Extension Protocol for the Investigation on Nutrition Status and Clinical Outcome of Patients with Common Cancers in China(INSCOC)Study:2021 update显示文摘Background Malnutrition is common in patients with cancer, and this adversely affects the survival and quality of life of patients.Chinese Society for Nutritional Oncology issued a multi-center,large-scale,long-term follow-up prospective study, the Investigation on Nutrition Status and Clinical Outcome of Patients with Common Cancers in China(INSCOC study)since 2013.This is an extension to the previous 2013-2020 study protocol. This study still sought to:①address the prognostic impact of nutritional factors and quality of life on cancer patient survival;②describe the overall and cancer-specific incidence and/or distribution of malnutrition and different measurements of patient quality of life. Methods and study design This is an observational,multi-centered,hospital-based prospective cohort study.Data collection will be performed at baseline(within 48 hours after patient admission),during the hospital stay and 30 days after hospital admission.Follow-up will be conducted for 1-20 years after enrollment. The primary outcome will be the all-cause mortality/overall survival, and secondary outcomes will be the length of hospital stay and costs of hospitalization.Study factors will include demographic characteristics, tumor characteristics, information about chronic diseases, hematological measurements(e.g.,red blood cell count, total lymphocyte counts,hemoglobin,albumin,prealbumin,creatinine,C-reactive protein,IL-6),anthropometric measurements(e.g.,height,weight,arm circumference, arm muscle circumference, triceps skinfold thickness, and waist circumference),body composition parameters,PG-SGA scores, quality of life(as indicated by the QLQ-C30 questionnaire),muscle mass(as indicated by the calf circumference),muscle strength(as indicated by the handgrip strength),muscle function(as indicated by the six-meter walking speed test)and physical status assessments(as indicated by the Karnofsky Performance Status scores). This clinical study protocol was approved by local Ethics Committees of all the participating hospitals.Written informed consent is required for each subject included.Discussion This multi-center,large-scale,long-term follow up prospective study will help improve the diagnosis of malnutrition in cancer patients and identify the risk factors associated with adverse clinical outcomes. The anticipated results of this study will highlight the need for a truly scientific appraisal of nutrition therapy in Chinese oncology populations, and finally help treat the potentially reversible elements of malnutrition in cancer patients to improve their clinical outcomes in the future. | Hong Xia Xu Chun Hua Song Liang Yu Yin Chang Wang Zhen Ming Fu Zeng Qing Guo Yuan Lin Ying Ying Shi Wen Hu Yi Ba Su Yi Li Zeng Ning Li Kun Hua Wang Jing Wu Ying He Jia Jun Yang Cong Hua Xie Fu Xiang Zhou Xin Xia Song Gong Yan Chen Wen Jun Ma Su Xia Luo Zi Hua Chen Zhi Kang Chen Ming Hua Cong Hu Ma Chun Ling Zhou Wei Wang Qi Luo Yong Mei Shi Yu Mei Qi Hai Ping Jiang Wen Xian Guan Jun Qiang Chen Xiang Hua Wu Jia Xin Chen Yu Fang Lan Zhou Yong Dong Feng Rong Shao Tan Tao Li Jun Wen Ou Qing Chuan Zhao Jian Xiong Wu Min Weng Qing Hua Yao Ya Yin Yu Quan Jun Lyu Hong Qiu Ming Liu Wei Li Han Ping Shi The Investigation on Nutrition Status and Clinical Outcome of Common Cancers(INSCOC)Group | 2022 | Journal of Nutritional Oncology2022,7,2: | 3
Efficacy and safety of darbepoetin alfa injection replacing epoetin alfa injection for the treatment of renal anemia in Chinese hemodialysis patients:A randomized,open-label,parallel-group,noninferiority phase III trial显示文摘Background:This study was to explore the clinical efficacy and safety of darbepoetin alfa injection replacing epoetin alfa injection(recombinant human erythropoietin injection,rHuEPO)for the treatment of anemia associated with chronic kidney failure in Chinese patients undergoing hemodialysis.Method:This study was a multicenter,randomized,open-label,intergroup parallel control phase III noninferiority trial from April 19,2013 to September 9,2014 at 25 sites. In this study, the members of the darbepoetin alfa group underwent intravenous administration once per week or once every two weeks. The members of the control drug epoetin alfa group underwent intravenous administration two or three times per week. All subjects underwent epoetin alfa administration during the 8-week baseline period. After that, subjects were randomly assigned to the darbepoetin alfa group or epoetin alfa group. The noninferiority in the changes of the average Hb concentrations from the baseline to the end of the evaluation period(noninferiority threshold:-1.0 g/dl)was tested between the two treatments. The time-dependent hemoglobin(Hb)concentration and the maintenance rate of the target Hb concentration(the proportion of subjects with Hb concentrations between 10.0 and 12.0 g/dl)were also evaluated.Iron metabolism, including changes in the serum iron, total iron-binding capacity, ferritin, transferrin saturation, and comparisons of the dose adjustments between the two groups during the treatment period were analyzed further.Adverse events(AEs)were also observed and compared, and the safety was analyzed between the two treatment groups. The conversion rate switching from epoetin alfa to darbepoetin alfa was also discussed.SAS?software version 9.2 was used to perform all statistical analyses.Descriptive statistics were used for all efficacy, safety, and demographic variable analyses, including for the primary efficacy indicators. Results:Four hundred and sixty-six patients were enrolled in this study, and ultimately 384 cases were analyzed for safety, including 267 cases in the darbepoetin alfa group and 117 cases in the epoetin alfa group. There were 211 cases in the per-protocol set, including 152 cases in the darbepoetin alfa group and 59 cases in the epoetin alfa group. The changes in the average Hb concentrations from the baseline to the end of the evaluation period were-0.07 and-0.15 g/dl in the darbepoetin alfa group and epoetin alfa group respectively. The difference between the two groups was 0.08 g/dl(95%confidence interval[CI]:-0.22 to 0.39),and the lower limit of the 95%CI was-0.22>-1.0 g/dl.The average Hb concentrations of the two groups were 10.88-11.43 g/dl(darbepoetin alfa)and 10.91-11.38 g/dl(epoetin alfa)during the study period of Weeks 0-28,with the maintenance rates of the target Hb concentration ranging within 71%-87%and 78%-95%in the darbepoetin alfa group and epoetin alfa group respectively.During the period of comparison between the two groups, the incidence of AEs in the darbepoetin alfa group was 61.42%,while in the epoetin alfa group it was 56.41%.All of the adverse events and reactions in the study were those commonly associated with hemodialysis. Conclusion:The overall efficacy and safety of darbepoetin alfa for the treatment of Chinese renal anemia patients undergoing hemodialysis are consistent with those of epoetin alfa. | Bicheng Liu Nan Chen Jinghong Zhao Aiping Yin Xiongfei Wu Changying Xing Gengru Jiang Junzhou Fu Mei Wang Rong Wang Jianying Niu Ping Fu Zhaohui Ni Fanfan Hou Jiuyang Zhao Jing Chen Yuqing Chen Wei Shi Jianghua Chen Wenge Li Gang Xu Ling Zhong Wenhu Liu Guohua Ding Yuichiro Kondo Changhe Yue Changlin Mei | 2022 | Chronic Diseases and Translational Medicine2022,8,2: | 1
Darbepoetin alfa injection versus epoetin alfa injection for treating anemia of Chinese hemodialysis patients with chronic kidney failure:A randomized,open-label,parallel-group,non-inferiority Phase III trail显示文摘Background:Erythropoietin is a glycoprotein that mainly regulates erythropoiesis. In patients with chronic renal failure with anemia, darbepoetin alfa can stimulate erythropoiesis, correct anemia, and maintain hemoglobin levels. This study was designed to demonstrate the efficacy and safety of darbepoetin alfa injections as being not inferior to epoetin alfa injections(Recombinant Human Erythropoietin injection,rHuEPO)when maintaining hemoglobin(Hb)levels within the target range(10.0-12.0 g/dL)for the treatment of renal anemia.Methods:Ninety-five patients were enrolled in this study from April 15,2013 to April 10,2014 at 25 sites. In this study,patients(n=95)aged 18-70 years were randomized into a once per week intravenous darbepoetin alfa group(n=56)and a twice or three times per week intravenous epoetin alfa group(n=39)for 28 weeks, who had anemia with hemoglobin levels between 6 g/dL and 10 g/dL due to chronic kidney disease(CKD)and were undergoing hemodialysis or hemofiltration with ESA-naive(erythropoiesis stimulating agent-naive).The primary efficacy profile was the mean Hb level(the non-inferiority margin was-1.0 g/dL,week 21-28);the secondary efficacy profiles were the Hb increase rate(week 0-4),the target Hb achievement cumulative rate and time, the change trends of the Hb levels, and the target Hb maintenance ratio.Adverse events(AEs)were observed and compared, and the efficacy and safety were analyzed between the two treatment groups.Additionally, the frequencies of dose adjustments between the darbepoetin alfa and epoetin alfa groups were compared during the treatment period.SAS?software version 9.2 was used to perform all statistical analyses.Descriptive statistics were used for all efficacy, safety, and demographic variable analyses, including for the primary efficacy indicators. Results:The mean Hb level was 11.3 g/dL in the darbepoetin alfa group and 10.7 g/dL in the epoetin alfa group, respectively;the difference of the lower limits of the 95%confidence intervals(CI)between the two groups was 0.1 g/dL(>-1.0 g/dL),and non-inferiority was proven;the Hb levels started to increase in the first four weeks at a similar increase rate;no obvious differences were observed between the groups in the target Hb achievement cumulative rates, and the Hb levels as well as the target Hb level maintenance rate changed over time. The incidence of AEs was 62.5%in the darbepoetin alfa group and 76.9%in the epoetin alfa group. All the adverse events observed in the study were those commonly associated with hemodialysis. Conclusion:Darbepoetin alfa intravenously once per week can effectively increase Hb levels and maintain the target Hb levels well, which makes it not inferior to epoetin alfa intravenously twice or three times per week.Darbepoetin alfa shows an efficacy and safety comparable to epoetin alfa for the treatment of renal anemia. | Nan Chen Changying Xing Jianying Niu Bicheng Liu Junzhou Fu Jiuyang Zhao Zhaohui Ni Mei Wang Wenhu Liu Jinghong Zhao Ling Zhong Xiongfei Wu Wenge Li Yuqing Chen Wei Shi Jianghua Chen Aiping Yin Ping Fu Rong Wang Gengru Jiang Fanfan Hou Guohua Ding Jing Chen Gang Xu Yuichiro Kondo Yuliang Su Changlin Mei | 2022 | Chronic Diseases and Translational Medicine2022,8,1: | 1
AIWW:a new nutrition-screening tool for the oncologic population显示文摘Malnutrition is a common comorbidity among patients with cancer. However,no nutrition-screening tool has been recognized in this population.A quick and easy screening tool for nutrition with high sensitivity and easy-to-use is needed.Based on the previous 25 nutrition-screening tools, the Delphi method was made by the members of the Chinese Society of Nutritional Oncology to choose the most useful item from each category.According to these results,we built a nutrition-screening tool named age, intake, weight, and walking(AIWW).Malnutrition was defined based on the scored patient-generated subjective global assessment(PG-SGA).Concurrent validity was evaluated using the Kendall tau coefficient and kappa consistency between the malnutrition risks of AIWW,nutritional risk screening 2002(NRS-2002),and malnutrition screening tool(MST).Clinical benefit was calculated by the decision curve analysis(DCA),integrated discrimination improvement(IDI),and continuous net reclassification improvement(c NRI).A total of 11,360 patients(male,n=6,024(53.0%)were included in the final study cohort, and 6,363 patients had malnutrition based on PG-SGA.Based on AIWW,NRS-2002,and MST,7,545,3,469,and1,840 patients were at risk of malnutrition, respectively. The sensitivities of AIWW,NRS-2002,and MST risks were 0.910,0.531,and 0.285,and the specificities were 0.768,0.946,and 0.975.The Kendall tau coefficients of AIWW,NRS-2002,and MST risks were 0.588,0.501,and 0.326,respectively.The area under the curve of AIWW,NRS-2002,and MST risks were0.785,0.739,and 0.630,respectively.The IDI,c NRI,and DCA showed that AIWW is non-inferior to NRS-2002(IDI:0.002(-0.009,0.013),c NRI:-0.015(-0.049,0.020)).AIWW scores can also predict the survival of patients with cancer. The missed diagnosis rates of AIWW,NRS-2002,and MST were 0.09%,49.0%,and 73.2%,respectively.AIWW showed a better nutritionscreening effect than NRS-2002 and MST for patients with cancer and could be recommended as an alternative nutritionscreening tool for this population. | Yi-Zhong Ge Zhen-Ming Fu Qi Zhang Meng-Meng Song Guo-Tian Ruan Xi Zhang Xiao-Wei Zhang Xiang-Rui Li Kang-Ping Zhang Meng Tang Xiao-Yue Liu Ming Yang Tong Liu Hai-Lun Xie He-Yang Zhang Zi-Wen Wang Chun-Lei Hu Shi-Qi Lin Rui Zhang Hong-Xia Xu Wei Li Chun-Hua Song Ming Liu Jun-Qiang Chen Kun-Hua Wang Li Bo Ming-Hua Cong Zeng-Ning Li Zeng-Qin Guo Xiao-Bin Wang Bin-Yan Wang Benjamin Xu Xian-Hui Qin Xi-Ping Xu Rocco Barazzoni Qing-Hua Yao Min Weng Xian Shen Han-Ping Shi The Investigation on Nutrition Status and Clinical Outcome of Common Cancers(INSCOC)Group | 2023 | Science China(Life Sciences)2023,66,8: | 1
The nearest neutron star candidate in a binary revealed by optical time-domain surveys显示文摘The near-Earth(within~100 pc)supernova explosions in the past several million years can cause the global deposition of radioactive elements(e.g.,60Fe)on Earth. The remnants of such supernovae are too old to be easily identified. It is therefore of great interest to search for million-year-old near-Earth neutron stars or black holes, the products of supernovae. However, neutron stars and black holes are challenging to find even in our Solar neighbourhood if they are not radio pulsars or X-ray/γ-ray emitters. Here we report the discovery of one of the nearest(127.7±0.3 pc)neutron star candidates in a detached single-lined spectroscopic binary LAMOST J235456.73+335625.9(hereafter J2354).Utilizing the time-resolved ground-based spectroscopy and space photometry,we find that J2354 hosts an unseen compact object with M_(inv)being 1.4-1.6 M_(⊙).The follow-up Swift ultraviolet(UV)and X-ray observations suggest that the UV and X-ray emission is produced by the visible star rather than the compact object.Hence,J2354 probably harbours a neutron star rather than a hot ultramassive white dwarf.Two-hour exceptionally sensitive radio follow-up observations with Five-hundred-meter Aperture Spherical radio Telescope fail to reveal any pulsating radio signals at the 6σflux upper limit of 12.5μJy.Therefore,the neutron star candidate in J2354 can only be revealed via our time-resolved observations.Interestingly, the distance between J2354 and our Earth can be as close as~50 pc around 2.5 million years(Myrs)ago,as revealed by the Gaia kinematics. Our discovery demonstrates a promising way to unveil the hidden near-Earth neutron stars in binaries by exploring the optical time domain, thereby facilitating understanding of the metal-enrichment history in our Solar neighbourhood. | Ling-Lin Zheng Mouyuan Sun Wei-Min Gu Tuan Yi Zhi-Xiang Zhang Pei Wang Junfeng Wang Jianfeng Wu Shan-Shan Weng Song Wang Sen-Yu Qi Jia Zhang Chun-Qian Li Jian-Rong Shi Yong Shao Xiang-Dong Li Jin-Bo Fu Fan Yang Zhongrui Bai Yu Bai Haotong Zhang Jifeng Liu | 2023 | Science China(Physics,Mechanics & Astronomy)2023,66,12: | 0

Renal tubule-targeted dexrazoxane suppresses ferroptosis in acute kidney injury by inhibiting ACMSD显示文摘Acute kidney injury(AKI)is a heterogeneous clinical complication with no existing definite or particular therapies. Therefore, molecular mechanisms and approaches for treating acute kidney injury are in urgent need.Herein,we demonstrated that dexrazoxane(DXZ),a U.S.Food and Drug Administration(FDA)-approved cardioprotective drug, can both functionally and histologically attenuate cisplatin or ischemia-reperfusion injury-induced AKI in vitro and in vivo via inhibiting ferroptosis specifically. This effect is characterized by decreasing lipid peroxidation, shown by the biomarker of oxidative stress 4-hydroxynonenal(HNE)and prostaglandinendoperoxide synthase 2(Ptgs2),while reversing the downregulation of glutathione peroxidase 4(GPX4)and ferritin 1(FTH-1).Mechanistically,the results revealed that DXZ targeted at the renal tubule significantly inhibits ferroptosis by suppressingα-amino-β-carboxymuconate-ε-semialdehyde decarboxylase(ACMSD).Furthermore, the conjugation of dexrazoxane and polysialic acid(DXZ-PSA)is specifically designed and utilized to enhance the therapeutic effect of DXZ by long-term effect in the kidney, especially retention and targeting in the renal tubules. This study provides a novel therapeutic approach and mechanistic insight for AKI by inhibiting ferroptosis through a new type drug DXZPSA with the enhanced renal distribution. | Yunjing Zhang Jicheng Wu Quanlin An Huanhuan Zhu Xinwan Su Ying Wang Xishao Xie Jian Zhang Xi Yao Chunhua Weng Shi Feng Jianhua Mao Xianghui Fu Fei Han Xin Cao Ben Wang Weiqiang Lin | 2023 | Nano Research2023,16,7: | 0
