3Radiofrequency ablation of hepatocellular carcinoma sized> 3 and ≤ 5 cm: Is ablative margin of more than 1 cm justified?显示文摘AIM:To investigate whether an ablative margin(AM)>1.0 cm might reduce chance of recurrence for patients with hepatocellular carcinoma(HCC)tumors 3.1to 5.0 cm in size,compared with an AM of 0.5-1.0 cm.METHODS:From October 2005 to December 2012,936 consecutive patients with HCC who received radiofrequency ablation were screened.Of these,281 patients,each with a single primary HCC tumor of 3.1 to5.0 cm in size on its greatest diameter,were included in the study.Based on the AM width,we categorized patients into the 0.5-1.0 cm group and the>1.0 cm group.Local tumor progression(LTP)-free survival,intrahepatic distant recurrence(IDR)-free survival and overall survival(OS)rates were obtained using the Kaplan-Meier method.RESULTS:The 1-,2-,3-,4-,and 5-year LTP-free survival rates and IDR-free survival rates were significantly higher in the>1.0 cm group compared with the 0.5-1.0cm group(97.5%,86.3%,73.6%,49.5%and 26.4%vs 91.3%,78.4%,49.5%,27.8%,and 12.8%;95.1%,90.3%,77.0%,61.0%and 48.3%vs 95.2%,85.9%,62.6%,47.2%and 28.5%;P<0.05).The 1-,2-,3-,4-,and 5-year OS rates were 98.6%,91.5%,69.2%,56.0%and 42.2%,respectively,in the 0.5-1.0 cm group and 100%,98.9%,90.1%,68.7%and 57.4%,respectively,in the>1.0 cm group(P=0.010).There were no significant differences in complication rates between the two groups.Both univariate and multivariate analyses identified AM as an independent prognostic factor linked to LTP,IDR,and OS.CONCLUSION:For HCC tumors>3.0 cm and≤5.0cm,AM>1.0 cm could reduce chances of recurrence compared with AM of 0.5-1.0 cm,emphasizing the need for a more defensive strategy using AMs>1.0 cm for ablating HCC tumors of 3.1 to 5.0 cm.Shan Ke Xue-Mei Ding Xiao-Jun Qian Yi-Ming Zhou Bao-Xin Cao Kun Gao Wen-Bing Sun 2013World Journal of Gastroenterology2013,19,42:21

6Exogenous NADPH ameliorates myocardial ischemia–reperfusion injury in rats through activating AMPK/mTOR pathway显示文摘Our previous study shows that nicotinamide adenine dinucleotide phosphate(NADPH)plays an important role in protecting against cerebral ischemia injury.In this study we investigated whether NADPH exerted cardioprotection against myocardial ischemia/reperfusion(I/R)injury.To induce myocardial I/R injury,rats were subjected to ligation of the left anterior descending branch of coronary artery for 30 min followed by reperfusion for 2 h.At the onset of reperfusion,NADPH(4,8,16 mg·kg?1·d?1,iv)was administered to the rats.We found that NADPH concentrations in plasma and heart were signi?cantly increased at 4 h after intravenous administration.Exogenous NADPH(8?16 mg/kg)signi?cantly decreased myocardial infarct size and reduced serum levels of lactate dehydrogenase(LDH)and cardiac troponin I(cTn-I).Exogenous NADPH signi?cantly decreased the apoptotic rate of cardiomyocytes,and reduced the cleavage of PARP and caspase-3.In addition,exogenous NADPH reduced mitochondrial vacuolation and increased mitochondrial membrane protein COXIV and TOM20,decreased BNIP3L and increased Bcl-2 to protect mitochondrial function.We conducted in vitro experiments in neonatal rat cardiomyocytes(NRCM)subjected to oxygen–glucose deprivation/restoration(OGD/R).Pretreatment with NADPH(60,500 nM)signi?cantly rescued the cell viability and inhibited OGD/R-induced apoptosis.Pretreatment with NADPH signi?cantly increased the phosphorylation of AMPK and downregulated the phosphorylation of mTOR in OGD/R-treated NRCM.Compound C,an AMPK inhibitor,abolished NADPH-induced AMPK phosphorylation and cardioprotection in OGD/R-treated NRCM.In conclusion,exogenous NADPH exerts cardioprotection against myocardial I/R injury through the activation of AMPK/mTOR pathway and inhibiting mitochondrial damage and cardiomyocyte apoptosis.NADPH may be a potential candidate for the prevention and treatment of myocardial ischemic diseases.Jiang Zhu Yi-fei Wang Xiao-ming Chai Ke Qian Ling-wei Zhang Peng Peng Pei-min Chen Jian-fang Cao Zheng-hong Qin Rui Sheng Hong Xie 2020Acta Pharmacologica Sinica2020,41,4:19
7Virus-induced accumulation of intracellular bile acids activates the TGR5-β-arrestin-SRC axis to enable innate antiviral immunity显示文摘The mechanisms on metabolic regulation of immune responses are still elusive. We show here that viral infection induces immediate-early NF-kB activation independent of viral nucleic acid-triggered signaling, which triggers a rapid transcriptional induction of bile acid (BA) transporter and rate-limiting biosynthesis enzymes as well as accumulation of intracellular BAs in divergent cell types. The accumulated intracellular BAs activate SRC kinase via the TGR5-GRK-卩-arrestin axis, which mediates tyrosine phosphorylation of multiple antiviral signaling components including RIG-I, VISA/MAVS, MITA/STING, TBK1 and IRF3. The tyrosine phosphorylation of these components by SRC conditions for efficient innate antiviral immune response. Consistently, TGR5 deficiency impairs innate antiviral immunity, whereas BAs exhibit potent antiviral activity in wild-type but not TGR5-deficient cells and mice. 10Downregulation of serum mi R-17 and mi R-106b levels in gastric cancer and benign gastric diseases显示文摘Altered micro RNA(mi RNA) associated with gastric cancer(GC) development and mi R-17 and mi R-106 b were differentially expressed in GC tissues. This study detected serum levels of mi R-17 and mi R-106 b expression in GC, benign gastric disease(BGD) and healthy controls to assess them as tumor markers for GC. Serum samples from 40 GC, 32 BGD(10 gastric ulcer, 14 gastric polyps, and 8 gastric ulcer with polyps) and 36 healthy individuals were subjected to quantitative reverse transcription polymerase chain reaction(q RT-PCR) analysis of mi R-17 and mi R-106 b expression. The data showed that the serum levels of mi R-17 and mi R-106 b were significantly reduced in healthy individuals and BGD patients compared to GC patients. There was a significant association of mi R-17 and mi R-106 b expression with age, but not with other clinicopathological features, such as gender, tumor differentiation, stage and lymphatic metastasis. Further analysis showed that, in discriminating GC patients from healthy controls, mi R-17 could yield a receiver-operating characteristic(ROC) area under the curve(AUC) of 0.879 with 80.6% sensitivity and 87.5% specificity and mi R-106 b could yield an AUC of 0.856 with 75.0% sensitivity and 92.5% specificity. The combined AUC of mi R-17 and mi R-106 b was 0.913 with 83.3% sensitivity and 87.5% specificity. 11A novel colloidal gold immunochromatography assay strip for the diagnosis of schistosomiasis japonica in domestic animals显示文摘Background:Schistosomiasis remains a major public health concern in China and an epidemiological survey has revealed that schistosome-infected bovines and goats are the main transmission sources for the disease.Therefore,development of a sensitive technique for the diagnosis of schistosomiasis in domestic animals is necessary.Method:A novel colloidal gold immunochromatography assay(GICA)strip was developed for detecting Schistosoma japonicum in domestic animals.The colloidal gold was conjugated with recombinant streptococcal protein G(rSPG).As the test and control lines,the schistosome soluble egg antigen and rSPG,respectively,were blotted on nitrocellulose membrane.Results:The lowest detectable serum dilution was 1∶640 for schistosome-infected buffaloes.The cross-reaction rate of GICA was 14.29%with Paramphistomum sp.in buffaloes,16.67%with Haemonchus sp.in goats,and 33.33%with Orientobilharzia sp.in goats.These results were slightly lower and similar to those obtained through ELISA.Moreover,the strips for detecting S.japonicum in mice,rabbits,buffaloes,and goats showed high sensitivity(100.00%,100.00%,100.00%,and 100.00%,respectively)and specificity(100.00%,100.00%,94.23%,and 88.64%,respectively).And the sensitivity or specificity of the GICA strips did not present any significant differences after storage for 12 months at room temperature.When compared with ELISA,the GICA strips exhibited similar sensitivity and specificity in the diagnosis of schistosomiasis in mice,rabbits,buffaloes,and goats.Besides,only 5μl of serum are required for the test and the detection can be completed within 5 min.Conclusion:This study is the first to develop a GICA strip using gold-rSPG conjugate for the diagnosing of schistosomiasis in domestic animals,and preliminary results showed that the developed strip may be suitable for large-scale screening of schistosomiasis in endemic areas.Rui Xu Jintao Feng Yang Hong Chao Lv Dengyun Zhao Jiaojiao Lin Ke Lu Hao Li Jinming Liu Xiaodan Cao Tao Wang Jinli Zai Zhaozhe Wang Bingguang Jia Qian Han Chuangang Zhu 2017Infectious Diseases of Poverty2017,6,1:12

14Characteristics of secondary inorganic aerosol and sulfate species in size-fractionated aerosol particles in Shanghai显示文摘Sulfate, nitrate and ammonium(SNA) are the dominant species in secondary inorganic aerosol, and are considered an important factor in regional haze formation. Size-fractionated aerosol particles for a whole year were collected to study the size distribution of SNA as well as their chemical species in Shanghai. SNA mainly accumulated in fine particles and the highest average ratio of SNA to particulate matter(PM) was observed to be 47% in the fine size fraction(0.49–0.95 μm). Higher sulfur oxidation ratio and nitrogen oxidation ratio values were observed in PM of fine size less than 0.95 μm. Ion balance calculations indicated that more secondary sulfate and nitrate would be generated in PM of fine size(0.49–0.95 μm). Sulfur K-edge X-ray absorption near-edge structure(XANES) spectra of typical samples were analyzed. Results revealed that sulfur mainly existed as sulfate with a proportion(atomic basis) more than 73% in all size of PM and even higher at 90% in fine particles. Sulfate mainly existed as(NH4)2SO4 and gypsum in PM of Shanghai. Compared to non-haze days, a dramatic increase of(NH4)2SO4 content was found in fine particles on haze days only, which suggested the promoting impact of(NH4)2SO4 on haze formation. According to the result of air mass backward trajectory analysis, more(NH4)2SO4 would be generated during the periods of air mass stagnation. 16Development and validation of an endoscopic images-based deep learning model for detection with nasopharyngeal malignancies显示文摘Background:Due to the occult anatomic location of the nasopharynx and frequent presence of adenoid hyperpla-sia,the positive rate for malignancy identification during biopsy is low,thus leading to delayed or missed diagnosis for nasopharyngeal malignancies upon initial attempt.Here,we aimed to develop an artificial intelligence tool to detect nasopharyngeal malignancies under endoscopic examination based on deep learning.Methods:An endoscopic images-based nasopharyngeal malignancy detection model(eNPM-DM)consisting of a fully convolutional network based on the inception architecture was developed and fine-tuned using separate training and validation sets for both classification and segmentation.Briefly,a total of 28,966 qualified images were collected.Among these images,27,536 biopsy-proven images from 7951 individuals obtained from January 1st,2008,to December 31st,2016,were split into the training,validation and test sets at a ratio of 7:1:2 using simple randomiza-tion.Additionally,1430 images obtained from January 1st,2017,to March 31st,2017,were used as a prospective test set to compare the performance of the established model against oncologist evaluation.The dice similarity coef-ficient(DSC)was used to evaluate the efficiency of eNPM-DM in automatic segmentation of malignant area from the background of nasopharyngeal endoscopic images,by comparing automatic segmentation with manual segmenta-tion performed by the experts.Results:All images were histopathologically confirmed,and included 5713(19.7%)normal control,19,107(66.0%)nasopharyngeal carcinoma(NPC),335(1.2%)NPC and 3811(13.2%)benign diseases.The eNPM-DM attained an overall accuracy of 88.7%(95%confidence interval(CI)87.8%-89.5%)in detecting malignancies in the test set.In the prospective comparison phase,eNPM-DM outperformed the experts:the overall accuracy was 88.0%(95%CI 86.1%-89.6%)vs.80.5%(95%CI 77.0%-84.0%).The eNPM-DM required less time(40 s vs.110.0±5.8 min)and exhibited encouraging performance in automatic segmentation of nasopharyngeal malignant area from the background,with an average DSC of 0.78±0.24 and 0.75±0.26 in the test and prospective test sets,respectively.Conclusions:The eNPM-DM outperformed oncologist evaluation in diagnostic classification of nasopharyngeal mass into benign versus malignant,and realized automatic segmentation of malignant area from the background of nasopharyngeal endoscopic images.Chaofeng Li Bingzhong Jing Liangru Ke Bin Li Weixiong Xia Caisheng He Chaonan Qian Chong Zhao Haiqiang Mai Mingyuan Chen Kajia Cao Haoyuan Mo Ling Guo Qiuyan Chen Linquan Tang Wenze Qiu Yahui Yu Hu Liang Xinjun Huang Guoying Liu Wangzhong Li Lin Wang Rui Sun Xiong Zou Shanshan Guo Peiyu Huang Donghua Luo Fang Qiu Yishan Wu Yijun Hua Kuiyuan Liu Shuhui Lv Jingjing Miao Yanqun Xiang Ying Sun Xiang Guo Xing Lv 2018Cancer Communications2018,38,1:9
17Progressive failure analysis of slope with strain-softening behaviour based on strength reduction method显示文摘Based on the strength reduction method and strain-softening model,a method for progressive failure analysis of strain-softening slopes was presented in this paper.The mutation is more pronounced in strain-softening analysis,and the mutation of displacement at slope crest was taken as critical failure criterion.An engineering example was provided to demonstrate the validity of the present method.This method was applied to a cut slope in an industry site.The results are as follows:(1) The factor of safety and the critical slip surface obtained by the present method are between those by peak and residual strength.The analysis with peak strength would lead to non-conservative results,but that with residual strength tends to be overly conservative.(2) The thickness of the shear zone considering strain-softening behaviour is narrower than that with non-softening analysis.(3) The failure of slope is the process of the initiation,propagation and connection of potential failure surface.The strength parameters are mobilized to a non-uniform degree while progressive failure occurs in the slope.(4) The factor of safety increases with the increase of residual shear strain threshold and elastic modulus.The failure mode of slope changes from shallow slip to deep slip.Poisson's ratio and dilation angle have little effect on the results.Ke ZHANG Ping CAO Rui BAO 2013Journal of Zhejiang University-Science A(Applied Physics & Engineering)2013,14,2:9

19Accumulated chilling hours during endodormancy impact blooming and fruit shape development in peach(Prunus persica L.)显示文摘Winter chill is essential for the growth and development of deciduous species. To understand the relationship between accumulated chilling hours during endodormancy and blooming and fruit shape development, we controlled chilling hours and investigated their effects on blooming date and fruit shape of peaches. The results showed that the number of days to full bloom date and the heat requirement for blooming were negatively correlated with accumulated chilling hours. Accumulated chilling hours were significantly negatively correlated with fruit shape index and fruit tip lengths, suggesting that the number of chilling hours affect the fruit shape development. Fewer accumulated chilling hours may be the major reason for longer fruit shape and protruding fruit tips. In conclusion, our results indicate specifically that decreased winter chilling hours can delay the bloom date and may lead to aberrant fruit shape development in peaches. 20Influence of chronic HBV infection on superimposed acute hepatitis E显示文摘AIM:To investigate the influence of chronic hepatitis B virus(HBV)infection[based on the status of hepatitis B e antigen(HBeAg),HBV DNA,and cirrhosis]on superimposed acute hepatitis E.METHODS:A total of 294 patients were recruited from the Department of Infectious Diseases of the Third Affiliated Hospital,Sun Yat-sen University,from January 2003 to January 2012.The patients were classified into two groups:an HBV+hepatitis E virus(HEV)group(a group with chronic HBV infection that was superinfected with acute hepatitis E,n=118)and an HEV group(a group with acute hepatitis E,n=176).We retrospectively analyzed and compared the clinical features of the two groups.Statistical analyses were performed using theχ2test or Fisher’s exact test for categorical variables and the Student’s t test forcontinuous variables.A P value<0.05 was considered statistically significant.RESULTS:The peak values of prothrombin time,serum total bilirubin,and Model for End-Stage Liver Disease scores were significantly higher in the HBV+HEV group.More patients in the HBV+HEV group had complications(39.8%vs 16.5%,P=0.000)and developed liver failure(35.6%vs 8.5%,P=0.000).Additionally,the mortality of the HBV+HEV group was significantly higher(20.3%vs 7.4%,P=0.002).Further analysis of the HBV+HEV group showed that there were no significant differences in complication occurrence,liver failure incidence,or mortality between patients with different HBeAg and HBV DNA statuses.However,in patients with underlying cirrhosis,complication occurrence and liver failure incidence significantly increased.In total,12.7%of the patients in the HBV+HEV group received anti-HBV treatment,but this therapy failed to reduce mortality in patients who developed liver failure.CONCLUSION:The presence of underlying cirrhosis in chronic HBV infection results in more severe clinical outcomes with superimposed acute hepatitis E.AntiHBV treatment cannot improve the prognosis of liver failure caused by HBV-HEV superinfection.Si-Hong Cheng Li Mai Feng-Qin Zhu Xing-Fei Pan Hai-Xia Sun Hong Cao Xin Shu Wei-Min Ke Gang Li Qi-Huan Xu 2013World Journal of Gastroenterology2013,19,35:8
