2A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. 32018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 8China National Medical Products Administration approval summary:anlotinib for the treatment of advanced non-small cell lung cancer after two lines of chemotherapy显示文摘Background:On May 8,2018,the China National Medical Products Administration(NMPA)approved anlotinib,an orally administered anti-angiogenesis inhibitor,for the treatment of patients with advanced non-small cell lung can-cer(NSCLC)who have progressed after treatment with two or more lines of prior systemic chemotherapy.Main body of the abstract:China NMPA reviewed and inspected a regional double-blinded,placebo-controlled,Phase III trial comparing the overall survival(OS)of NSCLC patients between the anlotinib and placebo arms.A total of 437 patients were randomized(2:1)to receive either anlotinib(n=294)or placebo(n=143)once daily on a 2-week on and 1-week off schedule.Patients with epidermal growth factor receptor(EGFR)or activating anaplastic lymphoma kinase(ALK)genomic tumor aberrations should have disease progression on NMPA-approved therapy.Anlotinib is the first NMPA-approved drug for patients with advanced NSCLC who have progressed on at least two lines of prior systemic chemotherapies in China.The approval was based on a statistically and clinically significant improvement in median OS with anlotinib(9.46 months)compared with placebo[6.37 months;hazard ratio(HR])=0.70,95%confidence interval(CI)=0.55-0.89;two-sided log-rank P=0.002].The confirmed objective response rate(ORR)was 9.2%in the anlotinib arm and 0.7%in the placebo arm.The median duration of response(DoR)was 4.83 months,with a 95%CI of 3.31-6.97 months.The toxicity profile of anlotinib was consistent with that of known anti-angiogenesis inhibitors.Common adverse drug reactions(ADRs)in anlotinib-treated patients included hypertension(67.4%),hand-foot syndrome(43.9%),hemoptysis(14.0%),thyroid stimulating hormone(TSH)elevation(46.6%),and corrected QT interval(QTc)prolongation(26.2%).Short conclusion:Anlotinib demonstrated a clinically significant OS prolongation as a novel therapeutic option for advanced or metastatic NSCLC following at least two lines of chemotherapy.Ming Zhou Xiaoyuan Chen Hong Zhang Lin Xia Xin Tong Limin Zou Ruimin Hao Jianhong Pan Xiao Zhao Dongmei Chen Yuanyuan Song Yueli Qi Ling Tang Zhifang Liu Rong Gao Yuankai Shi Zhimin Yang 2019Cancer Communications2019,39,1:35
9Sr-Nd isotopic compositions of the Changjiang sediments: Implications for tracing sediment sources显示文摘The suspended particulate and fine-grained floodplain sediments were collected from the main stream and tributaries of the Changjiang River for Sr-Nd isotopic measurements. The εNd(0) values gradually decrease downstream from -10.8 on average in the upper reaches to -12.3 in the lower reaches, whereas the 87Sr/86Sr ratios increase correspondingly, averaging 0.721899 and 0.725826 respectively in the upper and middle-lower reaches. The compositional variations primarily reflect the complex con- trols of provenance rocks, chemical weathering, and sediment characters between different catchments, among which the abnormal Sr-Nd isotopic compositions of the Yalong, Fujiang, Tuojiang and Yuanjiang rivers indicate the sediment provenance contributions from the Emeishan Basalt in the upper reaches and the old metamorphic and siliceous rocks in the middle-lower reaches. The Sr-Nd isotopic ratios of the Changjiang sediments can better reflect the average composition of weathered continental crust compared to other major rivers in the world because of the unique source rock types in the Changjiang drainage basin. 12The Association of Maternal Body Composition and Dietary Intake with the Risk of Gestational Diabetes Mellitus during the Second Trimester in a Cohort of Chinese Pregnant Women显示文摘Objective To investigate the association of maternal body composition and dietary intake with the risk of gestational diabetes mellitus(GDM). Methods A total 154 GDM subjects and 981 controls were enrolled in a prospective cohort study in 11 hospitals from May 20, 2012 to December 31, 2013. Bioelectrical impedance analysis and dietary surveys were used to determine body composition and to evaluate the intake of nutrients in subjects at 21-24 weeks' gestation(WG). Logistic regression analysis was applied to explore the relationships of maternal body composition and dietary intake with the risk of GDM morbidity. Results Age, pre-pregnant body weight(BW), and body mass index(BMI) were associated with increased risk of GDM. Fat mass(FM), fat mass percentage(FMP), extracellular water(ECW), BMI, BW, energy, protein, fat, and carbohydrates at 21-24 WG were associated with an increased risk of GDM. In contrast, fat free mass(FFM), muscular mass(MM), and intracellular water(ICW) were associated with a decreased risk of GDM. 13Spatio-temporal trend and statistical distribution of extreme precipitation events in Huaihe River Basin during 1960-2009显示文摘Based on the daily precipitation data of 27 meteorological stations from 1960 to 2009 in the Huaihe River Basin, spatio-temporal trend and statistical distribution of extreme precipitation events in this area are analyzed. Annual maximum series (AM) and peak over threshold series (POT) are selected to simulate the probability distribution of extreme precipitation. The results show that positive trend of annual maximum precipitation is detected at most of used stations, only a small number of stations are found to depict a negative trend during the past five decades, and none of the positive or negative trend is significant. The maximum precipitation event almost occurred in the flooding period during the 1960s and 1970s. By the L-moments method, the parameters of three extreme distributions, i.e., Generalized extreme value distribution (GEV), Generalized Pareto distribution (GP) and Gamma distribution are estimated. From the results of goodness of fit test and Kolmogorov-Smirnov (K-S) test, AM series can be better fitted by GEV model and POT series can be better fitted by GP model. 14Induction of apoptosis of human gastric carcinoma SGC-7901 cell line by 5,7-dihydroxy-8-nitrochrysin in vitro显示文摘AIM: To investigate the effect of 5, 7-dihydroxy-8- nitrochrysin (NOChR) on apoptosis of human gastric carcinoma SGC-7901 cell line. METHODS: SGC-7901 cells were cultured in vitro and the inhibitory effect of NOChR on proliferation of SGC-7901 cells was measured by using an MTT assay. NOChR-induced apoptosis rate of SGC-7901 cells was detected using flow cytometry (FCM) with PI staining. DNA ladder bands were observed by DNA agarose gel electrophoresis. The influence of NOChR on the proxisome proliferator-activated receptor-γ (PPARγ), Bcl-2 and Bax protein expression of SGC-7901 cells was analyzed by Western blot. RESULTS: MTT assay showed that NOChR markedly inhibited proliferation of SGC-7901 cells in a dose- dependent manner, and when IC50 was 4.14 μmol/L, the potency of NOChR was 10 times than that of lead compound, chrysin (ChR, IC50 was 40.56 μmol/L), and was similar to 5-fluorouracil (5-FU, IC50 was 4.51 μmol/L). FCM with propidium iodide (PI) staining demonstrated that the apoptosis rates of SGC-7901 cells treated with 1.25, 5.00 and 20.00 μmol/L NOChR for 48 h were 9.8% ± 0.2%, 36.8% ± 1.9% and 45.5% ± 3.5%, respectively, and were significantly higher when treated with 5.00 and 20.00 μmol/L NOChR than that with 20.00 μmol/L ChR (12.9% ± 1.5%). DNA agarose gel electrophoresis showed that treatment of SGC-7901 cells with 20.00 μmol/L NOChR for 48 h resulted in typical DNA ladder bands of DNA of SGC-7901 cells, which could be eliminated by treating with 10.00 μmol/L GW9662, a blocker of PPARγ. Western blot analysis revealed that after 24 h of treatment with 20.00 μmol/L NOChR, PPARgamma and Bax protein expression of SGC-7901 cells increased but Bcl-2 expression decreased; however, pre-incubation with 10.00 μmol/L GW9662 could efficiently antagonize and weaken the regulatory effect of 20.00 μmol/L NOChR on Bax and Bcl-2 protein expression of SGC-7901 cells. 15Genome analysis of Taraxacum kok-saghyz Rodin provides new insights into rubber biosynthesis显示文摘The Russian dandelion Taraxacum kok-saghyz Rodin(TKS), a member of the Composite family and a potential alternative source of natural rubber(NR) and inulin, is an ideal model system for studying rubber biosynthesis. Here we present the draft genome of TKS, the first assembled NR-producing weed plant. The draft TKS genome assembly has a length of 1.29 Gb, containing 46 731 predicted protein-coding genes and68.56% repeats, in which the LTR-RT elements predominantly contribute to the genome enlargement. We analyzed the heterozygous regions/genes, suggesting its possible involvement in inbreeding depression.Through comparative studies between rubber-producing and non-rubber-producing plants, we found that enzymes of the mevalonate(MVA) pathway and rubber elongation might be critical for rubber biosynthesis, and several key isoforms have been isolated and shown to be predominantly expressed in the latex, indicating their crucial functions in rubber biosynthesis. Moreover, for two important families in rubber elongation, the CPT/CPTL and REF/SRPP families, diverse evolutionary tracks have been revealed. Effect of S-ODNson SMMC-7721 human hepatoma cell growth invitro was determined using microculturetetrazolium assay. In vivo antitumor activitiesof S-ODNs were monitored by measuring tumorweight differences in treated and control micebearing SMMC-7721 xenografts. Induction of cellapoptosis was evaluated by fluorescence-activated cell sorter (FACS) analysis.RESULTS Antisense S-ODN treatment led toreduced AFP gene expression. Specificantisense S-ODNs, but not control S-ODNs,inhibited the growth of heaptoma cells in vitro.In vivo. only antisense S-ODNs exhibitedobvious antitumor activities. 17Therapy for acute pancreatitis with platelet-activating factor receptor antagonists显示文摘Acute pancreatitis (AP) causes release of platelet- activating factor (PAF), which induces systemic effects that contribute to circulatory disturbances and multiple organ failure. PAF is a cell surface secretion of bioactive lipid, which could produce physiological and pathological effects by binding to its cell surface receptor called platelet-activating factor receptor (PAF-R). Studies showed that PAF participates in the occurrence and development of AP and administration of platelet-activating factor receptor antagonists (PAF-RAs) could significantly reduce local and systemic events after AP. PAF has also been implicated as a key mediator in the progression of severe AP, which can lead to complications and unacceptably high mortality rates. Several classes of PAF-RA show PAF- RAs significant local and systemic effects on reducing inflammatory changes. As a preventive treatment, PAF-RA could block a series of PAF-mediated inflammatory injury and thus improve the prognosis of AP. 18BMI, WC, WHtR, VFI and BFI: Which Indictor is the Most Efficient Screening Index on Type 2 Diabetes in Chinese Community Population显示文摘Objective Obesity is a major risk factor for type 2 diabetes, many indexes can be used to describes obesity and predict diabetes. This research attempts to identify the best indicator of obesity to screening diabetes in Chinese population. Methods A cross-sectional data of 8121 subjects aged 35-60 years were included in this research belongs to the Diabetes Appropriate Technology Intervention Study. Anthropometric indicators including body weight, height, waist circumferences (WC), body fat index (BFI) and visceral fat index (VFI) and blood biochemical indicators after an overnight fast [fasting blood glucose, total cholesterol, high-density lipoprotein (HDL) cholesterol, and triacylglycerol] were measured. BMI (body mass index) and Weight to Height Ratio was calculated. Results Subjects with obesity had a higher risk of physician diagnosed diabetes (OR=2.50, 95% CI 1.83-3.43), new diagnosed diabetes (OR=4.23, 95% CI 2.91-6.15) and pre-diabetes (OR=1.75, 95% CI 1.31-2.34) compared to those with normal Body mass index (BMI). There was a significant trend of increased risk of all diabetes status with increased waist circumference (WC). The waist-to-height ratio (WHtR) yielded the most significant association with new diagnosed diabetes and physician diagnosed diabetes than other indices. Conclusion Central obesity is significantly correlated with diabetes. 20Theoretical Analysis and Experimental Verification on Flow Field of Piezoelectric Pump with Unsymmetrical Slopes Element显示文摘Regular valveless piezoelectric pumps have rectifying elements outside their chambers to produce net flow.These rectifying elements outside the chamber will increase the overall volume of the pump and prevent its minimization.Valveless piezoelectric pump with unsymmetrical slopes elements(USE),proposed in this paper,differs from other valveless pumps in that it is easy to be minimized by developing the chamber bottom as such a rectifying element.In this research,the working principle of the proposed pump was analyzed first.Numerical models were thereby established and numerical simulation was conducted to the chamber flow field with the method of time-dependent velocity.The effects of the USEs on the flow field in the chamber were shown clearly in simulation.And the particular feature of flow field in the chamber was discovered.It behaves a complex flow field,in which strong turbulent occurs companying a lot of vortexes in different directions and different sizes.This feature is just opposite to what regular piezoelectric pumps expect:a moderate flow field.The turbulent flow could be used to have different liquids stirred and well mixed in the chamber to produce homogeneous solution,emulsion or turbid liquid.Meanwhile,numerical simulation also presents the effect of the angles difference of the two slopes upon the flow field,and upon the flow rate of the pump,which fits to the theoretical analysis.Experiments with the proposed pump were also conducted to verify the numerical results.In these experiments,six USEs with different slope angles were used for efficiency tests,which proved the validity and reliability of the numerical analysis.The data obtained from numerical analysis agree well with that from the experiments.The errors ranged from 4.4% to 14.8% with their weighted average error being 9.7%.XIA Qixiao ZHANG Jianhui LEI Hong CHENG Wei 2009Chinese Journal of Mechanical Engineering2009,22,5:18
