3Addition of hepatectomy decreases liver recurrence and leads to long survival in hilar cholangiocarcinoma显示文摘AIM:To evaluate hepatic recurrence and prognostic factors for survival in patients with surgically resected hilar cholangiocarcinoma in a single institution over the last 13 years. METHODS:From 1994 to 2007,all patients with hilar cholangiocarcinoma referred to a surgical clinic were evaluated.Demographic data,tumor characteristics, and outcome were analyzed retrospectively.Outcome was compared in patients who underwent additional liver resection with resection of the tumor. RESULTS:Of the 69 patients submitted to laparotomy for tumor resection,curative resection(R0 resection) was performed in 40 patients,and palliative resection in 29.Thirty-one patients had only duct resection,and 38 patients had combined duct resection with liver resection including 34 total or part caudate lobes. Curative rates with the combined hepatectomy were significantly improved compared with those without additional hepatectomy(27/38 vs 13/31;χ2=5.94,P <0.05).Concomitant liver resection was associated with a decreased incidence of initial recurrence in liver one year after surgery(11/38 vs 23/31;χ2=13.98, P<0.01).The 3-year survival rate after R0 resection was 30.7%and was 10.5%for palliative resection.R0 resection improved the 3-year survival rate(30.7%vs 10.5%;χ2=12.47,P<0.01).CONCLUSION:Hepatectomy,especially including the caudate lobe combined with bile duct resection should be considered standard treatment to cure hilar cholangiocarcinoma.Zheng Shi Ming-Zhi Yang Qing-Liang He Rong-Wen Ou You-Ting Chen 2009World Journal of Gastroenterology2009,15,15:12

10Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Lung Cancer Patients显示文摘The prevalence of malnutrition is high among oncology patients in China. Although the Patient- Generated Subjective Global Assessment (PG-SGA) is widely recognized as an important nutritional assessment tool, it has not been validated for Chinese cancer patients. The purpose of this study was to conduct an analysis of nutrition screening and assessment data to validate the Chinese version of the PG-SGA in Chinese patients with lung cancer. This was an observational, cross-sectional study of 2,000 consecutive adult patients with lung cancer treated at several tertiary hospitals in China. Anthropometric and patient descriptive data were collected. The PG-SGA generated a score for the nutritional risk and a global rating for the nutritional status. The internal consistency, external consistency, and construct validity were evaluated, and a known-groups comparison and principal component analyses were conducted to evaluate the reliability and validity of the PG-SGA. The known-groups comparison demonstrated the ability of the PG-SGA to differentiate lung cancer patients of different nutritional and body mass index (BMI) groups. A confirmatory factor analysis supported the original four-factor structure of the PG-SGA. In terms of the internal and external consistency, the PG-SGA demonstrated intra-class correlation coefficients (ICC) up to 0.987. Alternative form validity was also demonstrated between Box 4 of the PG-SGA and the Karnofsky Performance Status score, with excellent external consistency (r = 0.972, P < 0.001). The construct validity was supported, and selected questionnaire dimensions were evident in the principal component analysis. Significant Spearman correlations (P < 0.001) were demonstrated. The Chinese version of the PG-SGA demonstrated significant reliability and sufficient exploratory properties to support its validity. It seems to be a valid tool that can be used to access the nutritional status of Chinese lung cancer patients. 11Protective Effects of Salvianolate on Myocardial Injury or MyocardiaMnfarction after Elective Percutaneous Coronary Intervention in NSTE-ACS Patients:A Randomized Placebo-Controlled Trial显示文摘Objective:To evaluate the protective effects of salvianolate on percutaneous coronary intervention(PCI)related myocardial injury or myocardial infarction after elective PCI in non-ST-segment elevation acute coronary syndrome(NSTE-ACS)patients.Methods:A total of 149 patients with NSTE-ACS who underwent elective PCI were enrolled.The patients were randomly allocated in a 1:1 ratio to the salvianolate group(74 cases)or the control group(75 cases).After exclusion criteria of coronary angiography,60 patients with PCI therapy remained in the salvianolate group and 68 in the control group.The incidence and the severity of PCI related myocardial injury or myocardial infarction,in addition to major adverse cardiac events(MACEs)during 1 year follow-up after PCI were studied between the two groups.Multivariate logistic regression analysis was used to determine the independent factors for PCI related myocardial injury or myocardial infarction after elective PCI.Results:Compared with the control group,salvianolate treatment reduced the incidence of PCI related severe myocardial injury or myocardial infarction(11.7%vs.26.5%,P=0.035).The rate of MACEs or all-cause death within 1 month or 1 year after the procedure was not significantly different between the two groups.Conclusions:Periprocedural treatment with salvianolate reduces the incidence of PCI related severe myocardial injury or myocardial infarction,although it does not influence clinical prognosis.OU Yang SUN Sheng-jia SHI Hai-ming LUO Jian-feng LUO Xin-ping SHEN Yun-zhi CHEN Yu-fei FAN Wei-hu LIU Hong-ying SHEN Wei 2020Chinese Journal of Integrative Medicine2020,26,9:6
12A nomogram to predict prognosis for gastric cancer with peritoneal dissemination显示文摘Objective: To identify independent prognostic factors to be included in a nomogram to predict the prognosis of gastric cancer patients with peritoneal dissemination.Methods: This is a retrospective study on 684 patients with a histological diagnosis of gastric cancer with peritoneal dissemination from the Sun Yat-sen University Cancer Center as the development set, and 62 gastric cancer patients from the Sixth Affiliated Hospital of Sun Yat-sen University as the validation group. Chi-square test and Cox regression analysis were used to compare the clinicopathological variables and the prognosis of gastric cancer patients with peritoneal dissemination. The Harrell's concordance index(C-index) and calibration curve were determined for comparisons of predictive ability of the nomogram.Results: Univariate and multivariate analyses showed that serum carcinoembryonic antigen(CEA) level(P=0.032), ascites grading(P=0.008), presence of extraperitoneal metastasis(P<0.001), seeding status(P=0.016) and performance status(P=0.009) were independent prognostic factors for gastric cancer patients with peritoneal dissemination in the development set. The nomogram model was constructed using these five factors. Internal validation showed that the C-index of the model was 0.641. For the external validation, the C-index of this model was 0.709.Conclusions: We developed and validated a nomogram to predict the prognosis for gastric cancer patients with peritoneal dissemination. 13Validity of the Chinese Version of the Patient-Generated Subjective Global Assessment (PG-SGA) in Gastric Cancer Patients显示文摘Purpose Malnutrition is severe among gastric cancer patients in China. While the Patient-Generated Subjective Global Assessment (PG-SGA) is a widely used nutritional assessment tool, the validation for Chinese patients with gastric cancer has not been performed yet. The aim of this research is to validate the Chinese version of the PGSGA in Chinese gastric cancer patients by conducting analyses of nutrition screening and assessment data. Methods Two thousand consecutive adult patients with gastric cancer were included in this observational, cross-sectional study in several tertiary hospitals in China. A survey was made among those patients, which included several questionnaires and some anthropometric parameters. We calculated the parameters to validate the use of PG-SGA categorizing the nutritional status of gastric cancer patients. Then we performed statistical analysis to evaluate the internal consistency, external consistency, and construct validity. Results This study verified the ability of the PG-SGA to differentiate gastric cancer patients into different nutritional groups. The confirmatory factor analysis verified the original four-factor structure of the PG-SGA. The internal and external consistency was measured by a concordance analysis which showed an intraclass correlation coefficients (ICC) of 0.987. The correlations of the Box 4 score with the Karnofsky performance status score demonstrated alternative form validity with good external consistency (Pearson, r = 0.643, P < 0.001). All three anthropometric dimensions of the PG-SGA were supported by principal component analysis. Spearman correlations (P < 0.001) were significant in the majority of tests. 14Lipid Distribution in Marine Sediments from the Northern South China Sea and Association with Gas Hydrate显示文摘The distributions of lipids in surface and subsurface sediments from the northern South China Sea were determined. The n-alkanes were in bimodal distribution that is characterized by a centre at n-C16 –n-C20 with maximum at C18(or C19) and n-C27 –n-C31 as well as at C29(or C31). The short-chain alkanes suffered from significant losses due to their slow deposition in the water column, and their presence with a slight even carbon predominance in shallow seafloor sediments was ascribed mainly to the direct input from the benthos. The long-chain alkanes with odd predominance indicate transportion of terrigenous organic matter. Immature hopanoid biomarkers reflect the intense microbial activity for bacteria–derived organic matter and the gradual increase of maturity with burial depth. Abundant n-fatty acid methyl esters(n-FAMEs) that are in distributions coincident with fatty acids were detected in all samples. We proposed that the observed FAMEs originated from the methyl esterification of fatty acids; methanol production by methanotrophs and methanogenic archaea related to the anaerobic oxidation of methane, and sulfate reduction provided an O–methyl donor for methylation of fatty acids. 15Immunomodulatory properties of mesenchymal stem cells/dental stem cells and their therapeutic applications显示文摘Mesenchymal stem/stromal cells(MsCs)are widely distributed in the body and play essential roles in tissue regeneration and homeostasis.MsCs can be isolated from discarded tissues,expanded in vitro and used as therapeutics for autoimmune diseases and other chronic disorders.MsCs promote tissue regeneration and homeostasis by primarily acting on immune cells.At least six different types of MsCs have been isolated from postnatal dental tissues and have remarkable immunomodulatory properties.Dental stem cells(DsCs)have been demonstrated to have therapeutic effects on several systemic inflammatory diseases.Conversely,MsCs derived from nondental tissues such as the umbilical cord exhibit great benefits in the management of periodontitis in preclinical studies.Here,we discuss the main therapeutic uses of MSCs/DSCs,their mechanisms,extrinsic inflammatory cues and the intrinsic metabolic circuitries that govern the immunomodulatory functions of MSCs/DSCs.Increased understanding of the mechanisms underpinning the immunomodulatory functions of MSCs/DSCs is expected to aid in the development of more potent and precise MSC/DSC-based therapeutics.Peishan Li Qianmin Ou Songtao Shi Changshun Shao 2023Cellular & Molecular Immunology2023,20,6:4

17RGF1-RGI1,a Peptide-Receptor Complex,Regulates Arabidopsis Root Meristem Development via a MAPK Signaling Cascade显示文摘Root growth is maintained by the continuous division of cells in the apical meristem.ROOT MERISTEM GROWTH FACTOR 1(RGF1)is a critical peptide hormone regulating root stem cell niche maintenance.Previous studies discovered that five closely related leucine-rich repeat receptor-like protein kinases(LRRRLKs),named RGF1 INSENSITIVES(RGIs)or RGF1 RECEPTORS(RGFRs),are able to perceive the RGF1 signal and redundantly control root stem cell niche maintenance.RGF1 regulates root meristem activity mainly via two downstream transcription factors,PLETHORA 1(PLT1)and PLT2.Regulatory proteins connecting cell surface RGF1-RGI1 and nuclear PLTs,however,were not identified.Here,we report that the mitogen-activated protein(MAP)kinase kinase 4(MKK4)and MAP kinase 3(MPK3)were coimmunoprecipitated with RGI1-FLAG after Arabidopsis seedlings were treated with RGF1.Genetic and biochemical assays confirmed that MKK4 and MKK5,and their downstream targets MPK3 and MPK6,are essential RG卜dependent regulators of root meristem development.In addition,we found that the MKK4/MKK5-MPK3/MPK6 module functions downstream of YDA,a MAPKKK.Our results demonstrate that RGF1-RGI1 regulate the expression of PLT1/PLT2 via a YDA-MKK4/MKK5-MPK3/MPK6 signaling cascade.Xiaoting Lu Hongyong Shi Yang Ou Yanwei Cui Jinke Chang Liang Peng Xiaoping Gou Kai He Jia Li 2020Molecular Plant2020,13,11:3

19Revised Super-long Down-regulation Protocol Improves the Outcome of Infertile Patients with Repeated Implantation Failure in IVF/ICSI-ET显示文摘Objective To investigate the outcome of revised super-long down-regulation protocol(RSDP) for in vitro fertilization / intracytoplasmic sperm injection-embryo transfer(IVF/ICSI-ET) in the special infertile patients with repeated implantation failure(RIF).Methods Patients with RIF were divided into RSDP group and routine long downregulation protocol(RLDP) group. In RSDP group, gonadotropin releasing hormone agonist(GnRHa) was injected intramuscularly by 2.5 mg in mid-luteal phase for the first time and 1.25 mg after 28 d; gonadotropin(Gn) was started 14 d later after the second GnRHa dose. IVF/ICSI-ET was performed according to the routine procedure.
