| 1 | Association of Fusobacterium nucleatum with immunity andmolecular alterations in colorectal cancer显示文摘The human intestinal microbiome plays a major role in human health and diseases, including colorectal cancer. Colorectal carcinogenesis represents a heterogeneous process with a differing set of somatic molecular alterations, influenced by diet, environmental and microbial exposures, and host immunity. Fusobacterium species are part of the human oral and intestinal microbiota. Metagenomic analyses have shown an enrichment of Fusobacterium nucleatum(F. nucleatum) in colorectal carcinoma tissue. Using 511 colorectal carcinomas from Japanese patients, we assessed the presence of F. nucleatum. Our results showed that the frequency of F. nucleatum positivity in the Japanese colorectal cancer was 8.6%(44/511), which was lower than that in United States cohort studies(13%). Similar to the United States studies, F. nucleatum positivityin Japanese colorectal cancers was significantly associated with microsatellite instability(MSI)-high status. Regarding the immune response in colorectal cancer, high levels of infiltrating T-cell subsets(i.e., CD3+, CD8+, CD45RO+, and FOXP3+ cells) have been associated with better patient prognosis. There is also evidence to indicate that molecular features of colorectal cancer, especially MSI, influence T-cell-mediated adaptive immunity. Concerning the association between the gut microbiome and immunity, F. nucleatum has been shown to expand myeloid-derived immune cells, which inhibit T-cell proliferation and induce T-cell apoptosis in colorectal cancer. This finding indicates that F. nucleatum possesses immunosuppressive activities by inhibiting human T-cell responses. Certain micro RNAs are induced during the macrophage inflammatory response and have the ability to regulate host-cell responses to pathogens. Micro RNA-21 increases the levels of IL-10 and prostaglandin E2, which suppress antitumor T-cell-mediated adaptive immunity through the inhibition of the antigen-presenting capacities of dendritic cells and T-cell proliferation in colorectal cancer cells. Thus, emerging evidence may provide insights for strategies to target microbiota, immune cells and tumor molecular alterations for colorectal cancer prevention and treatment. | 3 | Present status of endoscopic mastectomy for breast cancer显示文摘Endoscopy is now being used for breast cancer surgery. Though it is used for mastectomy, lymph node dissection, and breast reconstruction, its prime use is for mastectomy. Because an incision can be placed inconspicuously in the axillary site, a relatively largeincision can be created. A retractor with an endoscope, CO2, and an abrasion device with the endoscope are used for operation space security. It is extremely rare that an endoscope is used for lymph node dissection. For breast reconstruction, it may be used for latissimus muscle flap making, but an endoscope is rarely used for other reconstructions. Endoscopic mastectomy is limited to certain institutions and practiced hands, and it has not been significantly developed in breast cancer surgery. On the other hand, endoscopic surgery may be used widely in breast reconstruction. | 4 | Arterial steroid injection therapy can inhibit the progression of severe acute hepatic failure toward fulminant liver failure显示文摘AIM: To utilize transcatheter arterial steroid injection therapy (TASIT) via the hepatic artery to reduce hepatic macrophage activity in patients with severe acute hepatic failure.METHODS: Thirty-four patients with severe acute hepatic failure were admitted to our hospital between June 2002 to June 2006 providing for the possibility of liver transplantation (LT). Seventeen patients were treated using traditional liver supportive procedures, and the other 17 patients additionally underwent TASIT with 1000 mg methylprednisolone per day for 3 continuous days. RESULTS: Of the 17 patients who received TASIT, 13 were cured without any complications, 2 died, and 2 underwent LT. Of the 17 patients who did not receive TASIT, 4 were self-limiting, 7 died, and 6 underwent LT. Univariate logistic analysis revealed that ascites, serum albumin, prothrombin time, platelet count, and TASIT were significant variables for predicating the prognosis. Multivariate logistic regression analysis using stepwise variable selection showed that prothrombin time, platelet count, and TASIT were independent predictive factors. | 5 | Imaging pitfalls of pancreatic serous cystic neoplasm and its potential mimickers显示文摘The aim of this article is to clarify diagnostic pitfalls of pancreatic serous cystic neoplasm(SCN) that may result in erroneous characterization. Usual and unusual imaging findings of SCN as well as potential SCN mimickers are presented. The diagnostic key of SCN is to look for a cluster of microcysts(honeycomb pattern), which may not be always found in the center. Fibrosis in SCN may be mistaken for a mural nodule of intraductal papillary mucinous neoplasm(IPMN). The absence of cyst wall enhancement may be helpful to distinguish SCN from mucinous cystic neoplasm. However, oligocystic SCN and branch duct type IPMN may morphologically overlap. | 6 | Mutations in carboxy-terminal part of E2 including PKR/eIF2αphosphorylation homology domain and interferon sensitivity determining region of nonstructural 5A of hepatitis C virus 1b:Their correlation with response to interferon monotherapy and viral load显示文摘AIM: To study the amino acid substitutions in the carboxy (C)-terminal part of E2 protein and in the interferon (IFN) sensitivity determining region (ISDR) and their correlation with response to IFN and viral load in 85 hepatitis C virus (HCV)-lb-infected patients treated with IFN. METHODS: The C-terminal part of E2 (codons 617-711) including PKR/eIF2a phosphorylation homology domain (PePHD) and ISDR was sequenced in 85 HCV-1b-infected patients treated by IFN monotherapy. RESULTS: The amino acid substitutions in PePHD detected only in 4 of 85 patients were not correlated either with response to IFN or with viral load. The presence of substitutions in a N-terminal variable region (codons 617-641) in the C-terminal part of E2 was significantly correlated with both small viral load (33.9% vs 13.8%, P = 0.0394) and sustained response to IFN (25.0% vs 6.9 %, P = 0.0429). Four or more substitutions in ISDR were significantly correlated with both small viral load (78.6% vs 16.2%, P < 0.0001) and sustained response to IFN (85.7% vs 2.9%, P < 0.0001). In multivariate analysis, ISDR in nonstructural (NS) 5A (OR = 0.39, P < 0.0001) and N-terminal variable region (OR = 0.51, P = 0.039) was selected as the independent predictors for small viral load, and ISDR (OR = 39.0, P < 0.0001) was selected as the only independent predictor for sustained response. | 7 | A case of asymptomatic intraductal papillary neoplasm of the bile duct without hepatolithiasis显示文摘A 65-year-old woman was found to have dilatation of the intrahepatic bile duct in the right anterior segment during a general health. Laboratory data were within normal ranges and no solid mass was detected in her abdominal computer tomography (CT) or nuclear magnetic resonance imaging (MRI). However, endoscopic retrograde cholangiopancreatography (ERCP) demonstrated an obstruction of the right bile duct. Intraoperative cholangiography showed stenosis of the intrahepatic bile duct in the anterior inferior segment (B5) and narrowness of the intrahepatic bile duct in the anterior superior segment (B8), so that we strongly suspected intrahepatic cholangiocarcinoma (ICC). Histologically, surgically resected liver specimens, without tumor mass by macroscopic observation, showed intraductal papillary proliferation with fibrovascular cores and intraductal spreading of carcinoma in situ throughout a considerable area, especially in bile ductules around the peripheral small portal area. Furthermore, the immunohistochemical profile of the tumor (MUC5AC+/CK7+) was compatible with an intraductal papillary neoplasm of the bile duct (IPN-B). Consequently, this case was diagnosed as IPN-B with spreading CIS, stageⅠ(pT1, pN0, P0, H1, M0). | 11 | Image quality and diagnostic performance of free-breathing diffusion-weighted imaging for hepatocellular carcinoma显示文摘AIM To retrospectively evaluate the diagnostic performance of free-breathing diffusion-weighted imaging(FB-DWI) with modified imaging parameter settings for detecting hepatocellular carcinomas(HCCs).METHODS Fifty-one patients at risk for HCC were scanned with both FB-DWI and respiratory-triggered DWI with the navigator echo respiratory-triggering technique(RTDWI).Qualitatively,the sharpness of the liver contour,the image noise and the chemical shift artifacts on each DWI with b-values of 1000 s/mm2 were independently evaluated by three radiologists using 4-point scoring.Wecompared the image quality scores of each observer between the two DWI methods,using the Wilcoxon signed-rank test.Quantitatively,we compared the signal-to-noise ratios(SNRs) of the liver parenchyma and lesion-to-nonlesion contrast-to-noise ratios(CNRs) after measuring the signal intensity on each DWI with a b-factor of 1000 s/mm2.The average SNRs and CNRs between the two DWI methods were compared by the paired t-test.The detectability of HCC on each DWI was also analyzed by three radiologists.The detectability provided by the two DWI methods was compared using Mc Nemar's test.RESULTS For all observers,the averaged image quality scores of FB-DWI were:Sharpness of the liver contour [observer(Obs)-1,3.08 ± 0.81;Obs-2,2.98 ± 0.73;Obs-3,3.54 ± 0.75],those of the distortion(Obs-1,2.94 ± 0.50;Obs-2,2.71 ± 0.70;Obs-3,3.27 ± 0.53),and the chemical shift artifacts(Obs-1,3.38 ± 0.60;Obs-2,3.15 ± 1.07;Obs-3,3.21 ± 0.85).The averaged image quality scores of RTDWI were:Sharpness of the liver contour(Obs-1,2.33 ± 0.65;Obs-2,2.37 ± 0.74;Obs-3,2.75 ± 0.81),distortion(Obs-1,2.81 ± 0.56;Obs-2,2.25 ± 0.74;Obs-3,2.96 ± 0.71),and the chemical shift artifacts(Obs-1,2.92 ± 0.59;Obs-2,2.21 ± 0.85;Obs-3,2.77 ± 1.08).All image quality scores of FB-DWI were significantly higher than those of RT-DWI(P < 0.05).The average SNR of the normal liver parenchyma by FB-DWI(11.0 ± 4.8) was not significantly different from that shown by RT-DWI(11.0 ± 5.0);nor were the lesion-to-nonlesion CNRs significantly different(FB-DWI,21.4 ± 17.7;RT-DWI,20.1 ± 15.1).For all three observers,the detectability of FB-DWI(Obs-1,43.6%;Obs-2,53.6%;and Obs-3,45.0%) was significantly higher than that of RT-DWI(Obs-1,29.1%;Obs-2,43.6%;and Obs-3,34.5%)(P < 0.05).CONCLUSION FB-DWI showed better image quality and higher detectability of HCC compared to RT-DWI,without significantly reducing the SNRs of the liver parenchyma and lesionto-nonlesion CNRs. | Yukihisa Takayama Akihiro Nishie Yoshiki Asayama Kousei Ishigami Daisuke Kakihara Yasuhiro Ushijima Nobuhiro Fujita Ken Shirabe Atsushi Takemura Hiroshi Honda | 2017 | World Journal of Hepatology2017,9,14: | 3 |
