| 1 | High prevalence of vitamin A deficiency in Crohn's disease patients according to serum retinol levels and the relative dose-response test显示文摘AIM:To assess the vitamin A status of patients with Crohn's disease(CD) by evaluating serum retinol levels and the relative dose response(RDR) test(liver retinol stores).METHODS:Vitamin A nutritional status was measured by serum retinol obtained by high performance liquid chromatography and the RDR test for evaluation of the hepatic stores.Body composition was performed by densitometry by dual-energy X-ray absorptiometry.Vitamin A dietary intake was assessed from a semiquantitative food frequency questionnaire.RESULTS:This study included 38 CD patients and 33 controls.Low serum retinol concentrations were detected in 29% of CD patients vs 15% in controls(P < 0.005).The RDR test was positive in 37% of CD patients vs 12% in controls,which indicated inadequate hepatic vitamin A stores(P < 0.005).Individuals with hypovitaminosis A had lower BMI and body fat compared with those without this deficiency.There was no association between vitamin A deficiency and its dietary intake,ileal location,presence of disease activity and prior bowel resections.CONCLUSION:Patients with CD have higher prevalence of vitamin A deficiency,as assessed by two independent methods. | Márcia Soares-Mota Tianny A Silva Luanda M Gomes Marco AS Pinto Laura MC Mendon?a Maria Lúcia F Farias Tiago Nunes Andrea Ramalho Cyrla Zaltman | 2015 | World Journal of Gastroenterology2015，21，5： | 4 |
| 2 | Juvenile hemochromatosis:HAMP mutation and severe iron overload treated with phlebotomies and deferasirox显示文摘Juvenile hemochromatosis(JH) is a rare condition classified as an autosomal recessive disorder that leads to severe iron absorption. JH usually affects people under the age of 30 and presents symptoms such as chronic liver damage,hypogonadotropic hypogonadism,cardiac diseases and endocrine dysfunctions. The present case reports a 29-year-old Brazilian woman with JH condition due to HAMP mutation(g.47G>A),treated with phlebotomies and deferasirox. She presented symptoms such as weakness,skin hyperpigmentation,joint pain in the shoulders and hands and amenorrhea. First laboratory tests showed altered biochemical parameters [serum ferritin(SF): 5696 ng/mL,transferrin saturation(TS): 85%]. After sessions of phlebotomies(450 mL every 15 d),the patient presented partial symptomatic improvements and biochemical parameters(SF: 1000 ng/mL,Hb: 11 g/dL). One year later,deferasirox(15 mg/kg per day) was introduced to the treatment,and the patient showed total symptomatic improvement,with significant clearing of the skin,SF: 169 ng/mL,and TS: 50%. Furthermore,after the combined deferasirox-phlebotomy therapy,magnetic resonance imaging measurements revealed normalized level for liver iron(30 μmol/g; reference value < 36 μmol/g). | 4 | Decomposition and stabilization of organic matter in an old-growth tropical riparian forest:effects of soil properties and vegetation structure显示文摘Background:Nutrient cycling in tropical forests has a large importance for primary productivity,and decomposition of litterfall is a major process influencing nutrient balance in forest soils.Although large-scale factors strongly influence decomposition patterns,small-scale factors can have major influences,especially in old-growth forests that have high structural complexity and strong plant-soil correlations.Here we evaluated the effects of forest structure and soil properties on decomposition rates and stabilization of soil organic matter using the Tea Bag Index(TBI)in an old-growth riparian forest in southeastern Brazil.These data sets were described separately using Principal Components Analysis(PCA).The main axes for each analysis,together with soil physical properties(clay content and soil moisture),were used to construct structural equations models that evaluated the different parameters of the TBI,decomposition rates and stabilization factor.The best model was selected using Akaike’s criterion.Results:Forest structure and soil physical and chemical properties presented large variation among plots within the studied forest.Clay content was strongly correlated with soil moisture and the first PCA axis of soil chemical properties,and model selection indicated that clay content was a better predictor than this axis.Decomposition rates presented a large variation among tea bags(0.009 and 0.098 g·g^(−1)·d−1)and were positively related with forest structure,as characterized by higher basal area,tree density and larger trees.The stabilization factor varied between 0.211–0.426 and was related to forest stratification and soil clay content.Conclusions:The old-growth forest studied presented high heterogeneity in both forest structure and soil properties at small spatial scales,that influenced decomposition processes and probably contributed to small-scale variation in nutrient cycling.Decomposition rates were only influenced by forest structure,whereas the stabilization factor was influenced by both forest structure and soil properties.Heterogeneity in ecological processes can contribute to the resilience of old-growth forests,highlighting the importance of restoration strategies that consider the spatial variation of ecosystem processes. | Pedro Henrique de Godoy Fernandes Andréa Lúcia Teixeira de Souza Marcel Okamoto Tanaka Renata Sebastiani | 2021 | Forest Ecosystems2021，8，1： | 1 |

The pull method of tube placement was used. Five months after the procedure, the patient was referred to the hospital with a hard palpable tumour at the ostomy site. The hystologic exam revealed an abdominal wall metastasis of the esophageal cancer. The authors present this case because of the rarity of metastasis in ostomy after placement of PEG in patients with tumours located in the head and neck. In this particular context and judging by the rarity of situation, the clinical impact of this phenomenom is limited.
