyll Meter and Leaf Colour Chart显示文摘The need to maintain high rice yields and improve fertilizer nitrogen(N)-use efficiency has fueled the use of tools such as leaf colour chart(LCC) and chlorophyll meter(SPAD meter) in managing fertilizer N based on colour of the leaf. Field experiments were conducted during 2011 to 2013 at Ludhiana, India to assess the need for basal N application and to establish critical threshold values of leaf greenness as measured by LCC and SPAD meter for formulating strategies for in-season management of fertilizer N in dry direct-seeded rice(DDSR). Avoiding application of N at sowing did not adversely affect rice grain yield, indicating that basal N application in DDSR was not necessary and might lead to reduced N-use efficiency. Monitoring N uptake rate during the growing season of DDSR suggested that N uptake rate peaked at the two growth stages: maximum tillering(42 to 56 days after sowing(DAS))and panicle initiation stages(70 to 84 DAS). Using the Cate-Nelson procedure, critical LCC and SPAD meter values for fertilizer N application worked out to be 4 and 37, respectively. Real-time fertilizer N management strategy based on applying 30 kg N ha-1whenever SPAD meter or LCC readings fell below the critical values maintained optimum rice yields along with higher N-use efficiency than that observed by following blanket recommendation for fertilizer N in the region. The fixed-time variable-dose strategy consisted of applying prescriptive doses of 20 kg N ha-1at 14 DAS and 30 kg N ha-1at 28 DAS and corrective doses of 30, 40 or 50 kg N ha-1at 49 and 70 DAS depending upon LCC shade to be ≥ 4, 4–3.5, or < 3.5 and SPAD meter readings to be ≥ 40, 40–35, or< 35, respectively. This strategy also resulted in optimal rice yield along with higher N-use efficiency as compared to the blanket recommendation. 6Gender differences in caregiving among family- caregivers of people with mental illnesses显示文摘All over the world women are the predominant providers of informal care for family members with chronic medical conditions or disabilities, including the elderly and adults with mental illnesses. It has been suggested that there are several societal and cultural demands on women to adopt the role of a family-caregiver. Stress-coping theories propose that women are more likely to be exposed to caregiving stressors, and are likely to perceive, report and cope with these stressors differently from men. Many studies, which have examined gender differences among family-caregivers of people with mental illnesses, have concluded that women spend more time in providing care and carry out personal-care tasks more often than men. These studies have also found that women experience greater mental and physical strain, greater caregiver-burden, and higher levels of psychological distress while providing care. However, almost an equal number of studies have not found any differences between men and women on these aspects. This has led to the view that though there may be certain differences between male and female caregivers, most of these are small in magnitude and of doubtful clinical significance. Accordingly, caregivergender is thought to explain only a minor proportion of the variance in negative caregiving outcomes. A similar inconsistency characterizes the explanations provided for gender differences in caregiving such as role expectations, differences in stress, coping and social support, and response biases in reporting distress. Apart from the equivocal and inconsistent evidence, there are other problems in the literature on gender differences in caregiving. Most of the evidence has been derived from studies on caregivers of elderly people who either suffer from dementia or other physical conditions. Similar research on other mental illnesses such as schizophrenia or mood disorders is relatively scarce. With changing demographics and social norms men are increasingly assuming roles as caregivers. However, the experience of men while providing care has not been explored adequately. The impact of gender on caregiving outcomes may be mediated by several other variables including patient-related factors, socio-demographic variables, and effects of kinship status, culture and ethnicity, but these have seldom been considered in the research on gender differences. Finally, it is apparent that methodological variations in samples, designs andassessments between studies contribute a great deal to the observed gender differences. 7Adherence to self-care practices, glycemic status and influencing factors in diabetes patients in a tertiary care hospital in Delhi显示文摘AIM To assess the adherence to self-care practices,glycemic status and influencing factors in diabetes patients.METHODS This was a cross-sectional observational analysis of baseline data from a quasi-experimental study conducted among 375 diabetic patients aged between 18 to 65years at a major public tertiary care centre in New Delhi,India during February-September’2016.The Summary of Diabetes Self-care activities measure was used to assess medical adherence in diabetic patients.Open ended questions were used to identify facilitators and inhibitors of medical adherence.RESULTS Mean age of the study subjects was 49.7±10.2 years.A total of 201 men and 174 women were enrolled in the study.Three hundred nine(82.4%)subjects were adherent to their intake of anti-diabetic medication.On binary logistic regression,education level below primary school completion and absence of hypertensioncomorbidity were found to be independent predictors of medication non-adherence.Sociocultural resistance was an important factor impeding outdoor exercise among younger women.Knowledge of diabetes in the study subjects was low with mean score of 3.1±2(maximum score=10).Suboptimal glycemic control was found in259(69%)subjects which was significantly more likely in patients on Insulin therapy compared to those on Oral Hypoglycemic agents alone(P<0.006).DISCUSSION Our study found a large gap existed between selfreported medication adherence and glycemic control.This suggests the need for enhanced physician focus for diabetic patient management.Saurav Basu Suneela Garg Nandini Sharma M Meghachandra Singh Sandeep Garg 2018World Journal of Diabetes2018,9,5:4
8OsbZIP76 interacts with OsNF-YBs and regulates endosperm cellularization in rice (Oryza sativa)显示文摘Following double fertilization,plant endosperm nuclei undergo syncytial divisions,followed by synchro-nous cellularization.Cellularization is a key event during endosperm development,but our understanding of its regulation is limited to Arabidopsis.In this study we show that OsbZIP76 regulates cellularization in rice(Oryza sativa).Activation of OsbZIP76 coincided with the initiation of cellularization,and its knockdown or knockout mutants exhibited precocious cellularization.Genes involved in en-dosperm development or starch biosynthesis were pre-maturely activated in the osbzip76 caryopsis.As a putative transcription factor,OsbZIP76 alone lacked transcriptional activation activity;however,it interacted with the nuclear factor Y(NF-Y)family transcription factors OsNF-YB9 and OsNF-YB1 in yeast and in planta.OsbZIP76 and OsNF-YB9 were predominantly expressed in the endosperm and the proteins colocalized.Seeds of osnf-yb1 and osbzip76 mu-tants showed reduced size and reduced apparent amylose content.The parent-of-origin-dependent expression of OsbZIP76 is variable in different rice accessions.In sum-mary,OsbZIP76 is an endosperm-expressed imprinted gene that regulates endosperm development in rice.Baixiao Niu Hui Deng Tingting Li Sandeep Sharma Qianbin Yun Qianru Li Zhiguo E Chen Chen 2020Journal of Integrative Plant Biology2020,62,12:4

14Ultra-performance liquid chromatography electrospray ionization-tandem mass spectrometry method for the simultaneous determination of itraconazole and hydroxy itraconazole in human plasma显示文摘A highly sensitive,selective,and precise ultra-performance liquid chromatography tandem mass spectrometry method was developed and validated for simultaneous quantification of itraconazole and hydroxy itraconazole in human plasma by a single liquid-liquid extraction step.The precursor to product ion transitions of m/z 705.3/392.3.m/z 721.2/408.3 and m/z 708.2/435.4 were used to detect and quantify itraconazole,hydroxy itraconazole and itraconazole-d3 respectively.The lower limit of quantitation was found to be 0.500 ng/mL for itraconazole and 1.00 ng/mL for hydroxy itraconazole.The mean recoveries for itraconazole and hydroxy itraconazole were found to be 100.045% and 100.021%,respectively.This developed method with a chromatographic run time of 2.0 min was successfully applied to a bioequivalence study of 100 mg itraconazole capsule.Ashish Dwivedi Bhupinder Singh Sandeep Sharma R.S.Lokhandae Naveen Dubey 2014Journal of Pharmaceutical Analysis2014,4,5:1
