1A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. 2Persistence and clearance of viral RNA in 2019 novel coronavirus disease rehabilitation patients显示文摘Background:A patient’s infectivity is determined by the presence of the virus in different body fluids,secretions,and excreta.The persistence and clearance of viral RNA from different specimens of patients with 2019 novel coronavirus disease(COVID-19)remain unclear.This study analyzed the clearance time and factors influencing 2019 novel coronavirus(2019-nCoV)RNA in different samples from patients with COVID-19,providing further evidence to improve the management of patients during convalescence.Methods:The clinical data and laboratory test results of convalescent patients with COVID-19 who were admitted to from January 20,2020 to February 10,2020 were collected retrospectively.The reverse transcription polymerase chain reaction(RT-PCR)results for patients’oropharyngeal swab,stool,urine,and serum samples were collected and analyzed.Convalescent patients refer to recovered non-febrile patients without respiratory symptoms who had two successive(minimum 24 h sampling interval)negative RT-PCR results for viral RNA from oropharyngeal swabs.The effects of cluster of differentiation 4(CD4)+T lymphocytes,inflammatory indicators,and glucocorticoid treatment on viral nucleic acid clearance were analyzed.Results:In the 292 confirmed cases,66 patients recovered after treatment and were included in our study.In total,28(42.4%)women and 38 men(57.6%)with a median age of 44.0(34.0-62.0)years were analyzed.After in-hospital treatment,patients’inflammatory indicators decreased with improved clinical condition.The median time from the onset of symptoms to first negative RT-PCR results for oropharyngeal swabs in convalescent patients was 9.5(6.0-11.0)days.By February 10,2020,11 convalescent patients(16.7%)still tested positive for viral RNA from stool specimens and the other 55 patients’stool specimens were negative for 2019-nCoV following a median duration of 11.0(9.0-16.0)days after symptom onset.Among these 55 patients,43 had a longer duration until stool specimens were negative for viral RNA than for throat swabs,with a median delay of 2.0(1.0-4.0)days.Results for only four(6.9%)urine samples were positive for viral nucleic acid out of 58 cases;viral RNA was still present in three patients’urine specimens after throat swabs were negative.Using a multiple linear regression model(F=2.669,P=0.044,and adjusted R2=0.122),the analysis showed that the CD4+T lymphocyte count may help predict the duration of viral RNA detection in patients’stools(t=-2.699,P=0.010).The duration of viral RNA detection from oropharyngeal swabs and fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(15 days vs.8.0 days,respectively;t=2.550,P=0.013)and the duration of viral RNA detection in fecal samples in the glucocorticoid treatment group was longer than that in the non-glucocorticoid treatment group(20 days vs.11 days,respectively;t=4.631,P<0.001).There was no statistically significant difference in inflammatory indicators between patients with positive fecal viral RNA test results and those with negative results(P>0.05).Conclusions:In brief,as the clearance of viral RNA in patients’stools was delayed compared to that in oropharyngeal swabs,it is important to identify viral RNA in feces during convalescence.Because of the delayed clearance of viral RNA in the glucocorticoid treatment group,glucocorticoids are not recommended in the treatment of COVID-19,especially for mild disease.The duration of RNA detection may relate to host cell immunity.Yun Ling Shui-Bao Xu Yi-Xiao Lin Di Tian Zhao-Qin Zhu Fa-Hui Dai Fan Wu Zhi-Gang Song Wei Huang Jun Chen Bi-Jie Hu Sheng Wang En-Qiang Mao Lei Zhu Wen-Hong Zhang Hong-Zhou Lu 2020Chinese Medical Journal2020,,9:58

7Global leaf nitrogen and phosphorus stoichiometry and their scaling exponent显示文摘Leaf nitrogen(N) and phosphorus(P) concentrations constrain photosynthetic and metabolic processes,growth and the productivity of plants. Their stoichiometry and scaling relationships regulate the allocation of N and P from subcellular to organism, and even ecosystem levels, and are crucial to the modelling of plant growth and nutrient cycles in terrestrial ecosystems. Prior work has revealed a general biogeographic pattern of leaf N and P stoichiometric relationships and shown that leaf N scales roughly as two-thirds the power of P. However, determining whether and how leaf N and P stoichiometries, especially their scaling exponents, change with functional groups and environmental conditions requires further verification. In this study, we compiled a global data set and documented the global leaf N and P concentrations and the N:P ratios by functional group, climate zone and continent. The global overall mean leaf N and P concentrations were 18.9 mg g^(-1) and 1.2 mg g^(-1), respectively, with significantly higher concentrations in herbaceous than woody plants(21.72 mg g^(-1) vs. 18.22 mg g^(-1) for N; and 1.64 mg g^(-1) vs. 1.10 mg g^(-1) for P). Both leaf N and P showed higher concentrations at high latitudes than low latitudes. Among six continents, Europe had the highest N and P concentrations(20.79 and 1.54 mg g^(-1)) and Oceania had the smallest values(10.01 and 0.46 mg g^(-1)). These numerical values may be used as a basis for the comparison of other individual studies. Further, we found that the scaling exponent varied significantly across different functional groups,latitudinal zones, ecoregions and sites. The exponents of herbaceous and woody plants were 0.659 and0.705, respectively, with significant latitudinal patterns decreasing from tropical to temperate to boreal zones. 8Bacterial flora concurrent with Helicobacter pylori in the stomach of patients with upper gastrointestinal diseases显示文摘AIM:To investigate the non-Helicobacter pylori(H.pylori) bacterial flora concurrent with H.pylori infection.METHODS:A total of 103 gastric biopsy specimens from H.pylori positive patients were selected for bacterial culture.All the non-H.pylori bacterial isolates were identified by matrix-assisted laser desorption ionization time-of-flight mass spectrometry(MALDI-TOF MS).RESULTS:A total of 201 non-H.pylori bacterial isolates were cultivated from 67(65.0%) of the 103 gastric samples,including 153 isolates identified successfully at species level and 48 at genus level by MALDI-TOF MS.The dominant species were Streptococcus,Neisseria,Rothia and Staphylococcus,which differed from the predominantly acid resistant species reported previously in healthy volunteers.The prevalence of non-H.pylori bacteria was higher in non-ulcer dyspepsia group than in gastric ulcer group(100%vs 42.9%,P < 0.001).Six bacterial species with urease activity(Staphylococcus epidermidis,Staphylococcus warneri,Staphylococcus capitis,Staphylococcus aureus,Brevibacterium spp.and Klebsiella pneumoniae) were also isolated.CONCLUSION:There is a high prevalence of the non-H.pylori bacteria concurrent with H.pylori infection,and the non-H.pylori bacteria may also play important as-yet-undiscovered roles in the pathogenesis of stomach disorders.Yuan Hu Li-Hua He Di Xiao Guo-Dong Liu Yi-Xin Gu Xiao-Xia Tao Jian-Zhong Zhang 2012World Journal of Gastroenterology2012,18,11:20

11Significance of platelet activating factor receptor expression in pancreatic tissues of rats with severe acute pancreatitis and effects of BN52021显示文摘AIM:To investigate the dynamic changes and significance of platelet activating factor receptor (PAF-R) mRNA and protein in pancreatic tissues of rats with severe acute pancreatitis (SAP) and effects of BN52021 (Ginkgolide B). METHODS:Wistar male rats were randomly assigned to the negative control group (NC group),SAP model group (SAP group),and BN52051-remedy group (BN group),and each of the groups was divided into 6 subgroups at different time points after operation (1 h,2 h,3 h,6 h,12 h,and 24 h) (n=10 in each). PT-PCR and Western blot methods were used to detect PAF-RmRNA and protein expression in pancreatic tissues of rats respectively. Pathological examination of pancreatic tissues was performed and the serum amylase change was detected. RESULTS:Serum amylase and pathological results showed the that SAP model was successfully prepared,BN52021 was able to decrease serum amylase,and the pathological ratings in BN group at 3 h,6 h,and 12 h significantly decreased compared with those in the SAP group (8.85 ± 0.39 vs 5.95 ± 0.19,9.15 ± 0.55 vs 5.55 ± 0.36,10.10 ± 0.65 vs 6.72 ± 0.30,P < 0.05). The result of PAF-mRNA showed dynamic changes in SAP and BN groups,which increased gradually in early stage,reached a peak at 3 h (0.71 ± 0.14 vs 0.54 ± 0.14,0.69 ± 0.13 vs 0.59 ± 0.04,P < 0.05),and decreased gradually later. There were significant differences at each time point except 1 h and 2 h,when compared with those in the NC group (0.71 ± 0.14 or 0.69 ± 0.13 vs 0.47 ± 0.10,0.38 ± 0.08 or 0.59 ± 0.04 vs 0.47 ± 0.09,0.25 ± 0.07 or 0.29 ± 0.05 vs 0.46 ± 0.10,0.20 ± 0.06 or 0.20± 0.04 vs 0.43 ± 0.09,P < 0.05),whereas there was no significant difference between BN and SAP groups at each time point. The result of PAF-R protein showed that the change of PAF-R protein in the SAP group and the BN group was consistent with that of PAF-R mRNA. There were significant differences at each time point except 1 h,when compared with those in the NC group (0.90 ± 0.02 or 0.80 ± 0.05 vs 0.48 ± 0.02,1.69 ± 0.06 or 1.58 ± 0.02 vs 0.48 ± 0.03,1.12 ± 0.10 or 0.98 ± 0.03 vs 0.49 ± 0.09,1.04 ± 0.14 or 0.87 ± 0.02 vs 0.52 ± 0.08,0.97 ± 0.16 or 0.90 ± 0.05 vs 0.49 ± 0.10,P < 0.05),whereas there was no significant difference between the BN group and the SAP group. CONCLUSION:PAF-R plays an important role in occurrence and development of SAP. 13Effect of BN52021 on NFκ-Bp65 expression in pancreatic tissues of rats with severe acute pancreatitis显示文摘AIM: To investigate dynamic changes and significance of expression of NF-κBp65 in pancreatic tissues of rats with severe acute pancreatitis (SAP), as well as BN52021 effects. METHODS: Wistar male rats were randomly divided into negative control group (NC group, n = 60), SAP-model group (SAP group, n = 60), and BN52021-treated group (BN group, n = 60), and each of the above groups was respectively divided into 6 subgroups at different time points after operation (1 h, 2 h, 3 h, 6 h, 12 h, and 24 h) (n = 10). By RT-PCR and Western blot, NF-κBp65 mRNA and its protein expression in pancreatic tissues of rats were detected respectively. RESULTS: The expression of NF-κBp65 mRNA dynamically changed in both SAP groups and BN groups. The mRNA level was higher in SAP groups than NC groups at 2 h, 3 h, 12 h, and 24 h after operation (P < 0.05), higher in BN groups than NC groups at all time points (P < 0.05), and higher in BN groups than SAP group at 1 h (P < 0.05). The NF-κBp65 protein level was higher in SAP groups than NC groups at 1 h, 3 h, and 6 h (P < 0.01), and 2 h, 12 h, and 24 h (P < 0.05), higher in BN groups than NC groups at all time points (P < 0.05), and lower in BN groups than SAP groups at 1 h, 3 h, and 6 h (P < 0.05). CONCLUSION: The expression of NF-κBp65 in pancreatic tissues is dynamically changed and the changes play an important role in pathogenesis of SAP. 14Protective effect of recombinant human IL-1Ra on CCl_4-induced acute liver injury in mice显示文摘AIM: To evaluate the effects of positive regulation of recombinant human interleukin 1 receptor antagonist (rhIL-1Ra) on hepatic tissue recovery in acute liver injury in mice induced by carbon tetrachloride (CCl 4 ). METHODS: Acute liver damage was induced by injecting 8-wk-old mice with CCl 4 1 mL/kg (1:3 dilution in corn oil) intraperitoneally (ip). Survival after liver failure was assessed by injecting 8-wk-old mice with a lethal dose of CCl 4 2.6 mL/kg (1:1 dilution in corn oil) ip. Mice were subcutaneously injected with 1 mg/kg recombinant human IL-1Ra twice a day after CCl 4 treatment for 5 d. Serum alanine amino transferase (ALT) and aspartate aminotransferase (AST) levels were determined with a commercial assay kit. Serum IL-1β, IL-1Ra levels were measured by enzyme-linked immunosorbent assay kit. Quantitative real-time polymerase chain reaction was used to determine liver IL-1β, IL-1Ra and IL-6 expression during CCl 4-induced acute liver injury. Liver sections were stained with hematoxylin-eosin. A histology-injury grading system was used to evaluate the degree of necrosis after acute liver injury. Proliferating cell nuclear antigen (PCNA) staining was used to evaluate the role of rhIL-1Ra in promoting hepatocyte proliferation. RESULTS: Quantitative analysis showed a higher level of IL-6 mRNA expression and reduced serum AST and ALT levels in the livers of the rhIL-1Ra-treated group at the early phase of CCl 4-induced acute liver injury. Histological examination indicated a decrease in centrilobular necrotic areas in mice treated with rhIL-1Ra, and a novel role of rhIL-1Ra in promoting hepatocyte proliferation was also supported by an increase of PCNA staining. All these results, accompanied by a strong survival benefit in rhIL-1Ra-treated vs PBS-treated groups, demonstrated that rhIL-1Ra administration ameliorated the histological damage and accelerated the regeneration and recovery process of the liver. 16The enhanced X-ray Timing and Polarimetry mission—eXTP显示文摘In this paper we present the enhanced X-ray Timing and Polarimetry mission—eXTP. eXTP is a space science mission designed to study fundamental physics under extreme conditions of density, gravity and magnetism. The mission aims at determining the equation of state of matter at supra-nuclear density, measuring effects of QED, and understanding the dynamics of matter in strong-field gravity. In addition to investigating fundamental physics, eXTP will be a very powerful observatory for astrophysics that will provide observations of unprecedented quality on a variety of galactic and extragalactic objects. 17Science Letters:IGFBP7 plays a potential tumor suppressor role against colorectal carcinogenesis with its expression associated with DNA hypomethylation of exon 1显示文摘Insulin-like growth factor binding-protein-7 (IGFBP7) was obtained from our previous colonic adenocarcinoma (CRC) and normal mucosa suppression subtraction hybridization (SSH) cDNA libraries. By RT-PCR and immunohistochemistry, we found that IGFBP7 was overexpressed in CRC tissue compared to normal tissue. However, our in vitro experiments performed in 10 CRC cell lines showed that IGFBP7 expressed only in SW480 and Caco2 cell lines, which implied an underlying reversible regulatory mechanism. Using methylation-specific PCR (MSP) and bisulfite sodium PCR (BSP), we found that its expression was associated with DNA hypomethylation of exon1. This was further supported by the in vitro study which showed restored IGFBP7 expression after demethylation agent 5-aza-2’-deoxycytidine treatment. Correlation analysis between IGFBP7 expression and prognosis indicated that overexpression of IGFBP7 in CRC tissue correlated with favourable survival. Investigation of the func-tional role of IGFBP7 through transfection studies showed that IGFBP7 protein could inhibit growth rate, decrease colony for-mation activity, and induce apoptosis in RKO and SW620 cells, suggesting it a potential tumor suppressor protein in colorectal carcinogenesis. 18Biomass-derived porous carbon highly efficient for removal of Pb(Ⅱ) and Cd(Ⅱ)显示文摘The utilization of abundant and renewable biomass to fabricate advanced functional materials is considered a promising route for environmental applications.Herein,Lignin-based porous carbon with layered graphene-like structure(LPC)is successfully synthesized and applied to efficiently remove Pb(Ⅱ)and Cd(Ⅱ).The as-synthesized LPC materials are systematically characterized and these results show that LPC has a porous graphene-like structure,facilitating the diffusion and immobilization of heavy metal ions.The influence of different reaction parameters(solution pH,initial concentration of metal ions,contact time and adsorbent amount)on the adsorption performance is investigated in details.The results demonstrate that LPC can achieve superior adsorption capacities of 250.5 mg·g^-1 for Pb(Ⅱ)and 126.4 mg·g^-1 for Cd(Ⅱ),which are far superior to the previously reported adsorbents.Pseudo-second order kinetics model and Freundlich isotherm model describe the adsorption process well.Furthermore,the exhausted LPC can be regenerated easily and exhibits the removal efficiency of 96%and 92%for Pb(Ⅱ)and Cd(Ⅱ)after five continuous runs,respectively.This study shows a sustainable strategy for the design of porous carbon material from na?ve biomass and highlights the great potential in wastewater treatment.Anqi Wang Zhikeng Zheng Ruiqi Li Di Hu Yiran Lu Huixia Luo Kai Yan 2019Green Energy & Environment2019,4,4:10

20Application of chemical fractionation and X-ray powder diffraction to study phosphorus speciation in sediments from Lake Hongfeng,China显示文摘Based on the techniques of sequential extraction and X-ray powder diffraction(XRD) ,the distribution characteristics of phosphorus speciation were investigated in 13 sediment samples from Lake Hongfeng,an important drinking water source for Guiyang with a typical karstic limestone environment.The total phosphorus(TP) concentrations in the sediments ranged from 973.36 to 2334.18 mg/kg with a mean concentration of 1635.87 mg/kg(dry weight) ,and it was evident that the phosphorus level in the sediments was high compared with other eutrophic lakes such as Lake Chaohu and Lake Xihu.The results of the two methods,sequential extraction and XRD,demonstrated the presence of different phosphorus species in the lake sediments.Being compatible with local geophysical characteristics,the results of sequential extraction techniques indicate that the average percentages of dissolved phosphorus(DP) ,aluminum-bound phosphorus(Al-P) ,iron-bound phosphorus(Fe-P) ,calcium-bound phosphorus(Ca-P) and organic phosphorus(OP) in the sediments were 0.40%,7.06%,5.07%,45.48%and 38.49%,respectively.Notably,the concentrations of inorganic phosphorus(IP) were higher than those of OP,and the IP consisted primarily of Ca-P.The high concentrations of Ca-P may temporarily control the release of phosphorus from the sediments because it is a relatively stable,inert and non-bioavailable phosphorus fraction.The results of XRD analysis indicated that,among phosphorus species,AlPO4 was found in all sediment samples at relatively high concentrations.Moreover,the TP concentrations in the sediments were high;thus the release potential and bioavailability of phosphorus in the sediments was important.Further statistical analyses of the results revealed significant correlations between phosphorus species in the sediments with two extractable principal components and four selectable cluster levels allowing interpretation of possible origins of phosphorus loading and transformation mechanisms of phosphorus species.Furthermore,available remediation measures were briefly assessed for the lake while considering its distinct environmental features.JIANG CuiHong WU Di HU JiWei LIU Feng HUANG XianFei LI CunXiong JIN Mei 2011Chinese Science Bulletin2011,56,20:9
