1Impact of alanyl-glutamine dipeptide on severe acute pancreatitis in early stage显示文摘AIM:To evaluate the therapeutic effect of alanyl- glutamine dipeptide (AGD) in the treatment of severe acute pancreatitis (SAP) in early and advanced stage. METHODS: Eighty patients with SAP were randomized and received 100 mL/d of 20% AGD intravenously for 10 d starting either on the day of (early treatment group) or 5 d after (late treatment group) admission. Groups had similar demographics, underlying diseases, Ranson score, Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score, and Balthazar’s computed tomography (CT) score at the beginning of the study and underwent similar other medical and nutritional management. RESULTS: The duration of acute respiratory distress syndrome (2.7 ± 3.3 d vs 12.7 ± 21.0 d, P < 0.01), renal failure (1.3 ± 0.5 d vs 5.3 ± 7.3 d, P < 0.01), acute hepatitis (3.2 ± 2.3 d vs 7.0 ± 7.1 d, P < 0.01), shock (1.7 ± 0.4 d vs 4.8 ± 3.1 d, P < 0.05), encephalopathy (2.3 ± 1.9 d vs 9.5 ± 11.0 d, P < 0.01) and enteroparalysis (2.2 ± 1.4 d vs 3.5 ± 2.2 d, P < 0.01) and hospital stay (28.8 ± 9.4 d vs 45.2 ± 27.1 d, P < 0.01) were shorter in the early treatment group than in the late treatment group. The 15-d APACHE Ⅱ score was lower in the early treatment group than in the late treatment group (5.0 ± 2.4 vs 8.6 ± 3.6, P < 0.01). 6Thermal history of the Sichuan Basin,SW China:Evidence from deep boreholes显示文摘The Sichuan Basin is a superimposition basin composed of terrestrial and marine sediments that is well known for its abundant petroleum resources. Thermal history reconstruction using paleogeothermal indicators, including vitrinite reflectance and thermochronological data, shows that different structural subsections of the Sichuan Basin have experienced various paleogeothermal episodes since the Paleozoic. The lower structural subsection comprising the Lower Paleozoic to Middle Permian(Pz–P_2) successions experienced a high paleogeothermal gradient(23.0–42.6°C/km) at the end of the Middle Permian(P_2), whereas the upper structural subsection comprising Late Permian to Mesozoic strata underwent a relatively lower paleogeothermal gradient(13.2–26.9°C/km) at the beginning of the denudation(Late Cretaceous or Paleocene in the different regions). During the denudation period, the Sichuan Basin experienced a successive cooling episode. The high paleogeothermal gradient resulted from an intensive thermal event correlated to the Emeishan mantle plume. The heat flow value reached 124.0 m W/m^2 in the southwestern basin near the center of the Emeishan large igneous province. The low geothermal gradient episode with heat flow ranging from 31.2 to 70.0 m W/m^2 may be related to the foreland basin evolution. 8Association of Overlapped and Un-overlapped Comorbidities with COVID-19 Severity and Treatment Outcomes: A Retrospective Cohort Study from Nine Provinces in China显示文摘Objective Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.Methods A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients’ medical records. Multivariable logistic regression models were used to estimate the odds ratio(OR) and 95% confidence interval(95% CI) of the associations between comorbidities(cardiometabolic or non-cardiometabolic diseases), clinical severity, and treatment outcomes of COVID-19.Results Overall, 158(13.6%) patients were diagnosed with severe illness and 32(2.7%) had unfavorable outcomes. Hypertension(2.87, 1.30–6.32), type 2 diabetes(T2 DM)(3.57, 2.32–5.49),cardiovascular disease(CVD)(3.78, 1.81–7.89), fatty liver disease(7.53, 1.96–28.96), hyperlipidemia(2.15, 1.26–3.67), other lung diseases(6.00, 3.01–11.96), and electrolyte imbalance(10.40, 3.00–26.10)were independently linked to increased odds of being severely ill. T2 DM(6.07, 2.89–12.75), CVD(8.47,6.03–11.89), and electrolyte imbalance(19.44, 11.47–32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission(5.46,3.25–9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes(6.58, 1.46–29.64) within two weeks.Conclusion Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. 9Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. 10Coagulation factors VII, IX and X are effective antibacterial proteins against drug-resistant Gram-negative bacteria显示文摘Infections caused by drug-resistant“superbugs”pose an urgent public health threat due to the lack of effective drugs;however, certain mammalian proteins with intrinsic antibacterial activity might be underappreciated. Here, we reveal an antibacterial property against Gram-negative bacteria for factors VII, IX and X, three proteins with well-established roles in initiation of the coagulation cascade. These factors exert antibacterial function via their light chains (LCs). Unlike many antibacterial agents that target cell metabolism or the cytoplasmic membrane, the LCs act by hydrolyzing the major components of bacterial outer membrane, lipopolysaccharides, which are crucial for the survival of Gram-negative bacteria. The LC of factor VII exhibits in vitro efficacy towards all Gram-negative bacteria tested, including extensively drug-resistant (XDR) pathogens, at nanomolar concentrations. It is also highly effective in combating XDR Pseudomonas aeruginosa and Acinetobacter baumannii infections in vivo. Through decoding a unique mechanism whereby factors VII, IX and X behave as antimicrobial proteins, this study advances our understanding of the coagulation system in host defense, and suggests that these factors may participate in the pathogenesis of coagulation disorder-related diseases such as sepsis via their dual functions in blood coagulation and resistance to infection. 11miR-124 suppresses multiple steps of breast cancer metastasis by targeting a cohort of pro-metastatic genes in vitro显示文摘Metastasis is a multistep process involving modification of morphology to suit migration, reduction of tumor cell adhesion to the extracellular matrix, increase of cell mobility, tumor cell resistance to anoikis, and other steps. MicroRNAs are well-suited to regulate tumor metastasis due to their capacity to repress numerous target genes in a coordinated manner, thereby enabling their intervention at multiple steps of the invasion-metastasis cascade. In this study, we identified a microRNA exemplifying these attributes, miR-124, whose expression was reduced in aggressive MDA-MB-231 and SK-3rd breast cancer cells. Down-regulation of miR-124 expression in highly aggressive breast cancer cells contributed in part to DNA hypermethylation around the promoters of the three genes encoding miR-124. Ectopic expression of miR-124 in MDA-MB-231 cells suppressed metastasis-related traits including formation of spindle-like morphology, migratory capacity, adhesion to fibronectin, and anoikis. 12Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Gastric Cancer Patients显示文摘Purpose Malnutrition is severe among gastric cancer patients in China. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P < 0.001) were significant in the majority of tests. 13Optimal Waist Circumference Cut-off values for Identifying Metabolic Risk Factors in Middle-aged and Elderly Subjects in Shandong Province of China显示文摘Objective To study the optimal waist circumference(WC) cut-off values for identifying metabolic risk factors in middle-aged and elderly subjects in Shandong Province of China. Methods A total of 2 873 men and 5 559 women were included in this cross-sectional study. Metabolic syndrome(MetS) was diagnosed according to the definition of Chinese Diabetes Society in 2004. The relation between WC and MetS was analyzed by multivariate logistic regression analysis. The optimal WC cut-off values were identified using the area under the ROC curve and the different diagnostic criteria for central obesity were compared. Results The WC was the risk factor for MetS independent of BMI, blood glucose, blood lipid, and blood pressure. The optimal WC cut-off value was 83.8 cm and 91.1 cm for identifying MetS in women and men, respectively. Compared with 80 cm and 85 cm for women and men, 85 cm and 90 cm had a higher Youden index for identifying all metabolic risk factors and MetS in women and men. 15Infrared-transmission spectra and hydrogen content of hydrogenated amorphous silicon显示文摘In this paper, two kinds of methods of calculating the hydrogen content of a-Si:H thin film by means of the wagging mode and the stretching modes of infrared-transmission spectra , are investigated. The reason for the difference in these two calculation results is analyzed. If the contents of SiH2 and (SiH2)n are indicated in terms of a structure factor F=(I840+I880)/I2000, it is shown that the calculation results obtained from the two different methods are almost equal when the refractive index n is approximately 3.4 or the fitting thickness is between 0.71 and 0.89 mm in the case of a small F. It is shown that the ways of fabrication of thin film can influence silicon-hydrogen bonding configuration of a-Si:H film, and different ways of fabrication can lead to different contents of SiH2 and (SiH2)n. The uniformity of the thin film with a big F is bad. In this case, there is great difference between the thickness measured by the SurfCom408A surface profile apparatus and the thickness obtained by fitting the fringes; and the hydrogen contents of a-Si:H films obtained by means of the wagging mode and the stretching modes are different, too. 19Chinese Medicine Shensong Yangxin Capsule(参松养心胶囊)Ameliorates Myocardial Microcirculation Dysfunction in Rabbits with Chronic Myocardial Infarction显示文摘Objective:To investigate the effect of Chinese compound Shensong Yangxin Capsule(参松养心胶囊,SSYX)on myocardial microcirculation in myocardial-infarcted rabbits.Methods:Myocardial infarction(MI)was established in rabbits by ligation of the left circumflex coronary.Thirty rabbits were randomly divided into the control group,the MI group(model),and the MI treated with SSYX group(MI+SSYX)by a random number table method.After 4 weeks of administration,low-energy real-time myocardial contrast echocardiography(RT-MCE)was conducted to assess the microcirculatory perfusion.Immunofluorescence double staining was used to detect the capillary density.The endothelial ultrastructure was observed with a transmission electron microscope.The m RNA expression levels of vascular endothelial growth factor(VEGF),endothelin 1(ET-1),prostaglandin I2(PGI2)and endothelial nitric oxide synthase(e NOS)were measured by real-time quantitative polymerase chain reaction(Real-time PCR).The plasmic levels of ET-1,thromboxane A2(TXA2),nitric oxide(NO)and von willebrand factor(v WF)were examined with enzyme-linked immunosorbent assays(ELISA).Results:SSYX significantly improved the myocardial blood volume,myocardial micro bubble velocity,and myocardial inflow according to the examination of RT-MCE,and it visibly ameliorated the capillary endothelial structure.Furthermore,compared with the MI group,the plasma levels of TXA2,ET-1 and v WF contents significantly decreased in the MI+SSYX group,and the ET-1 mRNA expression levels of myocardium in the border zone significantly decreased,and the VEGF,PGI2 and e NOS m RNA expression levels significantly increased(all P<0.05).Conclusions:SSYX has favorable advantages in ameliorating the impaired myocardial microcirculation following MI.The mechanisms of the effect are related to the ability of SSYX in balancing the endothelial-derived vasodilators and vasoconstrictors,and up-regulating the expression of VEGF and e NOS.JIANG Chan WANG Xi DANG Song WANG Xin DENG Qing HU Juan HUANG Cong-xin 2021Chinese Journal of Integrative Medicine2021,27,1:2
