| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 3 | Precise nanomedicine for intelligent therapy of cancer显示文摘Precise nanomedicine has been extensively explored for efficient cancer imaging and targeted cancer therapy, as evidenced by a few breakthroughs in their preclinical and clinical explorations. Here, we demonstrate the recent advances of intelligent cancer nanomedicine, and discuss the comprehensive understanding of their structure-function relationship for smart and efficient cancer nanomedicine including various imaging and therapeutic applications, as well as nanotoxicity. In particular, a few emerging strategies that have advanced cancer nanomedicine are also highlighted as the emerging focus such as tumor imprisonment, supramolecular chemotherapy, and DNA nanorobot. The challenge and outlook of some scientific and engineering issues are also discussed in future development. | 6 | Assessment of liver fibrosis by Fibroscan as compared to liver biopsy in biliary atresia显示文摘AIM: To evaluate liver stiffness measurement(LSM)using non-invasive transient elastography(Fibroscan)in comparison with liver biopsy for assessment of liver fibrosis in children with biliary atresia(BA).METHODS: Thirty-one children with BA admitted to the Department of Pediatric Surgery of Beijing Children's Hospital from March 2012 to February 2013 were included in this study. Their preoperative LSM, liver biopsy findings, and laboratory results were studied retrospectively.RESULTS: The grade of liver fibrosis in all 31 patients was evaluated according to the METAVIR scoring system, which showed that 4 cases were in group F2, 20 in group F3 and 7 in group F4. There were 24non-cirrhosis cases(F2-F3) and 7 cirrhosis cases(F4).In groups F2, F3 and F4, the mean LSM was 9.10 ±3.30 kPa, 11.02 ± 3.31 kPa and 22.86 ± 12.43 kPa,respectively. LSM was statistically different between groups F2 and F4(P = 0.002), and between groups F3 and F4(P = 0.000), however, there was no statistical difference between groups F2 and F3(P = 0.593). The area under the receiver operating characteristic curve of LSM for ≥ F4 was 0.866. The cut-off value of LSM was 15.15 kPa for ≥ F4, with a sensitivity, specificity,positive predictive value and negative predictive value of 0.857, 0.917, 0.750 and 0.957, respectively.CONCLUSION: Fibroscan can be used as a noninvasive technique to assess liver fibrosis in children with BA. | 11 | Genetic bases of source-,sink-,and yield-related traits revealed by genome-wide association study in Xian rice显示文摘The source-sink relationship determines the ultimate grain yield.We investigated the genetic basis of the relationship between source and sink and yield potential in rice.In two environments,we identified quantitative trait loci(QTL)associated with sink capacity(total spikelet number per panicle and thousand-grain weight),source leaf(flag leaf length,flag leaf width and flag leaf area),source-sink relationship(total spikelet number to flag leaf area ratio)and yield-related traits(filled grain number per panicle,panicle number per plant,grain yield per plant,biomass per plant,and harvest index)by genome-wide association analysis using 272 Xian(indica)accessions.The panel showed substantial variation for all traits in the two environments and revealed complex phenotypic correlations.A total of 70 QTL influencing the 11 traits were identified using 469,377 high-quality SNP markers.Five QTL were detected consistently in four chromosomal regions in both environments.Five QTL clusters simultaneously affected source,sink,source–sink relationship,and grain yield traits,probably explaining the genetic basis of significant correlations of grain yield with source and sink traits.We selected 24 candidate genes in the four consistent QTL regions by identifying linkage disequilibrium(LD)blocks associated with significant SNPs and performing haplotype analysis.The genes included one cloned gene(NOG1)and three newly identified QTL(qHI6,qTGW7,and qFLA8).These results provide a theoretical basis for high-yield rice breeding by increasing and balancing source–sink relationships using marker-assisted selection. | Yun Wang Yunlong Pang Kai Chen Laiyuan Zhai Congcong Shen Shu Wang Jianlong Xu | 2020 | The Crop Journal2020,8,1: | 8 |
| 12 | Microstructures and mechanical properties of Ti3Al/Ni-based superalloy joints brazed with AuNi filler metal显示文摘For the purpose of high-temperature service and the weight reduction in aviation engineering applications, the dissimilar joining of Ti3Al-based alloy to Ni-based superalloy (GH536) was conducted using Au-17.5Ni (wt%) brazing filler metal. The microstructure and chemical composition at the interfaces were investigated by scanning electron microscope, X-ray diffraction and transmission electron microscope. The diffusion behaviors of elements were analyzed as well. The results indicated that the Ti3Al/GH536 joint microstructure was characterized by multiple layer structures. Element Ni from Au-Ni filler metal reacted with Ti3Al base metal, leading to the formation of AlNi2Ti and NiTi compounds. Element Ni from Ti3Al base metal reacted with Ni and thus Ni3Nb phase was detected in the joint central area. Due to the dissolution of Ni-based superalloy,(Ni,Au) solid solution ((Ni,Au)ss) and Ni-rich phase were visible adjacent to the superalloy side. The average tensile strength of all the joints brazed at 1253 K for 5-20 min was above 356 MPa at room-temperature. In particular, the joints brazed at 1253 K/15 min presented the maximum tensile strength of434 MPa at room-temperature, and the strength of 314 MPa was maintained at 923 K. | 13 | Effect of Ni addition on the glass-forming ability and soft-magnetic properties of FeNiBPNb metallic glasses显示文摘The effect of Ni addition on the glass-forming ability (GFA) and soft-magnetic properties of an (Fe1-xNix)75.5B14.5P7Nb3 (x=0-0.6) alloy system were investigated. We found that the addition of Ni was effective in allowing the alloy to approach a eutectic point as well as increasing the thermal stability of the supercooled liquid. By increasing the amount of Ni,the supercooled liquid region (ΔTx),the reduced glass transition temperature Trg (Tg/Tl) and the Y parameter [Tx/(Tg+Tl)] increased from 49 to 75 K,0.540 to 0.594 and 0.373 to 0.405,respectively. As a result,novel (Fe1-xNix)75.5B14.5P7Nb3 bulk metallic glass (BMG) rods with a maximum diameter of 1 mm were synthesized with a composition of x=0.5 and x=0.6 by copper mold casting. | 15 | High mortality associated with gram-negative bacterial bloodstream infection in liver transplant recipients undergoing immunosuppression reduction显示文摘BACKGROUND Immunosuppression is an important factor in the incidence of infections in transplant recipient.Few studies are available on the management of immunosuppression(IS)treatment in the liver transplant(LT)recipients complicated with infection.The aim of this study is to describe our experience in the management of IS treatment during bacterial bloodstream infection(BSI)in LT recipients and assess the effect of temporary IS withdrawal on 30 d mortality of recipients presenting with severe infection.AIM To assess the effect of temporary IS withdrawal on 30 d mortality of LT recipients presenting with severe infection.METHODS A retrospective study was conducted with patients diagnosed with BSI after LT in the Department of Liver Surgery,Renji Hospital from January 1,2016 through December 31,2017.All recipients diagnosed with BSI after LT were included.Univariate and multivariate Cox regression analysis of risk factors for 30 d mortality was conducted in the LT recipients with Gram-negative bacterial(GNB)infection.RESULTS Seventy-four episodes of BSI were identified in 70 LT recipients,including 45 episodes of Gram-positive bacterial(GPB)infections in 42 patients and 29 episodes of GNB infections in 28 patients.Overall,IS reduction(at least 50%dose reduction or cessation of one or more immunosuppressive agent)was made in 28(41.2%)cases,specifically,in 5(11.9%)cases with GPB infections and 23(82.1%)cases with GNB infections.The 180 d all-cause mortality rate was 18.5%(13/70).The mortality rate in GNB group(39.3%,11/28)was significantly higher than that in GPB group(4.8%,2/42)(P=0.001).All the deaths in GNB group were attributed to worsening infection secondary to IS withdrawal,but the deaths in GPB group were all due to graft-versus-host disease.GNB group was associated with significantly higher incidence of intra-abdominal infection,IS reduction,and complete IS withdrawal than GPB group(P<0.05).Cox regression showed that rejection(adjusted hazard ratio 7.021,P=0.001)and complete IS withdrawal(adjusted hazard ratio 12.65,P=0.019)were independent risk factors for 30 d mortality in patients with GNB infections after LT.CONCLUSION IS reduction is more frequently associated with GNB infection than GPB infection in LT recipients.Complete IS withdrawal should be cautious due to increased risk of mortality in LT recipients complicated with BSI. | Fang Chen Xiao-Yun Pang Chuan Shen Long-Zhi Han Yu-Xiao Deng Xiao-Song Chen Jian-Jun Zhang Qiang Xia Yong-Bing Qian | 2020 | World Journal of Gastroenterology2020,26,45: | 5 |

| 18 | Prevalence, risk factors, and survival associated with pulmonary hypertension and heart failure among patients with underlying coronary artery disease: a national prospective, multicenter registry study in China显示文摘Background: Coronary artery disease (CAD) is the commonest cause of heart failure (HF), whereas pulmonary hypertension (PH) has not been established or reported in this patient population. Therefore, we assessed the prevalence, risk factors, and survival in CAD-associated HF (CAD-HF) complicated with PH.Methods: Symptomatic CAD-HF patients were continuously enrolled in this prospective, multicenter registry study. Echocardiography, coronary arteriography, left and right heart catheterization (RHC), and other baseline clinical data were recorded. Patients were followed up and their survival was recorded.Results: One hundred and eighty-two CAD-HF patients were enrolled, including 142 with HF with a preserved ejection fraction (heart failure with preserved ejection fraction [HFpEF];left ventricular ejection fraction [LVEF] ≥50%) and 40 with a reduced ejection fraction (heart failure with reduced ejection fraction [HFrEF];LVEF < 50%). PH was diagnosed with RHC in 77.5% of patients. Patients with PH showed worse hemodynamic parameters and higher mortality. HFrEF-PH patients had worse survival than HFpEF-PH patients. CAD-HF patients with an enlarged left ventricular end-diastolic diameter and reduced hemoglobin were at higher risk of PH. Nitrate treatment reduced the risk of PH. Elevated creatinine and mean pulmonary arterial pressure (mPAP), diastolic pressure gradient (DPG) ≥7 mmHg, and previous myocardial infarction (MI) entailed a higher risk of mortality in CAD-HF patients with PH.Conclusions: PH is common in CAD-HF and worsens the hemodynamics and survival in these patients. Left ventricle enlargement and anemia increase the risk of PH in CAD-HF. Patients may benefit from nitrate medications. A phase-fi eld method based on the full threaded tree(FTT) for the simulation of casting solidifi cation microstructure was proposed in this paper, and the structure of the full threaded tree and the mesh refi nement method was discussed. During dendritic growth in solidifi cation, the mesh for simulation is adaptively refi ned at the liquid-solid interface, and coarsened in other areas. The numerical results of a threedimension dendrite growth indicate that the phase-fi eld method based on FTT is suitable for microstructure simulation. Most importantly, the FTT method can increase the spatial and temporal resolutions beyond the limits imposed by the available hardware compared with the conventional uniform mesh. At the simulation time of 0.03 s in this study, the computer memory used for computation is no more than 10 MB with the FTT method, while it is about 50 MB with the uniform mesh method. In addition, the proposed FTT method is more effi cient in computation time when compared with the uniform mesh method.
