| 2 | Handgrip Strength as a Predictor of Nutritional Status in Chinese Elderly Inpatients at Hospital Admission显示文摘Objective To assess nutritional status and define gender-and age-specific handgrip strength(HGS) cut-point values for malnutrition or nutritional risk in elderly inpatients. Methods A cross-sectional study of 1,343 elderly inpatients was conducted in the Chinese PLA General Hospital. Nutrition Risk Screening(NRS 2002) and Subjective Global Assessment(SGA) were administered. Anthropometric measurements and blood biochemical indicators were obtained using standard techniques. The gender-and age-specific receiver operating characteristic(ROC) curves were constructed to evaluate the HGS for nutritional status by SGA and NRS 2002. Sensitivity, specificity, and areas under the curves(AUCs) were calculated. Results According to NRS 2002 and SGA, 63.81% of elderly inpatients were at nutritional risk and 28.22% were malnourished. Patients with higher HGS had an independently decreased risk of malnutrition and nutritional risk. The AUCs varied between 0.670 and 0.761. According to NRS 2002, the optimal HGS cut-points were 27.5 kg(65-74 years) and 21.0 kg(75-90 years) for men and 17.0 kg(65-74 years) and 14.6 kg(75-90 years) for women. According to SGA, the optimal HGS cut-points were 24.9 kg(65-74 years) and 20.8 kg(75-90 years) for men and 15.2 kg(65-74 years) and 13.5 kg(75-90 years) for women. Conclusion Elderly inpatients had increased incidence of malnutrition or nutritional risk. | 4 | Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). | 18 | Development and Assessment of Two Highly Pathogenic Avian Influenza(HPAI) H5N6 Candidate Vaccine Viruses for Pandemic Preparedness显示文摘Objective In China, 24 cases of human infection with highly pathogenic avian influenza(HPAI) H5 N6 virus have been confirmed since the first confirmed case in 2014. Therefore, we developed and assessed two H5 N6 candidate vaccine viruses(CVVs).Methods In accordance with the World Health Organization(WHO) recommendations, we constructed two reassortant viruses using reverse genetics(RG) technology to match the two different epidemic H5 N6 viruses. We performed complete genome sequencing to determine the genetic stability. We assessed the growth ability of the studied viruses in MDCK cells and conducted a hemagglutination inhibition assay to analyze their antigenicity. Pathogenicity attenuation was also evaluated in vitro and in vivo.Results The results showed that no mutations occurred in hemagglutinin or neuraminidase, and both CVVs retained their original antigenicity. The replication capacity of the two CVVs reached a level similar to that of A/Puerto Rico/8/34 in MDCK cells. | 20 | Deciphering transcriptome alterations in bone marrow hematopoiesis at single-cell resolution in immune thrombocytopenia显示文摘Immune thrombocytopenia(ITP)is an autoimmune disorder,in which megakaryocyte dysfunction caused by an autoimmune reaction can lead to thrombocytopenia,although the underlying mechanisms remain unclear.Here,we performed single-cell transcriptome profiling of bone marrow CD34+hematopoietic stem and progenitor cells(HSPCs)to determine defects in megakaryopoiesis in ITP.Gene expression,cell-cell interactions,and transcriptional regulatory networks varied in HSPCs of ITP,particularly in immune cell progenitors.Differentially expressed gene(DEG)analysis indicated that there was an impaired megakaryopoiesis of ITP.Flow cytometry confirmed that the number of CD9+and HES1+cells from Lin−CD34+CD45RA−HSPCs decreased in ITP.Liquid culture assays demonstrated that CD9+Lin−CD34+CD45RA−HSPCs tended to differentiate into megakaryocytes;however,this tendency was not observed in ITP patients and more erythrocytes were produced.The percentage of megakaryocytes differentiated from CD9+Lin−CD34+CD45RA−HSPCs was 3-fold higher than that of the CD9−counterparts from healthy controls(HCs),whereas,in ITP patients,the percentage decreased to only 1/4th of that in the HCs and was comparable to that from the CD9−HSPCs.Additionally,when co-cultured with pre-B cells from ITP patients,the differentiation of CD9+Lin−CD34+CD45RA−HSPCs toward the megakaryopoietic lineage was impaired.Further analysis revealed that megakaryocytic progenitors(MkP)can be divided into seven subclusters with different gene expression patterns and functions.The ITP-associated DEGs were MkP subtype-specific,with most DEGs concentrated in the subcluster possessing dual functions of immunomodulation and platelet generation.This study comprehensively dissects defective hematopoiesis and provides novel insights regarding the pathogenesis of ITP. | Yan Liu Xinyi Zuo Peng Chen Xiang Hu Zi Sheng Anli Liu Qiang Liu Shaoqiu Leng Xiaoyu Zhang Xin Li Limei Wang Qi Feng Chaoyang Li Ming Hou Chong Chu Shihui Ma Shuwen Wang Jun Peng | 2022 | Signal Transduction and Targeted Therapy2022,7,11: | 0 |
